두 개의 수술용 가이드와 Milled-bar를 이용한 임플란트 피개 의치 수복 증례 (Implant-supported milled bar overdenture with two implant surgical guides)

  • 완전 무치악 환자에게서 잔존 치조골의 흡수가 심할 경우 의치의 저작효율과 안정성 향상을 위해 임플란트로부터 지지를 얻는 피개의치를 고려해볼 수 있다. 임플란트 식립 위치를 선정할 때 디지털 진단 프로그램을 이용하면 환자의 해부학적 요소와 최종보철물의 형태를 고려한 수술용 가이드를 만들어 임플란트 수술의 정확도를 높일 수 있다. 임플란트 지지 피개의치에서 다양한 부착장치 중 milled bar 경우 임플란트간의 연결고정 효과뿐만 아니라 견고한 지지를 얻을 수 있는 이점이 있다. 이에 본 증례에서는 치조골 삭제 전, 후의 상황을 가정한 두 개의 수술용 가이드를 이용하여 임플란트를 식립한 후 milled bar를 이용한 임플란트 피개의치를 장착해주어 기능적이고 심미적인 결과를 얻었다.

Clinical evaluation of mandibular implant overdentures via Locator implant attachment and Locator bar attachment

  • PURPOSE. The aim of this study was to evaluate the clinical findings and patient satisfaction on implant overdenture designed with Locator implant attachment or Locator bar attachment in mandibular edentulous patients. MATERIALS AND METHODS. Implant survival rate, marginal bone loss, probing depth, peri-implant inflammation, bleeding, plaque, calculus, complications, and satisfaction were evaluated on sixteen patients who were treated with mandibular overdenture and have used it for at least 1 year (Locator implant attachment: n=8, Locator bar attachment: n=8). RESULTS. Marginal bone loss, probing depth, plaque index of the Locator bar attachment group were significantly lower than the Locator implant attachment group (P<.05). There was no significant difference on bleeding, peri-implant inflammation, and patient satisfaction between the two denture types (P>.05). The replacement of the attachment components was the most common complication in both groups. Although there was no correlation between marginal bone loss and plaque index, a significant correlation was found between marginal bone loss and probing depth. CONCLUSION. The Locator bar attachment group indicates lesser marginal bone loss and need for maintenance, as compared with the Locator implant attachment group. This may be due to the splinting effect among implants rather than the types of Locator attachment.

  • 2급 악간관계를 갖는 환자는 1급 악간관계인 환자와는 달리 불안정한 교합을 가질 수 있으며, 무치악인 경우 총의치를 사용할 때 불안정한 교합을 보이면서 의치의 움직임이 커질 가능성이 있다. 이런 환자에 있어 임플란트 피개의치를 사용함으로써 의치의 안정을 향상시킬 수 있다. 임플란트 피개의 치에서 attachment 종류는 크게 bar type과 solitary type으로 나눌 수 있는데 각각 적응증이 있어 상황에 따라 적용을 달리 하여야 한다. 본 증례는 2급 악간관계를 가진 하악 무치악 환자에서 임플란트 피개의치를 계획함에 있어 디지털 기술을 이용함으로써 더 적합한 attachment를 선택하고 임플란트의 위치 및 깊이를 미리 계획하여 계획한 위치에 식립하였다. 상악은 국소의치로 수복하였고 하악은 임플란트 피개의치로 수복하여 만족할만한 결과를 얻었기에 이를 보고하고자 한다.

  • 잔존골 흡수가 진행된 상악 무치악 환자의 수복 계획시 전통적인 총의치로의 치료보다는 임플란트를 식립 한 뒤 milled bar와 가철성 보철물을 제작하여 attachment로 연결하는 임플란트 피개의치를 우선적으로 고려할 수 있다. Milled bar와 attachment를 사용하면 상악 의치의 유지력 및 안정성의 증가뿐만 아니라 구개 피개 부위를 일부 감소시킬 수 있어 환자의 불편감을 줄여줄 수 있다는 장점이 있다. 또한 소수의 임플란트를 식립 할 경우 상부 보철물을 milled bar로 제작하면 임플란트 고정체 간에 연결고정되는 효과가 있어 저작과 같은 기능 하에서 역학적으로 유리해진다. 본 증례의 환자는 기존의 실패한 임플란트를 제거한 뒤 잔존 치조골이 상당량 소실된 상악 무치악 환자로 양측에 각 3개의 임플란트를 식립한 후 milled bar와 ADD-TOC attachment를 사용하여 임플란트 피개의치를 제작하였다. Attachment 장착을 통해 얻어진 부가적인 유지력을 바탕으로 구개부 피개량 감소하여 총의치 제작 가능하였고 이렇게 milled bar와 attachment를 병용하여 충분한 유지력 뿐만아니라 지지 및 안정을 얻음으로써 심미성 및 기능성 모두에서 만족할 만한 결과를 얻었기에 보고하는 바이다.

Trueness of 3D printed partial denture frameworks: build orientations and support structure density parameters

  • PURPOSE. The purpose of the study was to assess the influence of build orientations and density of support structures on the trueness of the 3D printed removable partial denture (RPD) frameworks. MATERIALS AND METHODS. A maxillary Kennedy class III and mandibular class I casts were 3D scanned and used to design and produce two 3D virtual models of RPD frameworks. Using digital light processing (DLP) 3D printing, 47 RPD frameworks were fabricated at 3 different build orientations (100, 135 and 150-degree angles) and 2 support structure densities. All frameworks were scanned and 3D compared to the original virtual RPD models by metrology software to check 3D deviations quantitatively and qualitatively. The accuracy data were statistically analyzed using one-way ANOVA for build orientation comparison and independent sample t-test for structure density comparison at (α = .05). Points study analysis targeting RPD components and representative color maps were also studied. RESULTS. The build orientation of 135-degree angle of the maxillary frameworks showed the lowest deviation at the clasp arms of tooth 26 of the 135-degree angle group. The mandibular frameworks with 150-degree angle build orientation showed the least deviation at the rest on tooth 44 and the arm of the I-bar clasp of tooth 45. No significant difference was seen between different support structure densities. CONCLUSION. Build orientation had an influence on the accuracy of the frameworks, especially at a 135-degree angle of maxillary design and 150-degree of mandibular design. The difference in the support's density structure revealed no considerable effect on the accuracy.

BIOMECHANICS OF ABUTMENTS SUPPORTING REMOVABLE PARTIAL DENTURES UNDER UNILATERAL LOADING

  • Statement of problem. In distal extension removable partial denture, the preservation of health of abutment teeth is very important, but abutment teeth are subjected to unfavorable stress under unilateral loading specially. Purpose. The purpose of this study was to investigate the biomechanical effects of mandibular removable partial dentures with various prosthetic designs under unilateral loading, using strain gauge analysis. Material and methods. Artificial teeth of both canines were anchored bilaterally in a mandibular edentulous model made of resin. Bilateral distal extension removable partial dentures with splinted and unsplinted abutments were fabricated. Group 1: Clasp-retained mandibular removable partial denture with unsplinted abutments Group 2: Clasp-retained mandibular removable partial denture with splinted abutments by 6-unit bridge. Group 3: Bar-retained mandibular removable partial denture Strain gauges were bonded on the labial plate of the mandibular resin model, approximately 2 mm dose to the abutments. Two unilateral vertical experimental loadings (30N and 100N) were applied subsequently via miniature load cell that were placed at mandibular left first molar region. Strain measurements were performed and simultaneously monitored from a computer connected to data acquisition system. For within-group evaluations, t-test was used to compare the strain values and for between-group comparisons, a one-way analysis of variance (ANOVA) was used and Tukey test was used as post hoc comparisons. Results. The strain values of group 1 and 2 were tensile under loadings. In contrast, strain values of group 3 were compressive in nature. Strain values increased as the applied load in increased from 30N to 100N (p<.05) except for right side in group 1. Under 30N loading, in left side, group 1 showed higher strain values than groups 2 and 3 in absolute quantity (p<.05). And group 2 showed higher strain values than group 1 (p<.05). In right side, group 1 and 2 showed higher strain values than group 3 in absolute quantity (p<.05). Under 100N loading in left side, group 1 showed higher strain values than groups 2 and 3 in absolute quantity (p<.05). And group 2 showed higher strain values than group 1 (p<.05). In right side, group 1 and 2 showed higher strain values than group 3 in absolute quantity (p<.05). Under 30N loading, group 2 and 3 showed higher strain values in right side than in left side. Under 100N loading, right side strain values were higher than left side ones for all groups. Conclusion. Splinting of two isolated abutments by bridge reduced the peri-abutment strain in comparison with unsplinted abutments under unilateral loading. Bar-retained removable partial denture showed the lowest strain of three groups, and compressive nature.

세 가지 방식으로 제작한 하악 총의치의 중합 전후에 따른 인공치아 위치 3차원 분석 (Three-dimensional analysis of artificial teeth position according to three type complete mandibular denture before and after polymerization)

  • Purpose: The aim of this study was to evaluate accuracy of three type complete mandibular denture of before and after polymerization. Methods: Mandibular edentulous model was selected as the master model. 15 study models were made by Type IV stone. Wax complete mandibular dentures were produced by the denture base and artificial teeth. Before and after curing, STL files were obtained using a blue scanner. By superimposing the digitized complete mandibular denture data(after curing) with the CAD-reference(before curing) three-dimensionally, visual fit-discrepancies were drawn by calculating the root mean square (RMS) and visualized on a color-difference map. Each calculated RMS-value was statistically analyzed by 1-way analysis of variance(ANOVA) (${\alpha}=.05$). Results: Mean(SD) RMS-values was OM group $88.98(6.10){\mu}m$, BM group $82.35(13.46){\mu}m$, BDM group $77.83(9.46){\mu}m$. The results of the 1-way ANOVA showed no statistically significant differences in the RMS values of the Three groups for the material (P > .241). Conclusion : Deformation of artificial teeth position was observed in all groups after resin polymerization. But the values, all group were within the clinically acceptable range. The values of BDM group showed the least deformation than the other two groups.

주 연결장치의 설계변화에 따른 하악 유리단 국소의치의 광탄성 응력 분석에 관한 연구 (A PHOTOELASTIC STUDY ON THE STRESS ANALYSIS UNDER MADIBULAR DISTAL-EXTENSION REMOVABLE PARTIAL DENTURE WITH DIFFERENT DESIGN OF THE MAJOR CONNECTOR)

  • The purpose of this study was to analyze the magnitude and distribution of stress using a photoelastic model from a distal extension removable partial dentures with three kinds of mandibular major connectors, that is, lingual bar, linguoplate, and swing-lock attachment. A photoelastic model was made of the epoxy resin(PC-1) and hardener(PCH-1) and coated with plastic cement-1 (PC-1) at the lingual surface of the epoxy model and set with three kinds of chrome-cobalt removable partial dentures. A bilateral vertical load of 15kg to the middle portion of the metal bar crossing both the first molars of the right and the left, and a unilateral vertical load of 12.5kg to the right first molar were applied with the use of specially designed loading device and the reflective circular polariscope was used to analyze the photoelastic model under each condition. The following results were obtained : 1. When the bilateral vertical load was applied, the magnitude and distribution of the stress concentration of the edentulous area and the terminal abutment or adjacent teeth was in the order of lingual bar, linguoplate, swing-lock attachment. 2. When the unilateral vertical load was applied, the magnitude and distribution of the stress concentration of the edentulous area and the terminal abutment or adjacent teeth was in the order of lingual bar, linguoplate, swing-lock attachment. 3. When the unilateral vertical load was applied, the magnitude and distribution of the stress concentration of the termial abutment or adjacent teeth on the non-loaded side showed the least stress distribution in case of swing-lock attachment. 4. When the bilateral vertical load and the unilateral vertical load were applied the swing-lock attachment showed the mildest uniform stress distribution on the edentulous area and the alveolar bone around the abutment teeth.

유리단 국소의치에서 Suprabulge Clasp와 Infrabulge Clasp가 지대치 동요에 미치는 영향에 관한 연구 (Effect of Suprabulge Clasp and Infrabulge Clasp on the Mobility of Abutment Teeth for Distal Extension Removable Partial Dentures)

  • Distal-extension removable partial dentures have long been implicated in the increase in mobility and the destruction of the supporting structures of the primary abutment teeth. Various clasping systems have traditionally been used to retain distal extension removable partial dentures, and other designs have been proposed to minimize torquing forces on the abutment teeth. Most recent studies investigating the effects of removable partial dentures on abutment teeth have been performed in it laboratory setting. Results obtained from in vitro research have given dentists insight into removable partial denture design, but laboratory test model cannot be constructed that simulates actual functional or parafunctiona1 movements and forces. The purpose of this study was to clinically evaluate the degree of tooth mobility produced by two clasping systems (suprabulge type and infrabulge type) used for distal extension removable partial dentures. Akers clasp and R.P.I. system were selected for the evaluation, and four patients required a distal extension removable partial denture on the mandibular arch were selected for participation in the study. Two partial dentures were constructed in the same condition expect the design of clasp. All abutments in the study were mandibular first or second premolars. Measurements of mobility were made with a research tool designed by $M\"{u}hlemann$. This instrument, periodontometer, measures tooth mobility in the mouth by means of a dial gauge accurated to 0.01mm when the tooth is stressed with a force meter. Lingual and buccal deflection of abutment tooth was measured using buccal and lingual pressure. The amount of force applied was 500gm. Tooth mobility tests were made at four key stages; 1. Before insertion of the first removable partial denture, baseline mobility was establsihed. 2. After wearing of the first prosthesis, measurement was made at weekly intervals for 4 weeks. 3. The removable partial denture was then taken from the patient, and tooth mobility was measured again at weekly intervals until the patient's established baseline mobility had returned. 4. The second prosthesis of different clasp design was worn for a month and evaluated in the same manner as the first. The sequence of placement of clasping system was alternated between patients. The following results were obtained from this study; 1. The mobility of abutment tooth increased during the initial stage of wear and returned to baseline mobility after removal of removable partial dentures. 2. The mobility of abutment tooth showed no difference between Akers clasp and I-bar clasp during the 4-week test period. 3. All teeth tested showed greater mobility toward the buccal than the lingual direction.

Partial denture metal framework may harbor potentially pathogenic bacteria

  • PURPOSE. The aim of this study was to characterize and compare bacterial diversity on the removable partial denture (RPD) framework over time. MATERIALS AND METHODS. This descriptive pilot study included five women who were rehabilitated with free-end mandibular RPD. The biofilm on T-bar clasps were collected 1 week ($t_1$) and 4 months ($t_2$) after the RPD was inserted ($t_0$). Bacterial 16S rDNA was extracted and PCR amplified. Amplicons were cloned; clones were submitted to cycle sequencing, and sequences were compared with GenBank (98% similarity). RESULTS. A total of 180 sequences with more than 499 bp were obtained. Two phylogenetic trees with 84 ($t_1$) and 96 ($t_2$) clones represented the bacteria biofilm at the RPD. About 93% of the obtained phylotypes fell into 25 known species for $t_1$ and 17 for $t_2$, which were grouped in 5 phyla: Firmicutes ($t_1=82%$; $t_2=60%$), Actinobacteria ($t_1=5%$; $t_2=10%$), Bacteroidetes ($t_1=2%$; $t_2=6%$), Proteobacteria ($t_1=10%$; $t_2=15%$) and Fusobacteria ($t_1=1%$; $t_2=8%$). The libraries also include 3 novel phylotypes for $t_1$ and 11 for $t_2$. Library $t_2$ differs from $t_1$ (P=.004); $t_1$ is a subset of the $t_2$ (P=.052). Periodontal pathogens, such as F. nucleatum, were more prevalent in $t_2$. CONCLUSION. The biofilm composition of the RPD metal clasps changed along time after RPD wearing. The RPD framework may act as a reservoir for potentially pathogenic bacteria and the RPD wearers may benefit from regular follow-up visits and strategies on prosthesis-related oral health instructions.
