상악 부분 무치악 환자에서 Milled-bar와 어태치먼트를 이용한 임플란트 부분 피개의치 수복 증례 보고 (Implant-assisted removable partial denture using Milled-Bar and Attachment in partially edentulous maxilla: A case report)

  • 부분 무치악 환자의 구강회복을 위해서는 임플란트를 이용한 치료 방법이 널리 활용되고 있다. 임플란트를 이용한 치료방법은 크게 고정성 임플란트 보철물과 가철성 임플란트 보철물로 나뉠 수 있으며, 골 흡수가 심한 환자의 경우 심미적이고 경제적인 이점을 갖는 가철성 임플란트 보철물이 치료 옵션으로 활용될 수 있다. 특히 소수의 임플란트에 Milled-bar와 부가적인 유지장치를 이용한 보철물 제작 시, 임플란트 고정체 간의 연결고정 효과 및 의치의 유지력과 안정성이 증가할 수 있다. 또한 의치상을 통한 구순 지지를 통해 심미적인 보철물 제작이 가능하다. 본 증례의 환자는 상악골의 심한 골 흡수를 동반한 소수의 잔존치를 갖는 부분 무치악 환자로 전치부 및 소구치부위에 임플란트를 3개 식립 후 캐드캠을 이용하여 제작된 Milled-bar와 ADD-TOC 어태치먼트를 이용한 임플란트 부분 피개의치로 치료하였다. 그 결과 어태치먼트를 이용한 clasp 제거 및 의치상을 이용한 구순 지지를 통해 심미성을 높였으며, Milled-bar를 통한 교합력 지지와 의치 안정성을 확보하였다. 또한 환자의 이물감 해소를 위해 구개부 피개량을 줄인 금속 구조물의 피개의치를 제작하였으며, 만족스러운 결과를 얻었기에 보고하는 바이다.

  • Treatment with removable partial denture is effective for partially edentulous patients who are unable to obtain sufficient retention and stability for functional and esthetic restoration. There are several cases reporting the improvement of retention and stability of the partial denture using a small number of implants. However, there are limited studies on the implant-assisted removable partial denture using a small number of remaining teeth and the bar locator system. The bar locator system has an advantage in that it could compensate the angle of insertion of removable prosthesis on implant with inconsistent placement angle due to anatomical constraints compared to when using the locator only. This case report describes the patient with $Parkinson^{\circ}$Øs disease who was treated with the Locator bar system using two previously placed implants and two remaining teeth on maxilla. No additional implants could be placed because of the medical and economic condition of the patients, and the angle of one of two implants could not be matched with the direction of the removable partial denture insertion. Considering the angle of the implants, the patient was treated with implant-assisted RPD using the Locator bar system and had satisfactory results in the aspect of esthetics, masticatory function, and oral hygiene maintenance.

무치악 환자에서 기성 조립식 bar를 이용한 임플란트 피개의치 증례 (Implant-retained overdentures with pre-fabricated bar attachment system in edentulous patients)

  • 치아 상실에 이어 심각한 치조골 소실을 보이는 무치악 환자들은 전통적인 방식의 총의치를 사용하는데 어려움이 많다. 이러한 문제를 보완할 수 있도록 임플란트를 이용한 피개의치가 사용되고 있다. SFI-bar$^{(R)}$는 진료실에서 직접 조정이 가능한 기성품으로 복잡한 기공과정을 필요로 하는 기존의 바 시스템보다 간결하여 시간과 비용을 절감할 수 있다. 첫 번째 증례는 55세 남자 환자로 기존 하악 임플란트-지지 피개의치가 파절되었다는 주소로 내원하였고 내구성이 있으며 통증이 없는 의치를 원하였다. 파절된 Locator$^{(R)}$ 어태치먼트를 제거 후 바를 이용한 피개의치로 치료하였다. 두 번째 증례는 파킨슨병을 앓고 있는 77세 여자 환자로 하악 치조골이 심각하게 흡수되어 의치가 자주 탈락한다는 주소로 내원하였고 통증이 없고 유지력이 있는 의치를 원하였다. 하악 전치부에 두개의 임플란트를 식립한 후 임플란트에 연결된 어댑터에 바를 연결하였다. 그 후 바에 맞는 여성부를 새로운 의치에 장착하여 치료하였다. 2년의 추적 관찰 결과 상기 환자 모두 심미적, 기능적으로 만족할만한 결과를 보였으므로 증례를 보고하고자 한다.

  • The scars and contracture around the oral-facial region may cause difficulty in prosthodontic treatment to restore esthetics and function for the patients, who suffered severe burns. This article presents a technique that uses a fixed partial denture prepared with a conventional milling technique and an attachment to support anterior cantilever removable partial denture, thereby providing a more esthetically acceptable and functional result.

Immediate loading on mandibular edentulous patient with SFI Bar$^{(R)}$ overdenture

  • Despite the greater retention and low maintenance of bar attachment system, the longer clinical time and increased number of visits were the assignments to overcome in bar attachment system. This case report describes SFI-Bar$^{(R)}$ (Cendres et Me$\'{e}$taux, Biel/Bienne, Switzerland) to be solve those problems. A 65-year-old female, who had severely absorbed mandible, hoped to wear a stable mandibular denture without pain. As soon as two implants were placed on mandible, a tube bar was connected to two adaptors connected to each implant. The length of the tube bar was adjusted considering inter implant distance, and reconnected to the adaptors. Finally a female part was seated beneath the denture. This case report showed that a satisfactory clinical result was achieved by delivering bar overdenture immediately after implant placement without laboratory procedure.

하악 견치 잔존시 의치 설계에 따른 치아 지지조직 및 잔존치조제의 광탄성 응력분석 (A PHOTOELASTIC STRESS ANALYSIS ON TOOTH SUPPORTING STRUCTURE AND RESIDUAL RIDGE ACCORDING TO DENTURE DESIGN FOR REMAINING MANDIBULAR CANINES)

  • The purpose of this study was to evaluate the stress patterns developed in abutment and residual ridge according to removable denture design in case of remaining mandibular canines. The removable denture designs in this study were as foolows : 1. Removable partial denture with non-splinted abutment 2. Removable partial denture with splinted abutment 3. Overdenture with telescopic crown 4. Overdenture with O-Ring attachment 5. Overdenture with combination bar attachment Photoelastic stress analysis was used to record the isochromatic fringe patterns and to calculate principal stress components at measuring points. The results were as follows : 1. In case of removable partial denture with non-splinted abutment, stress of root area at the loaded side was the largest. No significant differences in stress of root area were observed between loaded side and unloaded side. 2. No significant differences in stress of residual ridge at the loaded side were observed between removable partial denture with splinted abutment and removable partial denture with non-splinted abutment. 3. In case of combination bar attachment retained overdenture, stress of root area was the largest and in case of telescopic crown retained overdenture, stress of root area was the lowest. 4. In case of attachment retained overdenture, stress of residual ridge was lower but stress of root area was larger than in case of removable partial denture.

구순구개열 환자에서 Locator® 유지장치가 장착된 milled titanium bar를 이용한 가철성 임플란트 피개 국소의치의 보철수복증례 (Removable implant-supported partial denture using milled bar with Locator® attachments in a cleft lip & palate patient: A clinical report)

  • 본 증례는 구순구개열 환자에서 상순의 반흔, 상악 잔존치조제의 과도한 흡수, 협소한 구개 및 상악골의 열성장을 동반한 3급 악간관계로 인해 통상의 가철성 국소의치 수복에 한계를 보이기 때문에 상악 무치악 부위에 4개의 임플란트를 식립 후, CAD/CAM 시스템으로 밀링한 titanium bar를 이용한 상악 가철성 임플란트 피개 국소의치를 계획하였다. 통상의 주조방법은 주조 후 수축을 하게 되지만 CAD/CAM 시스템으로 밀링한 titanium bar는 높은 정밀성과 경제성 그리고 가벼운 장점이 있다. 하지만 실제 임상에서 milled bar는 아주 균등하고 정확한 마찰결합을 얻기 힘들며 의치의 반복적인 착탈에 의해 마찰 유지력이 감소 할 수 있다. 이에 milled bar와 함께 사용할 수 있는 다양한 보조 유지장치(e.g. friction pin, $ERA^{(R)}$, $CEKA^{(R)}$, magnetics, $Locator^{(R)}$ 등)를 같이 사용함으로써 부가적인 유지력, 경제성 및 교체의 용이성 등을 추가적으로 얻을 수 있다. 본 증례에서 milled titanium bar의 상부면에 drill and tapping 방법으로 $Locator^{(R)}$ 유지장치를 연결하였다. milled bar에 screw hole을 형성하고 3개의 $Locator^{(R)}$ 유지장치를 20 Ncm으로 체결하여 추가적인 유지력을 확보하였다. 보철수복 후, 환자가 원하는 심미적인 안모, 저작 기능 및 의치 유지에 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.

Stress analysis of mandibular implant overdenture with locator and bar/clip attachment: Comparative study with differences in the denture base length

  • PURPOSE. The design of the attachment must provide an optimum stress distribution around the implant. In this study, for implant overdentures with a bar/clip attachment or a locator attachment, the stress transmitted to the implant in accordance with the change in the denture base length and the vertical pressure was measured and analyzed. MATERIALS AND METHODS. Test model was created with epoxy resin. The strain gauges made a tight contact with implant surfaces. A universal testing machine was used to exert a vertical pressure on the mandibular implant overdenture and the strain rate of the implants was measured. RESULTS. Means and standard deviations of the maximum micro-deformation rates were determined. 1) Locator attachment: The implants on the working side generally showed higher strain than those on the non-working side. Tensile force was observed on the mesial surface of the implant on the working side, and the compressive force was applied to the buccal surface and on the surfaces of the implant on the non-working side. 2) Bar/clip attachment: The implants on the both non-working and working sides showed high strain; all surfaces except the mesial surface of the implant on the non-working side showed a compressive force. CONCLUSION. To minimize the strain on implants in mandibular implant overdentures, the attachment of the implant should be carefully selected and the denture base should be extended as much as possible.

편측성으로 설계된 하악 유리단 국소의치에서 직접유지장치의 설계 변화에 따른 광탄성 응력 분석에 관한 연구 (A PHOTOELASTIC STRESS ANALYSIS IN MANDIBULAR DISTAL EXTENSION REMOVABLE PARTIAL DENTURE DESIGNED UNILATERALLY WITH DIFFERENT DIRECT RETAINERS)

  • The purpose of this study was to analyze the magnitude and distribution of stress using a photoelastic model from a unilateral distal extention removable partial dentures with five kinds of the direct retainers, that is, the bilaterally designed bar clasp of the cross-arch lingual bar and the unilaterally designed bar clasp, circumferential clasp, mini-Dalbo attachment, and telescope retainer. A photoelastic model for mandible was made of the epoxy resin(PL-1) and hardner (PLH-1) with the acrylic resin teeth used and was coated with plastic cement-1 at the lingual surface of the model, and then five kinds of removable partial dentures were set, A unilateral vertical load of about 16Kg was applied on the first molar and the stress pattern of the photoelastic model under each condition was analyzed by the reflective circular polariscope. The following results were obtained: 1. The conventional removable partial denture with the bilaterally cross arch lingual bar produced the most favorable stress distribution on the residual ridge and supporting structure of abutment teeth than the unilaterally designed removable partial dentures. 2. The unilaterally designed removable partial denture with the bar clasp produced the stress distribution on the residual ridge, except sligtly higher stress concentration on the supporting structure of the abutment teeth, similar to the conventional removable partial denture with the bilaterally designed cross arch lingual bar. 3. On the unilaterally designed removable partial dentures, the bar clasp produced greater stress distribution on the residual ridge and supporting structure of the abutment teeth than the circumferential clasp. 4. On the unilaterally designed removable partial dentures, the mimi-Dalbo attachment produced relatively higher stress concentration on the residual ridge, but produced lesser stress concentration on the supporting structure of the abutment teeth than the other direct retainers. 5. On the unilaterally designed removable partial dentures, the telescope retainer produced uniform stress distribution on the residual ridge, but produced higher stress concentration at the root apex of the terminal abutment tooth than the other direct retainers. 6. On the unilaterally designed removable partial dentures the circumferential clasp and telescope retainer produced slightly higher stress concentration on the residual ridge and supporting structure of the abutment teeth than the bar clasp and mini- Dalbo attachment.
