Transosseous Tie Fixation Using Krackow Sutures and Bone Tunnels in a Comminuted Fracture of the Upper Pole of the Patella: A Case Report

  • Comminuted fractures of the patella mostly occur at the inferior pole and require appropriate reduction and fixation to restore the extensor mechanism. Conventional methods such as tension-band wiring are not enough to gain proper fixation strength. Numerous methods have been reported, including circumferential cerclage wiring, osteosynthesis, and suture anchors depending on the fracture pattern. Herein, the author reports a relatively rare case of a comminuted fracture of the upper pole of the patella, for which we used augmented Krackow sutures in the quadriceps and fixation with tying of the suture limbs through patellar bone tunnels. Satisfactory results were obtained in terms of reduction and extensor mechanism recovery.

Application of Hinged Transarticular External Skeletal Fixator (HTAESF) for Proximal Tibial Physeal Fracture in a Dog (개의 근위 경골 성장판 골절에서 경첩 관절경유 외고정의 적용)

  • An 8-month-old, 3.5 kg intact female Toy Poodle was presented for non-weight-bearing lameness on left hindlimb. In radiological testing, left proximal tibal type II Salter-Harris physeal fracture and fibular fracture were seen. Following open reduction, the fracture was stabilized with cross-pins, tension band wires, and a hinged transarticular external skeletal fixator (HTAESF). The range of the HTAESF was increased to $25^{\circ}$ at 7 days postsurgery and to $70^{\circ}$ at 14 days post-surgery. The HTAESF was removed 3 weeks after surgery. At 6 weeks post-surgery, the fracture was successfully healed with no complications and the patient recovered a normal gait. Seven months post-surgery, the patient had a normal gait and a normal stifle joint range of motion compared to the contralateral normal limb. This is a case in which the combined use of cross-pins, tension band wires, and HTAESF was successful for treatment of a proximal tibial physeal fracture in a dog. It is thought that these methods are beneficial for stability of fracture site and recover of joint's normal range of motion through early joint movement.

  • Recently, polymer insulators that are used for high voltage applications have some advantages such as light weight, small size, vandalism resistance, hydrophobicity and easy making process. During outdoor service of polymer insulators, the surface of the insulating material is frequently subjected to moisture and contamination that lead to dry band arcing. Their tracking resistance, erosion resistance, end sealing and shed design are very important because dry band arcing causes degradation of polymer surface. Aging test to estimate life property of polymer insulator is executed through several international standard such as IEC 61109 and CEA tracking wheel test, but is not getting clear conclusion yet. There are two methods in the diagnosis method of polymer insulator such as off-line and on-line. The diagnosis methods in off-line are external condition analysis by the eye, contaminant analysis on surface, surface analysis, pollution withstand voltage test, power frequency flashover voltage test, lightning impulse flashover test, tensile fracture load test and flexural load test. Polymer material is also investigated it's tracking resistance by adding surface active agent in IEC 587. In this paper, the tracking performance of polymer insulator with salt solution which is added surface active agent. The diagnosis of insulator sample has been analyzed by leakage current and visual examination, STRI guide and thermal image camera.

High Transparent Planar Dipole Antenna using Ionized Salt-water of ASA Structure (이온화된 소금물을 이용한 ASA 구조의 고 투명 평면형 다이폴 안테나)

  • This feasibility study evaluated an optically transparent planar antenna using liquid salt-water as the conducting material. The most significant reason behind using liquid salt-water for transparent antenna applications is its excellent average optical transparency (OTav) (> 95% at a salinity of 40 ppt) compared to other typical solid transparent thin-film electrodes, such as indium tin oxide (ITO:> 73%) or multi-layer films (MLF: > 78%). Each conductive arm of the proposed dipole is constructed from a salt-water layer held between two clear planar acrylic layers (��r = 2.61, tan�� = 0.01, OTav > 90%) (acrylic/salt-water/acrylic; ASA) due to surface tension. To examine the electrical and optical properties of the ASA structure, the surface tension was measured to determine the thickness of the salt-water layer that finalized its sheet resistance and OTav. The average gain and efficiency of the antenna were 1.72 dBi and 74%, respectively, in the operating UHF (Ultra high frequency) band (470-771 MHz). Therefore, the proposed antenna can be a good candidate for applications as a transparent planar antenna using salt-water.

  • Sound absorption characteristics of membrane system which are used in stadiums and arenas were investigated. Theoretical studies on acoustic properties of single and double leaf permeable membrane conducted. Also, experimental studies on sound absorption characteristics of combined membrane system that is composed of outer and inner membrane material were conducted. In this study, sound absorption characteristics of each membrane were investigated by experiments in reverberation chamber. 4 types of permeable membranes and a non-permeable membrane were used for experiments. Air space behind membrane material and tension on the membrane was varied. Sound absorption performance of permeable membrane materials was confirmed. As increasing air space behind the membrane material, sound absorption coefficient was increased. In a resonance absorption frequency band sound absorption coefficient varied more dramatically. Sound absorption characteristics were flat in mid and high frequency range and sound absorption coefficient was from 0,3 to 0,5. Also sound absorption coefficient was increased by the increment of surface density and air permeability of membrane. However, over the certain value of air permeability, sound absorption coefficient was decreased. These results can be used as design factors and method for the room acoustic design of dome-stadiums and large free-form buildings.

The Effects of Task-oriented Training on Kinetic Factors and Muscle Activities of CVA Patients (과제지향성 훈련이 뇌졸중 환자의 운동역학적 변인 및 근활성도에 미치는 영향)

  • The purpose of this study was to examine the effects of Task-oriented training on the balance, lapse time and MVC(Maximum Voluntary Contraction) of CVA(cerebral vascular accident) patients. The active balance equipment was used to measure of the static balance, lapse time and task-oriented training. The EMG technique was used to record muscle activitie of affected side of gluteus medius and vastus medialis. The raw EMG data were filtered with band pass filter (60Hz) to remove artifacts and then low pass filtered (20Hz) to find the linear envelope which resemble muscle tension curve. The experiment had been conducted at the department of physical therapy in J hospital in M city during 8weeks. The thirty patients experienced the stroke were participated: training group (15), control group (15). They were ambulatory with or without an assistive device. They were assessed on central perturbation(mm) in the static balance, lapse time(s) and MVC test(mV). The data were analyzed using repeated measured ANOVA. The results were as follows: After Task-oriented training, central perturbation and lapse time was significantly differences in both groups(p<.001), and MVC in gluteus medius were no significant differences in both groups, but vastus medialis was significant differences in both groups (p<.001).

Evaluation of the Weldability of Cu Sheet through the Ultrasonic Metal Welding Experiment (Cu박판의 초음파 금속 용착 실험을 통한 용착성 평가)

  • The Ultrasonic metal welding is used in the solid-phase welding method at room temperature or low temperature state. In welding process, the high frequency vibration energy is delivered to the welding part under the constant pressure for welding. In this study, we aimed to design and manufacture a 40,000 Hz band horn through finite element analysis. By performing modal analysis and harmonic response analysis, the modal analysis result is that the horn frequency was 39,599Hz and the harmonic response result that the horn frequency was 39,533Hz. These results were similar. In order to observe the designed horn's performance, about 4,000 voltage data was obtained from a light sensor and was analyzed by FFT analysis using Origin Tool. The result RMS amplitude was approximately $8.5{\mu}m$ at 40,000Hz, and maximum amplitude was $12.3{\mu}m$. Using this manufactured horn along with an ultrasonic metal welder and tension tester, the weldability of Cu sheets was evaluated. The maximum tensile force was 66.53 N in the welding condition of 2.0 bar pressure, 60% amplitude, and 0.32 s welding time. In excessive welding conditions, it was revealed that weldability is influenced negatively.

Radicular Pain due to Subsidence of the Nitinol Shape Memory Loop for Stabilization after Lumbar Decompressive Laminectomy

  • A number of dynamic stabilization systems have been used to overcome the problems associated with spinal fusion with rigid fixation recently and the demand for an ideal dynamic stabilization system is greater for younger patients with multisegment disc degeneration. Nitinol, a shape memory alloy of nickel and titanium, is flexible at low temperatures and regains its original shape when heated, and the Nitinol shape memory loop (SML) implant has been used as a posterior tension band mostly in decompressive laminectomy cases because the Nitinol implant has various characteristics such as high elasticity and a tensile force, flexibility, and biological compatibility. The reported short-term outcomes of the application of SMLs as posterior column supporters in cervical and lumbar decompressive laminectomies seem to be positive, and complications are minimal except for the rare occurrence of pullout and fracture of the SML. However, there was no report of neurological complications related to neural compression in spite of the use of the loop of SML in the epidural space. The authors report a case of delayed development of radiating pain caused by subsidence of the SML resulting epidural compression.

  • Distal clavicular fracture frequently requires operative treatment due to high rate of non-union. The operative technique includes the tension band wiring, K- wire fixation, and cerclage wiring etc. Each method has disadvantages somewhat like pin migration or acromioclavicular joint injury and so on. For the distal clavicular fracture type 2a, because of its oblique fracture line, the cerclage wiring is suitable. We performed the cerclage wiring percutaneously under minimal incision without injury to periosteum for the patient who had the distal clavicular fracture type 2a, and the result was favorable.

  • Since the olecranon fractures are caused by relatively low-energy injuries, such as a fall from standing height, they are usually found without comminution. Less commonly they can be developed by high-energy injuries and have severe concomitant comminution or injuries to surrounding structures of the elbow. Because the fracture by nature is intra-articular with the exception of some avulsion-type fracture, a majority of olecranon fractures are usually indicated for surgical treatment. Even if there is minimal displacement, surgical treatment is recommended because there is a possibility of further displacement by the traction force of triceps tendon. The most common type of olecranon fracture is displaced, simple non-comminuted fracture (that is, Mayo type IIA fractures). Although tension band wiring was the most widespread treatment method for these fractures previously, there is some trends toward fixation using locking plates. Primary goal of the surgery is to restore a congruent joint and extensor mechanisms by accurate reduction and stable fixation so that range of motion exercises can be performed. The literature has shown that good clinical outcomes are achieved irrespective of surgical fixation technique. However, since the soft tissue envelope around the elbow is poor and the implants are located at the subcutaneous layer, implant irritation is still the most common complication associated with surgical treatment.
