Understanding the Biomechanical Factors Related to Successful Balance Recovery and Falls: A Literature Review

  • Background: Despite fall prevention strategies suggested by researchers, falls are still a major health concern in older adults. Understanding factors that differentiate successful versus unsuccessful balance recovery may help improve the prevention strategies. Objects: The purpose of this review was to identify biomechanical factors that differentiate successful versus unsuccessful balance recovery in the event of a fall. Methods: The literature was searched through Google Scholar and PubMed. The following keywords were used: 'falls,' 'protective response,' 'protective strategy,' 'automated postural response,' 'slips,' 'trips,' 'stepping strategy,' 'muscle activity,' 'balance recovery,' 'successful balance recovery,' and 'failed balance recovery.' Results: A total of 64 articles were found and reviewed. Most of studies included in this review suggested that kinematics during a fall was important to recover balance successfully. To be successful, appropriate movements were required, which governed by several things depending on the direction and characteristics of the fall. Studies also suggested that lower limb muscle activity and joint moments were important for successful balance recovery. Other factors associated with successful balance recovery included fall direction, age, appropriate protective strategy, overall health, comorbidity, gait speed, sex and anticipation of the fall. Conclusion: This review discusses biomechanical factors related to successful versus unsuccessful balance recovery to help understand falls. Our review should help guide future research, or improve prevention strategies in the area of fall and injuries in older adults.

열린 사슬 운동과 닫힌 사슬 운동이 정적균형 능력에 미치는 영향 (The Effect of Static Balance Recovery by Open Kinetic Chain and Closed Kinetic Chain Exercises)

  • Purpose : This study was investigated the effect of static balance recovery during open kinetic chain exercise (OKC) and closed kinetic chain exercise(CKC). Methods : The paticipants were consisted of forty-one, was perform 3 sets, 3 times per week for 6 weeks, balance was measured by GOOD BALANCE. Statistical analysis was used repeated measure two-way ANOVA and independent t-test. Results : In CKC group, Center of presure(COP) medial-lateral(ML) velocity was significantly increased post 6 week test than pre-test, post 2 week test. COP anterior-posterior(AP) velocity was significantly increased post 6 week test than pre-test (p<.05). Conclusion : It was found that both OKC and CKC was significantly increased balance recovery in normal younng adults. In further study, it was suggested that was regard patient with muscle weakness.

Influence of Interferential Current Therapy and Laser Therapy on Functional Recovery after Total Knee Replacement

  • Purpose: The purpose of this study is to investigate the effects of interference current therapy and laser therapy on functional recovery after total knee arthroplasty by measuring the Berg balance scale and range of motion. Methods: Subjects were 30 patients who were admitted to G Hospital after total knee arthroplasty. They were randomly assigned to experimental group I in which interference current therapy was applied (n=10), experimental group II in which laser therapy was applied (n=10), or the control group (n=10). The Berg balance scale and range of motion of the subjects were measured before, after 2 weeks, and after 4 weeks of therapy. Results: There was a statistically significant change (p<0.05) in the Berg balance scale and range of motion before and after therapy intervention among the laser therapy group and the interference current therapy group. There was also a significant change between the groups in the Berg balance scale and range of motion. Tukey's post hoc comparison showed a statistically significant difference between the control group and experimental group I and between the control group and experimental group II (p<0.05). Conclusion: The application of interference current therapy and laser therapy resulted in a significant change in both the Berg balance scale and range of motion among patients with total knee arthroplasty. The findings of this study can be used as preliminary clinical data in evaluating functional recovery in patients with total knee arthroplasty in a post-clinic setting.

High Efficiency Alternating Current Driver for Capacitive Loads Using a Current-Balance Transformer

  • This paper proposes a new alternating current driving method for highly capacitive loads such as plasma display panels or piezoelectric actuators, etc. In the proposed scheme, a current balance transformer, which has two windings with the same turn-ratio, provides not only a resonance inductance for energy recovery but also a current balance among all of the switching devices of the driver for current stress reduction. The smaller conduction loss than conventional circuits occurs due to the dual conduction paths which are parallel each other in the current balance transformer. Also, the leakage inductances of the transformer are utilized as resonant inductors for energy recovery by the series resonance to the capacitive load. Furthermore, the resonance contributes to the small switching losses of the switching devices by soft-switching operation. To confirm the validity of the proposed circuit, prototype hardware with a 12-inch mercury-free flat fluorescent lamp is implemented. The experimental results are compared with a conventional energy-recovery circuit from the perspective of luminance performances.

  • Appropriate physical position and balance means giving the least stress and the most useful biomechanically to the body. As this fails, one would have functional recovery problem regardless of disability. There reported better effective on Dynamic training rather than Static training for a proper recovery of physical position, and additionally required Sensory Feedback. Those who have disability of balance, especially Central Nervous System lesion should he provided with variety of Sensory Feedback, and also Dynamic Balance training used by is quite effective.

시각적 바이오피드백 균형 훈련이 뇌졸중 환자의 균형 회복에 미치는 영향 (The Effect of Visual Biofeedback Exercise on the Recovery of Balance in Stroke Patients)

  • Purpose: The purpose of this study is to determine the effects of visual biofeedback training on the recovery of balance function in stroke patients. Methods: A total of 30 patients with stroke were chosen as the subjects of this study. The subjects were randomly divided into either the visual biofeedback balance training group (experimental group; n=15) or the general balance training group (control group; n=15). The visual biofeedback balance training and general balance training were implemented for 30 minutes a day, three times a week, for a total of four weeks. The subjects' balance ability was measured before and after the interventions. Results: The shift length and surface area of the center of the body decreased in both the experimental group and the control group, with the difference being statistically significant. The shift length and surface area of the center of the body both decreased more in the experimental group than in the control group, and there was a statistically significant between-group difference. Conclusion: The experimental group showed a greater improvement in terms of the balance ability of patients with stroke than the control group. Therefore, we believe that visual biofeedback balance training can be effectively applied for the improvement of balance ability in patients with stroke.

생체모사를 통한 보행로봇의 균형감에 관한 연구 (Biomimetic Balancing Mechanism for Walking Robot)

  • A cat is able to quickly recover balance from unstable posture. To observe the balance recovery procedure of the cat, an impulse is applied to the cat while walking on a narrow bridge. We find that it rotates its tail toward the falling direction. In our previous research, the balance recovery procedure is analyzed based on the law of the angular momentum conservation and then a key equation is derived to maintain the balance. However, it did not consider the gravity, so the performance is not good. In this paper, a new dynamic model is proposed using the Lagrangian mechanics. In the method, the gravity is included in the potential energy. Through the proposed dynamic model, controlling the balance of a walking robot is possible.

가상현실 프로그램이 감각이상이 있는 뇌졸중 환자의 균형능력에 미치는 영향 (Effects of Virtual Reality Program on Standing-Balance in Stroke with Sensory Deficit)

  • Purpose : The purpose of this study was to investigate the effects of virtual reality(VR) program on standing balance in stroke with sensory deficit. Methods : Subjects were assigned randomly to either VR group (n=28) or the control group (n=31) when study began. Both groups were conducted electricity therapy, exercise therapy 5 times per week during six weeks, and a group who have VR group 3 times per week during six weeks. Both groups assessed for standing balance before and after virtual reality program. Results : In standing balance, the virtual reality program which have sensory deficit in stroke group, open eyes from the center area of the body and center line significantly reduced the training period(p<.05). The virtual reality program to instantly move the weight through maximum voluntary safety limits to sensory deficit of virtual reality program(p<.05). Conclusion : Stroke patients with sensory deficit affects the recovery of the standing balance. In addition, virtual reality program was stroke patients with sensory deficit affects the recovery of standing balance.

뇌졸중 환자의 균형능력 회복을 위한 가정운동프로그램 효과 (The Effects of Home Based Exercise Program on Balance Recovery in a Post-Stroke Population)

  • 본 연구는 가정운동프로그램이 뇌졸중 환자의 균형능력회복에 미치는 효과를 알아보기 위하여 실시하였다. 본 연구는 2013년 9월에서 2013년 12월까지 실시하였으며, 대상자는 총 20명으로 일반적인 통원 물리치료를 실시한 조군 10명, 일반적인 통원 물리치료를 실시하고 추가적으로 가정운동프로그램을 실시한 운동군 10명으로 나누어 배정하였다. 가정운동프로그램은 신경계 물리치료사가 대상자 집을 방문하여 8주동안 주 2회씩 매회 30분간 시행하였으며 체중이동훈련, 지구력훈련, 운동재훈련, 감각재훈련, 하지운동으로 구성되었다. 균형능력을 평가하기 위하여 Biorescu를 이용하여 족저압, 안정성한계, 동요속도를 측정하였고 임상적인 측정방법 중 하나인 기능적 팔뻗기 검사를 실시하였다. 연구결과 대조군과 운동군은 안정성한계, 동요속도, 기능적 팔뻗기 검사에서 유의한 차이가 나타났다. 본 연구는 가정운동프로그램이 뇌졸중 환자의 균형회복을 회복하는데 효과적인 치료전략 중 하나라는 것을 나타낸다.

Isokinetic Exercise에 의한 중풍 재활교육이 인체기능회복에 미치는 영향 (Effect of Rehabilitation Education Using Isokinetic Exercise on Physical Function Recovery)

  • This study was performed to figure out effects of stroke rehabilitation on education using isokinetic exercise on physical function recovery. It is considered isokinetic exercise will playa primary role in muscle strength, ROM of joint, and body balance recovery for stroke rehabilitation and so far can be used as a basic references to increase the health of all people. The study consisted of 42 stroke patient(21 training group, 21 control group) diagnosed as cerebral hemorrhage from Oriental Rehabilitation Department of Kyung Hee University. Upper extremity and lower extremity exercise was performed in the training group using isokinetic ergometer. The recovery of physical function(muscle strength, ROM of joint, body balance) data between the two groups were compared and ana lysed by paired t-test are as followed. 1. Muscle testing record showed increased in the strength of elbow flexion, knee flexion, knee extension, ankle extension of the training group com paired to control group(p < .05). In the measurement of ROM, however other parts of the body motion showed no significant changes, only shoulder extension of the training group was increased(p < .05). 2. Body balance increase was highly significant in all training group compaired to control group(p<001). Based on these findings, stroke rehabilitation education with isokinetic ergometer showed available effects on recovery of physical function rehabilitation program with isokinetic exercise will play a primary role in the recovery of physical function of stroke or brain injury patients as well as to promote the health of all people.
