The Analysis of Research Trend about Management of Low Back Pain (요통관리에 관한 연구동향 분석)

  • The result of studying papers about management of lower back pain published in domestic and foreign nursing and medical magazines in these 10 years is as follows ; 1. General characteristic of lower back pain ; 1) In sex distribution, there were more men than women in 5 papers and more women than men in 4 papers among 9 papers surveryed. 2) In age distribution, thirties to forties of patients had more lower back pain as surveyed by general hospitals and fifties to sixties of patients had more lower back pain as surveyed by oriental medical hospitals. 3) In cause factor, there were 50 to 65% of sprain, 32 to 44% of herniated intervertebral disc and 13 to 29% of degenerative changes. 4) In symtom distribution, there were 26 to 57% of lower back pain, 42 to 65% of lower back pain with radiating pain and 34 to 99% of paravertevral muscle spasm. 5) In period of pain management distribution, 18 to 40% of patients experienced pain for less than 6 months and 59 to 82% of them experienced pain for more than 6 months in 3 papers among 4 papers. 6) In surveying the treatment, 66 to 88% of patients had conservative treatment and there were treatments of general hospital, oriental medicine, self remedy and traditional practice in conservative treatment. 7) In job distribution, 12 to 50% of them were housekeepers, 23 to 31% office workers, 4.6 to 36% blue color workers and 11 to 15% students. 2. As psychological character lower back pain paients had anxiety, depression, anger-hostility, phobic anxiety, neurasthenia, hypochondriasis, and interpersonal sensitivity. 3. To distinguish the cause of lower back pain, plain lumbar roentgenogram, straight leg rasing test, eletromyelogic findings, somatosensory evoked potentials CT and MRI were performed. 4. To relieve lower back pain. epidural adhesiolysis, epidulal injection of local anesthetic in mixture with steroid, lumbar spinal root block, low level laser therapy, acupuncture like transcutaneous nerve stimulation(AL TENS), topical capsaicin and lumbar orthotics were used in medical field, and relaxation technique was used in nursing field. 5. Mckenzie's extension exercise and William's flexsion exercise for lower back pain were used in medical field and Yoga exercise was applied in nursing field. 6. The more school education and self efficacy were high, the better they had active coping lower back pain positively and the less self efficacy was the more they had serious pain. As a result of studying the paper there have been very little research for lower back pain in nursing fields of Korea and foreign countries. Because 60 to 80% of population expeience lower back pain at least more than once, it is necessary to develop the study and clinical practice for management of lower back pain.

Comparative Analysis of Surface-mounted and Interior Permanent Magnet Synchronous Motor (표면부착형 / 매입형 영구자석 동기 전동기의 비교 분석)

  • In this paper, we present a comparative analysis of surface-mounted permanent magnet synchronous motors (SPMSM) and interior permanent magnet synchronous motors (IPMSM). First, we use 2D finite element analysis (FEA) to analyze models satisfying the same rated conditions according to the torque-speed curve characteristics, which are determined from the operating conditions. Next, we manufacture an SPMSM and IPMSM having good performances from an electromagnetic perspective based on analysis results, namely the cogging torque, torque ripple, and efficiency. We analyze both of the manufactured machines when they are connected back-to-back and when they are used as a motor and a generator, respectively. The motor is driven by a commercial inverter and the generator is connected to a three-phase resistance load bank. Finally, based on experimental results, which include the total harmonic distortion (THD) of the back electro-motive force (EMF), cogging torque, efficiency, and mass, we determine the motor that is most suitable under requirements.

Investigation on the Korean Cyclists' Body Type Through Anthropometric Measurements (사이클 선수들의 체형 특성에 관한 연구)

  • The purpose of this study was to compare the body measurements of cyclists and non-cyclists and to classify cyclists' body types to offer basic information for the bicycle apparel manufacturer in Korea. The anthropometric data was collected including both direct and indirect measurements of 81 cyclists (40 female, 41 male) aged from 19 to 24. Anthropometric measurements were analyzed using percentiles, T-test, factor and cluster analysis. The results were as follows; Comparison of anthropomeoic data between cyclist and non-cyclist was to clarify that cyclists have bigger size than non-cyclists; especially the thigh circumference shows big differences. As the result of factor analysis, 5 factors, which explain 74% of variance, were extracted from all items for male and female cyclists. The results of cluster analysis classified body types into 3 groups. Cluster 1 among three female cyclist groups has biggest torso and had an erect back. Cluster 2 has small size among three female group and drooping shoulders. Cluster 3 has the bended forward shoulders and shows the protrusion back. In case of male cyclists, cluster 1 has thin body type owing to big height measurements and small girth measurements. Cluster 2 among three male groups has the biggest torso and thigh circumference. Cluster 3 has big forward angle of shoulders and shows the protrusion of the back as female cyclist.

  • A novel method to measure the interaction movement, focusing direction and tracking direction in an optical pick-up, is proposed and the decoupling control for it is discussed. First, the basic principle of the coupline analysis method using back electromotive farce is introduced. Second, the interaction analysis between focusing direction and tracking direction fur commercial slim type actuator is performed using the proposed method. Finally, the coupling analysis and decoupling control for the slim type optical pick-up are discussed. From the brief simulation, we shows that the effectiveness and validity of the proposed method.

Parameter Optimization and Automation of the FLEXPART Lagrangian Particle Dispersion Model for Atmospheric Back-trajectory Analysis (공기괴 역궤적 분석을 위한 FLEXPART Lagrangian Particle Dispersion 모델의 최적화 및 자동화)

  • Atmospheric transport pathway of an air mass is an important constraint controlling the chemical properties of the air mass observed at a designated location. Such information could be utilized for understanding observed temporal variabilities in atmospheric concentrations of long-lived chemical compounds, of which sinks and/or sources are related particularly with natural and/or anthropogenic processes in the surface, and as well as for performing inversions to constrain the fluxes of such compounds. The Lagrangian particle dispersion model FLEXPART provides a useful tool for estimating detailed particle dispersion during atmospheric transport, a significant improvement over traditional "single-line" trajectory models that have been widely used. However, those without a modeling background seeking to create simple back-trajectory maps may find it challenging to optimize FLEXPART for their needs. In this study, we explain how to set up, operate, and optimize FLEXPART for back-trajectory analysis, and also provide automatization programs based on the open-source R language. Discussions include setting up an "AVAILABLE" file (directory of input meteorological fields stored on the computer), creating C-shell scripts for initiating FLEXPART runs and storing the output in directories designated by date, as wells as processing the FLEXPART output to create figures for a back-trajectory "footprint" (potential emission sensitivity within the boundary layer). Step by step instructions are explained for an example case of calculating back trajectories derived for Anmyeon-do, Korea for January 2011. One application is also demonstrated in interpreting observed variabilities in atmospheric $CO_2$ concentration at Anmyeon-do during this period. Back-trajectory modeling information introduced in this study should facilitate the creation and automation of most common back-trajectory calculation needs in atmospheric research.

Risk Factors Associated with Clinical Insomnia in Chronic Low Back Pain: A Retrospective Analysis in a University Hospital in Korea

  • Background: Insomnia is becoming increasingly recognized as a clinically important symptom in patients with chronic low back pain (CLBP). In this retrospective study, we have determined risk factors associated with clinical insomnia in CLBP patients in a university hospital in Korea. Methods: Data from four-hundred and eighty one CLBP patients was analyzed in this study. The Insomnia Severity Index (ISI) was used to determine the presence of clinical insomnia (ISI score ${\geq}15$). Patients' demographics and pain-related factors were evaluated by logistic regression analysis to identify risk factors of clinical insomnia in CLBP. Results: It was found that 43% of patients reported mild to severe insomnia after the development of back pain. In addition, 20% of patients met the criteria for clinically significant insomnia (ISI score ${\geq}15$). In a stepwise multivariate analysis, high pain intensity, the presence of comorbid musculoskeletal pain and neuropathic pain components, and high level of depression were strongly associated with clinical insomnia in CLBP. Among these factors, the presence of comorbid musculoskeletal pain other than back pain was the strongest determinant, with the highest odds ratio of 8.074 (95% CI 4.250 to 15.339) for predicting clinical insomnia. Conclusions: Insomnia should be addressed as an integral part of pain management in CLBP patients with these risk factors, especially in patients suffering from CLBP with comorbid musculoskeletal pain.

Dynamic Characteristics Analysis Considering the Effect of the Vortexes of Flux in a LIM for Railway Propulsion System (맴돌이 자속의 영향을 고려한 철도추진용 선형유도전동기의 동특성 연구)

  • In the case of a Linear Induction Motor (LIM), numerical analysis method like Finite Element Method (FEM) has been mainly used to analyze the travelling magnetic field problem which includes the velocity-induced electromotive force. If the problem including the velocity-induced electromotive force is analyzed by FEM using the Galerkin method, the solution can be oscillated according to the Peclet Number, which is determined by conductivity, permeability, moving velocity and size of mesh. Consequently, the accuracy of the solution can be low and the vortexes of flux can be occurred at the secondary back-iron. These vortexes of the flux occurred at the secondary back-iron does not exist physically, but it can be occurred in the analysis. In this case, the vortexes of the flux can be generally removed by using Up-Wind method which is impossible to apply a conventional S/W tool (Maxwell 2D). Therefore, in this paper, authors examined the vortexes of the flux occurred at the secondary back-iron of the LIM according to variations of the Peclet Number, and analyzed whether these vortexes of the flux affect on the dynamic force characteristics of the LIM or not.

Effects of Taping the Lower Back on the Lumbopelvic Region and Hip Joint Kinematics During Sit-to-Stand

  • Excessive lumbar flexion during sit-to-stand (STS) is a risk factor for lower back pain. Postural taping can prevent unwanted flexion of the lumbar spine. This study aimed to demonstrate the effect of taping the lower back on the lumbopelvic region and hip joint kinematics during STS. Sixteen healthy subjects participated. All subjects performed the STS with and without taping of the lower back. A three-dimensional motion analysis system was used to measure the kinematics of the lumbar spine, pelvis, and hip joint during STS. The angle of the peak lumbar flexion, pelvic anterior tilting, and hip flexion and angular displacement of the lumbar spine between starting position and maximal lumbar flexion were collected. Paired t-tests, or Wilcoxon's rank-sum test for non-parametric distribution, were used to assess differences in the measurements with and without taping. A p-value <.05 was taken to indicate a significant difference. Significant differences were observed in the angle of the peak lumbar flexion, pelvic anterior tilting, hip flexion and angular displacement of the lumbar spine (p<.05). Taping was associated with a significant decrease in the angle of peak lumbar flexion and angular displacement of the lumbar spine between the starting position and maximal lumbar spine flexion. In addition, the peak angle of pelvic anterior tilting and hip flexion were significantly increased with taping. The findings of this study suggest that taping the lower back can decrease excessive lumbar flexion, and increase the pelvic anterior tilting and hip flexion motion during STS.

Effects of Acupotomy on Pain and Functional Improvement in Acute Low Back Pain Patients: A Retrospective Study (도침치료가 급성 요추 염좌 환자의 통증 및 기능개선에 미치는 영향: 후향적 연구)

  • Objectives The aim of this study is to investigate the effect of Acupotomy on the pain reduction and functional improvement of patients with Acute low back pain. Methods We studied 60 patients with Acute low back pain with Numeric Rating Scale (NRS) 5 or higher in admission and had admitted to Department of Korean Medicine Rehabilitation of Daejeon University from March 1, 2017 to October 31, 2017. 30 patients had received Acupotomy combined Korean Medicine treatment(such as acupuncture, moxabustion, herbal medicine, etc) and the other 30 had received Korean Medicine treatment only. The analysis was conducted as a retrospective study which analyzes the patient's medical records. Statistical analysis was performed using the IBM SPSS Statistics 24 program. We used NRS to evaluate pain reduction, used Range of Motion (ROM) and Roland Morris Disability Questionnaire (RMDQ) to evaluate function improvement, and used EuroQol-5 Dimension (EQ-5D) and EuroQoL-Visual Analogue Scale (EQ-VAS) to assess quality of life. Finally, a five-point Likert scale was used to assess treatment satisfaction. Results The analysis revealed that patients who were treated with Acupotomy showed statistically significant NRS reduction, improvement of ROM and RMDQ, improvement of EQ-5D and EQ-VAS and satisfaction compared to those who just recevied Korean Medicine treatment. Conclusions In conclusion, we found that the Acupotomy showed a positive effect on pain resolving, functional rehabilitation and quality of life in patients with Acute low back pain.

A Basic Study on Plastic Suspension System for Automotive Seat under Consideration of Body Pressure Distribution (체압 분포를 고려한 자동차 시트용 플라스틱 서스펜션에 대한 기초적 연구)

  • This study investigates the plastic suspension assembly which is installed on inside of vehicle seat and support passenger's back to supply the comfortable ride performance. It aims to develop the structural design in order to support driver's back uniformly and assemble seat back frame with plastic suspension effectively. The part of suspension is designed by considering the body pressure distribution of driver and it has the same size as the practical model on simulation analysis. It is confirmed that the analysis result of plastic suspension approaches the practical measured values and the better body pressure distribution can be obtained as compared with the existing wire type.
