The Study of Core Muscle as Meridian Tendino-usculature ('코어(core)'근육에 대한 한의학적 소고(小考))

  • Obejectives : Slow-motion strengthening trainings build up muscles better than any other type of training, these exercises focus on balancing and strengthening the muscles in the anatomical center of the body - the core - which includes the back, hip and abdominal muscles. Methods : We had a concept of 'core', and compared core muscles with 12 meridian tendino-musculature based on Thomas Myers' myofascial meridian. Results : It has been suggested that the core muscles correspond to meridians of foot- three Yin and foot- three Yang. The study of core muscles strengthening exercise and movement therapy is requred.

Evaluation of Ultrasound for Prediction of Carcass Meat Yield and Meat Quality in Korean Native Cattle (Hanwoo)

  • Three hundred thirty five progeny testing steers of Korean beef cattle were evaluated ultrasonically for back fat thickness (BFT), longissimus muscle area (LMA) and intramuscular fat (IF) before slaughter. Class measurements associated with the Korean yield grade and quality grade were also obtained. Residual standard deviation between ultrasonic estimates and carcass measurements of BFT, LMA were 1.49 mm and $0.96cm^2$. The linear correlation coefficients (p<0.01) between ultrasonic estimates and carcass measurements of BFT, LMA and IF were 0.75, 0.57 and 0.67, respectively. Results for improving predictions of yield grade by four methods-the Korean yield grade index equation, fat depth alone, regression and decision tree methods were 75.4%, 79.6%, 64.3% and 81.4%, respectively. We conclude that the decision tree method can easily predict yield grade and is also useful for increasing prediction accuracy rate.

Suddenly fixed upward ocular deviation under general anesthesia

  • Various changes in ocular position are possible during general anesthesia as opposed to the awakening state. However, unexpected ocular deviation under general anesthesia is a disconcerting event as it can lead to difficult complications intraoperatively. To date, sudden fixed upward ocular deviation has been rarely reported previously. This phenomenon was observed in an 8-year-old boy during strabismus surgery. Suddenly fixed upward ocular deviation occurred when the speculum was inserted into the right eye. When the eyeball was pulled down, using forceps, there was some resistance, such as contracture of superior rectus. The eyeball sprang back into the upward position when the forceps was released. These changes could hamper the good exposition of the surgical field, leading to significant intraoperative difficulties and complications. Surgeons should be aware of this possibility, despite general anesthesia; if it occurs, proceed with the surgery as planned preoperatively, and both ophthalmic and anesthetic interventions should be used to solve this problem.

Delayed Retroperitoneal Hemorrhage due to Lumbar Artery Pseudoaneurysm after Lumbar Posterolateral Fusion

  • A 55-year-old female patient presented with lower back pain and neurogenic intermittent claudication and underwent L3-L4 posterolateral fusion. To prepare the bone fusion bed, the transverse process of L3 and L4 was decorticated with a drill. On the 9th post-operative day, the patient complained of a sudden onset of severe abdominal pain and distension. Abdominal computed tomography revealed retroperitoneal hematoma in the right psoas muscle and iatrogenic right L3 transverse process fracture. Lumbar spinal angiography showed the delayed hematoma due to rupture of the 2nd lumbar artery pseudoaneurysm and coil embolization was done at the ruptured lumbar artery pseudoaneusyrm. Since then, the patient's postoperative progress proceeded normally with recovery of the hemodynamic parameters.

  • It has been reported by some authors that caudal block with local anesthetic and steroid is the most effective therapeutic modality for piriformis syndrome; and the incidence ratio of female to male being 6 to 1. We treated 89 patients with piriformis syndrome in 1995. From those clinical experiences and anatomical studies we heave arrived at several conclusions different from other authors. Our results indicate the following: 1) Piriformis syndrome does not provoke lower back pain. 2) Our rate of incidence showed a very different profile as results showed a female to male ratio of 33:56. 3) Releasing the compressed nerves(gluteal, sciatic) with spasmolytic treatment on the piriformis muscle itself is thought to be the only therapeutic modality for piriformis syndrome.

  • Lumbar disc herniation is extremely uncommon in children below 10 years of age. A 7-year-old boy is reported who presented with low back pain and left leg radiating pain. The pain started seven days prior to presentation and was attributed to performing the jumping kick without any previous warm-up. Magnetic resonance imaging revealed a posterolateral disc herniation at the L3-4 level and multiple degenerative changes. The patient received conservative treatment including limitation of sports activities, anti-inflammatory and muscle relaxant medications as well as physical therapy. After three months of the aggressive treatment the child was symptom free. We present here a lumbar disc herniation in one of the youngest patient.

  • Piriformis syndrome is a syndrome of low back and leg pain thought to be due to chronic contracture of the piriformis muscle that causes irritation of the sciatic nerve. We have utilized an alternative approach using cauaal steroid in local anesthetics injection in the treatment of piriformis syndrome. I it was a very effective method without any complications.

  • The classic triad of diagnostic signs of submucosal cleft palate which may be present are: 1) bifid uvula 2) short palate with no muscle in the midline and 3) hard palate with a submucous notching defect in the posterior midline. The treatment of submucous cleft palate are V-Y push back palatorrhaphy, and superior based pharyngoplasty implant in the posterior pharynx. The best speech results were in those children operated upon in the younger age group (especially at or before 2 years of age), thus pointing up the importance of early diagnosis.

  • Today, many people are sitting on chair in their life. If a sitting posture is not correct, there is some trouble with the waist. And if it goes on long time, sometimes it cause the hurts of waist or the deformded spinal column. A crouched posture is an obstacle to breath and it give rise to drowsiness because the lack of oxygen. Sitting posture is a habit so people can't feel it oneself and look over some kind of risks. It analyzes a condition of muscles by measuring EMG of spinal both side of spin al-bones. It can have a right sitting posture by the analyzing that increasing of muscles tention in one or the other side of muscles when the posture inclines on one side or ahead.

An Ergonomics Evaluation on the Shovels of Korean Standard (삽에 대한 인간공학적 평가)

  • We Propose an improved shovel design to minimize physical discomfort and the risk of cumulative trauma disorder at shoveling tasks. The specifications of shovels in Korean standard data were scrutinized to check if they met the anthropometric specification of Korean population in 1997. We also applied "Body Map" pictograph and self-conscious checklist to field survey to analyze the discomfort of shoveling task. Thirty male subjects whose age were ranging from 18 to 65 voluntarily participated in the experiment. The measurement results show that shoveling workers were exposed to hazards of CTD's. Especially, when workers were involved in ditch digging and construction their trunk vend more than 120 degree. In order to compare the new designed shovel and the traditional K.S shovel, an analysis of variance was performed and we obtained very strong evidence that the new designed shovel was better in reducing physical discomfort. According to the results from electromyograrn experiment the new shovel contributed to improving subjective comfort level and reducing low back muscle fatigue. For futher study, the research may be extended to determine the relationship among shovel sharpness, type, digging motion and digging force.ing force.
