Dependences of Ultrasonic Parameters for Osteoporosis Diagnosis on Bone Mineral Density (골다공증 진단을 위한 초음파 변수의 골밀도에 대한 의존성)

  • Quantitative ultrasound technologies for osteoporosis diagnosis measure ultrasonic parameters such as speed of sound(SOS) and normalized broadband ultrasound attenuation(nBUA) in the calcaneus (heel bone). In the present study, the dependences of SOS and nBUA on bone mineral density in the proximal femur with high risk of fracture were investigated by using 20 trabecular bone samples extracted from bovine femurs. SOS and nBUA in the femoral trabecular bone samples were measured by using a transverse transmission method with one matched pair of ultrasonic transducers with a center frequency of 1.0 MHz. SOS and nBUA measured in the 20 trabecular bone samples exhibited high Pearson's correlation coefficients (r) of r = 0.83 and 0.72 with apparent bone density, respectively. The multiple regression analysis with SOS and nBUA as independent variables and apparent bone density as a dependent variable showed that the correlation coefficient r = 0.85 of the multiple linear regression model was higher than those of the simple linear regression model with either parameter SOS or nBUA as an independent variable. These high linear correlations between the ultrasonic parameters and the bone density suggest that the ultrasonic parameters measured in the femur can be useful for predicting the femoral bone mineral density.

Effects of Bovis Calculus, Ursi Fel Aqua-acupuncture on Adjuvant Arthritis in rats (우황(牛黃).웅담(熊膽) 약침(藥鍼)이 Adjuvant 관절염(關節炎)에 미치는 영향(影響))

  • In order to investigate experimentally that Bovis Calculus, Ursi Feli Aqua-acupuncture(BUA) have an effect on Adjuvant Arthritis in rats, the author inserted BUA at points corresponding with Chok-samni(ST36) and T'aegye(KI3), and observed an inhibitory rate of edema and pain, variations of White blood cell(WBC), Red blood cell(RBC), Hemoglobin(Hb) in blood. The author also observed the histological changes of joint tissue. The results were as follows : 1. The BUA group during the 6th and 9th were decreased with statistical significance in inhibitory rate of paw edema as compared with the control group. 2. The BUA group during the 3rd and 9th day were decreased with statistical significance in inhibitory rate of pain as compared with the control group. 3. The BUA group during the 3rd, 6th and 9th day were decreased with statistical significance in blood WBC as compared with the control group. The blood RBC and Hb didn't have statistical significance. 4. According to the histological studies, the synovial cells were necrotized at the 3rd, 6th and 9th day control group, but some synovial cells were necrotized at the 3rd day BUA group. The synovial cells of the the 6th and 9th day BUA group were recovered more than that of the 3th day group.

Correlations Among Speed of Sound, Broadband Ultrasonic Attenuation, Broadband Ultrasonic Reflection, and Bone Density in Bovine Cancellous Bone

  • Correlations between acoustic properties and bone density have been investigated in bovine cancellous bone. Speed of sound (SOS), broadband ultrasonic attenuation (BUA), and broadband ultrasonic reflection (BUR) were measured in 10 defatted bovine cancellous bone specimens in vitro. SOS showed a significant correlation with the apparent density of the bone. A comparable correlation was observed between BUA and the apparent density. BUR was rather highly correlated with the apparent density. It was shown that BUR had a weak correlation with BUA and a significant correlation with SOS. This indicates that the parameter BUR can provide important information that may not be contained in BUA and SOS and, therefore, can be useful as an alternative diagnostic parameter of osteoporosis. As expected, a linear combination of all three ultrasonic parameters in a multiple regression model resulted in a significant improvement in predicting the apparent bone density.

Acoustic Properties of Bovine Cancellous Bone in the Frequency Range of 0.5-2 MHz

  • Most previous studies using ultrasound for osteoporosis diagnosis have employed ultrasound in a frequency range of 0.2-1 MHz. In this study, acoustic properties of the 12 defatted bovine cancellous bone specimens were investigated in vitro. Speed of sound (SOS) and broadband ultrasonic attenuation (BUA) were measured using three matched pairs of transducers with the center frequencies of 1, 2.25, and 3.5 MHz, respectively, in order to cover a broad frequency range of 0.5-2 MHz. The relative orientation between ultrasonic beam and bone specimens was the mediolateral (ML) direction of the bovine tibia. SOS showed significant linear positive correlations with apparent density for all three pairs of transducers of 1 MHz, 2.25 MHz, and 3.5 MHz, respectively. BUA showed relatively weak correlations with apparent density for the pairs of transducers of 1 MHz and 2.25 MHz. Furthermore, in the measurement with the pair of 3.5 MHz transducers, BUA was independent of apparent density. SOS and BUA were only weakly correlated with each other. The linear combination of SOS and BUA showed significant correlations with apparent density. These results suggest that the frequency range up to 1.5 MHz may be also useful in the osteoporosis diagnosis.

  • The distilled water is used for the ultrasonic wave propagating material in the measurements of broadband ultrasonic attenuation (BUA) that is applied in industrial and medical applications, The acoustic impedance of water is significantly changed with its temperature. Therefore, the quantitative evaluation of BUA with temperature and the ultrasonic wave propagating distance is highly needed. In this study, we evaluated the variation of attenuation with change in temperature. To measure the variation of BUA in the low frequency region at the temperatures, 27$^{\circ}C$, 29$^{\circ}C$, and 31$^{\circ}C$, we tested the Plyethylene, Teflon, MC-Nylon, Urethane specimens and analyzed the center frequency, frequency bandwidth, spectral peak amplitude. The results showed that BUA value appeared to be lower with increasing temperature. This may be due to the fact that the frequency feature of ultrasonic wave is affected by not only the specific gravity, acoustic impedence, but material crystalline, porosity, the distance of ultrasonic wave propagation in water.

Effect of increased blood and tibia lead on the change of bone mineral density in retired male lead workers (과거의 직업적 납 노출에 의한 혈중납 및 골중납의 증가가 남성 골밀도 변화에 미치는 영향)

  • This study was designed to investigate the effect of increased blood and tibia lead on the change of bone mineral density in retired male lead workers. One hundred nine retired male lead workers who worked in 4 different lead industries and 51 nonoccupationally lead exposed male subjects were recruited from March 2004 to October 2004. Bone mineral density(BMD) was measured by broadband ultrasound attenuation(BUA) at left calcaneous bone area with broadband ultrasound attenuation method of QUS-2(Metra Biosystems Inc, USA). Tibia bone lead was measured for skeletal bone lead with K-xray fluorescence(K-XRF) and blood lead was analyzed with flameless atomic spectrophotometer. Hemoglobin, hematocrit, serum calcium and iron were also analyzed. In addition, information for smoking and drinking status and basic personal data such as age and lead exposure were also collected using questionnaire inquiry. Blood lead was correlated with tibia lead (r=0.711) and these two variables were negatively correlated with BUA in bivariate analysis. BUA and tibia lead showed significant main effects on the change of blood lead after adjusting covariates. The effect modification by the level of BMD (low: lower than the median of BUA and high: higher than the median of BUA) was observed between the association of tibia lead and blood lead after adjustment of covariates. The subjects who had higher BMD seemed to have lower blood lead by the increase of tibia lead than those of lower BMD. In the multiple regression analysis of blood lead and tibia lead on BUA after adjustment of covariates, only blood lead showed statistically significant effect on BUA. This study confirmed that BMD and blood lead were significantly associated. To verify the causal association of BMD on blood lead and vice versa, further longitudinal studies are needed.

Effects of Milk Consumption on Calcaneal Quantitative Ultrasound and Bone Turnover Markers of Women Living in Asan (아산시 거주 여성에서 우유섭취가 골초음파 상태와 골대사지표에 미치는 영향)

  • The objective of this study is to determine the effectiveness of 16-month milk consumption as a part of the health promotion community program for women in Asan. Subjects included 313 women belonging to the milk group (mean age = 69.1, range $47{\sim}89 y$) and 66 women to the control (mean age=43.6, range $20{\sim}69 y$) group. For those in the milk group, one cup (200 ml) of partially lactose-digested low-fat milk was provided everyday for 16 months. Each subject was interviewed to assess calcium intake by a 24-h recall after fasting blood was obtained for analyzing bone turnover markers, and calcaneus broadband ultrasound attenuation (BUA) was measured by quantitative ultrasound (QUS) on the left heel before and after the milk supplementation. After 16 months, the calcium intake levels changed from 55% of recommended dietary allowance (RDA) to 85% RDA in the milk group and from 73% RDA to 84% RDA in the control group. BUA were reduced from $67.9{\pm}8.1$ to $64.7{\pm}17.5$ dB/MHz for milk and from $90.4{\pm}13.0$ to $87.2{\pm}15.2$ dB/MHz for control groups. Paired t-test showed the changes of BUA for both groups (-3.24 and -3.15 dB/MHz for milk and control groups, respectively) were significant, but the two groups did not show any differences in absolute changes. When post-BUA was analyzed after age, initial BUA and menopausal status were controlled as covariates in ANCOVA model, the milk group showed significantly (p < 0.05) smaller changes than the control group (-3.50 vs -6.71 dB/MHz, respectively). According to a multiple regression analysis, milk consumption and initial BUA showed significant interaction meaning that those with lower initial BUA showed higher milk effects. We conclude that one-cup a day milk consumption for 16 month can prevent further bone loss and significantly improve calcium intake.

  • The speed of sound, transmission coefficient, and attenuation are measured around the center frequency 1 and 2 MHz in solid materials such as bone, sediment, rubber, and Lucite materials. Common and different characteristics of such materials in the sound speed, transmission coefficient, and attenuation are discussed. Ambiguities in estimating such acoustic characteristics we also addressed. Ultrasonic properties of the first and second kind waves are clarified for different materials. Discussions are concentrated on classes of sound speed, broadband ultrasonic attenuation (BUA), and correlations of sound speed and BUA with apparent density. New correlations of inverse sound speed square and BUA with apparent density are suggested.

A Study on the Habitat of Gidra Community in Papua New Guinea with Satellite Remote Sensing

  • Gidra speaking people living in Wonnie area of western province in Papua New Guinea are traditionally dependent on hunting and gathering. Sago is the main staple food and the vegetation consists of rain forest (Bua) and trees with savannah grass (Yap). The population density is very low and the people are very little affected by modernization. This study is an attempt to look at the people-resource interaction in the area for the past thirty years based on aerial photographs and various satellite data. The study showed the trends of yap versus bua distribution as well as the location of garden and Sago grove areas, which is important for studying the livelihood of the people.
