인지기능에 비만 역설은 존재하는가?: 고령화연구패널자료(2006-2016)를 이용하여 (Does the Obesity Paradox Exist in Cognitive Function?: Evidence from the Korean Longitudinal Study of Ageing, 2006-2016)

  • Background: There have been many studies on the associations between body mass index (BMI) and cognitive function. However, no study has ever compared the associations across the methods of categorizing BMI. In this study, we aimed to fill the gap in the previous studies and examine whether the obesity paradox is valid in the risk of cognitive function. Methods: Of the 10,254 people aged 45 and older from the Korean Longitudinal Study of Ageing from 2006 to 2016, 8,970 people were finalized as the study population. The dependent variable was whether a person has a normal cognitive function or not, and the independent variables of interest were BMI categorized by the World Health Organization Western Pacific Regional Office (WHO-WPRO) method, the WHO method, and a 10-group method. Covariates included sociodemographic factors, health behavior factors, and health status factors. A generalized linear mixed model analysis with a logit link was used. Results: In the adjusted model with all covariates, first, in the case of BMI categories of the WHO-WPRO method, underweight (odds ratio [OR], 1.16; 95% confidence interval [CI], 1.15-1.17), overweight (OR, 1.36; 95% CI, 1.35-1.36), and obese (OR, 1.34; 95% CI, 1.33-1.34) groups were more likely to have a normal cognitive function than a normal-weight group. Next, in the case of BMI categories of the WHO method, compared to a normal-weight group, underweight (OR, 1.15; 95% CI, 1.14-1.16) and overweight (OR, 1.06; 95% CI, 1.06-1.07) groups were more likely to have a normal cognitive function; however, obese (OR, 0.62; 95% CI, 0.61-0.63) group was less likely to have it. Lastly, in the case of the 10-group method, as BMI increased, the likelihood to have a normal cognitive function changed like a wave, reaching a global top at group-7 (26.5 kg/㎡ ≤ BMI <28.0 kg/㎡). Conclusion: The associations between BMI and cognitive function differed according to how BMI was categorized among people aged 45 and older in Korea, which suggests that cognitive function may be positively associated with BMI in some categories of BMI but negatively in its other categories. Health policies to reduce cognitive impairment need to consider this association between BMI and cognitive function.

모녀의 골밀도 예측요인분석 (Predictors of Bone Mineral Density in Mothers and Their Daughters)

  • Purpose: To evaluate physical characteristics, lifestyle related to bone-health, and bone mineral density (BMD) in mothers and their daughters and to determine the predictors of BMD. Method: BMDs at the forearm, lumbar spine, and femur were measured in 101 healthy, mother-daughter pairs by dual energy X-ray absorptiometry. Mother-daughter differences between general characteristics, means for BMDs were assessed by ${\chi}^2$-test, t-tests. Multiple regression analyses were used to identify predictors of BMD in each group. Results: Mothers had significantly higher BMD than their daughters at forearm, lumbar spine, and femur. The predictors of mothers' BMDs were body weight, body mass index (BMI) and percentage body fat, explaining 5.1~31.6% of the variation in BMDs. BMI, percentage body fat and their mother's BMD of the corresponding site bone were predictors in daughters, explaining 17.5~31.6% of the variations in BMDs. Conclusion: These results indicate the importance of weight on bone that the BMDs seems to be related to fat free mass both in young-adult daughters and in middle aged mothers. These also suggest the importance ofintervention for the development of BMD in daughter of mother with low BMD.

여대생의 섭식문제 관련요인 (Factors Influencing Eating Problems among Korean University Women)

  • Purpose: The purpose of the study was to investigate factors that influence university women with eating problems. Methods: Study participants were 307 women from two universities. Self-report questionnaires which included items on eating problems, satisfaction with body shape, self-esteem, body mass index, and weight control methods were used. Data were analyzed using independent t-tests, ${\chi}^2$ tests, and a multiple binary logistic regression. Results: About 20% of the undergraduate women were experiencing eating problems. The problematic eating group showed more dissatisfaction with body shape than the normal eating group, and used more diets, fasting, diet-products, and uretics/laxatives to control body weight. Significant predictors for the problematic eating group were diets, diet-products, BMI, self-esteem and dissatisfaction with body shape. The strongest predictors were diets; risk for women university students who had been on a diet was 15 times higher than their counterparts. Conclusion: It is pertinent for health professionals to start intervention programs to educate university women with eating problems. The contents should include information on healthy weight control methods, improving satisfaction with body shape and self esteem, as well as creating social atmosphere about healthy body shape for women.

중지 중절골과 경추를 이용한 골령 평가의 체질량 지수에 따른 일치도 (Measuring Agreement of Modified MP3 and CVMS according to BMI Percentile)

  • 이 연구의 목적은 골격 성숙도를 평가하기 위해 3번째 손가락의 중간 지골을 이용하는 방법과 경추를 이용하는 방법 사이의 체질량 지수 백분위 수에 따른 일치도를 조사하는 것이다. 후향의 차트 분석을 통하여 체질량 지수 자료, 수완부 방사선 사진, 측방 두부 방사선 사진이 같은 날에 채득된 경우를 조사 대상으로 하였다. 조사 대상은 체질량 지수 백분위 수에 따라 4개 군으로 나뉘었다. 2가지 방법 사이의 체질량 지수 백분위 수 군에 따른 일치도는 가중치 카파 통계량을 계산하여 측정되었다. 전체 군에 대해 2가지 방법 간에 좋은 일치도를 보였지만 성별 및 체질량 지수 백분위 수 군에 따라서 일치도가 달랐다. 남자는 과체중 군에서 가장 큰 일치도를 보였으나 여자의 경우 저체중에서 비만으로 감에 따라 일치도가 더 증가하는 경향을 보였다. 어린이와 청소년의 성장을 평가할 때 성별과 체중 상태가 골령을 분석하는 2가지 방법의 일치도에 영향을 줄 수 있기 때문에 소아치과 의사는 이를 고려해야 한다.

대사증후군 유발 인자에 따른 경동맥 내중막 두께의 변화 분석 (Analysis of Changes in Carotid Intima-media Thicknesses according to Metabolic Syndrome-inducing Factors)

  • 본 연구는 2022년 5월부터 2023년 2월까지 부산 지역 일개 의료기관을 내원한 환자들 중 경동맥 초음파 검사와 피검사를 모두 시행한 235명을 대상으로 후향적 분석을 시행하였다. 연구 목적은 경동맥 초음파를 통해 내중막 두께와 혈류속도를 측정하고 대상자들의 BMI 및 대사증후군 영향 인자들을 분석하여 내중막 두께와 혈류속도에 미치는 영향에 대하여 분석하고자 하였다. 경동맥 내중막 두께와 BMI의 상관관계 분석 결과 p=0.000으로 나타나 상관관계가 있는 것으로 확인되었다. 혈류속도와 BMI의 상관관계 분석 결과 p=0.015로 나타나 상관관계가 있는 것으로 확인되었다. 그밖에 연령(p=0.000), 공복혈당(p=0.002), 음주(p=0.006) 등이 경동맥 내중막 두께와 상관관계가 있는 것으로 나타났다. 경동맥 내중막 두께의 증가는 심뇌혈관질환으로 인한 이벤트의 증가를 발생시킨다. 따라서 경동맥 내중막 두께를 증가시키는 요인들을 배제 시키려는 노력이 반드시 필요하며 또한 다양한 예측 방법의 연구와 정기적인 검진을 통한 심혈관질환을 예방하는 것이 무엇보다 중요할 것이다.

Correlation between Obesity and Lumbar Lordosis in Obese Pre-Menopausal Korean Females

  • Objective : Obesity is associated with degenerative arthropathy giving stress on joints. It also amplifies loads of weight bearing joints by changing the gravity line of the body. Our aim is to investigate the correlation between obesity and lumbar lordosis in obese pre-menopausal Korean females. The hypothesis was tested that there is a correlation between obesity and lumbar lordosis. Methods : A cross-sectional evaluation of 44 Females (baseline age 30.77 ± 6.46) with BMI 31.53 ± 3.82 (kg/㎡) was done. Body composition was measured using bio-impedance analysis (BIA), and anthropometry was done by the same observer. A lateral whole spine X-ray was taken in standing position to measure the lumbar lordotic angle (LLA), Ferguson angle (FA) and lumbar gravity line (LGL). A Pearson correlation was used to measure the correlation between obesity and lumbar lordosis (SPSS 10.0 for windows). Results : Body mass index (BMI kg/㎡) had a negative relationship with LLA((equation omitted)=-0.469), FA((equation omitted) =-0.347) and LGL((equation omitted)=-0.389). Body fat rate had a negative relationship with LLA only(γ=-0.385). Waist circumference had a negative relationship with LLA((equation omitted)=-0.345) and LGL((equation omitted)=-0.346). WH ratio had no relationship with lumbar lordosis. Conclusion : These data show that obesity is related to mechanical structures, such as lumbar lordosis. BMI was the most useful index, which reflects a change of mechanical structure of lumbar, more than other variables in this study.

포도 식사가 일부 과체중 여대생들의 체중조절과 혈액성상에 미치는 영향 (The Effect of Grape Diet on Weight Control Serum Components in Korean Overweight Female College Students)

  • This study was to investigate the effect on weight control and changes in serum glucose, total protein , GPT, GOT , cholesterol, triglyceride, calcium, and phosphorus contents of seventeen overweight female college students during experimental periods [before grape diet(0 day-, after grape diet (3 day), after control diet(3 day), after normal diet(30 day)]. The mean weight, height , BMI, and Rohrer index of the subjects at the onset of the study was 58.91$\pm$5.22kg, 159.94$\pm$6.01cm, 22.96$\pm$1.17, and 20.06$\pm$1.48 respectively. During the study subjects, conducted grape diets, control diets, and normal diets to control their weight. They analyzed their changes in anthrophometric variables. The mean weight was significantly decreased from 58.29$\pm$7.85kg to 57.2$\pm$8.23kg after normal diet(p<0.05). Therefore BMI (body mass index) and Rohrer index was also significantly decreased after experimental periods. The analysis of dietary intake showed that the grape diet induced unbalanced low nutrient intake, but the intake of total calories and other nutrients were almost recovered their onset of the study during control and normal diets. The concentration of serum glucose, cholesterol, triglyceride GOT, and GTP were significantly(p<0.05) decreased and significantly(p<0.05) increased again during experimental periods. Calcium concentrations were not affected by grape diet, but phosphorus concentration was significantly reduced after grape and control diet.

제2형 당뇨환자의 개인별 맞춤영양교육의 효과 평가 (Individual Diabetes Nutrition Education Can Help Management for Type II Diabetes)

  • This study was designed to evaluate the effect of the individual nutrition education for type 2 diabetes who participate the diabetes buffet. The subjects were 66 patients and divided into education (n=34) and control groups (n=32). The mean age of education and control groups were 59.8 and 56.6 years old, respectively. There were no differences in age and body mass index (BMI) between two groups. Initial glycosylated hemoglobin (HbA1c), post prandial plasma glucose (PP2), total plasma cholesterol, and blood pressure were not different between two groups. But fasting blood sugar (FBS) was higher in education group than in control group. On completion of the study, the education group showed significant decreases in body weight, BMI, FBS and PP2, however, the control group showed no changes in body weight, BMI and PP2, and showed a significant increase in FBS. Initial calorie and protein intakes of the education group did not meet the prescribed amount, however, mineral and vitamin intakes were higher than estimated average requirement (EAR). By the end of study, calorie and protein intakes were significantly increased to meet the prescription. In order to evaluate the effect of diet education, awareness of calorie requirement was used as an index of understanding diet prescription. The degree of awareness of calorie requirement was dependent on age: younger patients showed higher awareness than older subjects. The subject who showed better understanding of diet prescription showed lower levels of HbA1c, FBS, and PP2 at the end of the experiment period. The results of this study clearly show that individual diabetes diet education is effective to make the patient understand their diet prescription, and is effective to control body weight and blood sugar level. Awareness of calorie requirement could be used as an index of understanding of prescribed diet. Since age is an important variable to determine the awareness of calorie requirement, different strategies of nutrition education should be developed for different age groups, especially patients over 70 years old.

전북지역 위암환자의 위절제술 전.후 영양상태 평가 (A Study on Nutritional Status after Gastrectomy of Gastric Cancer Patients in Jeonbuk Province)

  • The purpose of this study was to investigate the nutritional status before and after gastrectomy of gastric cancer patients in Jeonbuk Province. The subjects were 136 patients with gastrectomy of gastric cancer. We assessed the nutritional status before and after an operation by general characteristics (age, clinicopathological stage, type of operation, method of reconstruction), anthropometric characteristics (height, weight, skeletal muscle mass, body fat mass, body mass index (BMI), percent body fat, arm muscle circumference AMC, arm circumference AC) and biochemical characteristics (hemoglobin Hb, hematocrit Hct, mein corpuscular volume MCV, mean corpuscular hemoglobin MCH, total lymphocyte count TLC, serum albumin, serum transferrin). Mean ages were 60.0 yrs in males and 58.8 yrs in females. Age, clinicopathological stage, types of operation and reconstruction methods were not significantly different between males and females. Weight, skeletal muscle mass, body fat mass, BMI, percent body fat, AMC and AC significantly deteriorated by gastrectomy. There were severe weight losses in males and females after gastrectomy. Hemoglobin, Hct, MCV, MCH, TLC, albumin and transferrin significantly deteriorated by gastrectomy. After gastrectomy, subjects who were assessed as malnounrished in Hb and Hct were increased in male and those who were assessed as malnounrished in Hb were increased in females. These results suggest that a nutrition intervention, specially for energy, protein and iron, is necessary to improve the nutritional status of gastric cancer patients with gastrectomy.

여고생과 여대생의 체중, 자아존중감 및 우울에 대한 비교 연구 (Weight, Self-esteem, and Depression in High School and College Females)

  • The purpose of the study was to investigate Body Mass Index (BMI), the perception of weight problem and the level of self-esteem and depression in high school and college females. The convenience sample consisted of 303 high school females and 481 college females. Data were collected between November 1999 and December 1999. Subjects ranged in age from 15 years and 26 years. BMI was calculated based on the subject's self-reported body weight and height. Self-perception of having a weight problem was evaluated by a single item question. The Rosenberg's Self-Esteem Scale and the Center for Epidemiologic Studies Depression Scale were utilized to measure the level of self-esteem and depression symptoms. BMI mean scores were 19.92 and 19.83 in high school females and college females, respectively. Based on BMI, 79.2 percent of high school females and 85.2 percent of college females were in a normal weight range. Only 2.6 percent of high school females and 1.5 percent college females were obese. Both high school and college females perceived their body weight problem as moderately severe. The mean scores of depression were 20.33 in high school females and 21.69 in college females. Sixty percent of high school females were depressed and 73.4 percent of college students. There was no significant difference between two groups in BMI. However, high school females perceived their weight problem more severe than college females. College females had higher levels of self-esteem and depression than high school females. BMI and perceptions of weight problem revealed a positive relationship in both groups. Results of the study revealed that a perception of weight problem contributed significantly to predict the level of self-esteem and depression in both groups. BMI score had significant effect on only college females' self-esteem. In the study, even though the mean score of BMI was in a normal range, subjects perceived their weight problems as moderatley severe. Since the perception of a weight problem influenced the level of self-esteem and depression, health care providers need to teach them about the normal weight range in BMI criteria.
