재활승마가 노인의 균형과 노인 삶의 질척도에 미치는 영향 (Effects of Rehabilitation Horse Riding on the Balance and Geriatric Quality of Life Scale of the Elderly)

  • Purpose The purpose of this study was to investigate the effect of rehabilitation horse riding exercise on the balance and Geriatric Quality of Life Scale (GQOL) of the elderly. Methods The subjects were 14 elderly males who consisted of 7 in the experimental group and 7 in the control group. The subjected performed rehabilitation horse riding exercise twice a week for 12 weeks. For balance measurements, leftright and anteriorposterior weight bearing, and BBS were tested using GymPlate. In addition, the GQOL was evaluated. Results The results of this study show that in the experimental group, there were significant differences in balance ability and GQOL after the rehabilitation horse riding exercise (P<0.01), but there was no significant difference in the control group (P>0.05). Furthermore, the experimental and control groups showed significant differences in balance ability and GQOL (P<0.01). Conclusion Rehabilitation horse riding exercise is effective in improving the balance ability of the elderly, which also had an effect on the GQOL.

악성코드 동향에 따른 새로운 방어 전략 연구 (A Study of New Prevention Strategy According to the Trend of Malicious Codes)

  • 본 논문에서는 2018년에 성횡한 악성코드에 대한 피해 사례를 살펴본 후 이를 적극적으로 대응하기 위한 방안을 살펴본다. 특히 가상통화 거래소에 대한 해킹 사고 및 가상화폐에 대한 지속적인 해킹 시도가 탐지되면서 관련 소식들이 언론에 지속적으로 보도되었다. 또한 이와 관련하여 PC 및 서버 자원을 몰래 훔쳐 가상통화 채굴에 사용하는 크립토재킹 공격기법도 함께 주목받았다. 랜섬웨어 부문은 갠드크랩 관련 보도가 대부분을 차지할 정도로 국내에서 지속적으로 이슈가 되었다. 또한 미국 법무부에서 최초로 북한 해커조직의 일원을 재판에 넘기면서 해커 그룹에 대한 관심이 집중되기도 했다. 2018년 전반적으로 이러한 가상통화 거래소 해킹, 크립토재킹, 랜섬웨어, 해커 그룹의 4가지 키워드를 도출하였으며, 이 중 해커 그룹은 북한과 중국의 경우를 나누어 총 5가지 주제를 통해 악성코드에 대한 주요 이슈들을 살펴본다. 본 논문에서는 이러한 악성코드의 공격을 근본적으로 해결할 수 있는 방안으로 클라이언트 측에 USB형태의 BBS(Big Bad Stick) 하드웨어를 통하여 제안하는 환경을 제안하고 안전한 서비스가 제공됨을 증명하여 본 연구가 새로운 보안성을 갖춘 시스템임을 보인다.

고유수용성감각 운동프로그램이 편마비 환자의 균형 수행력에 미치는 효과 (The Effect of Proprioceptive Exercise Program on Balance Performance in Hemiplegic Patients)

  • Purpose : The purpose of this study is to analyze the effect of the proprioceptive exercise program on balance performance in order to the suggest hemiplegic patients the therapeutic intervention. Methods : In this study, Proprioceptive exercise program was applied to 11 hemiplegic patients for 6 weeks as follows: First of all, hemi leg flexion-extension pattern was applied in a supine position. Secondly, alternated leg flexion pattern was applied in a standing posture with one hand support on the unstable platform. Thirdly, rhythmic stabilization techniques were applied in a standing posture on the unstable platform. Results : Significant differences were observed the chronic low back pain patient for VAS, BBS. Chronic low back pain patient improved all test. The results of this study were summarized as follows: 1. After intervention, there was a statistically significant change in the dynamic balance (FSST, TUG, FRT) performance(p<.05). 2. After intervention, there was also a statistically significant change in the static balance (FICSIT-4) performance(p<.05). Conclusion : The result of the study suggests that muscle strength exercise by the intervention of proprioceptive exercise program improves the hemiplegic patients' static and dynamic balance performance.

통합관리 프로그램이 아급성 뇌졸중 환자의 운동기능, 인지기능, 우울에 미치는 효과 (The Effects of an Integrated Management Program on Physical Function, Cognitive Function, and Depression in Patients with Subacute stroke)

  • Purpose : This study examined the effects of an integrated management program on physical function, cognitive function, and depression in patients with subacute stroke. Methods : A nonequivalent control group design was adopted. The participants were assigned to either the experimental group (n=20) or control group (n=23). The experimental group received an 8-week integrated management program and standard rehabilitation service (i.e., physical therapy and occupational therapy), while the control group received the standard rehabilitation service only. Physical function was measured as gait speed and balance ability using the Berg Balance Scale (BBS). Cognitive function was measured with neuro-behavioral cognitive status examination (NCSE), and depression was measured using the Beck Depression Inventory-II (BDI-II). Repeated measure ANOVA was used to determine changes in physical function, cognitive function, and depression over 8-weeks. Results : The interaction between group and time was significant, indicating that the experimental group showed improvement in gait speed, balance ability, cognitive function (linguistic ability, linguistic memory, reasoning), and a decrease in depression compared to the control group. Conclusion : These results indicate that the integrated management program developed herein was beneficial in restoring physical function, cognitive function, and depression in subacute stroke patients.

Effect of Mirror Therapy on the Balance, Gait and Motor Function in Patients with Subacute Stroke

  • Purpose: This study examined whether mirror therapy could improve the balance, gait, and motor function of patients with subacute stroke. Methods: Thirty-three patients with subacute stroke were divided randomly into three groups: experimental group1, experimental group2, and the control group. The patients in experimental group1 performed a mirror therapy program on the unaffected side of the lower extremities, and the patients in experimental group2 performed mirror therapy on the affected side of the lower extremities. Both groups performed the exercise for 30 minutes per session, five times a week for four weeks. The control group did not receive mirror therapy. BBS, POMA, 10MWT, and the BRS were used to evaluate the balance, the quality of gait, gait speed, and the motor function before and after the intervention. Results: The gait speed increased significantly in the experimental groups1 and 2 after the intervention. The control group showed no significant difference in the gait speed after the intervention. The change in gait speed before and after the intervention showed a significant difference among the groups. Experimental group1 showed a significant increase in the gait speed compared to that of the control group. Conclusion: This study suggests that mirror therapy could be an effective intervention to improve the gait speed of patients with subacute stroke. On the other hand, there was no difference in the effectiveness of mirror therapy and therapeutic exercise on the balance, gait, and motor function.

지역사회 노인의 인지기능 향상 프로그램 개발에 대한 문헌적 고찰 (Literature Review on the Development of Cognitive Function Improvement Program for the Elderly in Community)

  • Objective: This study was to compares and analyzes programs applied to improve cognitive function in patients with mild cognitive impairment and early dementia in the community to find out their effectiveness. Methods: In this study, 12 papers were finalized by searching for "elderly", "cognitive", "community", and "program" using the database of the Research Information System (RISS), National Assembly Library, and Korean Studies Information (KISS). Results: Programs for cognitive function were in the order of cognitive stimulation program, arts and crafts, and exercise program. In the program, rather than applying the cognitive stimulation program alone, the program was operated by combining leisure or exercise, music, art, and handicraft. The time was shown to be 30 minutes. The most frequently used evaluation tool was MMSE, followed by GDS and BBS. By cognitive domain, cognitive stimulation program and memory, satisfaction in psychology, and balance ability in exercise were evaluated the most. In the cognitive area, various cognitive stimulation areas were included, and in the exercise area, basic exercise, muscle strength exercise, joint exercise, and balance exercise were applied. Conclusion: Therefore, developing a program to improve cognitive function for mild cognitive impairment, it will be possible to prepare guidelines to establish and development.

편측성 슬관절 골관절염이 있는 노인환자에서 동적균형운동의 효과 (Effect of Dynamic Balance Exercise in Elderly Patients with Unilateral Knee Osteoarthritis)

  • Objective: To evaluate the effectiveness of dynamic balance exercise after intra-articular injection of hyaluronic acid (HA) therapy in elderly patients with unilateral knee osteoarthritis. Method: 30 patients with unilateral knee osteoarthritis were enrolled in this study. The patients were randomly divided into two groups: dynamic balance exercise after HA injection group (group A) and only HA injection group (group B). Both groups administered intra-articular HA injection and group A patients continued 20-session exercises for 4 weeks. The assessments were measured before injection and 4 weeks after treatment using the visual analog scale (VAS) for pain, the Western Ontario and McMaster University Osteoarthritis Index (WOMAC) for physical function, Berg balance scale (BBS), and computerized dynamic posturography using SMART Balance Master system for balance function. Results: In both groups, significant improvements in VAS, WOMAC and balance function were observed. Compared between groups, group A showed significant improvements compared to group B in WOMAC and balance function. Conclusion: It is suggested that dynamic balance exercise may result in improved balance function and physical function in elderly patients with unilateral knee osteoarthritis.

PNF의 안정적 반전과 율동적 안정화 기법이 뇌졸중 환자의 보행 및 균형에 미치는 영향 (The Effect of Stabilizing Reversal and Rhythmic Stabilization in PNF on Walking and Balance in Patients with Stroke)

  • Purpose: The purpose of this study was to demonstrate the effects of proprioceptive neuromuscular facilitation (PNF) stability techniques on walking speed, trunk stability, and balance in stroke patients. Methods: Ten stroke patients volunteered to participate in the study, and each of subjects was randomly assigned to either the stability technique (ST) group (n=5) or to the treadmill (TM) group (n=5). Each therapeutic exercise program was provided for 30 minutes a day, 5 days per week for 4 weeks. The ST group performed a PNF pattern combined with stabilizing reversal and rhythmic stabilization of the PNF stability technique. Walking speed (measured using a 10-meter walking test), trunk stability (TIS), and balance (BBS, FRT) were evaluated before and after training. All data were analyzed using SPSS version 18.0. The significance level for statistical inspection was set at 0.05. Results: Both groups showed improvements on the 10-meter walking test, the trunk impairment scale, the Berg balance scale, and the functional reaching test. Conclusion: PNF stability techniques are effective for improving trunk stability, balance, and walking speed in stroke patients. For stroke patients, PNF stability techniques are very useful and effective, including in clinical practice.

불안정 지면에 앉아 추가적으로 수행한 몸통운동이 만성 뇌졸중 환자의 균형과 보행 능력에 미치는 영향 (Effects of Additional Trunk Exercises on an Unstable Surface on the Balance and Walking Ability of Individuals with Chronic Stroke)

  • Purpose: This study aimed to explore the effects of additional trunk exercises on an unstable surface on the balance and walking ability of individuals with chronic stroke. Methods: Sixteen patients with chronic stroke participated in this study. The participants were randomly assigned to two groups: experimental group (n=8) and control group (n=8). All the participants underwent a typical physical therapy program for 30 min a day. Moreover, the experimental group participated in a 30 min trunk exercise program on an unstable surface, whereas the control group participated in a 30 min trunk exercise program on a stable surface. Both groups performed the exercises five times a week for three weeks. The Berg Balance Scale (BBS) was used to measure changes in balance. The gait variables were measured using the GAITRite system (CIR System Inc., Clifton, NJ, USA) to examine changes in walking ability. Results: Both groups showed a significant intragroup improvement in balance, gait speed, cadence, stride length, and double support period (p<0.05). In the intergroup comparisons after the intervention, the experimental group showed significant improvements over the control group in balance, gait speed, cadence, stride length, and double support period (p<0.05). Conclusion: This study applied additional trunk exercises on an unstable surface to chronic stroke patients, and the results showed a significant improvement in the patients' balance and walking abilities. Therefore, trunk exercise on an unstable surface may be applicable as an intervention method to improve the balance and walking ability of chronic stroke patients.

착용형 하지 로봇을 이용한 편마비 보행 재활 훈련 효과에 관한 연구 (Study on Efficacy of Gait Training for Hemiplegia Patients Using Lower-Limb Wearable Robot)

  • Conventional gait rehabilitation requires at least three therapists in a traditional rehabilitation training program. Several robots have been developed to reduce human burden and increase rehabilitation efficacy. In this study, we present a lower-limb wearable robot (WA-H) for gait rehabilitation of hemiplegia patients, and propose a protocol of 12 weeks gait rehabilitation training program using WA-H. To identify the efficacy of the robot and protocols, we conducted a clinical study with two actual hemiplegia patients and observed a chronological change of ambulation ability through four assessments. We discovered the progression of results by 6 minute walking test, TUGT (Timed Up and Go Test), SPPB (Short Physical Performance Battery), BBS (Berg Balance Test), and Fugl-Meyer score. The torques generated in the normal side and paralyzed side of the patient became similar, indicating rehabilitation. The result also showed the walking of the paralysis patient improved and imbalance motion had considerable improved performance.
