Effect of Lamivudine Treatment on Chronic Hepatitis B Infection in Children Unresponsive to Interferon (인터페론 치료에 반응이 없었던 소아의 만성 B형 간염에 대한 라미부딘의 치료 효과)

  • Purpose: Interferon is a widely used treatment for chronic hepatitis B in children. However, additional treatment options are needed because more than 50% of hepatitis B patients are unresponsive to interferon. Although lamivudine is widely used to treat hepatitis B, there are few studies on the effect of lamivudine in hepatitis B patients unresponsive to interferon. Methods: Eight interferon unresponsive patients (6 males and 2 females) were treated with lamivudine (3 mg/kg/day, maximum 100 mg/day) from 6~12 months after interferon treatment was discontinued among 33 children with chronic hepatitis B. They were treated with interferon (interferon ${\alpha}$-2b, 10 MU/$m^2$ or pegylated interferon $1.5{\mu}g/kg$) for 6 months from January 2000 to December 2007 at the Pusan National University Hospital. The medical records were analyzed retrospectively. Results: The age at treatment with interferon and lamivudine was 4.9${\pm}$3.1 and 6.1${\pm}$3.2 years, respectively. The serum ALT level before treatment with interferon was 148.1${\pm}$105.8 IU/L and the log HBV-DNA PCR mean value was 6.95${\pm}$0.70 copies/mL. The serum ALT level after treatment with interferon was 143.1${\pm}$90.4 IU/L and the log HBV-DNA mean PCR value was 6.46${\pm}$2.08. HBeAg negativization occurred in 2 patients. For all patients, normalization of the serum ALT levels and HBeAg seroconversion (except 2 patients with HBeAg negativization) occurred at 7.4${\pm}$2.1 and 7.9${\pm}$2.1 months respectively after lamivudine treatment. The HBV-DNA PCR became negative in 7 patients (87.5%) at 2.4${\pm}$2.8 months. Complete response was achieved in 7 patients and no recurrence was observed in 2 patients for 3 years after the completion of treatment. Five patients are still under treatment for a mean treatment duration of 24.4${\pm}$9.1 months. In one patient, viral breakthrough occurred and the treatment was stopped. Conclusion: The number of patients was small, however, lamivudine treatment in patients with chronic hepatitis B who were unresponsive to interferon was highly effective.

Affinity Maturation of an Anti-Hepatitis B Virus PreS1 Humanized Antibody by Phage Display

  • In a previous study we generated an anti-Hepatitis B Virus (HBV) preS1 humanized antibody (HzKR127) that showed in vivo HBV-neutralizing activity in chimpanzees. However, the antigen-binding affinity of the humanized antibody may not be sufficient for clinical use and thus affinity maturation is required for better therapeutic efficacy. In this study, phage display technique was employed to increase the affinity of HzKR127. All six amino acid residues (Glu95-Tyr96-Asp97-Glu98-Ala99-Tyr100) in the heavy (H) chain complementary-determining region 3 (HCDR3) of HzKR127 were randomized and phage-displayed single chain Fv (scFv) library was constructed. After three rounds of panning, 12 different clones exhibiting higher antigen-binding activity than the wild type ScFv were selected and their antigen-binding specificity for the preS1 confirmed. Subsequently, five ScFv clones were converted to whole IgG and subjected to affinity determination. The results showed that two clones (B3 and A19) exhibited an approximately 6 fold higher affinities than that of HzKR127. The affinity-matured humanized antibodies may be useful in anti-HBV immunotherapy.

A Study on the Occurence Rate of HBsAg in a Dental School Population (치과대학생 및 전공 에 있어서 B형간염면항원의 발현빈도에 관한 연구)

  • Medical personnek are one of several groups that have been reported to have a high incidence of hepatiris B. It is also thought that the occurrence rate of hepatitis B surface antigen(HBsAg), aserologic marker for hepatitis B virus(HBV), is expected to be high in the dental personnel who are frequently exposed to the blood and saliva of the patients. Although many studies have been done to determine the HBsAg status of virus groups, limited investigations have been performed on dental personnel, especially in this country. The main purpose of this study was to identify HBsAg positivity among dental students, interns, and residents who would be expected to be a high risk group of hepatitis B infection. Screening test for HBsAg of a dental school population was performed by indirect hemagglutination(IHA)in 1982. The results were as follows : 1. Thirty four out of a total 362 persons(9.4%)tested in the study had positive response for HBsAg in their serum samples. 2. Twenty seven out of 320 dental students(8.4%)had positive for HBsAg, and in Senior class 12 out of 82 dental students (14.6%)had positive response that was the highest incidence among dental students group. 3. Seven out of 42 interns and residents(16.7%)had positive for HBsAg, and it was the highest incidence in this dental school population.

  • Children with abnormal liver function can often be seen in outpatient clinics or inpatients wards. Most of them have respiratory disease, or gastroenteritis by virus infection, accompanying fever. Occasionally, hepatitis by the viruses causing systemic infection may occur, and screening tests are required. In patients with jaundice, the tests for differential diagnosis and appropriate treatment are important. In the case of a child with hepatitis B virus infection vertically from a hepatitis B surface antigen positive mother, the importance of the recognition of immune clearance can't be overstressed, for the decision of time to begin treatment. Early diagnosis changes the fate of a child with Wilson disease. So, screening test for the disease should not be omitted. Non-alcoholic fatty liver disease, which is mainly discovered in obese children, is a new strong candidate triggering abnormal liver function. Muscular dystrophy is a representative disease mimicking liver dysfunction. Although muscular dystrophy is a progressive disorder, and early diagnosis can't change the fate of patients, it will be better to avoid parent's blame for delayed diagnosis.

  • We treated 1 case of primary infertility woman. She had three clinical problems. The first was endometrosis stage II, the second was tubar obstruction and the third was chronic persistent hepatitis B. Eometrosis and tubar obstruction are similar to 'blood stagnation'. And another clinical symptoms could be considered with 'asthenia of spleen and stagnation of wetness-evil'. So our prescriptions for this case were Heylbudu.Jcgeu-Tang and Samchulgunbi-Tang. And the prescription of acupuncture therapy was consist with Sp. 6, Ren. 4, Jagung, P. 6, and St. 36. So we got a successful result from above therapies. But we have to take continued follow-up plans for endometriosis and chronic persistent hepatits B.

  • The helicase genes and hypervariable regions (HVRs) of three avian hepatitis E viruses (HEVs) detected at three different farms were sequenced and characterized. Two isolates (DW-L and GI-B2) were classified as genotype 2 and one isolate (GR-B) was classified as genotype 1. A phylogenetic tree, based on the helicase gene and HVR nucleotide sequences, revealed the newly detected viruses and other avian HEVs were classified similarly. Unlike previously reported avian HEVs, the DW-L isolate detected in broiler breeders with characteristic lesions of avian HEV had no proline-rich motif in its HVR, suggesting that the proline-rich motif is non-essential for viral replication and infection.

Quality of Life and Related Factors in Patients with Chronic Hepatitis B* (만성 B형 간염환자의 삶의 질과 제요인)

  • Purpose: This study aimed to investigate LDQOL (Liver Disease Quality of Life) and its related factors of patients with Hepatitis B. Method: A cross-sectional descriptive study. The LDQOL was formally translated to Korean and reliability was examined. One hundred thirty eight patients following gastroenterology outpatient clinic of S. hospital in Seoul, Korea participated. Results: The mean age of the patients was 45.43 years and 87.7% were men. The mean score of LDQOL was 77.34, and mean scores of subscale were followed; symptom of liver disease (SxLD) (82.12), effect of liver disease (ELD) (25.50), concentration (Conc) (84.47), memory (Mem) (83.24), health discomfort, (HD) (75.18), sexual function (SFun) (75.71), sexual problem (SProb) (84.70), Loneliness (85.50), Hopeless (67.43), and stigma of liver disease (SLD) (91.64). Women had a lower LDQOL score for Loneliness (p=.034), and over 45 year-old patients had a lower LDQOL overall score (p=.000). Patients who were HBV carriers, or who had Chronic Hepatitis B or Liver Cirrhosis reported lower QOL respectively(p=.032). Conclusion: Although the liver disease itself seemed to be stable, patients with HBV experienced poor QOL in ELD, SProb, SFun, and Hopeless. Therefore nursing interventions in these aspects are needed.

The Effect of Health Care Program for Newly Married Couple in a Community Health Care Center (일 보건소의 신혼부부 건강프로그램의 효과)

  • Purpose: The research purpose was analysis of the effect of a health education program for newly-married executed from 2003 to 2005 as a part of health care service from community health center and providing the fundamental source for health education needed during newly-married. Method: The data collection with structured questionnaires was conducted during October to December in 2006. From the health center 106 brides who participate the program and 130 brides who didn't participate. Health care program for newly married couple were composed health examinations and health educations. Health examinations were CBC, LFT and Ag & Ab prevallence rates of hepatitis B, rubella. Health educations were done two times, the first individual health education was done at enrolled in health center for establishment of desirable couple relationship, family planning, contraception method and management of pre-pregnancy. The second individual health education was done for explanation of blood examination results, prevention of the congenital deformity and the vertical transmission of hepatitis B, management for pregnancy, breast feeding method, introduction to safety delivery method and encouraging self-study using by materials made by health center after two weeks at revisiting health center. SPSS/PC(ver. 12.0) and $X^2$-test, t-test was used to analyze the collected and tabulated data. Socio-demographic characteristics and regional characteristics of residence area of two groups shows no significant difference. Result: As a variables of experimental effect. The natural childbirth rates of participants group was significantly higher than Non-participants group(p=.012). Breast feeding rates for participants group was also statistically significant higher than Non-participants group. Contraception rates showed no significant difference. The rubella vaccination rates (p=.001) and The hepatitis B vaccination rates (p=.012) shows statistical differences. Conclusion: Therefore health care program for newly married couple in a community health care center was effective generally. We supposed that explore participation ways for nonparticipants, expand the program to community health care center in whole country.

Comparison of the Practical and Theoretical Knowledge of the Hepatitis B Virus among Dental Hygiene Students

  • Background: Hepatitis B is an important public health problem as one of the leading causes of morbidity and mortality. The aim of this study was to compare the theoretical and practical knowledge levels of dental assistant students about Hepatitis B Virus. Methods: The 1st and 2nd grade students of the Oral and Dental Health program were invited to participate in the survey. Out of the 68 invited students, 61 completed the questionnaire. The average ages of the male and female students surveyed were 20.27±1.45 and 19.56±1.16, respectively. A total of 34 questions were asked, of which 15 measured basic theoretical knowledge and 19 assessed basic practical knowledge. Results: There was no statistically significant difference between the students' knowledge for each question according to their sex and grade. When the questionnaire was grouped into basic theoretical and basic practical knowledge levels, both were observed to be high. While the lowest correct answer rate was 35.00% for the questions about practical applications, it was 31.14% for the questions measuring the level of theoretical knowledge. There was no statistically significant difference when the levels of knowledge of 1st and 2nd grade students were compared. Students answered the majority of the questions correctly, and ranged between 71% and 100%. Conclusion: Students' high level of basic theoretical knowledge can be a result of their in-class education on the fundamentals. However, their knowledge about the correct approaches in practical applications indicates the beneficial role of having well-defined criteria and prevention protocols that are required in hospitals and the effectiveness of their environmental orientations.

Association between Interferon-Inducible Protein 6 (IFI6) Polymorphisms and Hepatitis B Virus Clearance

  • CD8+T cells are key factors mediating hepatitis B virus (HBV) clearance. However, these cells are killed through HBV-induced apoptosis during the antigen-presenting period in HBV-induced chronic liver disease (CLD) patients. Interferon-inducible protein 6 (IFI6) delays type I interferon-induced apoptosis in cells. We hypothesized that single nucleotide polymorphisms (SNPs) in the IFI6 could affect the chronicity of CLD. The present study included a discovery stage, in which 195 CLD patients, including chronic hepatitis B (HEP) and cirrhosis patients and 107 spontaneous recovery (SR) controls, were analyzed. The genotype distributions of rs2808426 (C > T) and rs10902662 (C > T) were significantly different between the SR and HEP groups (odds ratio [OR], 6.60; 95% confidence interval [CI], 1.64 to 26.52, p = 0.008 for both SNPs) and between the SR and CLD groups (OR, 4.38; 95% CI, 1.25 to 15.26; p = 0.021 and OR, 4.12; 95% CI, 1.18 to 14.44; p = 0.027, respectively). The distribution of diplotypes that contained these SNPs was significantly different between the SR and HEP groups (OR, 6.58; 95% CI, 1.63 to 25.59; p = 0.008 and OR, 0.15; 95% CI, 0.04 to 0.61; p = 0.008, respectively) and between the SR and CLD groups (OR, 4.38; 95% CI, 1.25 to 15.26; p = 0.021 and OR, 4.12; 95% CI, 1.18 to 14.44; p = 0.027, respectively). We were unable to replicate the association shown by secondary enrolled samples. A large-scale validation study should be performed to confirm the association between IFI6 and HBV clearance.
