Genetic Diversity of Hepatitis C Virus in Korea (한국내 C형 간염바이러스의 유전적 다양성)

  • C형 간염바이러스 (HCV)는 각 개체간에 뉴클레오티드 서열상의 다양성을 나타내고, 이러한 유전적 다양성이 임상병리적 증상과 밀접한 연관이 있을 것으로 고려되어 왔다. 본 연구에서는 HCV E1과 NS5B 부위의 염기서열 분석을 통해 한국의 C형 간염바이러스의 분포와 다양성에 관해 분석하고, 발생계통도를 그려 HCV간의 진화적 거리를 확인하였다. 염기서열분석은 서울대학교 병원과 충남대학교 병원으로부터 얻은 56개의 HCV-양성 혈청을 대상으로 RT-PCR과 PCR 과정을 통해 얻은 유전자 산물을 클로닝하여 수행되었다. 56개의 혈청중 53개의 샘플에서 HCV RNA가 검출되었다. 이들 53개 샘플에 대한 분석 곁과, 유전형 1a, 1b, 2a, 2b, 7a가 각각 5.7, 45.3, 45.3, 1.9, 1.9%로 분포하고 있고, 1b형과 2a형이 한국에서의 주요한 HCV 유전형으로 밝혀졌다. 본 연구는 염기서열 분석을 통해 한국에서 1b형과 마찬가지로 2a형도 높은 빈도로 분포하고 있고, 비록 분포 빈도는 낮지만 1a 형과 7a 형도 존재하고 있음을 밝힌 최초의 보고이다.

  • 우리나라에서 만성 간질환으로 인한 사망자는 해마다 2만여 명에 가깝다. 이는 전체 사망자의 8.8%를 차지하는 것으로 단일 장기로 따지면 만성 간질환은 뇌혈관질환에 이어 사망 원인 2위다. 특히 40~50대 사망원인 중 3위를 차지한다고 한다. 간질환의 가장 큰 원인은 바이러스성 간염으로 B형 간염이 약 70%, C형 감염이 약 10~15%를 차지하고 있다.

  • 많은 항바이러스제의 개발에도 불구하고, B형간염바이러스 감염으로 인해 간 이식을 받은 환자를 대상으로 한 재발 억제 치료 시 항 B형간염바이러스 면역글로불린(Anti-hepatitis B immunoglobulin, HBIG) 치료는 아직도 가장 중요한 약물요법으로 여겨지고 있다. 본 연구진은 간 이식 환자에서의 6개월 간의 약동학 연구를 통해, HBIG의 약동학적 파라미터와 이에 대한 영향인자를 확인하였으며, 이를 이용해 용법용량 별로 유지요법 시기에 목표 농도 도달 가능성을 예측하는 시뮬레이션 tool을 개발하였다. 그 결과 95% 환자가 목표 농도 이상의 HBIG 농도를 유지하기 위한 용량은 '목표 농도 ${\times}$ 20'에 해당하는 것을 확인하였으며, 다수의 대학병원에서 통상적인 HBIG 유지 요법의 목표 농도를 최소 300 IU/L로 하고 있음을 고려할 때, 6,000 IU의 용량을 월 1회 투여하는 것이 바람직할 것으로 판단하였다.

A Study on the Funeral director's working Circumstances and Personal Protective Equipment on Funeral home (안치실 및 염습실 종사자의 주요 감염질병 조사 및 근무기간에 따른 개인위생용품 착용실태에 대한 연구)

  • We are investigated about the state of the working circumstances and infection blocking facility within Metropolitan, Chungcheong and Gyeongsang regions in Korea. The result is as follows. The HAV vaccination rate is 35% and the HBV vaccination rate is 50% among the funeral directors. We expected that the infection risk of funeral directors was very high. Significantly, tuberculosis prevalence rate of funeral director was four times as high as the public. The wear rate of gown, mask, gloves showed relatively high than the others. The wear rate of individual protective equipments has decreased with work experience. The funeral directors have shown that there is difficult to get safe protection from infection. The personal hygiene of funeral directors and the infection blocking equipment should be strengthened institutionally. We are thought to recommend institutional support about infection management, individual protective equipment, vaccination of funeral director and guideline to avoid reversion to previous habitual behavior.

  • 생명 현상을 시스템적으로 이해하기 위해서는 현상에 대한 수학적 모델링이 필수적이다. 여러 가지 수학적 모델 가운데 상미분 방정식(ODE) 모델은 여러 가지 생화학 반응을 모델링 하는데 널리 사용되고 있다. 본 논문에서는 신호전달 경로에 B형 간염 바이러스가 미치는 영향을 ODE로 모델링하고, 이를 시뮬레이션 한 결과를 보인다. 또한, ODE모델을 설계하는데 있어 보다 유연하고 확장 가능한 새로운 표현 방식을 제안한다.

Detection of Lamivudine-Resistant Mutations of HBV DNA Polymerase Gene Using PCR-Direct Sequencing

  • Treatment of hepatitis B virus (HBV) with lamivudine is effective in suppressing virus replication and results in reduced inflammatory activity. However the most troublesome problem of lamivudine treatment is the emergence of lamivudine-resistant strains with amino acid substitution in the YMDD motif of DNA polymerase gene during the treatment. The aim of this study was to determine the mutation of YMDD motif (codon 552) and codon 528 in chronic HBV patients with lamivudine therapy using PCR-direct sequencing and to investigate the relationship between lamivudine mediated HBV mutation and HBeAg. HBV DNA was extracted from serum samples of HBV patients and amplified by nested PCR with two sets of primer pairs selected in HBV DNA polymerase gene. Amplified PCR product was analyzed by 2% agarose gel electrophoresis and direct sequencing. HBV mutation was detected in 124 out of 207 samples (60%). Single mutation was 50.8% for M552I, 43.5% for M552V, 5.7% for M552I/V and the L528M mutation was 67.0%. Double mutation was 43.6% for M552V/L528M, 33.1% for M552I/L528(wild type), 17.7% for M552I/L528M and 5.6% for M552I/V/L528M. Serine mutation at YMDD motif (M552S) was not found and the L528M mutation frequently accompanied M552V type. In this study, the typical difference of frequencies for HBV mutation depending on HBeAg was not found. Moreover, the PCR-direct sequencing method used in this study might be a powerful tool for the mutation study in clinical reference laboratories with high volume.

  • Objectives: Since 1983, vaccination for Hepatitis B virus has been recommended for all neonates in Korea. The objectives of this study was to assess the changing trend of the positive rate of HBsAg and anti-HBs in university students in one province. Methods: The study population consisted of one university students who taken physical exam in 2004, which included 8,527 students (3,391 males and 5,136 females). Serum HBsAg and anti-HBs were detected by immunochromatography method. Results: The positivities of HBsAg and anti-HBs were 2.7% and 61.1%. The positivity of HBsAg was significantly higher in males(p< .05), while that of anti-HBs was significantly higher in females(p< .05). There was some differences of HBsAg positivities among age groups. But there was no correlation between age and the positivities. There was some differences of anti-HBs positivities among age groups. But there was no correlation between age and the positivities. Conclusion: The positivity of HBsAg has significantly decreased and that of anti-HBs has increased with years. It was suggested that the changes of positivity were caused by nationwide hepatitis B vaccination program.

  • The co-infection with hepatitis B virus (HBV) and hepatitis C Virus (HCV) is associated with a more severe liver disease and increased frequency in the development of hepatocellular carcinoma com-pared to those with single infection. Here, we demonstrated that HBV X protein (HBx) and HCV Core cooperatively up-regulated the level of p53 in human hepatoma HepG2 cells. The elevated p53 subsequently stimulated the expression of proapoptotic Bax whereas it repressed the expression of antiapoptotic Bcl2. These effects, however, were not observed in p53-negative Hep3B cells. Consistently to their cooperative regulation of apoptotic effectors, HBx and HCV Core additively stimulated cisplatin-mediated apoptotic cell death of HepG2 but not of Hep3B cells. These results may help to explain the development of a more severe liver disease in patients co-infection with HBV and HCV as well as some contradictory results on the roles of HBx and Core in apoptosis.

  • X-linked agammaglobulinemia (XLA) is a primary inherited B-cell immunodeficiency. The prevalence of neoplastic disease in patients with XLA is approximatedly 0.7%. The most frequent tumor is lymphoreticular malignancy. We report a case of hepatocellular carcinoma (HCC) in a 13-year-old boy with XLA, after probable maternal transmission of hepatitis B virus. The authors consider that the vertical transmission of hepatitis B virus might play an important role in the development of HCC in a child with XLA who could not eliminate hepatitis B virus effectively.
