One Case of Langer's Axillary Arch(Axillopectoral Muscle) (랑거씨 액와 아치 1예)

  • Langer's axillary arch(axillopectoral muscle) is a rare disease. It is a variant muscular structure of the axilla, which was described as Langer's muscle by Ramsay and Langer. It could evoke chronic ambiguous neck pain. Therefore, it should be considered, when the patient failed to find the origin of neck pain which do not react any treatment. If it were diagnosed, it can be easily managed. However, it is not well known to otorhinololaryngologitis. Here we reports the 53 years old female who developed pain on supraclavicular area as a result of an Langer's axillary arch.

COVID-19 Vaccine-Related Axillary and Cervical Lymphadenopathy in Patients with Current or Prior Breast Cancer and Other Malignancies: Cross-Sectional Imaging Findings on MRI, CT, and PET-CT

  • Breast radiologists are increasingly seeing patients with axillary adenopathy related to COVID-19 vaccination. Vaccination can cause levels I-III axillary as well as cervical lymphadenopathy. Appropriate management of vaccine-related adenopathy may vary depending on clinical context. In patients with current or past history of malignancy, vaccine-related adenopathy can be indistinguishable from nodal metastasis. This article presents imaging findings of oncology patients with adenopathy seen in the axilla or neck on cross-sectional imaging (breast MRI, CT, or PET-CT) after COVID-19 vaccination. Management approach and rationale is discussed, along with consideration on strategies to minimize false positives in vaccinated cancer patients. Time interval between vaccination and adenopathy seen on breast MRI, CT, or PET-CT is also reported.

The Square-Plus Flap: A Modification to Release Long Postburn Scar Contractures

  • The square flap method has been successful in releasing contracture bands at various body regions. However, the original square flap method alone may not be efficient in releasing long contracture bands. We, therefore, proposed an extended design to the traditional design, which is called the "square-plus flap." A 4-year-old girl presented with a postburn web-like contracture band over the right axilla. We marked a square flap technique at the center of the contracture band and then two additional Z-plasties were placed on both edges of the flap. After the release and securing of the square flap, the adjacent distal Z-plasty was then transposed and sutured in their new locations. We do not need to incise the proximal Z-plasty as we could achieve complete relaxation of the contracture band. This novel modification can be added to the plastic surgeon's armamentarium for releasing long postburn contracture bands involving distinct body regions.

A Clinical and Mycological Investigation on Tinea versicolor (전풍(?風)의 임상적(臨床的) 및 균학적(菌學的) 연구(硏究))

  • This investigation was undertaken to establish more accurately the clinical features of the desease as seen in Korea, and to investigate susceptibility factors and to study the ecology of P. orbiculare in this condition. All of 50 new patients with Tinea versicolor attending the Ewha University Hospital, during a period of 6 months were studied personally. 1. Clinical feature: The sex ratio of the whole series was 32 males to 18 females. Twenty of 50 patients (40%) were between the age of 20 to 29 years. The duration of disease was more than 4 months in 37 of 50 patients (74%). The cases of first onset in June, July and August were 33 of 50 patients (66%). Whereas, the developed in December, January and February were only 3 of 50 patients (6%). Seasonal fluctuation was remarkable in 16 of 25 patients (66.7%) who had the duration of more than 1 year. The course of disease was rapid in 33 of 50 patients (66%) and slow in 9 of 50 patients (18%). Thirty-three of 50 patients (66%) had not any subjective symptoms. Thirteen patients (26%) had mild itching sensation and 4 patients (8%) had severe itching. The frequency of the first affected were neck (27.4%), axilla (19.2%) anterior chest (15.0%) and groin (11.0%). 7) Hyperpigmented lesions were observed in 35 of 50 patients (70%) and hypopigmented lesions were observed in 11 of 50 patients (22%). Skin lesions were noticed mainly in the anterior chest, neck, back and axilla, and more severe in the anterior chest, back and abdomen than other areas. Tinea versicolor was particularly prevalent in those persons who had occupations of mental work and who perspired every day or every 2 or 3 days. A positive family history of Tinea versicolor was obtained in 7 of 50 patients (14%). Fourteen of 50 patients (28%) had association with superficial fungal infections. 2. Mycological features: Typical appearance of M. furfur was observed in all scrapings from 25 lesions and only 3 scrapings from 20 clinically skin of the patients. P. orbiculare was seen on direct examination of scrapings from 24 of 25 skin lesions, from 19 of 20. clinically normal skin of patients and from 18 of 29 control normal skin. The quantity of P. orbiculare was greater in skin lesions and clinically normal skin area than in control normal skin surfaces.

Effects of Caffeine on Nerve Conduction Velocity (카페인이 신경전도속도에 미치는 영향)

  • This study examined the effect of the nervous-system-stimulating caffeine on the nerve conduction velocity. The purpose of this study is to investigate the effect of caffeine that stimulates the nervous system on nerve conduction test. Although both measurement intervals did not show statistically significant differences when comparing the pre- and post-NCV values within the control and individual experimental groups, it was found that the nerve conductivity in the Axilla-Above Elbow section increased significantly after caffeine intake for the experimental group. Caffeine intake, which has increased the nerve conduction velocity (NCV), was determined to play roles in improving motor skills, muscle strength and nerve performance by temporarily increasing the nerve conduction velocity. Through this study, we learned that caffeine has an influence on the peripheral nervous system as it helps to improve the nerve conduction velocity. upon an appropriate amount of caffeine intake. We hope that these results will help develop treatment and diagnostic methods for patients with nerve dysfunction and myofunctional disorders.

Earwax of patients with hidradenitis suppurativa: A retrospective study

  • Background In Western nations, hidradenitis suppurativa (HS) typically affects the apocrine gland-bearing skin of people of African origin, women, smokers, and individuals with obesity. The clinical characteristics of HS in Korea and Japan, however, are reportedly different from those in the West. We therefore hypothesized that wet earwax is associated with HS because most East Asian people are genetically predisposed to produce dry earwax. Methods The medical charts of 53 Japanese patients with HS were reviewed retrospectively. Results Unlike the results of surveys conducted in Western nations, most of our patients were men (72%), whose buttocks were the most commonly affected site. Apocrine gland-bearing areas, such as the axilla, were affected less often. The proportion of HS patients with wet earwax was 51%, which was substantially higher than that found in the general Japanese population. Moreover, when patients with gluteal HS were excluded, the proportion of patients with wet earwax became even higher (68%). Conclusions Although the etiology of HS is unknown, our survey indicated that HS in apocrine gland-bearing skin, such as the axillary and anogenital areas, may be associated with wet earwax. As this study was conducted in a limited clinical setting, a nationwide, multicenter survey is warranted to clarify the clinical characteristics of HS in Japan.

Endoscopic transaxillary prepectoral conversion for submuscular breast implants

  • Background During breast augmentation, the transaxillary approach provides the advantage of allowing the mammary prosthesis to be placed through incisions that are remote from the breast itself, thereby reducing the visibility of postoperative scars. For patients experiencing capsular contracture who do not want additional scars, the previous transaxillary scar can be used for site change and implant exchange. Methods This study analyzed 17 patients (34 breasts) with submuscular breast implants with grade III-IV capsular contracture who received treatment from 2010 to 2015. The mean age of the patients was 29 years (range, 20-38 years). The inclusion criterion was a pinch test of more than 3 cm at the upper pole of the breast. Previous axillary scars were used to expose the pectoralis fascia, and submuscular breast implants were removed carefully. The dissection underneath the pectoralis fascia was performed with endoscopic assistance, using electrocautery under direct visualization. Results The mean follow-up period was 14 months (range, 6-24 months). The entire dissection plane was changed from the submuscular plane to the subfascial plane. Round textured gel implants were used, with a mean implant size of 220 mL (range, 160-300 mL). Two patients developed grade II capsular contracture. There were no cases of malposition or asymmetry. Three patients complained of minor implant palpability. None of the patients required additional surgery. Conclusions Endoscopic subfascial conversion may be an effective technique for treating capsular contracture and avoiding scarring of the breast in selected patients.

Expansile Duraplasty for Posttraumatic Syringomyelia - A Case Report - (외상후 척수공동증에 대한 확장성 경막성형술 - 증례보고 -)

  • A 35-year-old female patient who had been paraplegic since her upper thoracic cord injury one and a half years before was presented with a syringomyelia involving C7 to T2 cord segments on follow-up magnetic resonance imaging(MRI). Her pain on the chest and bilateral axilla worsened since about 2 years after initial injury, and MRI demonstrated a progressively enlarging cystic syrinx cavity of the cervicothoracic cord. The posttraumatic syringomyelia was managed operatively by lysis of the arachnoid adhesions, myelotomy, and expansile duraplasty. Post-operatively her pain was relieved, and MRI showed decrease in size of the syrinx, and digital infrared thermographic imaging(DITI) showed diminution of the hyperthermic area on the anterior chest. In conclusion, the posttraumatic syringomyelia can be managed successfully without shunt operation. It seems apparent that expansile duraplasty is effective in controlling the posttraumatic syrinx.

The Analysis of Manufactured Jacket Pattern for Obese Women in Their Middle Age (중년기 비만여성을 위한 기존 재킷의 패턴분석)

  • The ready-made clothes for obese women are not manufactured systematically due to the diversity and scarcity of their body shape. In this study, we first analyzed and classified 104 obese women's body shape to select four representative subjects. Then, the four subjects participated in wearing test to evaluate the performance of the jacket patterns which are available in the market and widely-used at the same time. The characteristics of the jacket patterns for obese women were also investigated. As to the highly rated jacket in terms of body motion, it was found that difference between the breast width to axilla and the breadth to anterior midaxilla is larger than the other jackets, and the breast width of front body is wider than what of back width. It was noted, however, that the armhole depth and the width of obese women' jackets should not be determined indirectly by the magnitude of the girth around bust, waist or hip as in the case of regular sized women's. It is because body proportion of obese women is different from that of standard sized women.
