  • A case of hepatic hemangioma is presented in which blood pool imaging by $^{99m}Tc$-HSA provided a definite diagnostic clue. Cavernous hemangiomas are the most common benign tumor of the liver. But the majority of them are asymptomatic and are found incidentally at laparotomy or autopsy. Selective hepatic arteriogrophy has been the most helpful technique for the preoperative diagnosis. Recently radioisotope scan and ultrasonogram proved to be a new, definite and non-invasive diagnostic procedure.

  • Diffuse pulmonary ossification is a rare entity which was reported just a case in Korea, defined as "wide spread heterotropic bone formation within the lung parenchyme", and it's pathogenesis remains unclear. Generally, diffuse pulmonary ossification is diagnosed by autopsy because most of case is asymptomatic, and classified as either nodular and dendriform diagnosed pathologic examination. We reviewed a case of diffuse dendriform pulmonary ossification with spontaneous pneumothorax.

Acute Toxicity Study on Ssanghwa-tang Extract Fermented with Paecilomyces Japonica in Mice (동충하초 자실체를 이용한 발효 쌍화탕의 급성독성 실험)

  • Objectives : This study was carried out to investigate the acute toxicity and safety of Ssanghwa-tang extract fermented with Paecilomyces japonica. Methods : To evaluate the acute toxity and safety, 0(control group), 1250, 2500 and 5000 mg/kg of Ssanghwa-tang and fermented Ssanghwa-tang extracts were orally administered to 35 male and 35 female ICR mice. After single administration, we observed number of death, general toxicity, changes of body weight, and autopsy. Results : Compared with the control group, we could not find any toxic alteration in all treated groups (1250, 2500 and 5000 mg/kg). Conclusions : $LD_{50}$ of Ssanghwa-tang and fermented Ssanghwa-tang extracts might be over 5000 mg/kg and it is very safe to ICR mice.

  • Tracheal tumors are uncommon comprising less than 0.1% of all malignancies. Metastatic tracheal tumors, especially from the extrathoracic sites, are exceedingly rare. Ovarian cancer tends to metastasize to the serous cavities and the lymph nodes. One large autopsy study reported tracheal involvement in 1% of patients who had died from ovarian cancer. Other studies have not mentioned tracheal involvement at all. Since the main symptoms of cough, hemoptysis, or wheezing are nonspecific, patients may be initially treated for other conditions including asthma or bronchitis. Here we describe a metastatic tracheal tumor from an ovarian carcinoma that was initially treated for bronchial asthma.

Yew Poisoning in 17 Dairy Cattle (젖소 17두의 주목나무 독성 중독)

  • We found seventeen dairy cattle with the abnormal signs in cardiovascular and gastrointestinal systems after feeding of the yew foilage. Among them three cattle were dead due to yew poisoning. Among the remaining fourteen dairy cattle, four cattle showed similar symptoms as did dead cattle. Although the remaining ten dairy cattle did not show any abnormal signs, we conducted a therapy of forced magnesium sulfate infusion because the yew consumption might have been occurred in all cattle. As a result of the therapy, we could not found further signs of yew poisoning. The performance of the therapy and the treatment procedures adapted by the therapy could be concluded as follows: The cardioselective toxicity and inhibition of peristaltic activity by the taxine in yew foilage might be involved in the symptoms of acute poisoning as anorexia, dullness, muscle tremor, dyspnea, and sudden death. We also performed the dose response relationship of taxine to the range of clinical symptoms and examined recovery performances. Through the autopsy of the cattle, we could confirm the presence of yew foilages that might have caused the poisoning in the gastrointestinal tract. However, we could not identify further abnormalities in other organs. In this case report, we demonstrated that practice of the forced magnesium sulfate infusion in yew poisoning was helpful for the attenuating the taxine poisoning by blocking the further proceeding of the toxic effect.

Acute Toxicity Study on Yeoldahanso-tang in Mice (Mouse(ICR)에 있어서 시험물질 열다한소탕(熱多寒少湯) 추출액의 급성독성 시험)

  • 1. Objectives This study was designed to andyze the acute toxicity and safety of Yeoldahanso-tang 2. Methods We investigated the acute toxicity for water-extracted Yeoldahanso-tang. 25 male and 25 female mice were observed for 14 days after one day oral administration of Yeoldahanso-tang at the respective doses of 0(control group), 2560, 3200, 4000 and 5000 mg/kg. 3. Results We observed survival rates, general toxicity, change of body weight and autopsy. 4. Conclusions The data confirmed that Yeoldahanso-tang is free from the toxicity and safety problems in oral route respectively. Compared with the control group, we could not find any toxic alteration in all treated groups(2560, 3200, 4000 and 5000 mg/kg). In conclusion, LD50 of Yeoldahanso-tang was over 5000 mg/kg and it is very safe to mice.

Postmortem Distribution of Methidathion in Human Specimens of a Acute Poisoning (Methidathion 중독사에 의한 사후혈액 및 조직중 분포)

  • Methidathion is one of the organophosphorus pesticides commonly used for stamping out harmful pests in farming areas. This paper presents a fatality due to methidathion intoxication and describes the distribution of methidathion in postmortem blood and tissues obtained at autopsy. Qualitative identification of methidathion was achieved by TLC, GC and GC/MS, and quantitative analysis was performed by GC with thermionic specific detector (TSD). The analytes in postmortem specimens were extracted by liquid-liquid extraction (LLE) with ethylether. After the ethylether layer was evaporated, the residue was partitioned into hexane and acetonitrile, and the acetonitrile layer was used for analysis. Tissue specimens were homogenized with 4% perchloric acid and applied for LLE. After extraction, the extracts were reconstituted 100 $\mu\textrm{g}$ pyraclofos (IS, 100 $\mu\textrm{g}$/ml in methanol) for GC and GC/MS analysis. On analysis of postmortem specimens, methidathion was identified and quantitated. The methidathion concentrations were 2.0 $\mu$l/ml in blood, 24.4 $\mu\textrm{g}$/g in liver, 13.9 $\mu\textrm{g}$/g in lung, 21.8 $\mu\textrm{g}$/g in kidney, respectively.

Uniqueness of radiographic patterns of the frontal sinus for personal identification

  • Purpose: This study was performed to evaluate the uniqueness and reliability of the frontal sinuses by comparing various patterns of frontal sinus as observed on Waters' radiographs for individual identification. Materials and Methods: Three Waters' radiographs of 100 individuals, taken on day one, after 6-8 months, and one radiograph with a slight variation in angulation, to mimic conditions out in the field or during autopsy. Three observers were randomly given radiographs from all there packets for comparisons and identification, by the method of superimposition and individual uniqueness. Results: The comparative identification by superimposition of the frontal sinus was 100% positive. The size, shape, unilateral or bilateral presence, absence, and septa were observed to be unique in each case; neither had the measurements changed over a period of time. Conclusion: The need to establish a reliable, low-cost, and easily reproducible method for human identification prompted the elaboration of technical, precise, and accessible parameters, such as the evaluation of the area, asymmetry, and shape of the frontal sinus. Comparison among each of the frontal sinuses of the 100 people in the sample revealed that no two sinuses are the same, that is, the sinus is unique to each individual.

  • Tracheopathia osteoplastica is a rare disease characterized by submucosal cartilaginous or bony projections into tracheobroncheal lumen with sparing of the posterior membranous portion of tracheobroncheal tree. The cause of this disorder is unknown. In the past, a majority of the cases were discovered incidentally at autopsy. But recently, antemortem diagnosis is increasingly reported after the introduction of computed tomography and bronchoscopy. We report a case of extensive tracheopathia osteoplastica diagnosed antemortem by computed tomography, bronchoscopic examination and biopsy.

Distribution and Characterization of Heavy Metals in Human Kidney Cortex and Kidney Medulla (인체 신장피질과 신장수질에서 중금속류의 분포 및 특성)

  • Heavy metals, such as Al, As, Cd, Cr Cu, Fe, Hg, Mn, Mo, Ni, Pb, Se, Si, Sn, V and Zn, were analysed on kidney cortex and medulla of Korean obtained from 154 forensic medical autopsy cadavers. Heavy metals were analysed by inductively coupled plasma atomic emission spectrometry In kidney cortex, the concentrations of Al, Cd, Cu, Mn, Mo, Pb, Se, Si and Zn were significantly higher than in the kidney medulla (Cd, Cu, Mn, Mo, Zn : p<0.01, AA, Pb, Se, Si : p<0.05). No significant local differences were found between kidney cortex and kidney medulla in the concentrations of As, Cr Fe, Hg, Ni, Sn and V. In kidney cortex and kidney medulla, Cd concentrations correlated positively with age, but Mn concentrations correlated negatively with age. A significantly positive correlation between Cd and Zn, Cd and Cu, Zn and Cu, Al and Si, Se and As was found in kidney cortex and kidney medulla. A significantly positive correlation between Hg and Se was only observed in kidney cortex. These results indicate that the distribution of hazardous heavy metals is similar to that of essential elements in the tissues.
