  • 본 논문에서는 개발된 biphasic 자동형 제세동기 시스템의 임상적 효용성을 동물실험을 통하여 검증하였다. 개발된 제세동기 시스템은 기존의 monophasic 제세동기와는 달리 낮은 에너지로 세동을 제거하는 biphasic 형으로, 이미 실험실 환경의 테스트를 통해 시스템의 안정성과 알고리즘의 탁월한 검출 능력이 확인되었다. 시스템의 임상적 안전성 및 효용성을 확인하기 위하여 8마리의 돼지를 사용하여 시스템의 적절한 세동 검출 및 세동제거능력을 실험하였고, 그 후 시스템의 효용성을 향상시키기 위한 연구로 같은 양의 에너지를 다른 전압 레벨에서 방전시켜 이에 따른 제 세동 효율을 조사하고, 또한 세동 신호의 규칙성과 방전시점에 따른 제 세동 효과를 평가하였다.

  • Usability study of the CU Medical "i-PAD" AED was conducted to validate the usability of the device, primarily focused on the trained user as well as the utility of the training material. Comparing these results of the trained participants to those obtained by Callejas et al. (2004), who studied video-trained users of two commercially available Philips AEDs (FR2 and HS1) using a male manikin only, the participants in this study were more successful (100% vs. 85.7%-FR2 vs. 88.9%-HS1) and faster to shock (58.5s-i-PADvs. 91s-FR2 vs. 90s- HS1). While the focus of this study was on validating ease of use for trained bystanders, the results of the untrained condition showed that, from a usability perspective, these devices perform as well as any other AEDs already approved and available for purchase for untrained bystander use, as evidenced by the comparison to various published research results

  • 심실세동은 환자의 생명을 위협하는 가장 치명적인 부정맥으로서, 심실세동의 발견즉시 특별한 조치를 취하지 못할 경우 환자는 급사한다. 심실세동을 정지시키는 유일한 방법은 전기적 제세동이며 이는 환자의 심장 부위에 전기적 에너지를 가해 정상 심장 리듬을 되찾도록 하는 방법이다. 심실세동의 발생은 예측할 수 없고, 이를 진단하기 위한 심전도 교육을 모든 의료인에게 할 수 없으므로 심전도를 자동 분석하여 심실세동을 조기에 검출하는 자동 분석장치의 개발은 심실세동에 의한 급사를 줄이는데 매우 중요하다. 본 연구에서는 교차간섭성분의 제거 능력이 뛰어나며 쉽게 구사할 수 있는 Choi-Williams distriguteion (CWD) 방법을 이용하여 급사를 초래할 수 있는 심실세동을 자동 검출하는 알고리즘을 개발하였다. 그리고 개발된 알고리즘의 성능을 검증하기 위하여 본 논문에서는 두 그룹의 심실세동 데이터를 사용하였다. 검출 알고리즘은 심실세동의 시간 및 주파수 특성을 일곱가지 조건들에 대한 만족 여부를 이용하여 심실세동을 판정하였고, 실험의 민감도와 선택도는 각각 92.1%, 97 .2% 이다.

  • 본 논문에서는 심장 돌연사(sudden cardiac death, SCD)의 주된 원인인 세동(fibrillation)을 낮은 에너지에서 효율적으로 제거할 수 있는 biphasic 자동형 제세동기를 개발하였다. 개발한 제세동기는 고전압 충 방전부와 신호처리부를 포함하는 하드웨어와 세동검출 알고리즘과 시스템 제어 알고리즘의 소프트웨어로 구성하였다. 개발한 시스템의 안정성과 효용성을 검증하기 위하여, 실험실 환경에서 160 번의 연속적인 충 방전 테스트를 통하여 시스템의 안정성을 확인하였으며, ECG simulator 에서 발생되는 6 종의 세동신호를 적용하여 100%의 실시간 검출능력을 확인하였다.

Knowledge, Self-confidence, and Intention of BLS of Clinical Nurses Who Work at Small-medium Sized Hospitals (중소병원 간호사의 BLS 지식, 자신감 및 수행의도)

  • Purpose: This study was performed to investigate the level of knowledge, self-confidence, and intention of basic life support (BLS) and identify the reason why clinical nurses lack confidence to initiate BLS at the cardiac arrest scene. Method: A cross-sectional survey was used with questionnaires which authors developed; a BLS knowledge questionnaire and a tool for measuring the level of self-confidence and intention of performing BLS. The subjects consisted of 396 nurses by convenient sampling from ten small to medium sized hospitals in Korea. Data were collected from March to August, 2008. Data were analyzed by the SPSS/WIN 14.0 program with t-test, ANOVA, and Pearson correlation coefficients. Results: The percentile score of BLS knowledge was 40.6%, self-confidence 62.1%, and intention 73.0%. Clinical career rather than BLS knowledge has a significant effect on increasing the self-confidence and intention of performing BLS. A major reason for not performing BLS was lack of confidence in operating an AED (automatic external defibrillator). Conclusion: An effective and continuous training program especially including AED for CPR should be provided with clinical nurses to increase self-confidence and intention of performing BLS and to improve the survival of cardiac arrest patients in the hospital.

Factors Influencing Cardiopulmonary Resuscitation Performance on a Stranger (일 지역 주민의 심폐소생술 수행에 미치는 영향요인)

  • Purpose: Bystander cardiopulmonary resuscitation (CPR) improves survival. However, there have been few studies on the performance of bystander CPR in Korea. This study was done to identify the reasons for being unwilling to perform CPR, and to investigate the factors associated with performing CPR on a stranger in an emergency situation. Method: The participants in this study were 444 local residents. Data were collected by using self-reported questionnaires from May 1 to June 30, 2010. For data analysis, descriptive statistics and multiple logistic regression were performed using SAS version 9.1. Result: Only 32.2% of respondents reported being willing to perform CPR on a stranger. The major reason for not being willing to perform CPR on a stranger was "have no confidence to perform CPR well". From multiple logistic regression analysis, statistically significant predictors of CPR performance on a stranger were found to be a higher educational level (OR=6.11, 95% CI 1.46-25.52), neighbors with angina pectoris or myocardial infarction (OR=6.65, 95% CI 3.93-11.24), and having confidence of performing CPR (OR=3.02, 95% CI1.78-5.14). Conclusion: CPR education (including automatic external defibrillator) should be offered to family members and neighbors. In addition, the Good Samaritan Law must be given wider publicity at the national level.

A survey on installation and management of automated external defibrillators (자동심장충격기 설치현황 및 관리 실태 조사)

  • This study was conducted to investigate the status of the installation, maintenance and management of automatic external defibrillators (AEDs) in all mandatory institutions and buildings of the providence to be equipped with AEDs. The study was conducted from November 20, 2017 to December 20, 2017, during which time 169 AED units were surveyed. The collected data was analyzed by frequency analysis and the percentage was determined using the SPSS/WIN 20.0 program. A total of 168 units (99.41%) of AEDs were installed inside. Additionally, 47 units (27.80%) were available 24 hours a day, and 44 units (26.00%) were available outside business hours of the institution. Moreover, 87 units (51.43%) of the responsible managers received rescue and first aid training. Evaluation of the management status of the AEDs revealed two (1.20%) that had problems with their battery state of charge battery charge and seven (4.10%) for which the AED pads were past the expiration date. A management system for the manager of AEDs should be established and a regular training program for their systematic maintenance should be developed and applied. It is also suggested that awareness of the necessity of continuous management be raised through reinforcement of laws and legal system reorganization.
