$Smartpeg^{TM}$의 고압멸균소독 및 재사용이 임플랜트 안정성 지수(ISQ) 측정에 미치는 영향 (THE EFFECT OF AUTOCLAVE STERILIZATION AND REUSE OF $SMARTPEG^{TM}$ ON THE IMPLANT STABILITY QUOTIENT (ISQ) MEASUREMENT)

  • Statement of problem: Resonance frequency analysis is evaluated as the non-invasive and objective method for the evaluation of implant stability and has been increasingly used. It is necessary to evaluate the factors affect the ISQ measurement stability. Purpose: This study was performed to evaluate the effect of the autoclave sterilization and reuse of $Smartpeg^{TM}$ on ISQ measurement. Material and methods: $SmartPeg^{TM}$ (Integration Diagnostics Ltd., $G\ddot{o}teborg$, Sweden) of autoclave group (A) was autoclave sterilized 9 times and $Smartpeg^{TM}$ of reuse group (R) was reused 9 times. Ten $SmartPeg^{TM}s$ were allocated to each group and after each autoclave sterilization and reuse, implant stability quotient (ISQ) values were measured 3 times from the two directions a and b at a right angle. $Osstell^{TM}$ mentor (Integration Diagnostics Ltd. $G\ddot{o}teborg$, Sweden) was used and type 1 (article no. 100353) $Smartpeg^{TM}$ was selected according to $Smartpeg^{TM}$ reference list. Osstem Implant US II future (Osstem Co., Seoul, Korea) in $4.0mm{\times}11.5mm$ was embedded in the self-curing acrylic resin ($Orthojet^{(R)}$, Lang Dental, U.S.A.). Data was statistically analyzed by one-way ANOVA $({\alpha}=.05)$ and scheffe test was done where a significant difference exist. Correlation test was also done between ISQ value and the number of autoclave sterilization or reuse. Results: 1. In autoclave group, the means and sd. of ISQ value before autoclave sterilization were $84.97{\pm}0.41,\;84.93{\pm}0.74$ at direction a and b. There was significant differences between autoclave groups at direction a and b (P=.000). 2. In reuse group, the means and sd. of ISQ value before reuse were $85.40{\pm}0.62,\;85.50{\pm}0.57$ at direction a and b. There was no significant difference between reuse groups at direction a and b (P>.05). 3. There was a weak positive correlation between the number of reuse and ISQ value at direction a and b (${\gamma}=.207$ and .246, P<.01). Conclusion: Within the limitations of this study, the following conclusions were drawn. Till ninth reuse of $Smartpeg^{TM}$, the ISQ measurement stability did not be affected. After twice autoclave sterilization of $Smartpeg^{TM}$ the ISQ measurement stability was affected.

Influence of autoclave sterilization procedures on the cyclic fatigue resistance of heat-treated nickel-titanium instruments: a systematic review

  • Objectives: This systematic review evaluated the influence of autoclave sterilization procedures on the cyclic fatigue resistance of heat-treated nickel-titanium (NiTi) instruments. Materials and Methods: A systematic search without restrictions was conducted in the following electronic databases: PubMed, Scopus, Web of Science, ScienceDirect, Cochrane, and Open Grey. The hand search was also performed in the main endodontic journals. The eligible studies were submitted to the methodological assessment and data extraction. Results: From 203 abstracts, a total of 10 articles matched the eligible criteria. After reading the full articles, 2 were excluded because of the absence of the heat-treated instruments in the experimental design and 3 due to the lack of a control group using heat-treated instruments without autoclave sterilization. From the 5 included studies, 1 presented a low risk of bias, 3 presented moderate and 1 high risk. It was observed heterogeneous findings in the included studies, with autoclave sterilization cycles increasing, decreasing or not affecting the cyclic fatigue life of heat-treated NiTi instruments. However, the retrieved studies evaluating the cyclic fatigue resistance of endodontic instruments presented different protocols and assessing outcomes, this variability makes the findings less comparable within and also between groups and preclude the establishment of an unbiased scientific evidence base. Conclusions: Considering the little scientific evidence and considerable risk of bias, it is still possible to conclude that autoclave sterilization procedures appear to influence the cyclic fatigue resistance of heat-treated NiTi instruments.

소독멸균 방법에 따른 초음파 스케일링 팁에 부착된 세균의 양상 (Morphological aspect of the attached bacteria by the sterilization method of the ultrasonic scaling tip)

  • Objectives: The purpose of the study was to investigate the bacterial morphology attached on ultrasonic scaler tips using no cleansing solution, alcohol cotton, liquid chemical disinfecting agent, and autoclave method. Methods: Scaling tip was applied to the mouth and the ultrasonic scaler tips were assigned to four groups. Group 1 was control group with no cleansing solution. Group 2 was treated with alcohol cotton. Group 3 was treated with 2% green Y-Na solution in liquid chemical disinfecting agent, and Group 4 was sterilized by autoclave method. Live bacteria were observed by phase contrast microscopy. The scanning electron microscopy(SEM) revealed the morphological characteristics of scaler surface. The type of attached bacteria were analyzed using SPSS 21.0 program. The data were analyzed by one-way analysis of variance(ANOVA) and Tukey's post-hoc test. Results: The types of sterilization methods had influences on the bacterial viability. The numbers of cocci, bacilli, spiral form bacteria, and filamentous bacteria was observed in $89.00{\pm}3.60%$, $29.67{\pm}3.51%$, $3.33{\pm}0.57%$ and $1.67{\pm}0.57%$ in control group, $31.67{\pm}3.51%$, $63.33{\pm}4.04%$, $2.00{\pm}1.00%$ and $1.67{\pm}0.57%$ in alcohol cotton group, $69.67{\pm}4.50%$, $12.33{\pm}2.51%$, 0% and 0% in liquid chemical disinfecting agent group, and 0.0%, 0.0%, 0.0% and 0.0% in autoclave method group. The clean surface of ultrasonic scaler tip was shown on SEM by autoclave method. Conclusions: The most effective sterilization method of ultrasonic scaler tip was the autoclave method. Autoclave method is the most effective sterilization method and can reduce the cross-infection in the dental clinic.

Antibacterial property of 4-hexylresorcinol incorporated in silk mat after sterilization

  • When 4-hexylresorcinol (4HR) incorporated silk mat is used for clinical application, sterilization process is essential. In this study, 4HR incorporated silk mat was sterilized with ethylene oxide (EO) gas or autoclave, and then antibacterial property was evaluated. For the examination of micro-structure after the sterilization, scanning electron microscopic (SEM) exam and Fourier Transform Infrared (FT-IR) spectroscopy were also done. There was no gross difference after sterilization in SEM exam and FT-IR spectra. Antibacterial property of 4HR was also maintained after the sterilization. In conclusion, EO gas or autoclave did not damage the antibacterial property of 4HR in the silk mat.

The effects of autoclave sterilization on the cyclic fatigue resistance of ProTaper Universal, ProTaper Next, and ProTaper Gold nickel-titanium instruments

  • Objectives: It was aimed to compare the cyclic fatigue resistances of ProTaper Universal (PTU), ProTaper Next (PTN), and ProTaper Gold (PTG) and the effects of sterilization by autoclave on the cyclic fatigue life of nickel-titanium (NiTi) instruments. Materials and Methods: Eighty PTU, 80 PTN, and 80 PTG were included to the present study. Files were tested in a simulated canal. Each brand of the NiTi files were divided into 4 subgroups: group 1, as received condition; group 2, pre-sterilized instruments exposed to 10 times sterilization by autoclave; group 3, instruments tested were sterilized after being exposed to 25%, 50%, and 75% of the mean cycles to failure, then cycled fatigue test was performed; group 4, instruments exposed to the same experiment with group 3 without sterilization. The number of cycles to failure (NCF) was calculated. The data was statistically analyzed by using one-way analysis of variance and post hoc Tukey tests. Results: PTG showed significantly higher NCF than PTU and PTN in group 1 (p < 0.05). Sterilization significantly increased the NCF of PTN and PTG (p < 0.05) in group 2. PTN in group 3 had significantly higher cyclic fatigue resistance than PTN group 4 (p < 0.05). Also, significantly higher NCF was observed for PTG in group 2 than in groups 3 and 4 (p < 0.05). Conclusions: PTG instrument made of new gold alloy was more resistant to fatigue failure than PTN and PTU. Autoclaving increased the cyclic fatigue resistances of PTN and PTG.

열멸균과정이 nickel titanium호선의 기계적 성질과 표면상태에 미치는 영향 (The Effect of Heat Sterilization on the Surface Topography and the Tensile Properties in Various Nickel Titanium Wires Including a Korean Product)

  • 본 연구의 목적은 수종의 nickel titanium 호선을 열멸균 처리 하였을 때 나타나는 물리적성질과 표면상태의 변화를 정량적으로 평가하고자 하는 것이다. 연구에 사용된 재료는 국산제품을 포함한 4종의 .$016\times.22$ 각형 nickel-titanium호선이며, 이들을 건열소독($180^{\circ}C$, 60분)군과 가압증기멸균($121^{\circ}C$, 15-20 psi, 30분)군, 그리고 아무런 처리를 하지 않은 대조군으로 구분하였다. 인정특성은 Inston (Model 4466, load cell capacity:1000kg)을 이용하여 표점거리 40mm, cross head는 분당 5mm의 속도, 실온의 조건에서 평가하였으며 쳐대인장강도와 연신율, 그리고 탄성률을 측정 및 계산하였다. 표면상태는 주사전자현미경과 profilometer로써 평가하였고, 1-test를 이용하여 통계적 분석을 하였다. 결과는 다음과 같다. 1. 실험에 사용된 nickel-titanium호선들은 모두 두가지 열멸균 처리에 의해 인장특성에 있어서 유의할 만한 변화를 보이지 않았다/ 2. 대조군에서 NiTi가 가장 높은 펴ㅛ면거칠기를 보였고, Align과 Sentalloy는 동일한 정도의 값을, Optimalloy는 가장 낮은 값을 지니고 있었다. 3. Align과 Optimalloy는 두가지 열멸균처리 후에 모두 유의할 만한 표면상태의 변화를 보이지 않았다. 그러나 NiTi는 건열소독후에 표면거칠기의 증가를 보였으며 Sentalloy는 두가지 열멸균후에 표면상태의 변화와 표면거칠기의 유위한 증가를 보였다.

Mycelial growth of Lentinula edodes in response to different mixing time, pressure intensity, and substrate porosity

  • Biological efficiency (BE), the ratio of fresh mushrooms harvested per dry substrate weight, expressed as the percentage of Lentinula edodes, also known as shiitake, was determined using the 'Sanjo 701' strain stored in the Department of Mushroom at the Korea National College of Agriculture and Fisheries. The mycelia were grown in glass columns with varying levels of moisture content and varying mixing periods of 0.5, 1, 2, and 3 hours. The substrate was sterilized using a steam pressure autoclave sterilizer at normal and high pressure to avoid contamination. The results showed that mycelial growth (126 mm/15 days) was optimized at 55% moisture content. The best mycelial growth of 117 mm/15 days was obtained with 2 hours of mixing time. Normal pressure sterilization yielded better results with mycelial growth of 96 mm/15 days at $100^{\circ}C$ compared to 88 mm /15 days with sterilization at $121^{\circ}C$. Mycelial density was higher, i.e. 3(+++), with normal pressure sterilization compared to 2(++) with high pressure sterilization. Furthermore, sawdust mixed with 5% woodchips increased the substrate porosity and yielded higher mycelial growth. Thus, we demonstrated that the optimum harvest or potential increased yield of shiitake can be obtained by modulating moisture content, mixing time, and substrate porosity.

인삼분말에 대한 살균방법 비교 (The Effective Sterilization of White Ginseng Powder)

  • This study was conducted to investigate to select the optimum condition for the sterilization of white ginseng powder. The powder heated at 100-121 $^{\circ}C$ for 1-30 minutes in a autoclave and irradiated at 2450 MHz microwave for 0.5-3.0 minutes was significantly changed in it's color density and become coagulated state, but their sterilizing effect on a general bacteria and 5. coli group were negligible. Meanwhile W- irradiation to the powder with thickness 1.5-10.5mm for 120 minutes was no changes in it's color. coagulated state and sterilizing effect, but 60Co-${\gamma}$ray irradiation at various dose with 0.1-2.0 Mrad was significant effect for sterilization and stability of the powder during the storage for 4 months at 25 $^{\circ}C$. The optimum dose of 60Co-${\gamma}$ ray irradiation was 0.5-0.7 Mrad.

Safety of Temporary Use of Recycled Autoclaved Femoral Components in Infected Total Knee Arthroplasty: Confirming Sterility Using a Sonication Method

  • Background: The purpose of this study was to evaluate the usefulness of sonication technique for microbiological diagnosis and the sterility of the recycled autoclaved femoral components from infected total knee arthroplasty (TKA) using a sonication method. Methods: Nineteen femoral implants explanted from patients with infected TKA were sterilized with a standard autoclave method. Standard culture of the fluid before and after sonication of the sterilized implants was performed to detect pathogenic microorganisms. Additional experiments were performed to evaluate the sterility of the recycled implant by inducing artificial biofilm formation. Methicillin-resistant Staphylococcus aureus (MRSA) was inoculated into 10 implants and sterilization in a standard autoclave was performed, and then the fluid was cultured before and after sonication. Results: Two of the 19 sterilized implants were positive for growth of bacteria after sonication, whereas no growth was detected in the cultured fluid from the sterilized implants before sonication. The bacteria were Staphylococcus species in all two cases. In one of 10 implants inoculated with MRSA, the culture was positive for growth of bacteria both before and after sonication. However, Staphylococcus epidermidis was cultured from both occasions and thus this implant was thought to be contaminated. Conclusions: We found sonication for identification of pathogens could be helpful, but this finding should be interpreted carefully because of the possibility of contamination. Sterilization of an infected femoral implant with an autoclave method could be a good method for using the temporary articulating antibiotic spacer in two-stage revision arthroplasty.
