삼생음(三生飮)이 실험동물(實驗動物)의 심혈관계(心血管系) 및 혈류장애개선(血流障碍改善)에 미치는 효과(效果) (Effects of Samsaengyeum on the Cardiovascular System in the Experimental Animal)

  • The present experiments were designed to investigate the effects of Samsaengyeum. water extracts on the Cardiovascular System in the Experimental Animals. Thus, the changes of blood pressure and heart rate were measured after oral administration. Measurement of Mortality rate was observed for measuring the effect of Samsaengyeum water extract Samsaengyeum water extract against pulmonary thromboembolism induced by collagen the mixture(0.1me/10g, 2mg/kg B.W) plus serotonin(5mg/kg B.W) in mouse. The effect of Samsaengyeiim water extract was examined by observing the change of collagen-induced platelet aggregation, coagulation activity, ex vivo and in vitro fibrinolytic activity of euglobulin fraction in rats. The results were summarized as followings. 1. Samsaengyeum dropped the blood pressure in spontaneous hypertensive rat. 2. The drug increased the auricular blood flow in rabbit. 3. The drug relaxed the artery contraction by pretreated norepinephrine in rat. 4. The drug inhibited the death rate of mouse which was led to thromboembolism by serotonin and collagen. 5. The drug inhibited the platelet aggregation in rat. 6. The drug prolonged the prothrombin time and activated partial thromboplastin time on the test of plasma coagulation factor activity in rat, but was not valuable. 7. The drug reduced the fibrinogen lyses time and increased the lyses area of rat. 8. Samsaengyeum reduced fibrinogen lyses time of rat in vitro assay. According to the above mentioned results, Samsaengyeum increased the blood flow and dropped the blood pressure by the dilation of blood vessel. And the drug inhibited the platelet aggregation.

악관절에 발생한 활액성 연골종증(Synovial Chondromatosis); 증례보고 (SYNOVIAL CHONDROMATOSIS OF THE TEMPOROMANDIBULAR JOINT: A CASE REPORT)

  • Synovial chondromatosis is an uncommon disease of cartilage transformation of synovial membrane with formation of loose bodies within the joint space. The knee and elbow are the most commonly involved sites and involvement of temporomandibular joint is very rare. Symtoms include swelling, pain, stiffness of the jaw, and inability to close the jaw. A case involving the temporomandibular joint(TMJ) is presented. A 28-year-old women had experienced pain of the left TMJ area and limitation of mouth opening. Radiographs of the left TMJ revealed calcified mass surrounding over the mandibular condyle and under the glenoid fossa. Treatment consisted of removal of calcified mass, reshaping of affected condyle and reconstruction with a auricular cartilage. After surgery, the patient's range of motion improved although deviation to the affected side. Until present after surgery there have been no recurrence of symtoms. We think that findings of this patitent agree with those of synovial synchondrmatosis in many aspects.

  • 재발성 다발성 연골염은 전신 연골에 염증반응을 특징으로 하는 드문 전신 질환으로, 대개 외이나 비부의 연골 침범이 가장 흔한 증상이나, 이러한 특징적 증상 없이 기도에 연골염의 형태로 발현하는 경우도 드물게 보고되어 있다. 저자들은 최근 64세 남자에서 흔히 발생하는 연골의 침범에 의한 전형적인 증상없이 미만성 기도 연골 침범으로 발현한 재발성 다발성 연골염 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

와우 수용기 모세포를 파괴한 가묘의 "전기와우" 삽입에 의한 "청각감" (Auditory Sensation by the Inserted "Electronic Cochlea" in the Cases of the Experim entally Destroyed Receptor Organ of Corti of the Cat)

  • 와우 수용기의 모세포를 파괴한 가묘에서 "전기 와우"의 내부 장치를 중이 골포내에 이입한 후에 그 전극을 고실계내에 삽입하고, "전기 와우"의 외부 장치에 폭로한 음향 자극에 대한 반응을 Electric Kymograph(Model 404 Harvard Apparastu Co.)에 묘기하여 다음과 같은 결과를 얻었다. 1) 음향 자극에 의하여 야기되는 가묘의 이개운동의 크기는 특정한 축위 안에서 자극의 강도와 평행하였다. 2) 반복 음향 자극에 의한 이개반사운동은 최초의 자극에 대해서는 약간 증대한 후, 점차 감소하는 경향이 있었다. 3)음향 자극에 의하여 야기되는 이개운동은, 절대불응기, 비교적 불응기 및 반응기 등이 관찰되었다. 4) 음향 자극에 의하여 야기되는 실험 가묘의 이개반사는 청수용기 모세포를 파괴한 후 "전기 와우"의 삽입에 의하여 "청각감"이 야기되고 있음을 시사한다.

동씨침법(董氏針法)과 십이정경(十二正經)의 상관성(相關性)에 관(觀)한 연구(硏究) (Bibliographic Study on the Interrelation between Dong Shi' Acupuncture and Twelve Regular Channels)

  • In the clinics of traditional medicine, various acupuncture methods, that is Sa-Am' acupuncture, scalp acupuncture, auricular acupuncture and Dong-Shi' acupuncture, have been introduced and increased gradually to treat the diseases. In this bibliographic study, we examined the interrelation between the Dong Shi' acupuncture and 12 regular channels. The theoretical origination of Dong Shi' acupuncture and 12 regular channels was applied from Yin and Yang, Five Element, State of Viscera and so on. As the treatment using by 12 regular channels have used to 'regulating Qi and Treating Shin (調氣治神)', Dong Shi' acupuncture have used to the itself defense mechanism and relative balance of human beings. Also, these methods have the same rules that was selected to the opposing needling (巨刺) of remote point selection, however, Ah-Shi' points (阿是穴) were not used for the selection. Of Dong-Shi' acu-points, there were many ones which have the same location as the acupoints of regular channels. However, these one was renamed according to the new characteristics and therapeutics found by Dong Shi. Dong Shi' acupuncture has divided human body to the 12 areas but not considered to the concept of a mutual connection of human being body. The needling manipulation methods were vary at the regular channels, but Dong Shi' acupuncture did not use the traditional manipulation method except for supplementary Dong-Qi acupuncture, To-Ma acupuncture and To-In acupuncture. From these study, we found that the Dong-Shi' acupuncture have something in common with 12 regular channels, although the Dong-Shi' acupuncture was not based on the theory of 12 regular channels.

Acupuncture Therapy and Herbal Medicine Accelerating Temporal Space Abscess after Tooth Extraction: A Case Report

  • Numerous oral and maxillofacial surgeons have found facial space infections after tooth extraction. Most of these infections can be managed easily, but some of them could be life-threatening. Among the facial infections, temporal space infections are rare. Most temporal space infections could be observed as secondary to maxillary third molar infections, maxillary sinusitis, and maxillary sinus fractures. Note, however, that there are insufficient studies on temporal space abscess due to mandibular second molars, especially with acupuncture. A 74-year-old female came to our hospital with severe trismus and facial swelling on the right temporal, buccal, posterior auricular, and cervical regions. The patient had undergone extraction of tooth #47 secondary to dental caries by a general dentist about a month ago. After the dental procedure, the patient had been treated with acupuncture therapy around the right temporomandibular joint area at the oriental medicine clinic. We performed emergency incision and drainage under general anesthesia and started antibiotic treatment with IV ampicillin/sulbactam 3 g every 24 hours and vancomycin 1 g every 24 hours for 5 days. The patient's symptoms subsided and ultimately disappeared. Temporal space abscess after mandibular molar extraction is quite rare. In this case, the spreading mechanism against gravity is considered to be acupuncture therapy.

구강악안면 영역의 말초신경 재생을 위한 비복신경의 외과적 해부학 (Surgical Anatomy of Sural Nerve for the Peripheral Nerve Regeneration in the Oral and Maxillofacial Field)

  • Peripheral nerve injuries in the oral and maxillofacial regions require nerve repairs for the recovery of sensory and/or motor functions. Primary indications for the peripheral nerve grafts are injuries or continuity defects due to trauma, pathologic conditions, ablation surgery, or other diseases, that cannot regain normal functions without surgical interventions, including microneurosurgery. For the autogenous nerve graft, sural nerve and greater auricular nerve are the most common donor nerves in the oral and maxillofacial regions. The sural nerve has been widely used for this purpose, due to the ease of harvest, available nerve graft up to 30 to 40 cm in length, high fascicular density, a width of 1.5 to 3.0 mm, which is similar to that of the trigeminal nerve, and minimal branching and donor sity morbidity. Many different surgical techniques have been designed for the sural nerve harvesting, such as a single longitudinal incision, multiple stair-step incisions, use of nerve extractor or tendon stripper, and endoscopic approach. For a better understanding of the sural nerve graft and in avoiding of uneventful complications during these procedures as an oral and maxillofacial surgeon, the related surgical anatomies with their harvesting tips are summarized in this review article.

The arterial blood supply of the temporomandibular joint: an anatomical study and clinical implications

  • Purpose: The aim of this study was to analyze three-dimensional images of the arterial supply to the temporo-mandibular joint. Materials and Methods: Ten patients (five men and five women, mean age 36 years) without signs or symptoms of temporomandibular disorders, who underwent contrast-enhanced computed tomographic (CT) scanning with intravenous contrast, were studied. The direct volume rendering technique of CT images was used, and a data set of images to visualize the vasculature of the human temporomandibular joint in three dimensions was created. After elaboration of the data through post-processing, the arterial supply of the temporomandibular joint was studied. Results: The analysis revealed the superficial temporal artery, the anterior tympanic artery, the deep temporal artery, the auricular posterior artery, the transverse facial artery, the middle meningeal artery, and the maxillary artery with their branches as the main arterial sources for the lateral and medial temporomandibular joint. Conclusion: The direct volume rendering technique was found to be successful in the assessment of the arterial supply to the temporomandibular joint. The superficial temporal artery and maxillary artery ran along the lateral and medial sides of the condylar neck, suggesting that these arteries are at increased risk during soft-tissue procedures such as an elective arthroplasty of the temporomandibular joint.

군진의학에서의 침치료 활용을 위한 연구동향 고찰 (A Study on Research for Utilization Acupuncture Treatment in Military Medicine)

  • Objectives Military medicine is the study of research which includes health, sanitation, clinic, and quarantine in military personnel. This study is focused on research the acupuncture treatment of military medicine. Methods The study on acupuncture research for Military Medicine was referred from via KoreaMed, OASIS, The Journal of Korean Military Medical Association, KMbase, KAMJE, NDSL, KISS, KISTI, PubMed, MEDLINE, EMBASE, and The Cochran library. As a result, 18 research papers were found and they were analyzed according to the year of publishment, the titles of journals, the nation of research center, the types of study, the purpose of study, the outcome of measurement, the type of acupuncture, and the assessment of acupuncture effect. And then assessed the risk of bias about RCT, NRCT, prospective cohort studies, and before and after studies. Results The number of the published research papers on acupuncture was increased since 2013. The studies were mainly published in the USA and eight out of 18 were RCT. The purpose of study is mostly for musculoskeletal and psychological disease and medical decision for acupuncture. The type of the acupuncture used is not only traditional acupuncture but also the auricular acupuncture. Even though the research results about the effect of acupuncture were excellent and positive, some of studies had a high risk of bias. Conclusions The studies state about acupuncture treatment as mostly positive and effective, however, further clinical studies are required to obtain objective clinical evidence of acupuncture treatment. Furthermore, future trials would be needed to find out effective acupuncture treatments on military duty.

Histological, Physical Studies after Xenograft of Porcine Ear Cartilage

  • Background: Because of the relatively similar size of organs to human and the physiological and structural similarities, the use of porcine as xenograft donors is progressing very actively. In this study, we analyzed the characteristics of porcine ear cartilage and evaluated its suitability as graft material in reconstructive and cosmetic surgery. Methods: The auricular cartilage was harvested from two pigs, and subjected to histological examination by immunohistochemical staining. To determine the collagen content, samples were treated with collagenase and weight changes were measured. After sterilization by irradiation, the samples were grafted into rats and stained with Hematoxylin and Eosin and Masson Trichrome to observe inflammation and xenograft rejection. Results: In IHC staining, extracellular matrices were mainly stained with type II collagen (20.69%), keratin sulfate (10.20%), chondroitin sulfate (2.62%), and hyaluronic acid (0.84%). After collagenase treatment, the weight decreased by 68.3%, indicating that about 70% of the porcine ear cartilage was composed of collagen. Upon xenograft of the sterilized cartilages in rats, inflammatory cells were observed for up to 2 months. However, they gradually decreased, and inflammation and reject-response were rarely observed at 5 months. Conclusion: The porcine ear cartilage was covered with perichondrium and cellular constituents were found to be composed of chondrocytes and chondroblasts. In addition, the extracellular matrices were mainly composed of collagen. Upon xenograft of irradiated cartilage into rats, there was no specific inflammatory reaction around the transplanted cartilage. These findings suggest that porcine ear cartilage could be a useful alternative implant material for human cosmetic surgery.
