Disrupted Association Between Empathy and Brain Structure in Attention-Deficit/Hyperactivity Disorder

  • Objectives: To investigate the relationship between brain structure and empathy in early adolescents with attention-deficit/hyperactivity disorder (ADHD). Methods: Nineteen early adolescents with ADHD and 20 healthy controls underwent 3T MRI. All the participants were assessed for different aspects of empathy using measures including the Interpersonal Reactivity Index and Empathy Quotient. Cortical thickness and subcortical structural volume based on T1-weighted scans were analyzed using FreeSurfer. Results: Cognitive empathy (t=-2.52, p=0.016) and perspective taking (t=-2.10, p=0.043) were impaired in the ADHD group compared with the control group. The cluster encompassing the left posterior insular, supramarginal, and transverse temporal cortices [cluster-wise p-value (CWP)=0.001], which are associated with emotional empathy, was significantly smaller in the ADHD group, and the volume of the left nucleus accumbens was greater than that of the control group (F=10.12, p=0.003, effect size=0.22). In the control group, the left superior temporal (CWP=0.002) and lingual cortical (CWP=0.035) thicknesses were positively associated with cognitive empathy, while the right amygdala volume was positively associated with empathic concern (Coef=14.26, t=3.92, p=0.001). However, there was no significant correlation between empathy and brain structure in the ADHD group. Conclusion: The ADHD group had a smaller volume of the cortical area associated with emotional empathy than the control group, and there was no brain region showing significant correlation with empathy, unlike in the control group.

Association between polycystic ovary syndrome and risk of attention-deficit/hyperactivity disorder in offspring: a meta-analysis

  • Background: There is evidence of a relationship between prenatal excess androgen exposure and central nervous developmental problems and attention-deficit/hyperactivity disorder (ADHD) in the offspring of mothers with polycystic ovary syndrome (PCOS). Purpose: Here we aimed to use a meta-analysis to investigate whether the offspring of mothers with PCOS are at an increased chance of developing ADHD. Methods: Three main English databases were searched for articles published through December 2020. The Newcastle-Ottawa Scale was used to assess study quality. Study heterogeneity was determined using I2 statistics and publication bias was assessed using Begg and Egger tests. The results are presented as odds ratio (OR) and relative ratio (RR) estimates with 95% confidence intervals (CIs) using a random-effects model. Results: Six articles (3 cohort and 3 case-control studies; 401,413 total ADHD cases) met the study criteria. Maternal PCOS was associated with an increased risk of ADHD in the offspring based on OR and RR (OR, 1.42; 95% CI, 1.27-1.57) and (RR, 1.43; 95% CI, 1.35-1.51), respectively. There was no heterogeneity among the included articles based on OR (I2=0.0%, P=0.588) and RR (I2=0.0%, P=0.878). Conclusion: Our study showed that maternal PCOS is a risk factor for ADHD. Therefore, screening their offspring for ADHD should be considered part of the comprehensive clinical care of women with PCOS.

  • Objectives : This study reports on the cases of four ADHD patients who treated with herbal medication based on Disease Pattern Identification Diagnostic System (DPIDS). Methods : Two children and two adults were diagnosed with ADHD according to DSM-V, and Galgeunhwangryeonhwanggeum-tang was administered. The parents of the two children completed the Korean ADHD Rating Scale (K-ARS) before and after treatment, while the other patients used the Korean ADHD Self Rating Scale (K-ASRS). The patients' somatic symptoms and other issues were evaluated through interviews. Results : In the first case, the K-ARS scale exhibited a change from 28 to 3. The patient demonstrated improvements in attention, concentration, and social behavior, along with the elimination of self-injurious behavior. In the second case, the K-ARS scale showed a change from 41 to 9. The patient in the third case demonstrated improved chronic fatigue, increased attention, work efficiency, and enhanced social skills, leading to maintaining employment for over six months for the first time in their life after the treatments. Additionally, the patient in the fourth case demonstrated improved work concentration and relief in nocturia. Conclusions : In this study, four patients exhibited improvements in symptoms of attention deficit, hyperactivity, and impulsivity before and after herbal treatment, leading to enhanced academic and social relationships. As the patients progressed in their health restoration, symptoms related to the genitourinary system also showed improvement.

상호작용식 메트로놈(Interactive Metronome) 훈련이 주의력결핍 과잉행동장애 아동의 자세조절과 글씨쓰기 수행에 미치는 영향: 단일사례연구 (Effect of Interactive Metronome Training on Postural Control and Hand Writing Performance of Children With Attention Deficit Hyperactivity Disorder (ADHD): Single Subject Research)

  • 목적 : 본 연구는 Attention Deficit Hyperactivity Disorder(ADHD) 아동에게 시행한 상호작용식 메트로놈(Interactive Metronome; IM) 중재가 자세조절과 글씨쓰기에 미치는 영향에 대해 알아보고자 하였다. 연구방법 : 본 연구는 ADHD로 진단받은 초등학교 3학년 아동 1명을 대상으로 시행하였다. 개별실험 연구방법 중에서 ABA 설계를 사용하였고, 총 30회기로 매주 3회기 씩 총 10주 진행하였다. 기초선과 재기초선 기간에는 글씨쓰기 과제의 측정만 실시하였고, 중재기 동안에 IM 훈련을 40~50분간 실시하고 난 후에 글씨쓰기의 명료도와 속도를 평가하였다. 기초선을 시작하기 전과 재기초선이 끝나고 한 달 이내에 Clinical Observation of Motor and Postural Skills(COMPS)를 실시하여 자세조절의 변화를 알아보았다. 결과 : IM 중재를 시행한 후 대상자의 자세조절 변화는 하위항목 중 슬로모션, 손가락-코 운동, 비대칭 경반사의 점수가 향상되었고, 글씨쓰기 명료도와 속도는 중재 기간 동안 상승하는 경향을 보였으나 유의하게 변화되지 않았다. 결론 : 본 연구를 통해 ADHD 아동을 대상으로 한 IM 훈련의 중재가 자세조절과 글씨쓰기 수행능력을 향상시키는데 긍정적인 효과를 확인할 수 있었고, 향후 연구에 새로운 방향에 접근할 수 있는 기회를 제공하였다는 근거로 사용될 수 있을 것이다.

주의력결핍 과잉행동장애 아동의 타이밍에 따른 운동기능과 실행기능의 차이 (Differences in Motor Functions and Executive Functions according to the Timing of Children With Attention Deficit Hyperactivity Disorder)

  • 목적 : 본 연구는 주의력결핍 과잉행동장애 아동(Attention Deficit Hyperactivity Disorder; ADHD)의 타이밍에 따른 운동기능과 실행기능의 차이를 알아보고자 하였다. 연구방법 : 연구 대상은 부산지역에 거주하며, ADHD 진단을 받은 만 6~12세 아동 32명을 대상으로 하였다. 연구도구로는 타이밍 기능 평가를 위해 상호작용식 메트로놈(Interactive Metronome; IM), 운동기능 평가를 위해 Bruininks-Oseretsky Test of Motor Proficiency, Second Edition(BOT-2)를 사용하였고, 실행기능 평가를 위해 스트룹 아동 색상 단어 검사와 아동 색 선로 검사를 사용하였다. 타이밍에 따른 운동기능과 실행기능의 차이를 알아보기 위해 Mann-Whitney U 검정을 실시하였다. 결과 : ADHD 아동의 타이밍에 따른 운동기능의 차이를 알아본 결과 BOT-2의 영역 중 미세한 손의 조절 영역에서 유의한 차이가 있었다(p<.05). 각 영역의 하위 항목에서는 미세운동 정확성에서 유의한 차이가 있었다(p<.05). ADHD 아동의 타이밍에 따른 실행기능의 차이를 알아본 결과 스트룹 아동 색상 단어 검사와 아동 색 선로 검사에서 유의한 차이를 보이진 않았다. 결론 : ADHD 아동의 결함들 중에서 타이밍 기능에 따른 운동과 실행기능의 차이를 알아 볼 수 있었다. 본 연구는 추후에 진행될 ADHD 아동의 타이밍에 관한 연구를 하는데, 기초자료로 사용될 수 있다는 점에서 의미가 있다.

ADHD 아동과 일반 아동의 감각처리능력과 시지각 기술의 비교 연구 (A Comparative Study of Sensory Processing Abilities and Visual Perception Skills in Children With Attention Deficit Hyperactivity Disorder and Typical Children)

  • 목적 : 본 연구는 주의력결핍 과잉행동장애(Attention Deficit Hyperactivity Disorder; ADHD) 아동과 일반 아동을 대상으로 감각처리능력과 시지각 기술을 비교하고자 하였다. 연구방법 : 연구대상은 지역적 환경이 동일한 아동으로 선정하였다. 소아정신과에서 ADHD로 진단받은 아동 25명과 초등학교 1, 2학년의 일반 아동 51명을 대상으로 감각처리능력과 시지각 기술을 평가하였다. 아동의 감각처리능력을 측정하기 위해 감각프로파일(Sensory Profile; SP)을 사용하였고 시지각 기술을 측정하기 위해 한국판 시지각 발달검사(Korean Developmental Test of Visual Perception-2; K-DTVP-2)를 사용하였다. 두 군 간의 감각처리능력과 시지각 기술의 차이는 독립표본 t 검정을 사용하여 분석하였다. 결과 : ADHD 아동과 일반 아동의 감각프로파일(SP)의 모든 하위 영역에서 통계적으로 유의한 차이를 보였다. 감각요인을 비교한 결과 감각예민을 제외한 8가지 요인에서 통계적으로 유의한 차이를 보였다. K-DTVP-2의 모든 시지각 지수에서 유의한 차이를 보였다. K-DTVP-2의 하위 항목 표준점수를 비교한 결과 공간위치(p=.031), 도형-배경(p=.001), 시각 통합(p=.005), 눈-손 협응(p=.000), 따라 그리기(p=.044), 공간관계(p=.006)에서 통계적으로 유의한 차이를 보였다. 결론 : 본 연구는 임상에서 ADHD 아동의 행동의 특성을 이해하는데 감각처리능력과 시지각 기술이 중요하다는 근거가 되며 ADHD 아동의 평가, 교육 및 연구에 도움을 제공할 수 있다는 점에서 의미가 있다.

  • 인터넷이 우리 생활에 많은 비중을 차지함에 따라 인터넷 중독과 관련하여 많은 정신질환들이 문제시되고 있다. 본 연구에서는 지속적인 주의력 산만, 과다활동, 충동성을 특징으로 하는 주의력결핍과잉행동장애와 인터넷 중독과의 관련성 여부를 알아보고 인터넷 중독 하위의 어떤 요인이 주의력결핍과잉행동장애에 영향을 미치는 지에 대하여 밝혀보고자 하였다. 특히 과잉행동장애가 감소하는 고등학생을 표본집단으로 신뢰도분석, 빈도분석, 교차분석, ANOVA분석, Pearson 상관분석을 통해 인터넷 중독과 주의력결핍 과잉행동장애의 관련성 연구에 필요한 적절한 결과를 도출했다.

주의력결핍 과잉운동장애에서 도파민 수송체 유전자 다형성 (Dopamine Transporter Gene Polymorphism in ADHD)

  • Patients with attention deficit hyperactivity disorder(ADHD) have symptoms of inattention, hyperactivity, impulsivity. Symptoms of ADHD are responsive to medications such as methylphenidate, dextroamphetamine, pemoline, and bupropion. The functional change of the dopamine transporter is related to the therapeutic effect of these drugs. This can be one reason for the dopamine transporter to be emphasized in the research field of ADHD. ADHD has a genetic tendency. Since dopamine transporter gene(DAT1) knock out mice were reported to be several times more active than normal mice in a novel situation, lights has been shed on DAT1 as a candidate gene for ADHD. Though there have been several studies which reported an association between DAT1 and ADHD, the association between DAT1 and ADHD is not conclusive. Since Vandenbergh reported the DAT1 polymorphism with variable number of tandem repeats(VNTR), and the racial differences in allelic frequencies of the DAT1, wide ethnic variation in the distribution of the DAT1 polymorphism had been confirmed. Wide ethnic variation in the distribution of the DAT1 suggested that there might be ethnic difference in the association between DAT1 and ADHD. Before applying previous findings to Koreans, verification might be needed for Korean patients with ADHD.

국민학교 아동의 설탕섭취량이 주의결핍 과잉활동장애와 미친 영향 (The Effect of Sugar Intake on Attention Deficit Hyperactivity Disorder of School Children)

  • The purpose of this study was to examine the relationship between sugar intake and behavioral problems in Korean school children. The subjects were 280 school children lived in Soung Nam city. Nutrient and sugar intakes were measured by 24-hour recall method and food frequency method through personal interview. Hyperactivity scores of children were measured by teacher and mother using conners Rating Scale-Revised. The average daily total sugar intake for the total sample was 63.0$\pm$37.4g(Range : 10g-220g). The result of stepwise multiple regression analysis using the sugar intake as dependent variable showed that energy, participation of school lunch program, Vit B2, protein, education of fathers, niacin, calcium are significant explanatory variables, explaining 44.8% of variation. As a result of the Conners scale test by teacher, 4% of children were Attention Deficit Hyperactivity Disorder(ADHD) children. A result of the Conners scale test by mother, 8% of children were ADHD children. The result of simple regression analysis had not shown significant relationship between sugar intake and ADHD scores. But the result of the Conners test by teacher, sugar intake(69.9$\pm$37.1g) of ADHD children were significantly higher than sugar intake(62.9$\pm$37.7g) of normal children(p<0.01).

주의력결핍 과잉행동장애 아동과 형제의 임상특징 및 신경심리학 소견 (Clinical Characteristics and Neuropsychological Profiles of the Children with ADHD and Their Siblings)

  • Objectives : This study aims to investigate the clinical characteristics and neuropsychological profiles of children with attention-deficit hyperactivity disorder (ADHD) and their siblings. Methods : Eighteen children (age $8.2{\pm}1.7$ years, 12 boys) with ADHD and their 18 siblings (age $7.8{\pm}1.6$ years, 8 boys) completed Continuous Performance (CPT), Stroop, Children's Trail Making, Rey-Kim Memory, and Kim's Frontal Executive Function tasks. The parents of these subjects underwent the Attention-Deficit/Hyperactivity Disorder Rating Scale (ARS), 10-item Parent General Behavior Inventory (P-GBI), and the Social Responsiveness Scale (SRS). Paired t-tests were used. Results : The inattention (p=.020), and hyperactivity-impulsivity (p=.001), scores of the ARS and the P-GBI score (p=.004) were significantly higher in children with ADHD than in their siblings. Deficits in social communication and motivation on SRS were higher in children with ADHD than in their siblings (p=.017 and p=.011, respectively). Z-scores of omission and commission errors as well as response time variability on visual CPT and omission errors on auditory CPT were in clinically significant range, and z-score of omission errors on auditory CPT was in borderline range in siblings. Omission (p=.018) and commission errors on Visual CPT (p=.007) were significantly higher in children with ADHD compared to their siblings. Recognition efficiency on Kim's Frontal Executive Function Task was lower in children with ADHD compared to their siblings, but in normal range in both groups. Stroop interference and figure fluency on Kims Frontal Executive Function Task were in borderline range in ADHD group, and figure fluency was in borderline range in siblings. Conclusion : Our results support a preliminary evidence for mild degree of attention deficit in ADHD siblings. Further studies are needed to examine the cognitive functions of siblings with ADHD in larger samples.
