ADHD 아동을 둔 어머니들의 ADHD 치료 및 서비스 경험에 관한 연구 (Mothers' Experience of Treatment Services for Attention Deficit Hyperactivity Disorder)

  • 본 연구는 ADHD 아동을 양육하는 어머니들의 체험을 통한 ADHD 아동에 대한 치료와 서비스 본질 의미를 탐구하고자 하였다. 본질을 탐색하기 위해 ADHD 아동에게 실시한 치료와 서비스의 경험을 Giorgi 4단계를 토대로 단계별 치료와 서비스 경험의 의미와 정보 채널의 과정을 연구문제 내용으로 기술하였다. 연구문제는 ADHD 아동 어머니들이 경험한 ADHD 치료 및 서비스 경험의 실태는 무엇인가?, ADHD 아동 어머니들이 경험한 ADHD 치료 및 서비스의 본질적 의미는 무엇인가? ADHD 아동 어머니들이 경험한 ADHD 치료 및 서비스 경험의 구조는 어떠한가?로 구성되었다. 이를 위해 2014년 6월부터 전화면접을 통한 접촉을 시작하였으며, 인터뷰는 2014년 12월부터 2015년 2월까지 실시되었다. 13명의 ADHD 아동 어머니를 대상으로 2~3회에 걸쳐 현상학적 연구로 진행되었다. 그 결과 ADHD 아동 어머니들이 경험한 ADHD 치료 및 서비스 경험의 본질적 의미를 통한 질적 구조는 '내가 변해야 우리 아이가 산다'는 것으로 인지의 변화를 가져온 '인식변화를 위한 동행과정'이었다. 이러한 과정은 '무지와 혼란 속에서 자기결정의 부재'를 가져온 경험을 통해 '지각한 현실과 불안에 대처하며 변화에의 욕구'를 가진 후 '변화를 시도하지만 아직 남은 미결의 과제'를 지닌 채 '긍정적 변화의 주체로' 맞서 나아가는 과정이었다. 이 연구결과를 통해 병합치료의 필요성과 치료를 지속하는데 있어 부모의 올바른 인식에 의한 치료 참여는 주요한 변인이 된다는 것을 알 수 있었다. 아울러 아동의 변화를 위한 어머니의 변화가 중요하지만 삶에 대한 어머니의 태도가 긍정적으로 변화될 때만이 이루어질 수 있는, 어머니의 인지의 변화 과정이었다는 것이다.

주의력 결핍 과잉행동 성향을 지닌 유아의 식행동과 식사균형도 조사 (A Survey of Dietary Behavior and Meal Balance of Preschool Children with ADHD Dispositions)

  • This study was conducted to investigate the relationship among dietary behavior, meal balance, and clinical symptoms related nutritional status and Attention Deficit Hyperactivity Disorder(ADHD) in preschool children. The survey was conducted using questionnaires and the subjects were 3~6 years old preschool children in Samcheok. Subjects were divided into an ADHD dispositions group(n=88) and a normal group(n=129) based on assessments conducted by the children's mother using the Abbreviated Conners's Parent Rating Scale. There was no significant difference in meal frequency, meal speed, meal regularity or meal balance between children with ADHD dispositions and the normal groups. The ADHD disposition group had a higher proportion of children with picky eating(p<0.05), too much snacking(p<0.05) and over eating (p=0.05) habits than the normal group. In addition, the ADHD dispositions group had a higher frequency of clinical symptoms such as 'breathlessness when going up stairs', 'sleeplessness', 'anxiety' and 'tiredness'. Overall, there was a significant relationship between ADHD disposition and dietary problems and clinical symptoms related to poor nutritional status in preschool children. Therefore, to prevent and treat ADHD in preschool children, proper dietary management such as correcting of picky eating, over snacking and overeating is needed.

Potential health effects of emerging environmental contaminants perfluoroalkyl compounds

  • Environmental contaminants are one of the important causal factors for development of various human diseases. In particular, the perinatal period is highly vulnerable to environmental toxicants and resultant dysregulation of fetal development can cause detrimental health outcomes potentially affecting life-long health. Perfluoroalkyl compounds (PFCs), emerging environmental pollutants, are man-made organic molecules, which are widely used in diverse industries and consumer products. PFCs are non-degradable and bioaccumulate in the environment. Importantly, PFCs can be found in cord blood and breast milk as well as in the general population. Due to their physicochemical properties and potential toxicity, many studies have evaluated the health effects of PFCs. This review summarizes the epidemiological and experimental studies addressing the association of PFCs with neurotoxicity and immunotoxicity. While the relationships between PFC levels and changes in neural and immune health are not yet conclusive, accumulative studies provide evidence for positive associations between PFC levels and the incidence of attention deficit hyperactivity disorder and reduced immune response to vaccination both in children and adults. In conclusion, PFCs have the potential to affect human health linked with neurological disorders and immunosuppressive responses. However, our understanding of the molecular mechanism of the effects of PFCs on human health is still in its infancy. Therefore, along with efforts to develop methods to reduce exposure to PFCs, studies on the mode of action of these chemicals are required in the near future.

Initial Impact of the COVID-19 Outbreak on ADHD Symptoms Among University Students in Japan

  • Objectives: The coronavirus disease 2019 pandemic and its associated societal responses are anticipated to have wide-ranging effects on youth development and mental health. Depression, anxiety, and attention deficit hyperactivity disorder (ADHD) are the three most common mental health problems among university students. Many factors that can threaten mental health during the outbreak affect these three conditions, as well as sleep conditions, in undergraduate students. Thus, determining how these abrupt changes in students' circumstances impact their mental health is very important from a public health perspective. Methods: We investigated the usual conditions and changes in ADHD symptoms during the outbreak, in relation to depressive and sleep-related symptoms among undergraduate students. A total of 252 students, primarily juniors, completed the online survey. Results: The results showed that 12% of the students exceeded the cut-off score of the ADHD questionnaire before the pandemic. Approximately 6%-21% of the university students, especially those with ADHD traits, rated their ADHD behaviors as worse during the outbreak than that before the outbreak. Conclusion: Female students and undergraduates with ADHD traits are more susceptible to experiencing further deterioration of ADHD (inattention) symptoms during the pandemic. In cases where it is difficult to intervene with ADHD symptoms, approaching circadian rhythm or depression will be of considerable clinical use.

한국형 주의력결핍 과잉행동장애 약물치료 알고리듬 개발을 위한 예비연구 (A Preliminary Study on the Development of Korean Medication Algorithm for Attention-Deficit Hyperactivity Disorder)

  • Objectives:This study was conducted to develop a Korean algorithm of pharmacological and non-pharmacological treatment strategies in attention-deficit hyperactivity disorder (ADHD) and its specific comorbid disorders (e.g. tic disorder, depressive disorder, anxiety disorder, bipolar disorder, and oppositional defiant disorder/conduct disorder). Methods:Based on a literature review and expert consensus, both paper- and web-based survey tools were developed with respect to a comprehensive range of questions. Most options were scored using a 9-point scale for rating the appropriateness of medical decisions. For the other options, the surveyed experts were asked to provide answers (e.g., duration of treatment, aver-age dosage) or check boxes to indicate their preferred answers. The survey was performed on-line in a self-administered manner. Ultimately, 49 Korean child & adolescent psychiatrists, who had been considered experts in the treatment of ADHD, vol untarily completed the questionnaire. In analyzing the responses to items rated using the 9-point scale, consensus on each option was defined as a non-random distribution of scores as determined by a chi-square test. We assigned a categorical rank (first line/preferred choice, second line/alternate choice, third line/usually inappropriate) to each option based on the 95% confidence interval around the mean rating score. Results:Specific medication strategies for key clinical situations in ADHD and its comorbid disorders were indicated and described. We organized the suggested algorithms of ADHD treatment mainly on the basis of the opinions of the Korean experts. The suggested algorithm was constructed according to the templates of the Texas Child & Adolescent medication algorithm Project (CMAP). Conclusion:We have proposed a Korean treatment algorithm for ADHD, both with and without comorbid disorders through expert consensus and a broad literature review. As the tools available for ADHD treatment evolve, this algorithm could be reorganized and modified as required to suit updated scientific and clinical research findings.

아동기에 주의력결핍 과잉행동증상을 경험한 성인의 정신병리와 기질 성격 요인 (Psychopathology, Temperament, and Character Factors of Adults with Childhood Attention-Deficit Hyperactivity Symptoms)

  • Objectives : The objective of this study is to find out the differences in psychopathology, temperament, and character factors between young adults with and without childhood attention-deficit hyperactivity disorder (ADHD) symptoms. Methods : The subjects consisted of 314 university students (male=72, female=242) in Gongju. They were divided into childhood ADHD group (N=54) and normal group (N=260) to the Wender-Utah Rating Scale. Participants were assessed using Korean Adult ADHD Scale, Beck Depression Inventory-II, Beck Anxiety Inventory, Kimberly Young's Internet Addiction Test, Barratt Impulsiveness Scale, Paranoia Scale, Korean-Schizotypal Ambivalence Scale, Lubben Social Network Scale, and Temperament and Character Inventory-Revised. Descriptive statistics, t-test, chi-square, spearman correlation, and stepwise multiple regression were applied to analyze the data. Results : Participants with childhood ADHD symptoms had high level of adult ADHD symptoms (p<.001), impulsiveness (p=.001), depression (p<.001), anxiety (p<.001), internet addiction (p<.001), paranoia tendency (p<.001) and low level of selfesteem (p<.001) compared to normal group. Self-directedness (p<.001) was lower, Harm avoidance (p=.001) and Self-transcendence (p=.029) were higher in the childhood ADHD group. In correlation and stepwise multiple regression, childhood ADHD symptoms were significantly associated with cooperativeness (r=-0.515 and ${\beta}$=-0.547, p<.001 respectively). Conclusion : This result showed that young adults with childhood ADHD symptoms had various forms of psychopathology and childhood ADHD symptoms were influenced by character such as cooperativeness. Therefore, more thorough evaluation regarding childhood ADHD symptoms is needed.

소아정신과 입원환자의 공존질병(Comorbidity) (COMORBIDITY OF CHILD AND ADOLESCENT INPATIENTS)

  • 본 논문의 목적은 소아정신과 입원환자에서 진단의 공존질병이 어느정도인지를 알아보기 위한 예비적 조사이다. 평균나이 12세인 56명의 입원환자를 대상으로 DSM-III-R에 의한 공존질병을 조사결과 64.3%에서 2개 이상의 진단이 있었으며 평균 2개의 진단이 있었다. 주의력결핍과잉운동장애, 품행장애, 반항장애, 정신분열증, 기분장애, 틱장애, 배설장애, 정신지체, 인격장애 9개의 진단군으로 나누어 조사하여 상당수에서 공존질병이 있었으며 특히 틱장애, 배설장애, 파탄적행동장애에서 평균진단 수 20개 보다 많았다. 앞으로의 연구에서 대상 군의 수를 늘려 공존질병을 조사하고 아울러 새로운 진단분류의 가능성에 대해서도 고려해야 될 것이다.

Obstructive sleep apnea syndrome in children: Epidemiology, pathophysiology, diagnosis and sequelae

  • The prevalence of pediatric obstructive sleep apnea syndrome (OSAS) is approximately 3% in children. Adenotonsillar hypertrophy is the most common cause of OSAS in children, and obesity, hypotonic neuromuscular diseases, and craniofacial anomalies are other major risk factors. Snoring is the most common presenting complaint in children with OSAS, but the clinical presentation varies according to age. Agitated sleep with frequent postural changes, excessive sweating, or abnormal sleep positions such as hyperextension of neck or abnormal prone position may suggest a sleep-disordered breathing. Night terror, sleepwalking, and enuresis are frequently associated, during slow-wave sleep, with sleep-disordered breathing. Excessive daytime sleepiness becomes apparent in older children, whereas hyperactivity or inattention is usually predominant in younger children. Morning headache and poor appetite may also be present. As the cortical arousal threshold is higher in children, arousals are not easily developed and their sleep architectures are usually more conserved than those of adults. Untreated OSAS in children may result in various problems such as cognitive deficits, attention deficit/hyperactivity disorder, poor academic achievement, and emotional instability. Mild pulmonary hypertension is not uncommon. Rarely, cardiovascular complications such as cor pulmonale, heart failure, and systemic hypertension may develop in untreated cases. Failure to thrive and delayed development are serious problems in younger children with OSAS. Diagnosis of pediatric OSAS should be based on snoring, relevant history of sleep disruption, findings of any narrow or collapsible portions of upper airway, and confirmed by polysomnography. Early diagnosis of pediatric OSAS is critical to prevent complications with appropriate interventions.

야뇨증의 병태생리: 뇌, 수면장애 그리고 정신적 효과 (Pathogenesis of Enuresis: Brain, Sleep Dysfunction and Psychological Effects)

  • The relationship between central nervous system (CNS) and enuresis has not been sufficiently elucidated despite the presence of several circumstantial evidences. Contrary to common belief, polysomnographic sleep analysis revealed that the disturbance of arousal rather than deep sleep was responsible for enuresis. Subsequent studies confirmed depressed sympathetic tone and retarded brainstem reflex indicating abnormal arousal threshold in enuretics. In accordance with the bladder-brain dialogue, chronic stimulation of bladder may modify the brainstem function elevating arousal threshold. Epidemiological studies have suggested the association between enuresis and various psychosomatic disorders like attention deficit hyperactivity disorder (ADHD), which has shown the abnormal brainstem reflex similar to enuresis. Taken together, CNS is assumed to play a crucial role in the pathogenesis of enuresis. Psychological assessment is vital to understand the psychodynamic effect of enuresis. Studies have shown that the prevalence of psychological problems was higher in enuretic children and externalization of the symptoms was usually found. Several explanations have been brought up regarding the development of enuresis and psychological problems. Enuresis may cause psychological problems and vice versa. Otherwise, both may be associated with other variables, such as socioeconomic status (SES).

미술치료가 ADHD 아동의 문제행동과 또래관계에 미치는 효과 (The effect of Art Therapy apply to ADHD child's problem behavior and child relationship)

  • This study shows that Art therapy has positive effects on ADHD child's problem behavior and child relationship. Also we want to provide more data to education field. We work with 3 children. They are recommended by their class teacher and they are attending J elementary school which is located in K city in Chunbuk. The experiment period was 7 weeks - 2 times a week, total is 14th- and each time required was 40-60 minutes when they finish their school. Since we want to see each ADHD child's results, we use ADHD teacher evaluating measure and Child relationship measure to compare the results after the fact. The results are Art therapy has positive effects on the ADHD child's problem behavior specially concentration, excess behavior, social skill, hostile behavior and aggregation with same ages, sociality, lack of sociality, exclusive, unstable, and hypersensitiveness.
