본 연구의 목적은 바이올린 교습 중심의 음악치료 활동이 주의력결핍과잉행동장애 아동들의 집중력과 충동성 조절에 변화를 가져오는지 연구하고 그러한 변화가 일상생활에 전이가 되는지 보고자 하는 것이다. 본 연구는 연령대가 비슷하고 종합병원에서 ADHD라고 진단을 받은 초등학교 2학년, 3학년, 4학년을 대상으로 선정하여, 본 연구자가 8주에 걸쳐 14번의 개별 세션과 마지막 회기의 발표회 연주로 총 15회기를 실시하였다. ADHD 아동의 집중력 향상과 충동성 조절의 변화를 살펴보기 위하여 15회 음악치료 활동 전 후에 실시한 대상 아동의 문제행동측정리스트(K-CBCL) 검사도구 중 주의 집중문제 항목, 단축형 Conners 부모용 평정척도(ACRS), 가정환경 설문지 개정판(CHSQ-R), 개정판 Conners 교사 평정척도(CCTRS-28) 검사 도구의 점수를 비교 분석하였다. 또한, 세션 안에서 보이는 '주의산만행동'과 '학습 외 행동'은 빈도수 기록으로 나타내었으며, '학습 외 행동' 중 가만히 있지 못하고 손발을 움직이는 '손발 행동'은 등간 기록법, '주의집중시간'은 지속 시간 기록법으로 표시하였다. 위의 척도들이 가지는 양적인 데이터의 단점을 보완하기 위하여 아동들이 음악활동에서 보여 준 집중력과 충동 조절에 관한 반응을 질적으로 분석하였다. 그 결과 바이올린 교습 중심의 음악치료 실시 후 ADHD 아동의 집중력과 충동성 조절이 향상되었으며 가정이나 학급에서 발생하는 문제 행동 감소에 효과가 있음이 확인되었다. 본 연구는 선행 연구가 빈약한 분야인 바이올린 교습을 치료적으로 적용하여 실제 ADHD 아동을 대상으로 음악치료 활동의 효과를 입증한 점에서 의의가 있다. 즉 기존의 약물치료와 인지행동치료의 제한점으로 인해 모든 ADHD 아동들에게 그러한 치료를 적용할 수 없다는 한계와 현실적인 한국 상황 내 치료 관행을 고려해 볼 때, 바이올린 교습 중심의 음악치료적 접근이 ADHD 아동에 대한 치료적인 대안이 될 수 있음을 시사한다.

Objectives: This study was conducted to investigate the reliability and validity of the Korean version of the DSM-5 Level 2 Cross-Cutting Symptom Measure-inattention [Swanson, Nolan and Pelham, version IV (SNAP-IV)] and anger [Patient-Reported Outcome Measurement Information System (PROMIS) Anger] for parents and guardians of children aged 6-17 years. Methods: We included 104 children and adolescents diagnosed with attention-deficit/hyperactivity disorder (ADHD), ADHD with anxiety and depression, depressive disorder, anxiety disorder, and tic disorder with somatic symptoms (ADHD=41, depression=9, anxiety=14, ADHD+anxious depression=11, tic+somatic symptoms=29). Their ages ranged from 8 years to 15 years. The participants' mothers completed the SNAP-IV, PROMIS Anger scale, Korean version of the IOWA Conners Rating Scale (K-IOWA), and Korean ADHD Rating Scale (K-ARS) so that the reliability and validity of the SNAP-IV and PROMIS Anger scales, which are DSM-5 scales for assessing inattention and anger of children and adolescents, could be examined. Results: The reliability coefficient of SNAP-IV (Cronbach's α) was 0.94. The correlation coefficients between SNAP-IV, K-IOWA inattention, and K-ARS inattention scores ranged from 0.73 to 0.86. The mean SNAP-IV scores of the ADHD and the ADHD+anxious depression groups were significantly higher than those of the anxiety and the tic+somatic symptoms groups. The reliability coefficient of the PROMIS Anger was 0.91. The correlation coefficient between PROMIS Anger and K-IOWA oppositional/defiant scores was 0.75. The PROMIS Anger mean score of the ADHD+anxious depression group tended to be higher than that of the other groups. Conclusion: These results suggest that the Korean version of the DSM-5 Level 2 Cross-Cutting Symptom Measure-inattention and anger for parent and guardian of child age 6-17 might be a reliable and valid test and may be useful for screening children and adolescents with ADHD.

Objectives : This study aimed to investigate the efficacy and safety of Metadate CD (MCD) when given to Korean children and adolescents with attention-deficit hyperactivity disorder (ADHD). We also explored the effects of the drug on diverse neuro-cognitive functions. Methods : Ninety-one subjects with ADHD (mean age 8.6${\pm}$2.2 years) were recruited at 6 outpatient clinics in Seoul, Korea. We used the ADHD Rating Scale (ARS), Clinical Global Impression (CGI), and comprehensive attention test (CAT) to measure the drug's effects. Results : After 0.92${\pm}$0.32mg/kg/day of MCD were administered for 57.4${\pm}$7.6 days, there was a 48.5% reduction in the mean total ARS scores (p<.001). Fifty-seven subjects (64.8%) showed either much improved or very much improved outcomes on the CGI-Improvement scale. The CGI-Severity scale also decreased from an average of 4.7 to an average of 2.9 (p<.001). Errors and response time standard deviations of the CAT, sustained attention test-to-response tasks, the flanker test, and divided attention test scores decreased after treatment (p<.05). The forward memory span of the spatial working memory test scores increased (p<.05). Thirty-five patients (39.8%) experienced side effects, of which the most common were headache (14.8%), nausea (12.5%), and anorexia (9.1%). Conclusion : This open-label study suggests that MCD is effective and safe in improving the symptoms and neurocognitive functions of Korean children and adolescents with ADHD.

주의력 결핍장애아동의 치료는 악물치료가 중요하지만 중복장애아동과 같은 원칙에서 인지-행동적, 교육적 접근과 상호보완이 필요하다. 치료의 첫단계는 진단을 확립하고, 아동, 부모, 교사에게 이장애의 특성을 설명해 주고 치료의 대책을 세운다. 약물치료는 주로 중추신경자극제-그중에서도 메칠페니데이트를 중심으로 논의하였고, 그외에 삼환계 항우울제, 클로니딘, 항정신병약물이 소개되었다. 약물치료이외의 방법들로는 정신요법, 인지행동요법, 교육적 방법과 부모및 가족상담의 원칙들을 논의하였다. 이들 전통적인 치료방법이외에 논란되고 있는 식이요법, 비타민요법, 저당분요법, 미네랄요법, 정제된 당분의 투여, 신경학적 조직의 이론에 근거한 운동요법들을 소개하고 설명하였다.
Learning disabilities (LD) are heterogeneous group of disorders with evidences of genetic or familial trait, intrinsic to the individual and presume to be due to central nervous dysfunction. Learning disabilities and attention deficit hyperactivity disorder (ADHD) are the two of the most common disorders in the population of school-age children. Typically academic achievements in children with learning disabilities are significantly lower than expected by their normal or above normal range of IQ. Although academic and cognitive deficits are hallmarks of children with LD, those children are also at risk for a broad range of behavioral and emotional problems. Almost all cases meet criteria for at least one additional diagnosis such as ADHD, developmental coordination disorder, depression, anxiety, obsessive compulsive disorder, tic disorder, among which ADHD is particularly predominant. Because of the response to the therapeutic intervention program is promising and positive when applied early, it is critical to recognize patients as early as possible. Pediatricians often are the first to hear from parents worried about a childs academic progress. It is not the responsibility of pediatrician to make a diagnosis, referring children for a diagnostic evaluation of LD is a reasonable first step. Pediatricians can make early referral of suspicious children by asking some serial short questions about basic and processing skills. With a basic knowledge about the clinical characteristics, diagnostic and therapeutic procedures of LD, pediatricians also can provide primary counseling and education for parents at their outpatient clinical settings.

Electroencephalography (EEG) measures neural activation due to various cognitive processes. EEG and event-related potentials (ERPs) are widely used in studies investigating psychopathology and neural substrates of psychiatric diseases in children and adolescents. The present study aimed to review recent ERP studies in child and adolescent psychiatry. ERPs are non-invasive methods for studying synaptic functions in the brain. ERP might be a candidate biomarker in child-adolescent psychiatry, considering its ability to reflect cognitive and behavioral functions in humans. For the EEG study of psychiatric diseases in children and adolescents, several ERP components have been used, such as mismatch negativity, P300, error-related negativity (ERN), and reward positivity (RewP). Regarding executive functions and inhibition in patients with attention-deficit/hyperactivity disorder (ADHD), P300 latency, and ERN were significantly different in patients with ADHD compared to those in the healthy population. ERN showed meaningful changes in patients with anxiety disorders, such as generalized anxiety disorder, separation anxiety disorder, and obsessive-compulsive disorder. Patients with depression showed significantly attenuated RewP compared to the healthy population, which was related to the symptoms of anhedonia.
This study aimed to examine the relationship between nutrient intake and attention deficit hyperactivity (ADH) of children suffering atopic dermatitis. We examined clinical symptoms as well as the status of ADH and the dietary intake of 47 children with atopic dermatitis aged between 4 and 6 against 84 children in the control group. Family history of the atopic dermatitis group was significantly higher than the control group. There was no significant difference between the diet habit of the two groups. The daily energy intake of the atopic dermatitis group and the control group were 1,189.7 kcal and 1245.0 kcal, respectively. There was no significant difference between the nutrient intake of the two groups. In regards to the clinical symptom items, the point for 'anxieties' was 0.5 in the atopic dermatitis group, which was significantly higher than 0.3 of the control group (p<0.05). In terms of the detailed items in ADH assessed by kindergarten teachers, the point for 'changeable feeling' was 0.8 in the atopic dermatitis group, which was significantly higher than 0.5 of the control group (p<0.05). As for the correlation among nutrient intake, the ADH score and clinical symptoms, total fat intake (p<0.05) and animal fat intake (p<0.05) showed a significantly negative correlation with the point of ADH assessed by the teachers, while the intake of fiber (p<0.05) displayed a significantly negative correlation with the point of ADH assessed by the parents. Since atopic dermatitis and ADH may be caused by the same dietary factors, it will be necessary to conduct more studies in this field in the future. In addition, appropriate nutrition management will be necessary for children in the growing period who suffer from atopic dermatitis.

Attention deficit hyperactivity disorder (ADHD) affects 4-12% of chool-age children worldwide and is characterized by three core symptoms: hyperactivity, inattention, and impulsivity. Although standard pharmacological treatments, such as methylphenidate and atomoxetine, are available, concerns about drug-induced psychological and cardiovascular problems, as well as growth retardation and sleep disturbances, highlight the continuing need for new therapeutic interventions. Using a neonatal hypoxia-induced hyperactivity model in rats, the potential positive role that oral administration of red ginseng extract may have in relation to the hyperactive phenotype was investigated. Hypoxia was induced in 2-day-old male Sprague-Dawley (SD) rat pups by placing them in a nitrogen chamber for 15 min. The neonatal hypoxia-induced rats showed a significant increase in hyperactivity phenotype, such as increased movement duration, movement distance, and rearing frequency, which was determined by monitoring their spontaneous locomotor activity using the Ethovision video tracking system. One week of oral treatment with red ginseng extract decreased the hyperactivity phenotype of the neonatal hypoxia-induced rats and increased the locomotor activity of the control rats. In the neonatal hypoxia-induced rats, expression of the norepinephrine transporter in the forebrain was increased, and red ginseng treatment partially prevented its up-regulation, while increasing its level in the control rats. Taken together, these results suggest that red ginseng extract decreased the neonatal hypoxia-induced hyperactivity phenotype, although it increased locomotor activity in normal animals.

목 적:본 연구에서는 주의력 결핍/과잉운동 장애(ADHD)와 학습장애(LD), 그리고 이 두 장애의 공존질병(ADHD+LD)을 보이는 혼합형 장애 아동 집단을 대상으로 여러 가지 기억 기능을 평가하는 신경심리검 사상에서 세 장애 집단의 수행 차이를 비교하였고, 지능수준과 기억 책략의 사용 여부가 이들의 기억능력 향상에 어떤 영향을 미치는지를 알아보고자 하였다. 방 법:ADHD 아동 11명, LD 아동 5명, 두 장애를 함께 가진 혼합형 장애(ADHD+LD) 아동 9명, 그리고 정상 아동 8명에게 기억 기능과 인지능력을 평가하기 위해 개별적으로 기억력 평가 검사(MAS)와 웩슬러 아동용 지능검사를 실시한 후, 집단간 수행 차이를 비교하였다. 결 과:언어 및 시각 과제에 대한 재인검사를 제외한 제반 기억 검사상에서 통제집단에 비해서 세 장애집단군이 저조한 수행을 보였으며, 특히 ADHD+LD 집단이 가장 저조한 수행을 보였고, 그 다음으로 LD, ADHD 집단의 순으로 전반적인 기억 검사상에서 저조한 수행을 보였다. 이러한 수행 양상은 언어 기억 소검사를 제외한 MAS 검사의 다른 하위검사에서도 동일하게 나타났으며, 제반 기억 검사에서의 우수한 수행은 검사시 아동이 사용하였던 기억 책략 및 오류 반응의 사용정도 유의미한 상관이 있었다. 논 의:ADHD, LD의 공존질병을 가질 경우 기억 및 학습에 더 어려움이 있으며, ADHD, LD, 혼합형 집단을 변별하는데 있어 기억기능을 측정하는 것이 각 장애를 감별진단하는데 도움이 될 가능성이 시사되었다.
Objectives: The purpose of this study is to investigate the clinical trial research trends in regards to the neurofeedback training (NFT) in Korean journals. Methods: We researched articles published in Korean journals from 2000 up to 2013 and were related to the NFT. We searched six electronic databases to find relevant articles, using the term 'neurofeedback'. Results: 1) 6 single group comparative studies and 31 randomized controlled trials were found. 2) Healthy volunteers and students were most frequently studied with neurofeedback training. Other studies included attention deficit hyperactivity disorder (ADHD), Poststroke, panic disorder, premenstrual dysphoric disorder (PDD), temporomendibular disorder (TMD), and obesity. NFT interventions were attempted in diverse training protocols and assessed with many different outcome measurements. 3) Most studies showed effective results after NFT. Conclusions: NFT is increasingly studied and used in various clinical fields. Also, there have been efforts to adopt NFT in Korean medical clinics and researches, and more rigorous and innovative studies are needed in the future.
