Characteristics of Atrial Natriuretic Peptide Release in Renal Hypertensive Rats (신성 고혈압 백서 심방의 심방이뇨 호르몬분비 특성)

  • It has long been suggested that the cardiac atrium is a low pressure volume receptor controlling body fluid volume and blood pressure. Recently, the cardiac atrium has been found to contain a family of powerful peptides. To clarify the relationship between high blood pressure and the biologically active atrial peptides, experiments were done to define the characteristics of atrial natriuretic peptide secretion in the isolated perfused atria of renal hypertensive rats. Higher concentrations of plasma atrial natriuretic peptide and renin activity were observed in the two-kidney, one clip hypertensive rat compared to the normotensive rat. Atrial volume changes in response to pressure elevations were attenuated in hypertensive rats compared to normotensive rats. Incremental response to atrial volume changes in ANP secretion was accentuated in hypertensive rats. These date suggest that the accentuated atrial natriuretic peptide response to volume changes of hypertensive rats may be a physiological or pathphysiological adaptation to the high blood pressure and may be, at least in part, responsible for the elevated levels of plasma atrial natriuretic peptide observed in hypertensive rats.

Correlation of Left Atrial Enlargement and Metabolic Syndrom Risk Factors in Echocardiography (심장초음파검사에서 좌심방 확장과 대사증후군 요인과의 상관관계)

  • This study investigated the relationship between metabolic syndrom risk factors to patients with left atrial enlargement according to the results of echocardiography. Male were 269(8.9%) and female were 46(1.5%) that of the total 315 patients (10.5%) with left atrial enlargement. In the obesity factors analysis, BMI, WC, and WHtR were associated with left atrial enlargement. Among them, WHtR included the many obese people with left atrial enlargement. In the analysis of blood pressure, The systolic blood pressure was the highest in the borderline hypertension group in the left atrial enlargement and the diastolic blood pressure was the highest in the normal range group in the left atrial enlargement. In the blood result analysis, there was no correlation with left atrial enlargement that total cholesterol, HDL, LDL cholesterol, triglyceride, and fasting blood glucose level were within the normal range. Therefore, obesity and systolic blood pressure were associated with the left atrial enlargement group. However, the systolic blood pressure did not show pathological condition due to borderline hypertension, and the distribution of the left atrial enlargement group was smaller than that of obesity. Obesity, which includes the largest number of left atrial enlargement group, has been identified to be the highest risk factor for left atrial enlargement.

Modulation of Subcellular Ca2+ Signal by Fluid Pressure in Rat Atrial Myocytes

  • Atrial chambers serve as mechanosensory systems during the haemodynamic or mechanical disturbances, which initiates arrhythmia. Atrial myocytes, lacking t-tubules, have two functionally separate sarcoplasmic reticulums (SRs): those at the periphery close to the surface membrane, and those at the cell interior (center) not associated with the membrane. To explore possible role of fluid pressure (FP) in the regulation of atrial local $Ca^{2+}$ signaling we investigated the effect of FP on subcellular $Ca^{2+}$ signals in isolated rat atrial myocytes using confocal microscopy. FP was applied to whole area of single myocyte with pressurized automatic micro-jet (200-400 $mmH_2O$) positioned close to the cell. Application of FP enhanced spontaneous occurrences of peripheral and central $Ca^{2+}$ sparks with larger effects on the peripheral release sites. Unitary properties of single sparks were not altered by FP. Exposure to higher FP often triggered longitudinal $Ca^{2+}$ wave. These results suggest that fluid pressure may directly alter excitability of atrial myocytes by activating $Ca^{2+}$-dependent ionic conductance in the peripheral membrane and by enhancing spontaneous activation of central myofilaments.

Relationship between atrial pressures and the interventricular pressure in the moving actuator type total artificial heart (심실간 공간 압력을 이용한 이동작동기형 완전이식 인공심장에서의 좌, 우심방압 추정)

  • The right and left atrial pressures are important parameters in automatic control of a total artificial heart (TAH) within normal physiological ranges. Our TAH is composed of a moving actuator, right and left ventricles and the interventricular space enclosed by a semi-rigid housing. During operation of the TAH, the jnterventpicular space's volume is changed dynamically by the difference between the ejection volume of one ventricle and the inflow volume of the other. Therefore, the changes in pressure of the interventricular space is related to both atrial pressures. We measured the interventricular pressure (IVP) waveform using a pressure sensor and attempted to indirectly estimate the changes of atrial pressures. This method has an advantage that the sensor does not contact the blood directly. Furthermore, the IVP waveforms have its zero baseline in each pump cycle, thus the pressure measurements are free from the transducer drift problems by measuring the peak pressure from these baseline values. From the In vitro experiments, we found that the IVP waveform contained several useful parameters such as negative peak, dP/dT on the initial break, the area enclosed by the profile in each stroke, which are associated with atrial pressures and the filling conditions of the ventricles. The measured atrial pressures were linearly related to the negative peak of the interventricular pressure.

Clinical Studies on Congenital Atrial Septal Defects in Koreans (심방중격결손증에 대한 임상적 고찰)

  • Atrial septal defect is one of the most frequently encountered congenital heart disease. Up to December 31, 1976, 1682 cardiac patients received cardiac catheterization in the cardiac department of Yonsei university medical college. Out of the 1682 cardiac patients 723 cases had congenital heart disease and only 116 cases had congetial atrial septal defect. This amounted to 16.04% of all those with congenital heart disease. 58 cases of congenital atrial septal defect operated in the chest surgery department were presented. Of these 58 cases of atrial septal defect, 27 cases were male and 31 cases were female. Their ages ranged from 5 years to 54 years. The systolic pressure of the main pulmonary artery of 40 out of the 58 cases of atrial septal defect was below 40% of that of the systemic blood pressure: in 6 cases, the range of the systolic pressure of the main pulmonary artery was 50-90mmHg; in 12 cases, the range of the systolic pressure of the main pulmonary artery was 40-50mmHg. Average age of these was 30. 1 years. This study tends to show that Korean patients with atrial septal defect even though younger have a slight higher systolic pressure of the main pulmonary artery than Western patients have. The pulmonary blood is 1.5-2.5 times of systemic blood flow in 52 cases out of 58 cases of atrial septal defect.In only one of the 58 cases of atrial septal defect, the Rp was found to be as high as 45% of Rs. All other cases were below this level.51 cases had ostium secundum defect, 4 out of these cases had ostium secundum defect combined with mitral incompetence and 6 out of them had double ostium secundum defect. The remaining 7 cases had ostium primum defect. Their atrial defects were repaired under direct vision utilizing extracorporeal circulation, by hemodilution technic combined with moderate hypothermia. 44 cases [2nd atrial septal defect] were repaired by direct sutures while 14 cases, including the 7 cases ostium primum defects needed patches [1 pericardium and 13 teflon patch]. In 4 cases there were single defects while showed two defects. However the associated septal defect was so small that it could be closed by direct sutures. The size of the defect ranged between 6.0cm2and 10.0cm2 in 19 cases[33.7%]: the smallest being 0. 5cm2 and the largest 24cm2. The surgical mortality was 2 cases [3.4%]. These one case with ostium primum defect, could not be resuscitated on operation table. The cause of death in this case was myocardial failure and MI. The other, a case of ostium primum defect had a second operation on the first operative day due to massive bleeding from LV vent-line insertion site.The patient died on 26th post-operative day due to sepsis.

  • The aim of this experiments was to investigate the effect of Lycii Fructus water extracts on the blood pressure, plasma renin activity and plasma levels of atrial natriuretic peptide in two-kidney one clip Goldblatt hypertensive rats. Systolic blood pressure was decreased significantly after administration of Lycii Fructus water extracts. Plasma levels of atrial natriuretic peptide was increased significantly after adminstration of Lycii Fructus water extracts. Plasma renin activity was not changed after adminstration of Lycii Fructus water extracts. Thease results suggested that the depressor response after adminstration of Lycii Fructus water extracts were related with the changes of the plasma levels of atrial natriuretic peptide.

Hemodynamic effects of induced overtransfusion of blood in dogs (과량수혈이 혈역학에 미치는 영향)

  • Appreciation of the large volume deficits which may occur in surgical or trauma patients due to blood loss has led to vigorous transfusion techniques designed to overt hypovolemic shock and ischemic damage to vital organs which may develop in minutes during the hypovolemic state. In a significant proportion of patients treated with massive rapid blood or fluid transfusion, hypervolemia occurs and life threatening pulmonary edema may develop. Especially, hypervolemia may occur during transfusion for preventing development of the so-called low output syndrome following cardiac surgery. However, the most effective indicator which reveals the adequate level of transfusion is not settled yet. The present study was aimed to compare the effectiveness of the indicators suggested thus far and to determine the most sensitive one. Eight dogs were experimentally studied in terms of left atrial pressure, pulmonary arterial systolic pressure, central venous pressure, mean systemic arterial pressure and heart rate before and after induced hypervolemia with infusion of 600ml heparinized homologous blood. Immediately after induced overtransfusion of the blood, pulmonary arterial systolic pressure increased 75.0%, in omparison with the control before transfusion, left atrial pressure 58.8%, central venous pressure 44.6%, and mean systemic arterial pressure 10.1%, one hour after transfusion, pulmonary arterial systolic pressure 40.0%, left atrial pressure 21.2%, central venous pressure 14.5%, and mean systemic arterial pressure 3.2%, central venous pressure 14.5%, and mean systemic arterial pressure 3.2%, respectively. Heart rate showed no significant change throughout the experiment. These result suggested that the changes of the pulmonary arterial systolic pressure is the most sensitive indicator for detection of hypervolemia during blood transfusion.

Renal and Hormonal Responses to Atrial Natriuretic Peptide and Furosemide in the Freshwater Turtle, Amyda japonica (자라 신장기능에 미치는 Atrial Natriuretic Peptide의 효과)

  • Effects of synthetic atrial natriuretic peptide and furosemide on the cardiovascular and renal functions were examined in the freshwater turtle, Amyda japonica. Both atria and ventricle of turtle contained an immunoreactive atrial natriuretic peptide. Synthetic rat atrial natriuretic peptide (atriopeptin III) and turtle atrial extract caused a decrease in mean arterial blood pressure and the vasodepressor effect was dose-dependent. In hydrated turtles received either atriopeptin III or turtle atrial extract, no significant change in renal function was observed until 100 min except a slight natriuresis at 60 or 100 min after injection of 30 ug/kg atriopeptin III or atrial extract, respectively. However, furosemide, 2 mg/kg, caused marked diuresis, natriuresis and kaliuresis. In non-hydrated turtles, no significant change in renal function was observed until 6 hrs following injection of 30 ug/kg atriopeptin III. Plasma aldosterone decreased at 2 hr and increased at 24 hr after injection of atriopeptin III although plasma renin concentration did not change. But, furosemide caused persistent diuresis, natriuresis and kaliuresis. Additionally, plasma aldosterone and renin concentrations were significantly increased at 24 hrs after injection of furosemide. In conclusion, we suggest that the freshwater turtle may have an atrial natriuretic peptide in heart and vascular receptors for atrial natriuretic peptide, and that atrial natriuretic peptide is more important in the regulation of blood pressure rather than that of renal function in freshwater turtles. We also suggest that an increased plasma renin concentration caused by furosemide may not be due to the sodium concentration delivered to macula densa, but due to the dehydration caused by persistent diuresis and natriuresis.

Regulatory Role of Nitric Oxide on Atrial Natriuretic Peptide System in Normotensive and Hypertensive Rats

  • The present study was aimed to explore an interaction between endothelium-derived nitric oxide (NO) and atrial natriuretic peptide (ANP) systems in normotensive and hypertensive states. Rats were made two-kidney, one clip (2K1C) hypertensive and supplemented with either $N^G-nitro-L-arginine$ methyl ester (L-NAME, 5 mg/100 ml drinking water) or L-arginine hydrochloride (400 mg/100 ml drinking water). One group supplied with normal tap water served as control. Sham-clipped rats were also divided into the L-NAME, L-arginine, and control groups. The plasma levels and atrial contents of ANP were determined at day 28 following clipping the renal artery. In 2K1C rats, the plasma level of ANP was higher and the atrial content was lower than in the sham-clipped control. L-Arginine increased the atrial content of ANP in association with a decreased plasma ANP, whereas L-NAME significantly affected neither parameter. The increase of blood pressure in 2K1C rats was not affected by L-arginine or L-NAME. In sham-clipped rats, the plasma level of ANP was significantly increased by L-NAME along with an increase in blood pressure. On the contrary, L-arginine did not affect the blood pressure or plasma ANP. The atrial content of ANP was significantly altered neither by L-arginine nor by L-NAME. These results suggest that NO plays a tonic inhibitory role on the ANP release with concomitant increases of the atrial tissue content. In addition, hypertension is suggested to modify the release and tissue storage of ANP.
