영아기 영양과 소아 알레르기질환의 예방 (Dietary Prevention of Allergic Diseases in Children)

  • Previous infant feeding guidelines recommended a delayed introduction of solids to beyond 6 months of age to prevent atopic diseases. However, scientific evidence supporting a delayed introduction of solids for prevention of atopic diseases is scarce and inconsistent. Current evidence does not support a major role for maternal dietary restrictions during pregnancy or lactation in the prevention of atopic disease. In studies of infants at high risk of developing atopic disease, there is evidence that exclusive breastfeeding for at least 4 months compared with feeding intact cow milk protein decreases the incidence of atopic dermatitis, cow milk allergy, and wheezing in early childhood. For infants at high risk of developing atopic disease who are not breastfed exclusively for 4 to 6 months, there is modest evidence that atopic dermatitis may be delayed or prevented by the use of extensively or partially hydrolyzed formulas, compared with cow milk formula, in early childhood. There is no convincing evidence that a delayed introduction of solid foods beyond 4 to 6 months of age prevents the development of atopic disease. For infants after 4 to 6 months of age, there are insufficient data to support a protective effect of any dietary intervention for the development of atopic disease.

  • Purpose: The disease specific self-efficacy of mothers caring children with atopic dermatitis is a crucial factor for adherence to recommended treatments. This study investigated factors associated with disease specific self-efficacy of mothers of atopic dermatitis children. Methods: A cross-sectional study was used. One hundred twenty five mothers recruited from the out-patient clinic of three hospitals completed self-administrative questionnaires, including the disease specific self-efficacy, general parenting efficacy, and severity of atopic dermatitis. Descriptive statistics, ANOVA, and multiple regression analysis were performed. Results: The degree of disease specific self-efficacy of mothers was significantly different according to the disease severity, and parenting efficacy. 26.0% of the variance of disease specific self-efficacy was explained by the severity of disease and parenting efficacy. Conclusions: Future studies need to focus on the development of educational programs that will help to improve mothers' disease specific self-efficacy as well as parenting efficacy.

중증 성인형 아토피 피부염을 사상처방으로 치료하여 호전된 환자 15례에 대한 1년간의 추적 관찰 (One Year Follow up for Severe Adult Atopic Dermatitis of 15 Patients After Sasang Constitutional Therapy)

  • Background : Atopic Dermatitis is a chronic and recurrent skin disease. it is characterized as itch-scratch cycle and topical or systemic use of corticosteroids is required in western medicine. Though its effects on major symptoms are impressive, these treatments are prone to several side effects and tend to recur after months or years. So potential treatments have been actively studied in Estern medicine, especially Sasang constitutional therapy, which has fewer side effects. Objectives : The purpose of this study was to examine the effect of Sasang constitutional therapy on treating severe adult Atopic Dermatitis and preventing its recurrent symptoms. Methods : fifteen patients who had severe adult Atopic Dermatitis attended treatment of Sasang constitutional therapy and were followed up for one year. An oriental internal specialist first isolated Sasang constitution and a distinctive body type. Grading of Atopic dermatitis was measured by the guidelines offered by Rajka G. and Langeland T. Results : After treatment with Sasang constitutional therapy, the patients symptoms were improved and the mean Rajka and Langeland score reduced significantly from $7.73{\pm}0.79\;to\;3.4{\pm}10.91$. During the follow-up period of one year, the mean Rajka and Langeland score was $2.72{\pm}2.06$ which was lower than last treatment. Conclusions : The results of this study support Sasang constitutional therapy as an effective therapy for severe adult Atopic Dermatitis. Follow up data suggest that this Eastern medical approach is of greater clinical value than the western medical approach in the long term.

아토피 피부염 환자 발병률의 지역적 특성 분석 - 모란지수 방법을 활용하여 - (An Analysis on Characteristics of Spatial Distribution of the Atopic Dermatitis Patients : With an Application of the Moran Indices)

  • 산업화 이후 환경성 질환의 증가가 사회적인 문제로 대두되면서 이들의 지역적 차이와 특성에 대한 관심 또한 높아지고 있다. 본 연구의 목적은 국민건강보험공단 자료를 이용하여 환경성 질환의 지역분포와 지역적 특성을 알아보는 것이다. 본 연구에서는 환경성 질환 중 아토피 피부염을 대상으로 국지모란지수를 이용하여 아토피 피부염 환자의 지역별 군집성 정도와 유형을 분석하였다. 이에 따라 다음과 같은 연구결과를 도출하였다. 첫째, 아토피 피부염 환자의 분포는 지역적으로 차이를 나타내고 있었으며, 일반적 예상과 달리 서울시가 비교적 청정한 지역으로 분류되었다. 둘째, 수도권 지역 중 인천 서구 등 42곳은 아토피 피부염의 핫스팟 지역으로 나타났다. 셋째, 지방 중 단양군 등 39곳은 아토피 피부염의 콜드스팟이 도출되었고, 환경과의 연관성을 추론할 근거가 나타났다. 넷째, 제주시와 서귀포시가 독특하게 아토피 핫스팟 지역으로 나타났다. 이와 같은 아토피 피부염 환자의 지역분포 특성에 대한 분석을 바탕으로 차후 보건지리학 연구가 지속적으로 진행될 필요가 있다고 사료된다.

  • Introduction: Atopic dermatitis is chronic eczematous recurrent disease of skin and till now its mechanism is not cleary known. It is related to a family history of atopic disease. It has severe itching and it causes eczema secondarily. Atopic dermatitis has mostly severe symptoms and progresses to chronic relapsing. Objectives: The purpose of this study was to estimate the efficacy of herb- medicine treatment on atopic dermatitis. Subjects: 1 treated 5 patients who visited Chungbalsan Kyunghee oriental medical clinic with atopic dermatitis from September, 2001 to October, 2002 partiticipated in the study and were observed for over 2 months.   • We observed and treated a 19 year-old female who had atopic dermatitis. Even though she had been treated with western medicine, she still complained of severe itching, erythema, lichenification, scaling, dryness, oozing and insomnia especially on her face and neck. We treated her with Sasang Constitutional Therapies(Kupoong-san, Teumchoui-tang, Kalkunsungki-tang), apcupuncture, herbal irrigation and moisture emulsion. Having been treated for 24 days in hospital admitted, her symptoms were reduced and changed from severe to mild grade in SCORAD(SCORing Atopic Dermatitis)

  • Objective : This case report presents the effect of Hoiyeok-tang on atopic dermatitis. A patient was diagnosed with Lesser Yin disease based on Shanghanlun provisions. Methods : According to the disease pattern identification diagnostic system based on Shanghanlun provisions (DPIDS), we diagnosed the patient with Lesser Yin disease. The patient was treated with Hoiyeok-tang for 87 days. The changes in symptoms were estimated by the SCORAD index. Results : After treatment, the SCORAD Index improved from 57.3 to 33.5. The patient also body weight from 89.1 kg to 76.5 kg. Conclusions : This case study shows the effectiveness of Hoiyeok-tang on atopic Dermatitis according to DPIDS.

아토피성 질환관리의 최신 동향 (Recent Progress in the Care of Atopic Disease in Children)

  • Background: Atopic dermatitis starts as an early childhood type I hypersensitivity to environmental allergens and is often the first step in the atopic march to develop into asthma and allergic rhinitis. Despite progress that has been achieved in management, this health problem remains poorly controlled and cause great pain and suffering for many children and their parents To introduce nurses to recent progress in pathophysiology, management and preventive measure of atopic disease. Search method: Systemic search was done using the PubMed and CINAHL from 1980 to 2005. In addition, historical references were taken from standard medical textbook. Results: In total, 30 relevant publications were located including primary research and review articles that cover the pathophysiology, management, and preventive measure of atopy. The evidence emerging from literature indicate that non-medical approaches such as breastfeeding and probiotics would improve management outcomes. Conclusion: The review suggests that breastfeeding and probiotic approaches would be the most effective preventive measures for children with atopic diseases.

Genomic Susceptibility Analysis for Atopy Disease Using Cord Blood DNA in a Small Cohort

  • Atopic disease is caused by a complex combination of environmental factors and genetic factors, and studies on influence of exposure to various environmental factors on atopic diseases are continuously reported. However, the exact cause of atopic dermatitis is not yet known. Our study was conducted to analyse the association of SNPs with the development of atopic disease in a small cohort. Samples were collected from the Mothers' and Children's Environmental Health (MOCEH) study and 192 cord blood DNA samples were used to identify incidence of atopy due to influence of exposure to environmental factors. Genetic elements were analysed using a precision medicine research (PMR) array designed with various SNPs for personalized medicine. Case-control analysis of atopy disease revealed 253 significant variants (p<0.0001) and SNPs on five genes (CARD11, ZNF365, KIF3A, DMRTA1, and SFMBT1) were variants identified in previous atopic studies. These results are important to confirm the genetic mutation that may lead to the onset of foetal atopy due to maternal exposure to harmful environmental factors. Our results also suggest that a small-scale genome-wide association analysis is beneficial to confirm specific variants as direct factors in the development of atopy.

아토피 피부염 아동의 문제행동, 자아 역량 인식, 사회적 능력과 양육 스트레스와의 관계 (Behavioral Problem, Self-Perceived Competence, Social Competence, and Parental Stress in Children with Atopic Dermatitis)

  • Atopic dermatitis is the fastest growing skin disorder among children from infancy to adolescence in Korea. The side-effects of atopic dermatitis not only include physical discomfort, but also psychological trauma which ultimately affects the developmental growth of children. This study was conducted in order to investigate the behavioral characteristics of an atopic dermatitis sufferer. As part of this research, the relative influence of behavioral problems and, self-perceived competence were analyzed in relation to the social competence of an atopic child. In total, 301 atopic and non-atopic children, between 2 and 6 years of age, and their mothers and teachers participated in the study. From this number, 109 children had atopic dermatitis, while 192 children did not. Mothers were asked to complete a parent-report questionnaire that required information on parental stress, according to the scale parenting methodology of Abbdin(1990) and Cho(1999). Teachers were subjected to teacher-report questionnaires which included topics on social competence, socia-emotional assessment and behavioral problems of an atopic child. Furthermore, children also completed questionnaires on self-perceived competence. According to analysis on K-CBCL, ITSEA, social competence, self-perceived competence and parental stress, children with atopic dermatitis showed higher scores in depression/anxiety and depression/withdrawal, compared to children in the normal control group. In relation to parental stress, daily stress, parental role stress, and stress related disease, mothers with atopic dermatitis children exhibited higher scores. In establishing relationships among the related variables, atopic children who demonstrated more social competence were more likely to suffer less from withdrawal, attention problems, and depression/anxiety. Stress related disease in mothers with atopic children was positively related to attention problems of the child. In terms of relative influences, behavioral problems was the most significant variable, accounting for 23% of variance. Lower behavioral problems was positively related to more social competence. In summation, this study investigated the general characteristics of atopic children. In conclusion, atopic children and their mothers had difficulty in dealing with this disease. It is our belief that an atopic child would not only require physical treatment, but also need appropriate psychological care.
