Quality Assurance of Gastric Cancer Surgery (위암 수술의 질 관리)

  • Quality assurance may be defined as the complete set of systemic actions that is required to achieve a better treatment result by standardizing treatment and by using various audit programs. In general, application of a quality assurance program in surgery is considered to be more difficult than it is in chemotherapy or radiotherapy. However, recently, the importance of quality assurance in the surgical field has been emphasized in clinical trials comparing different surgical procedures and evaluating the role of postoperative adjuvant therapy. In the case of gastric cancer surgery, excellent quality assurance programs have rarely been applied in most large prospective clinical trials. Although the quality assurance in Dutch trial was conducted very systemically and strictly, the situation is quite different from ours. On the other hand, several quality assurance programs in Japanese trials comparing D2 and D2 plus para-arotic lymph node dissection seem to be applicable to Korean clinical trials. Several factors, including selection of appropriate surgeons based on personal experience and annual number of operations, standardization of surgical procedures by education and consensus, development of a unified database program, application of standardized perioperative management, and standardization of pathologic examination, are required to guarantee a successful multi-institutional prospective clinical trial. In contrast, one needs to realize that protocols that are too strict and sophisticated can make the enrollment of patients and surgeons more difficult and can promote protocol violation during the clinical trials.   • This report describes a preliminary study necessary to implement Measurement Assurance Program (MAP). The purpose of the study is to improve the measurement reliability of the laboratories which take environmental pollution measurement. We carried out the interlaboratory test or "Round-Robin" test program in which seven laboratories participated in order to obtain more reliable data about water pollution.pollution.

  • This study aimed to improve the reliability of the assurance pressure of a gun barrel due to the difference between the US Standard and Korean Standard. In addition, the reliability was found to differ according to the maximum pressure of the Ammunition, so restrictions are expected. During the development of the new bullet, the maximum pressure of the bullet was approximately 3,000 psi higher than the assurance pressure of the gun barrel. To solve this problem, the reliability of the cannon was analyzed when the assurance pressure of the gun barrel increased. First of all, the technical data from overseas were reviewed to check for cases of increased assurance pressure, and tests were performed to determine if it could withstand high pressure through a verification firing test. Finally, the simulation analyzed the stability of the recoil buffer. The study found no abnormal results in all items, suggesting that an increase in the assurance pressure for a gun barrel was possible. This study is expected to be used as basic data for future reliability studies of similar equipment.

A novel WOA-based structural damage identification using weighted modal data and flexibility assurance criterion

  • Structural damage identification (SDI) is a crucial step in structural health monitoring. However, some of the existing SDI methods cannot provide enough identification accuracy and efficiency in practice. A novel whale optimization algorithm (WOA) based method is proposed for SDI by weighting modal data and flexibility assurance criterion in this study. At first, the SDI problem is mathematically converted into a constrained optimization problem. Unlike traditional objective function defined using frequencies and mode shapes, a new objective function on the SDI problem is formulated by weighting both modal data and flexibility assurance criterion. Then, the WOA method, due to its good performance of fast convergence and global searching ability, is adopted to provide an accurate solution to the SDI problem, different predator mechanisms are formulated and their probability thresholds are selected. Finally, the performance of the proposed method is assessed by numerical simulations on a simply-supported beam and a 31-bar truss structures. For the given multiple structural damage conditions under environmental noises, the WOA-based SDI method can effectively locate structural damages and accurately estimate severities of damages. Compared with other optimization methods, such as particle swarm optimization and dragonfly algorithm, the proposed WOA-based method outperforms in accuracy and efficiency, which can provide a more effective and potential tool for the SDI problem.

  • Using micro-Raman spectrometer, we investigated the evaluation of microstress on silicon surface after the local thermal oxidation. The induced stress of silicon surface after local thermal oxidation shows maximum value at the interface of silicon oxide and active area. The smaller the size of active area, the larger stress. From the evaluation of three other device isolation processes, A, B and moB, whose active size has $0.45{\mu}m$ in length, moB process is turned out to have the lowest stress value and the smallest bird's beak effect.

  • Polysomnography is an essential methodology for diagnosing and following up sleep disorders and doing researches on human sleep. Sleep medicine, mainly with the utilization of polysomnographic techniques, has developed itself as one of the promising fields in the 21st century medicine. Korea is not an exception in importing and developing sleep medicine into the conventional medicine. However, it still remains to be clarified what polysomnography is for and how it should be done, considering the relatively recent introduction of sleep medicine into Korea. The author, being a board-certified sleep medicine specailist, having experienced spreading out sleep medicine within Korea for the past four years, and having recently set up a major sleep study facility in Korea at Seoul National University Hospital, attempts in this introductory critical article to review the essential issues related to quality assurance in polysomnographic study of human sleep. Also, unconditional introduction of "automated" sleep scoring system, which has been found to have significantly reduced reliability in various studies including the author's own, is critically reviewed. The author suggests that quality assurance and training program should be initiated and established by a responsible sleep medicine-related organization such as the Korean Association of Sleep Medicine and Psychophysiology.

Allocation of Design Assurance Level for KASS Based on International Standards (국제표준에 기반한 KASS 개발보증레벨 할당)

  • Since 2014, MOLIT (Ministry of Land, Infrastructure, and Transport) is carrying out a KASS project to develop and construct Korean SBAS. KASS can cause damage of human & properties if it has some problem during operation. Therefore, system safety assessment for KASS development is very important. Principal point of system safety assessment is the allocation of DAL(design assurance level) based on the hazard identification and classification. In this parer, the author conducts the allocation of DAL for KASS & its sub-systems based on the international standard(SAE ARP4761), which suggests a best practice of aviation system safety assessment. The result of this paper are the first step of system safety assessment, and can be used for further system safety assessment of KASS project.

  • This paper is presented to prepare NGN quality assurance management process under the quality system test methodology. The process should be drawn for NGN quality measurement framework of multimedia traffic. NGN test bed management process model are QoS measurement metrics, measurement interval meter above, and measuring tools, measuring equipment, measurement methods and measurement results from a series of processes for the analysis and methodology. This model, NGN quality assurance activities should be utilized in the future. Quality target level only when themselves constantly measured and managed, does not guarantee the communication quality of service. It is sensitive to the importance of NGN network technology paradigm for research on quality management in the NGN.

  • Recently, the top & down method with drilled shafts as a foundation of high rise building is often adopted for the purpose of construction period reduction and construction cost effectiveness. It is common to omit the loading test as a Quality assurance on account of the high capacity of drilled shafts for the top & down method. It seems that the capacity of drilled shaft in recent top & down method is beyond that of conventional loading test method.However, the quality assurance for the drilled shaft as foundation of high rise building becomes much more important since the drilled shaft should bear much higher working load. It may be a small scale test pile could be an alternative as a quality assurance for the drilled shaft with high capacities. Through a case study, this paper gives an idea for solving the limitation of the conventional loading test method for the quality assurance of drilled shaft with high capacities. In particular, this paper analyzed the scale effect for a small drilled shaft installed into bedrock, which could be used for an alternative.

Automatic Multileaf Collimation Quality Assurance for IMRT using Electronic Portal Imaging

  • More complex radiotherapy techniques using multi leaf collimation(MLC) such as intensity-modulated radiation therapy(IMRT) has been increasing the significance of verification of leaf position and motion. Due to the reliability and robustness, quality assurance(QA) of MLC is usually performed with portal films. However, the advantage of ease of use and capability of providing digital data of electronic portal imaging devices(EPIDs) have attracted many attentions as alternatives of films for routine quality assurance in spite of the concerns about their clinical feasibility, efficacy, and the cost to benefit ratio. In our work, the method of routine QA of MLC using electronic portal imaging(EPI) was developed. The verification of availability of EPI images for routine QA was performed by comparison with those of the portal films which were simultaneously obtained when radiation was delivered and known prescription input to MLC controller. Specially designed test patterns of dynamic MLC were applied to image acquisition. Quantitative off-line analysis using edge detection algorithm enhanced the verification procedure in addition to on-line qualitative visual assessment. In conclusion, the EPI is available enough for routine QA with the accuracy of portal films.
