중환자의 욕창 예방 연구 : 욕창 예방 QI팀을 중심으로 (CQI Action Team Approach to Prevent Pressure Sores in Intensive Care Unit of an Acute Hospital Korea)

  • Background : A pressure sore was defined as any skin lesion caused by unrelieved pressure and resulting in damage to underlying tissue. The health care institutions in the United States were reported the incident rate of pressure sores ranging from 6 to 14 %. Intensive Care Unit needed highest quality of care has been found over 40% incidence rate of pressure sore. Also, Annual expenditures for the care of pressure sores in patients in the United States have been estimated to be $7.5 billion; furthermore, 50 percent more nursing time is required to care for patients with pressure sore in comparison to the time needed to implement preventive measures against pressure sore formation. However, In Korea, there were little reliable reports, or researches, about incidence rates of pressure sore in health care institution including intensive care unit and about the integrated approach like CQI action team for risk assessment, prevention and treatment of pressure ulcers. Therefore, this study was to develop pressure sore risk assessment tool and the protocol for prevention of pressure sore formation through CQI action team activities, to monitor incident rate of pressure sore and the length of sore formation for patients at high risk, and to approximately estimate nursing time for sore dressing during research period as the effect of CQI action team. Method : CQI action team in intensive care unit, launched since early 1996, reviewed the literature for the standardized risk assessment tool, developed the pressure sore assessment tool based on the Braden Scale, tested its validity, compared on statistics including incidence rate of pressure sore for patients at high risk. Throughout these activities, CQI action team was developed the protocol, called as St. Marys hospital Intensive Care Unit Pressure Sore Protocol, shifted the emphasis from wound treatment to wound prevention. After applied the protocol to patients at high risk, the incident rate and the period of prevention against pressure development were tested with those for patients who received care before implementation of protocol by Chi-square and Kaplan-Meier Method of Survival Analysis. Result : The CQI action team found that these was significant difference of in incidence rate of pressure sores between patients at high risk (control group) who received care before implementation of protocol and those (experimental group) who received it after implementation of protocol (p<.05). 25% possibility of pressure sore formation was shown for the patients with 6th hospital day in ICU in control group. In experimental group, the patients with 10th hospital day had 10% possibility of pressure sore. Therefore, there was significant difference(p<.05) in survival rate between two groups. Also, nursing time for dressing on pressure sore in experimental group was decreased as much as 50% of it in control group. Conclusion : The collaborative team effort led to reduced incidence, increased the length of prevention against pressure sore, and declined nursing care times for sore dressing. However, there have had several suggestions for future study. The preventive care system for pressure sore should be applied to patients at moderate, or low risk throughout continuous CQI team activities based on Bed Sore Indicator Fact Sheet. Hospital-wide supports, such as incentives, would be offered to participants for keeping strong commitment to CQI team. Also, Quality Information System monitoring incidents and estimating cost of poor quality, like workload (full time equivalence) or financial loss, regularly in a hospital has to be developed first for supporting CQI team activities as well as empowering hospital-wide QI implementation. Being several limitations, this study would be one of the report cards for the CQI team activities in intensive care unit of an acute hospital and a trial of quality improvement of health care in Korea.

동종 조혈모세포 이식 전후 영양상태 평가 (Pre- and Post-Transplant Nutritional Assessment in Patients Undergoing Allogeneic Hematopoietic Stem Cell Transplantation)

  • Purpose: This study was performed to identify the pre-and post-transplant nutritional assessment for patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT). Methods: The subjects of this study were 25 patients undergoing allogeneic HSCT. The data collection was performed from January 31st to March 31st, 2011. The Patient-Generated Subjective Global Assessment (PG-SGA), anthropometrics and biochemical test were collected from the time they entered the isolation unit until they left. Results: Pre-transplant nutritional assessment status indicated moderate malnutrition which scored $7.32{\pm}1.68$ in PG-SGA. There were 22 patients (88.0%) with moderate malnutrition and 3 patients (12.0%) with severe malnutrition. Post-transplant nutritional assessment indicated severe malnutrition status which scored $11.92{\pm}3.26$ in PG-SGA. Pre-and post-transplant nutritional assessment displayed significant differences (p<.001) in PG-SGA score. Hematopoietic stem cell transplantation led to a deterioration of patients' nutritional status. Pre-transplant patients were already in malnutrition status and patients undergoing allogeneic HSCT were at risk for malnutrition. Conclusion: Pre-and post-transplant patients were categorized as having undernutritional and malnutritional status. Pre-transplant nutrition status impacted on post-transplant nutritional status. Health care personnel should pay attention to patient's nutrition status when undergoing allogeneic HSCT with appropriate nutritional assessment tools.

  • 우리나라 제7차 교육과정에서는 개별적 평가 외에 팀을 편성하여 팀 단위로 과제를 해결하도록 하는 집단적 평가 방법을 적용하여 문제를 해결하도록 유도하고 있다. 그러나 팀 단위의 협동적인 문제해결 과정을 통해 고등 사고력이나 사회적 기술 등을 기를 수 있도록 하는 팀 프로젝트에 대한 필요성은 증가하고 있는데 반해, 대다수의 교사들은 관리, 운영 등의 어려움을 이유로 팀 프로젝트 시행을 꺼려하는 경향이 있다. 따라서 본 논문에서는 팀 프로젝트 평가에 있어서의 현실적인 문제점을 인식하고, 교사의 팀 프로젝트 평가에 따른 부담을 덜어주면서 다면적인 평가를 통해 평가의 신뢰도와 타당도을 높이기 위해 자동 생성 웹 설문지를 이용한 팀 프로젝트 평가 시스템을 설계 및 구현하였다.

의료기관 환자안전과 환자중심성 간 관계 연구 (A Study on the Relationship between Patient Safety and Patient-Centeredness in Hospitals)

  • Purposes: This study purposed to analyze the relationship between patient safety and patient-centerendess. Methodology: The comprehensive scores from patient safety assessment program and patient experience survey conducted by Health Insurance Review & Assessment Service were used as independent variables and dependent variables. This study analyzed the relationship between 4 patient safety-related areas(i.e. risk standardized readmission ratio, intensive care unit, preventive antibiotic, the drug evaluation) and 6 patient experience areas(i.e. nurse services, doctor services, medication & treatment, hospital environment, patient's right, overall experience) by using robust regression analysis. Findings: According to results, the score in 'patient's right' and 'risk standardized readmission ratio' areas were found to have a significant relationship, and 'overall experience' and the 'preventive antibiotic' areas. The ratio of senior beds and specialists was a general characteristics of hospitals that had a significant relationship on patient experience assessment. Practical Implication: The relationships between patient safety and patient experience assessment were varied depending on areas. Further study is needed to make clear the supposed relationship.

초등 3~4학년군 국정 및 검정 과학 교사용지도서의 과학적 태도 분석: '지구와 우주' 영역을 중심으로 (The Analysis of Scientific Attitude in the Government-designated and Authorized Teacher's Guides of Elementary School Science for the 3rd~4th Grades)

  • 이 연구의 목적은 2015 개정 과학과 교육과정에 따라 개발된 3~4학년군 국정 및 검정 지도서 8종의 총론 그리고 각론의 지구와 우주 영역 4개 단원의 단원 목표, 단원 평가, 차시 목표 및 차시 평가에서 다루어진 과학적 태도의 행동 요소 분석을 통해 향후 지도서 개발을 위한 시사점을 도출하는 것으로, 연구 결과를 요약하면 다음과 같다: 첫째, 분석 대상 지도서 8종 중 5종(62.5%)이 총론에서 과학적 태도와 행동 요소에 대해 매우 간략하게 설명하고 있고 이에 대해 비교적 자세하게 설명한 지도서 3종도 이에 대한 지도 및 평가를 위한 실제적인 정보는 미흡한 편이다; 둘째, 지도서 각론 4개 단원의 단원 목표의 경우 지도서 5종(62.5%)은 호기심, 협동심, 객관성, 비판성, 증거의 존중 중 3~4가지 행동 요소를 포함하고 있고 그 나머지 지도서 3종(37.5%)은 행동 요소로 호기심만 포함하고 있거나 태도에 대한 내용 없이 지식과 탐구만을 포함하고 있거나 단원 목표 항목 자체가 없다; 셋째, 분석 대상 지도서 8종 중 1종(12.5%)만이 단원 평가에 해당하는 독자적인 항목을 포함하고 있고, 그 내용으로 과학적 태도의 행동 요소도 포함하고 있는데, 이 행동 요소는 단원 목표에서의 과학적 태도의 행동 요소와도 대체로 일치하는 편이다; 넷째, 차시 목표의 경우, 지도서 3종(37.5%)에는 호기심, 협동심, 객관성 및 비판성 중 3~4개의 행동 요소가 반영되어 있고 나머지 5종(62.5%)에는 호기심만 포함되어 있다; 다섯째, 차시 평가의 경우, 지도서 8종 중 5종(62.5%)에서 3~4가지 과학적 태도의 행동 요소가 평가되고 있고, 2종(25.0%)에는 호기심과 협동심 두 가지 행동 요소만 평가되고 있으며, 1종(12.5%)에는 전혀 평가되고 있지 않다.

  • The purpose of this study was to examine the current student assessment plan of subject domain and to propose ways to improve it. This study analyzed the history of our student assessment plan of subject domain and overviewed the student assessment cases of several countries. And then researchers discussed the improvement plans of student assessment of subject domain. The results of this study are as follows. First, we need to enlarge the use of detailed subject-ability record space or to assess student's subject-related ability according to subordinate perspectives of subject. Second, we need to establish the desirable evaluation-perspective and to develop absolute evaluation criteria of each subject and unit. Third, we need to diffentiate the scores of differentiated test items or equate the scores of differentiated groups.

An Array-Based Sensor for Seafood Freshness Assessment

  • This paper describes the development of an automated, hand-held sensor for the fast assessment of seafood freshness. The sensor developed here combined: an array-based chemical sensor, composed of incrementally different conducting polymer elements deposited on a small chip; a highly sensitive, custom-made electronics for the detection of very small signal changes; precise temperature control of the sensor chamber; and an on-board microcontroller for data collection, storage, automation, and analysis. The instrument was used to successfully test seafood samples with different degree of freshness and spoilage. A linear relationship between microbiological count and e-Nose signal for three different fish fillet was developed. Once the linear relationship is included into the hand-held unit software, the e-Nose signal can be used for assessment of seafood freshness without performing the microbiological count technique.

우리 나라의 도시생태계 평가제도 고찰 (Study on Assessment Institution of Urban Ecosystem in Korea)

  • 우리 나라의 도시생태계 평가제도는 도시계획법에 의해 시행되는 제도와 환경관련떱에 의해 시행된는 제도로 구분된다. 도시계획법에 의한 평가제도는 개발행위허가기준과 도시계획 환경성검토가 있으며, 환경관련법에 의해 시행되는 제도는 환경영향평가와 사전환경성검토가 있다. 도시생태계 평가제도는 그 동안 개발지향적인 토지이용정책으로 인해 체계적인 발달이 이루어지지 않았으며, 평가기법도 입목본수도 및 녹지자연도 등을 제외하고는 객관적인 평가기법이 개발되지 알았다. 또한 평가 결과 보전가치가 높은 지역에 대한 보전대책 및 계획과정과 사후에 대한 평가가 부족하다는 문제점을 가지고 있다 우리 나라의 도시생태계 평가제도가 활성화되기 위해서는 다음과 같은 개선방안 마련이 필요하다. (1) 비오톱지도와 같은 객관적인 도시생태계 평가기법 개발, (2) 평가결과 보전가치가 높은 지역에 대한 보전지구 지정 및 훼손에 대한 보상 방안 마련, (3) 도시개발계획과 생태계 보전계획 수립의 병행 및 사후평가 시행

역사발생적 원리에 따른 교수학습 모듈을 적용한 수행평가의 교수학적 효과 분석 (Pedagogical Effect of Learning-Teaching Module of Unit for the Logarithm According to Historico-Genetic Principle)

  • 본 연구에서는 지수의 역으로 정의되는 로그를 학습한 학생들을 대상으로 로그 개념과 성질에 대한 오류를 조사하고, 학생들 스스로 오류를 교정할 수 있도록 역사 발생적 수학 학습 지도 원리에 따른 교수-학습 모듈(module)을 개발하여 수행평가로 적용하고 그 교수학적 효과를 분석하였다. 교수-학습 모듈은 로그에 대한 수학적 분석, 역사발생적 과정에 대한 분석, 학생들의 오류에 대한 심리학적 분석을 바탕으로 로그의 정의와 기본 성질 단원을 중심으로 개발하였다. 교수-학습 모듈의 교수학적 효과는 로그의 뜻과 성질에 대한 사전 사후 지필 평가 결과의 정답률과 오류 발생률을 비교 분석하였다. 구체적으로 유의수준 .05에서 단일표본에 대한 t-검정 결과, 교수-학습 모듈을 적용한 수행평가가 짧은 시간동안 이루어졌음에도 효과가 있는 것으로 나타났다. 그리고 로그의 뜻과 성질 단원에서 나타난 5가지 오류 유형인 '기호나 용어, 개념에 대한 이해 부족으로 인한 오류(E1)', '정리나 성질에 대한 불완전한 이해로 인한 오류(E2)'와 '선행 지식의 부족으로 인한 오류(E3)', '계산 실수로 인한 오류(E4)', '풀이 과정의 중단으로 인한 오류(E5)' 중 교수-학습 모듈을 적용한 결과, E2와 E1 오류가 크게 개선되었다.

지역순환농업을 위한 분뇨단위 설정과 질소부하 평가 -여주지역 사례- (Assessment of N-Loading and Manure Units for Regional Recycling Farming -Case Study in Yeoju-Gun Region-)

  • In this study, the assessment of livestock manure nitrogen loading for recycling farming in Yeoju-Gun carried out comparing manure units based on the cultivation areas and the N-amount of manure that are generated from livestock manure. Manure units (MU) are used in the permitting, registration, because they allow equal standards for all animals based on manure nutrient production. An MU is calculated by multiplying the number of animals by manure unit factor for the specific type of animal. The manure unit factor for MU determination was determined by dividing amounts of manure N produced 80kg N/year. In this study, manure unit by nitrogen concentration and amount of animal manure was calculated as follows: Hanwoo multiplied by 0.36, dairy cows multiplied by 0.8. swine multiplied by 0.105. The laying hens and broilers multiplied by 0.0079, 0.0049, respectively. The analysis of liquid manure unit per ha shows that the N loading by LMU is quite different by region. When it comes to nitrogen loading, the LMU per ha of cultivated land in excess of the N-amount was the highest in the Bukne-myeon province with 2.76 MU/ha, which is higher than the appropriate level. The Ganam-myeon province came next with 2.53 LMU. To be utilized as a valid program to build the environmentally friendly agricultural system, diverse measures shall be mapped out to properly determine manure units, evaluate N-loading and to properly manage their nutrient balance of each region.
