Clinical Validation of a Protein Biomarker Panel for Non-Small Cell Lung Cancer

  • We validated the diagnostic performance of a previously developed blood-based 7-protein biomarker panel, $AptoDetect^{TM}$-Lung (Aptamer Sciences Inc., Pohang, Korea) using modified aptamer-based proteomic technology for lung cancer detection. Non-small cell lung cancer (NSCLC), 200 patients and benign nodule controls, 200 participants were enrolled. In a high-risk population corresponding to ${\geq}55years$ of age and ${\geq}30pack-years$, the diagnostic performance was improved, showing 73.3% sensitivity and 90.5% specificity with an area under the curve of 0.88. $AptoDetect^{TM}$-Lung (Aptamer Sciences Inc.) offers the best validated performance to discriminate NSCLC from benign nodule controls in a high-risk population and could play a complementary role in lung cancer screening.

  • The National Institute of Environmental Research (3rd KONEHs) conducted a survey of 280 residents in Ulsan, Suwon and Asan, and the concentration of heavy metals in the blood and urine were also analyzed. In case of blood lead concentration, Ulsan $2.27{\mu}g/dl$, Suwon $2.08{\mu}g/dl$, and Asan $1.75{\mu}g/dl$, the high peak in Ulsan, and the low peak in Asan. In case of cotinine concentration, Ulsan smoking($609.16{\mu}g/g{\_}ct$) is higher than nonsmoking($74.07{\mu}g/g{\_}ct$), as Suwon and Asan smoking($416.72{\mu}g/g{\_}ct$, $903.21{\mu}g/g{\_}ct$) is higher than nonsmoking($72.72{\mu}g/g{\_}ct$, $18.06{\mu}g/g{\_}ct$), smoking group is higher than nonsmoking group in all areas revealed statistically significant correlation(p<0.01). In considering results synthetically, these study results are an aid to constructing environmental health science-side heavy metal management measure education programs for normal residents.

Tuberculous Aortitis Complicated with Pseudoaneurysm Formation in the Descending Thoracic Aorta: A Case Report

  • A 51-year-old male with sustained fever was diagnosed with military tuberculosis and tuberculous aortitis complicated with pseudoaneurysm formation at the proximal descending aorta. A follow-up computed tomography evaluation showed an increased size of the pseudoaneurysm in this area, suggestive of a contained rupture. Consequently, the patient underwent emergency excision and replacement of the aorta using a left heart bypass. The patient was discharged without postoperative complications on post-operative day 12. During the one-year follow-up period, the patient was free of any complications or recurrence of tuberculosis. We report a case of pseudoaneurysm of the descending aorta that was successfully surgically repaired.

Mondor's Disease Developed after Ultrasound-assisted Liposuction for Treatment of Axillary Bromhidrosis

  • Mondor's disease is an uncommon condition characterized by a palpable, cord-shaped structure, which causes pain when pressed. Its known pathophysiology is thrombophlebitis of the superficial venous system. Although reported repeatedly, its definite cause is unknown and various possible causes have been identified, including surgery, irradiation, infection, malignancy, and trauma. We diagnosed this case to be Mondor's disease of the antecubital venous system, probably due to thermal injury of the proximal tributaries of the basilic or cephalic vein. Risk of thermal injury to the skin flap or the portal site remains a common complication, and as thermal injury to the blood vessel might also be considered, attention must be given when suctioning the area near a large superficial vessel.

  • Purpose: Mondor's disease is commonly known as a benign breast condition after augmentation mammaplasty, and some authors have also reported its association with other breast surgeries such as reduction mammaplasty or axillary lymph node biopsy. Here we report two cases of Mondor's disease after immediate breast reconstruction with silicone implant. Methods: Two women, 51-year-old and 36-year-old, underwent immediate breast reconstruction with silicone implants after nipple-areolar skin-sparing mastectomy. Results: Subcutaneous cord-like firm lesion appeared on upper abdomen, axillary area following surgery. The lesion was painless and spontaneously subsided with no medications. Conclusion: To our knowledge, this is the first report of Mondor's disease developed after immediate breast reconstruction using silicone implant.

A Study on the Molluscan Fauna in the Freshwater of Asan Area (아산시 일대 담수역의 연체동물상)

  • In order to understand freshwater molluscan fauna in Asan area, a survey was conducted on the watersheds of this area during the period of October 1999 to August 2000. For the purpose of easy overlook of the molluscan fauna in Asan area, present survey was undertaken on a total of 101 sampling sites of 4 major watersheds connected along Onyang River, Gokgyo River, Sapgyo Lake, and Asan Lake. The collected freshwater mollusks were analysed based on the environment of their habitats. The freshwater mollusks collected through out the present survey were 27 species, 12 families, 5 orders, and 2 classes. Of these, gastropods were 18 species, 10 families, 3 orders; and bivalves were 9 species, 2 families, and 2 orders. The dominant gastropods in this area were Radix auricularia coreana, Cipangopaludina chinensis malleata, Hippeutis cantori, Physa acuta, Gyraulus convexiusculus and Austropeplea ollula and the dominant bivalves were Anodonta arcaeformis, Anodonta arcaeformis flavotincta, and Anodonta woodiana. Dominant species in rice fields were Fossaria truncatula, Segmentina hemisphaerula and Physa acuta, dominant species in rivers were Radix auricularia coreana, Physa acuta, and Hippeutis cantori. Dominant species in reservoirs were Cipangopaludina chinensis malleata, Radix auricularia coreana, Austropeplea ollula, and Fossaria truncatula. Dominant species collected in lakes were Cipangopaludina chinensis malleata, Hippeutis cantori, Cipangopaludina japonica, and Radix auricularia coreana. Radix auricularia coreana, Cipangopaludina chinensis malleata, and Austropeplea ollula were dominantly inhabiting on the muddy bottoms. Anodonta woodiana, Cipangopaludina japonica, and Corbicula fluminea occurred mainly in the gravel areas. Radix auricularia coreana, Cipangopaludina chinensis malleata, and Hippeutis cantori were dominant in the muddy rock areas. Rock-dominant species were Radix auricularia coreana, Semisulcospira forticosta and Koreanomelania paucicincta. Sand-dominant species were Physa acuta, Radix auricularia coreana, and Hippeutis cantori. Sand gravel-dominant species were Physa acuta, Radix auricularia coreana, and oreanomelania paucicincta. Hippeutis cantori, Cipangopaludina chinensis malleata, and Gyraulus convexiusculus occurred in the sand-muddy areas. Cristaria plicata, appointed as an endangered species from the "Natural Environmental Conservation Law", was collected from 5 sampling sites (site Nos. 45, 47, 48, and 52). For the endangered species found in this survey should be make appropriate protection.

Feasibility of Reflecting Improvement of Tumor Hypoxia by Mild Hyperthermia in Experimental Mouse Tumors with $^18F-Fluoromisonidazole$ (저온온열치료에 의한 종양 내 저산소상태 개선효과를 $^18F$-Fluoromisonidazole의 섭취 변화를 이용한 평가)

  • Puporse: The aims of this study were to evaluate the change of $[^18F]fluoromisonidazole$($[^18F]FMISO$) uptake in C3H mouse squamous cell carcinoma-VII (SCC-VII) treated with mild hyperthermia ($42^{circ}C$) and nicotinamide and to assess the biodistribution of the markers in normal tissues under similar conditions. Methods and Materials: $[^18F]FMISO$ was producedby our hospital. Female C3H mice with a C3H SCC-VII tumor grown on their extremities were used. Tumors were size matched. Non-anaesthetized, tumor-bearing mice underwent control or mild hyperthermia at $42^{circ}C$ for 60 min with nicotinamide (50 mg/kg i.p. injected) and were examined by gamma counter, autoradiography and animal PET scan 3 hours after tracer i.v. injected with breathing room air, The biodistribution of these agents were obtained at 3 h after $[^18F]FMISO$ injection. Blood, tumor, muscle, heart, lung, liver, kidney, brain, bone, spleen, and intestine were removed, counted for radioactivity and weighed. The tumor and liver were frozen and cut with a cryomicrotome into 10- um sections. The spatial distribution of radioactivity from the tissue sections was determined with digital autoradiography. Results: The mild hyperthermia with nicotinamide treatment had only slight effects on the biodistribution of either marker in normal tissues. We observed that the whole tumor radioactivity uptake ratios were higher in the control mice than in the mild hyperthermia with nicotinamide treated mice for $[^18F]FMISO$ ($1.56{\pm}1.03$ vs. $0.67{\pm}0.30$; p=0.063). In addition, autoradiography and animal PET scan demonstrated that the area and intensity of $[^18F]FMISO$ uptake was significantly decreased. Conclusion: Mild hyperthermla and nicotinamide significantly improved tumor hypoxia using $[^18F]FMISO$ and this uptake reflected tumor hypoxic status.

Prospective validation of a novel dosing scheme for intravenous busulfan in adult patients undergoing hematopoietic stem cell transplantation

  • The objective of this study was to externally validate a new dosing scheme for busulfan. Thirty-seven adult patients who received busulfan as conditioning therapy for hematopoietic stem cell transplantation (HCT) participated in this prospective study. Patients were randomized to receive intravenous busulfan, either as the conventional dosage (3.2 mg/kg daily) or according to the new dosing scheme based on their actual body weight (ABW) ($23{\times}ABW^{0.5}mg\;daily$) targeting an area under the concentration-time curve (AUC) of $5924{\mu}M{\cdot}min$. Pharmacokinetic profiles were collected using a limited sampling strategy by randomly selecting 2 time points at 3.5, 5, 6, 7 or 22 hours after starting busulfan administration. Using an established population pharmacokinetic model with NONMEM software, busulfan concentrations at the available blood sampling times were predicted from dosage history and demographic data. The predicted and measured concentrations were compared by a visual predictive check (VPC). Maximum a posteriori Bayesian estimators were estimated to calculate the predicted AUC ($AUC_{PRED}$). The accuracy and precision of the $AUC_{PRED}$ values were assessed by calculating the mean prediction error (MPE) and root mean squared prediction error (RMSE), and compared with the target AUC of $5924{\mu}M{\cdot}min$. VPC showed that most data fell within the 95% prediction interval. MPE and RMSE of $AUC_{PRED}$ were -5.8% and 20.6%, respectively, in the conventional dosing group and -2.1% and 14.0%, respectively, in the new dosing scheme group. These findings demonstrated the validity of a new dosing scheme for daily intravenous busulfan used as conditioning therapy for HCT.

Extracorporeal Cardiopulmonary Resuscitation: Predictors of Survival

  • Background: The use of extracorporeal life support (ECLS) in the setting of cardiopulmonary resuscitation (CPR) has shown improved outcomes compared with conventional CPR. The aim of this study was to determine factors predictive of survival in extracorporeal CPR (E-CPR). Methods: Consecutive 85 adult patients (median age, 59 years; range, 18 to 85 years; 56 males) who underwent E-CPR from May 2005 to December 2012 were evaluated. Results: Causes of arrest were cardiogenic in 62 patients (72.9%), septic in 18 patients (21.2%), and hypovolemic in 3 patients (3.5%), while the etiology was not specified in 2 patients (2.4%). The survival rate in patients with septic etiology was significantly poorer compared with those with another etiology (0% vs. 24.6%, p=0.008). Septic etiology (hazard ratio [HR], 2.84; 95% confidence interval [CI], 1.49 to 5.44; p=0.002) and the interval between arrest and ECLS initiation (HR, 1.05 by 10 minutes increment; 95% CI, 1.02 to 1.09; p=0.005) were independent risk factors for mortality. When the predictive value of the E-CPR timing for in-hospital mortality was assessed using the receiver operating characteristic curve method, the greatest accuracy was obtained at a cutoff of 60.5 minutes (area under the curve, 0.67; 95% CI, 0.54 to 0.80; p=0.032) with 47.8% sensitivity and 88.9% specificity. The survival rate was significantly different according to the cutoff of 60.5 minutes (p=0.001). Conclusion: These results indicate that efforts should be made to minimize the time between arrest and ECLS application, optimally within 60 minutes. In addition, E-CPR in patients with septic etiology showed grave outcomes, suggesting it to be of questionable benefit in these patients.

Investigation into the Actual State of Sanitary Management and Recognition Degree and Infection Level of Ultrasonographic Probes (초음파 탐촉자(Probes)의 위생관리 실태와 감염 인식도 조사 및 세균 오염도 측정)

  • The gel, which is stained on probe after ultrasonography, is a good circumstances for proliferation of microbe. This study is to investigate into the actual state of sanitary management, recognition degree and infection level of ultrasonographic probes. We had performed a question with telephone to 42 hospitals in Seoul area from December in 2003. We also cultured to obtained a sample from three ultrasonographic units to investigate infection level of the probes. Sanitary management of the probes was performed in 21 hospitals with alcohol cotton. Sanitary management was performed daily in 14 hospitals. Most hospitals used cotton towel for clearing of gel stained on probes. Preventive management against infection was performed in 32 hospitals with vinyl cover, surgical glove, or alcohol sterilization etc. In the recognition degree on infection, the response that using method of ultrasonographic probes is insanitary were in 78.6%(33 hospitals), and 54.8%(23 hospitals) responded that bacteria can be infected through the probes. In the results of germiculture, bacteria and fungi were detected too number of to count, but escherichia coli was not detected. In conclusion, The gel stained on probe after ultrasonography must be cleared completely, and it is necessary that change of recognition on sanitary management.
