임플란트 표면에 따른 인공치면세균막 형성에 관한 연구 (THE STUDY OF THE DENTAL PLAQUE FORMATION ON DIFFERENT IMPLANT SURFACES)

  • The purpose of this study was to evaluate the artificial dental plaque by Streptococcus mutans on 4 different implant surfaces. In this study, the specimens were divided into 4 groups according to implant surface treatment. Uncoated implant group(n=5) which has an uncoated, smooth surfaced implant(Osstem, Korea), SLA implant group(n=5) which has an sandblasted large grit and acid-etched surface implant(Bicon, USA). Oxidized implant group(n=5) which has an oxidized surfaced implant (Osstem, Korea), and RBM implant group(n=5) which has resorbable blasting media(RBM) surfaced implant(Osstem, Korea). Acquired pellicle by human saliva and dental plaque by Streptococcus mutans were made on each implant surface. To analyze the plaque condition on implants surfaces, cell count and optical density were taken as a microbiologic method, and SEM(Scanning Electronic Microscope) findings was also taken for evaluation of surface condition. The following results were obtained. 1. Cell counting results of artificial dental plaque were Uncoated group($658.0{\pm}102.0$), RBM group($878.0{\pm}170.0$), SLA group ($946.0{\pm}42.0$), Oxidized group($992.0{\pm}40.0$), and there was difference between Oxidized group and Uncoated implant group(p<0.05). In case of modified cell counting results by v/w% were RBM group($197.8{\pm}45.2$), Oxidized group($207.04{\pm}8.34$), Uncoated group($261.6{\pm}40.6$), SLA group($315.4{\pm}14.0$), and there was difference between RBM group and SLA group(p<0.05). 2. Optical density results of artificial dental plaque after ultrasonic treatment was that there was difference among groups, and optical density of RBM group was higher than that of Uncoated group(p<0.05). In case of modified optical density results by v/w%, there was difference among groups, and the modified optical density of Uncoated group and SLA group was higher than those of Oxidized group and RBM group(P>0.05). 3. SEM findings of artificial dental plaque on the surfaces of implant as follows; there were artificial dental plaque on the surfaces of all test implants. Streptococcus mutans and by-product were observed at 10,000 times magnified condition on all test implants. Adhesion area of artificial dental plaque was about 1/2 of total surface after 24 hours incubate at $37^{\circ}C$. These results showed that there were differences among implant surfaces on the growth of Streptococcus mutans, and bacteria and by-product were covered about 1/2 area of total implant surfaces at 24 hours incubate at $37^{\circ}C$.

생체유사환경 하의 치과용 임플란트의 피로강도 평가 및 수명 향상법 (Fatigue Strength of Dental Implant in Simulated Body Environments and Suggestion for Enhancing Fatigue Life)

  • 치과용 임플란트를 대상으로 하여 건전환경 및 생체 내의 인자들을 유사하게 반영한 생체유사환경 하에서 피로시험을 수행하였다. 생체유사환경으로는 링거액 환경과 인공타액환경을 각각 사용하였다. 우선 건전환경과 생체유사환경 하에서의 피로수명 및 피로한도를 평가하였다. 또한 피로수명을 향상시킬 수 있는 한 가지 방법을 제안하였다. 인공타액 하에서의 피로수명이 링거환경보다 대체적으로 감소하였다. 시편에 대한 부식작용은 인공타액이 링거액보다 훨씬 크다. 이러한 부식의 차이가 두 경우의 피로수명의 차이로 나타난 것으로 판단된다. 질화처리된 지대주 나사의 질화경화층을 제거하고, 질소확산층을 최표면으로 하는 지대주 나사를 사용한 임플란트의 피로수명은 텅스텐카바이드를 코딩한 기존 제품에 비해 최대 8배의 피로수명 향상효과가 확인되었다.

키토산으로 표면처리된 인공치아의 충격전달에 관한 연구 (THE EFFECT OF A CHITOSAN COATING OF DENTAL IMPLANT ON THE SHOCK ABSORPTION UNDER IMPACT TEST)

  • With the object of providing a temporary artificial periodonal ligament-like membrane around the dental implant, 10 Branemark type implants were coated with commercially available chitosan(Fluka Co., Buchs, Switzerland) which has a molecular weight of 70,000 and 80% deacetylation degree. Once this bioactive hydrophillic polymer(chitosan) contacts with blood or wound fluids, it becomes swollen and penetrates into the adjacent cancellous bone. Thus the interface between implant and surrounding bone is completely filled with chitosan. This tight junction in early healing phase enhances primary stability. The chitosan coated dental implants were implanted into the fresh patella bones from porcine knees, since the thickness of cortical bone is relatively even and their cancellous structure is homogenous. To test the shock absorbing effect, 1mm delta-rogette strain gage was installed behind the implant. The results showed 1. The principal strain peak value directed to the impact of coated implant was 0.064 0.018(p<0.05) and that of uncoated implant was 0.095(0.032 p<0.05). 2. The peak time delay of coated implant was 0.056sec(0.011 p<0.05) and that of uncoated implant was 0.024sec(0.009 p<0.05). It can be reasoned from this results that the chitosan coating has a shock absorbing effect comparable with a temporary artificial periodontal ligament.

추간판 치환술이 요추분절에 미치는 생체역학적 영향 : 유한요소해석 (Biomechanical Effect of Total Disc Replacement on Lumbar Spinal Segment : A Finite Element Analysis)

  • The artificial discs have recently used to preserve the motion of the treated segment in lumbar spine surgery. However, there have been lack of biomechanical information of the artificial discs to explain current clinical controversies such as long-term results of implant wear and excessive facet contact forces. In this study, we investigated the biomechanical effects of three artificial implants on the lumbar spinal segments by finite element analysis. The finite element model of intact lumbar spine(L1-S) was developed and the three implants were inserted in L4-L5 segment of the spine model. 5 Nm of flexion and extension moments were applied on the superior plate of L1 with 400 N of compressive load. Excessive motions and high facet contact forces at the surgical level were generated in the all three implanted models. In the flexion, the peak von-Mises stresses in the semi-constrained type implant was higher than those in the un-constrained type implant which would cause wear on the polyethylene core. The results of the study would provide a biomechanical guideline for selecting optimal surgical approach or evaluating the current design of the implants, or developing a new implant.

고관절용 인공관절 시스템의 안정성 평가 (Stability Test of Artificial Joint for Hip Joint)

  • 인공관절은 관절부의 뼈가 파괴되거나 관절 손상을 입어 관절로서의 기능을 할 수 없게 된 경우에 사용된다. 고관절용 인공관절 시스템의 일차안정성을 평가하기 위해서는 인공관절이 삽입된 직후 인공관절에 반복하중을 가하여 대퇴골과 인공관절 사이의 상대변위를 측정하게 된다. 이때, 정확한 안정성을 평가하기 위해서는 인공관절이 대퇴골을 파고드는 변위와 대퇴골에서 인공관절이 회전한 각의 변화량을 정확히 측정하여야 한다. 본 연구에서는 고관절용 인공관절 시스템의 일차안정성을 평가하기 위해 새로운 상대 변위측정 방식을 제안하고 이를 이용하여 수술 방법의 차이 및 대퇴골 자체 강성의 차이에 따른 일차 안정성을 사체에서 얻은 대퇴골 실험을 통해 비교 평가하였다.

RBM 처리된 임플란트 표면의 인공치태 제거 효과 연구 (COMPARATIVE STUDY OF REMOVAL EFFECT ON ARTIFICIAL PLAQUE FROM RBM TREATED IMPLANT)

  • Purpose: This study was to evaluate the removal effect on artificial plaque from RBM treated implant surfaces that are exposed due to peri-implantitis. Materials and methods: Artificial plaque with Streptococcus mutans and acquired pellicle adhered to RBM treated implant discs. Study materials divided into one control and six test groups. In test groups, physical and chemical methods used to remove plaques. Prophyflex, Professional Mechanical Tooth Cleaning (PMTC) and interdental brush as mechanical treatments and 0.1% Chlorhexidine, Citric acid, HCl tetracycline as a chemical treatment were used. To analyses the study, disc weight was measured for remaining plaque quantities and SEM(Scanning Electronic Microscope) findings was taken for evaluation of surfaces. Results: 1. In weight changes, there was significant difference between each treatment group and the control group (p<0.05). Therefore all treatment methods using this study have good ability for remove plaques. 2. In weight changes, there was no significant difference between mechanical and chemical group, and there were no significant differences between each groups (p>0.05). 3. SEM findings after mechanical treatment disclosed as follows; Prophyflex group looked like sound implant surface, and there were some paste on implant surface at PMTC group, and there were some artificial plaque at interdental brush group. 4. SEM findings after chemical treatment disclosed as follows; there were some dark lesions which were supposed as the product from Streptococcus mutans at Chlorhexidine, Citric acid and HCl tetracycline groups. Conclusion: All six methods using in this study have good ability to remove artificial plaque on RBM treated implant. According to SEM findings, prophyflex is a superior method for removing of dental plaque among test groups.

다양한 유사체액과 인공타액에서 치과용 임플란트의 전기화학적 특성 (Electrochemical Characteristics of Dental Implant in the Various Simulated Body Fluid and Artificial Saliva)

  • Titanium and its alloy have been widely used in dental implant and orthopedic prostheses. Electrochemical characteristics of dental implant in the various simulated body fluids have been researched by using electrochemical methods. Ti-6Al-4V alloy implant was used for corrosion test in 0.9% NaCl, artificial saliva and simulated body fluids. The surface morphology was observed using scanning electron microscopy (SEM) and energy dispersive x-ray spectroscopy (EDX). The electrochemical stability was investigated using potentiosat (EG&G Co, 263A). The corrosion surface was observed using scanning electron microscopy (SEM). From the results of potentiodynamic test in various solution, the current density of implant tested in SBF and AS solution was lower than that of implant tested in 0.9% NaCl solution. From the results of passive film stability test, the variation of current density at constant 250 mV showed the consistent with time in the case of implant tested in SBF and AS solution, whereas, the current density at constant 250mV in the case of implant tested in 0.9% NaCl solution showed higher compared to SBF and AS solution as time increased. From the results of cyclic potentiodynamic test, the pitting potential and |$E_{pit}\;-\;E_{corr}$| of implant tested in SBF and AS solution were higher than those of implant tested in 0.9% NaCl solution.

디지털 복제로 만든 맞춤형 인공치로 제작한 임시의치와 임플란트 수술용 템플릿을 통한 임플란트 지지형 고정성 보철 수복 증례 (Rehabilitation with implant-supported fixed dental prostheses using digital duplication technique on customized artificial tooth, interim denture and implant surgical template: A case report)

  • 성공적인 임플란트 치료를 위해서는 식립부위의 골조직 및 연조직 상태(condition)도 중요하지만 식립 위치 또한 매우 중요하다. 잘못된 위치에 식립된 임플란트는 생물학적, 심미적, 기계적 문제점을 야기하게 된다. 올바른 위치에 임플란트를 식립하기 위해서는 최종 보철물을 고려해야 하며 이를 위해서는 진단 왁스업 과정이 필요하다. 발치 후 사용 할 임시 의치의 인공치를 진단 왁스업 형태대로 제작하면 구강내에서 진단 왁스업 형태를 평가해 볼 수 있으며, 잘 적응된 임시 의치를 복제해 제작한 임플란트 수술용 템플릿으로 계획된 위치에 임플란트를 식립하는 것이 가능하다. 본 증례에서는 진단 왁스업을 디지털 복제하여 의치용 인공치를 맞춤형으로 제작하였고, 이를 이용해 임시 의치를 제작하였다. 충분한 적응 기간을 거친 후 임시 의치를 복제하여 임플란트 수술용 템플릿을 제작한 다음, 이를 활용하여 계획된 위치에 임플란트를 식립하였다. 골유착을 위한 충분한 치유기간을 가진 다음 진단 왁스업 형태가 반영된 임플란트 지지형 고정성 보철물을 제작하였다. 최종 보철물 장착 후 6개월간의 경과 관찰 기간 동안 심미적, 기능적으로 만족할 만한 치료 결과를 보였기에 이를 보고하고자 한다.

치과 임플란트 국민건강보험 급여화 이후 노인의 치과 임플란트 이용에 대한 예측 모형: 사회경제적 요인 중심으로 (Prediction model for dental implants utilization in the elderly after the national health insurance coverage of dental implants: focusing on socioeconomic factors)

  • Objectives: The demand for dental care is expected to increase as the population ages. This study aimed to predict the utilization of dental implant care following the expansion of national health insurance benefits for dental implants. Methods: Multiple linear regression analysis was performed on HIRA big data open portal data and DNN-based artificial intelligence models to forecast the utilization of dental care in relation to the national health insurance coverage for dental implants. Results: National health insurance coverage of dental implants was found to be associated with the number of patients using dental implant services and demonstrated a statistical significance. The dental implant services utilization increased with the increased dental implant health insurance benefits for the elderly population, increased mean by region, increased number of dental institutions by region, and increased health insurance coverage rate for dental implants. However, the dental implant services utilization decreased with the increased number of older people living alone and increased size of dental institutions. Conclusions: With the expansion of the national health insurance coverage for dental implants, it is predicted that the utilization of dental implant medical services will increase in the future.

상악골 부분 절제술 시행한 환자에서 Artificial Palate로 안모지지를 재현한 폐색장치를 이용한 수복 증례 (Use of artificial palate for improving facial support in the fabrication of a maxillary obturator: A case report)

  • 상악골 부분 절제술을 시행한 많은 환자에서 안모의 변형으로 심리적 우울감에 취약할 뿐만 아니라 연하, 저작 기능이 저하되며 발음 장애가 발생한다. 폐색장치를 통한 악안면 보철적 치료는 저작, 연하, 발음 기능의 회복뿐 아니라 적절한 안모로 회복시켜주며 장치를 사용하는 환자들의 삶의 질을 향상해준다. 본 증례는 상악골 부분 절제술 시행한 환자에서 수술 부위 안정화 기간 사용한 임시 폐색장치로 적절한 안면 지지의 정도를 예측할 수 있었고 이를 반영하여 artificial palate를 제작하였다. 이는 적절한 안면 지지를 재현하여 최종 폐색장치를 제작하는 간단한 접근법을 제시하였고 기능적, 심미적 부분에서 만족할 만한 결과를 얻어 이를 보고하고자 한다.
