A New Impression Method Taken Simultaneously for the Complete Dentures (상하악 동시인상에 의한 새로운 총의치 제작법)

  • The movements of oral surrounding muscles, lips, and tongue play a very important role while masticating, swallowing, and speaking. In case of the edentulous patient wearing dentures as well, every movement of the tongue can be seen almost the same as that of the dentulous jaw. The crushed food during mastication is swallowed and sent to pharynx. At this course, the contact of the posterior portion of tongue with dentures affects the stabilization of denture. The retention and stability of the complete dentures are related with buccinator muscles of buccal parts, lips in front, and tongue of the lingual sides. Strack(1946) insisted that the arrangement of artificial teeth in the same place as that of the natural dentition is the best way to maintain the stability of denture. In this report, I would like to introduce the several advantages of a new impression method taken simultaneously, and its procedures and clinical cases.

Analysis of Korean and Foreign Special Effects Make-up in Representative Films by Period

  • The purpose of this study is to gather basic data in the special effects make-up field and to provide a source of inspiration for creative make-up design by examining how special effects make-up is developed according to representative films. Two approaches are used in this study: a literature review and an empirical case analysis. In Korea, color TV was introduced in the 1980s, with the demand for a realistic image. Since the 2000s, 3D printing technology has been introduced to special effects make-up. Special effects make-up was first introduced in Hollywood films in the 1890s, and by the 1900s, artificial teeth and eyebrows were added and presented using general make-up techniques. Since then, 3D and 4D technologies and animation performance capture methods have been introduced and distributed due to the rapid development of computer technology.

  • Proponents of bilaterally balanced occlusion believe that this occlusal concept is important for denture retention and stability. However, the critical appraisal of the literature reveals no scientific evidence to support the bilaterally balanced occlusion as the ideal occlusal concept for conventional complete dentures. Current evidence suggests that the occlusal concept has little influence on clinical outcomes or patient satisfaction. Additional randomized controlled clinical trials should be developed taking into account the influence of mucosal resiliency, alveolar ridge anatomy, and parafunctional activities on occlusal concept choice. In this paper, various types of complete denture occlusion (bilaterally balanced occlusion, lingualized occlusion, canine-guided occlusion) will be reviewed and compared.

Development of Root Perforation Filling Material using Demineralized Dentin Paste (치아들을 탈회동결건조처리하여 제작한 치근천공부 충전재 개발 I: 예비실험연구)

  • Purpose : This study was performed to evaluate the healing response around the root perforation restorative material. Materials and Methods : Four beagle dogs were used for experimental study. Endodontic treatment was performed at four maxillary premolars and artificial perforation was formed at furcation area of pulp chamber. Canal was filled with gutta percha cone and the perforation was sealed with MTA at group 1. At group 2, canal was filled and the perforation was sealed with dentin paste. Tooth paste was fabricated using extracted human teeth. Histologic examination of furcation area was performed 2, 4, 8 and 12 weeks after experiment. Results : New trabecular bone formation was observed around the MTA and tooth paste. Lamellar bone was observed as time is over. There were no inflammatory reaction in both groups. Conclusion : There is a possibility which endodontic filling material can be developed using extracted teeth.

  • Prosthodontic decision and management should be considered the patient's aesthetic need for anterior teeth in maxillary edentulism. This case report describes 10-year clinical outcome of a maxillary edentulous patient was rehabilitated by means of an implant-supported fixed dental prosthesis (ISFDP) for the anterior region and a distal-extension removable partial denture (RPD) for the posterior region. The ISFDP was designed to splint 4 endosseous implants as one unit and engaged by the RPD. The edentulous mandible was restored by means of an implant-supported overdenture. Over the course, no major adverse event occurred other than wear, discoloration, and breakage of resin veneers and artificial teeth.

Diagnosis and prediction of periodontally compromised teeth using a deep learning-based convolutional neural network algorithm

  • Purpose: The aim of the current study was to develop a computer-assisted detection system based on a deep convolutional neural network (CNN) algorithm and to evaluate the potential usefulness and accuracy of this system for the diagnosis and prediction of periodontally compromised teeth (PCT). Methods: Combining pretrained deep CNN architecture and a self-trained network, periapical radiographic images were used to determine the optimal CNN algorithm and weights. The diagnostic and predictive accuracy, sensitivity, specificity, positive predictive value, negative predictive value, receiver operating characteristic (ROC) curve, area under the ROC curve, confusion matrix, and 95% confidence intervals (CIs) were calculated using our deep CNN algorithm, based on a Keras framework in Python. Results: The periapical radiographic dataset was split into training (n=1,044), validation (n=348), and test (n=348) datasets. With the deep learning algorithm, the diagnostic accuracy for PCT was 81.0% for premolars and 76.7% for molars. Using 64 premolars and 64 molars that were clinically diagnosed as severe PCT, the accuracy of predicting extraction was 82.8% (95% CI, 70.1%-91.2%) for premolars and 73.4% (95% CI, 59.9%-84.0%) for molars. Conclusions: We demonstrated that the deep CNN algorithm was useful for assessing the diagnosis and predictability of PCT. Therefore, with further optimization of the PCT dataset and improvements in the algorithm, a computer-aided detection system can be expected to become an effective and efficient method of diagnosing and predicting PCT.

  • This study was designed to investigate the stress distribution, intensity and force mechanism derived from the MEAW by photoelastic stress analysis of the artificial teeth and surrounding bone composed of photoelastic material(PL-3) The findings of this study were as follows, 1. In case of no elastic on the MEAW with tip back, the moderate stress was observed on the molar and canine area, and the light stress was observed on the other area. 2. In case of the vertical elastic on the plain A.W, and the MEAW without tip back, the great stress was observed on the lateral incisor area, but on the MEAW with tip back, the moderate stress was observed on the anterior area and molar area. 3. In case of the C III elastic on plain A.W., the stress was concentrated on the anterior area hanged by elastic but on the MEAW without tip back, the stress was transmitted equally from the anterior area to the posterior teeth area. On the MEAW with tip back, the great stress was observed on the anterior and molar area. 4. In case of the C III elastic on the plain A.W., the stress was concentrated on the posterior area hanged by elastic but on the MEAW without tip back, the stress was transmitted equally from the posterior area to the anterior area. On the MEAW with tip back, the great stress was observed on the posterior area and the moderate stress was observed on the anterior area.

Implant hybrid prosthetic treatment in Down syndrome patient: a case report (다운증후군 환자에서 임플란트 하이브리드 보철 치료 증례)

  • Down syndrome patients generally have poor oral hygiene due to lack of awareness and ability to perform oral care, and there is a high risk of tooth loss. Also, they are characterized by small and irregular teeth and oligodontia, which makes it difficult to do prosthetic treatment. This article reports a case of implant hybrid prosthesis treatment of maxillary edentulous area in Down syndrome patient. The external shape of the final prosthesis was determined by using the mandibular artificial teeth to reflect the oligodontia of the mandible and the pontic areas were adjusted to facilitate oral hygiene care. As a result, aesthetically and functionally satisfactory results were obtained.

Neutral zone approach for rehabilitation of severely atrophic maxillary and mandibular ridges: A case report (치조제 흡수가 심한 환자에서 중립대를 이용한 총의치 수복 증례)

  • The neutral zone technique is an alternative approach for the construction of complete dentures on highly atrophic ridges with history of denture instability. This technique achieves two objectives. First, the teeth will not interfere with the normal muscle function, and second, the force exerted by the musculature against the denture is more favorable for stability and retention. In this case of a 78-years-old female patient with severely atrophic ridges who had been using unstable complete dentures, the neutral zone impression technique was used to increase the stability and the retention of dentures. The neutral zone for both arch was located with modeling compound. After the arrangement of artificial teeth within the neutral zone, the external impression was taken to determine the tissue surface. The final dentures showed enhanced stability and retention and the patient was satisfied with the new dentures with respect to functional and esthetic aspects.

  • Ectodermal Dysplasia is a genetic disorder caused by the abnormal development of two or more structures derived from the ectodermal layer. As an aspect in dentistry, ectodermal dysplasia is characterized by hypodontia, conical or peg-shaped teeth, reduced salivary secretion and decreased vertical dimension. These unfavorable oral conditions make children to have difficulties with mastication, esthetics, and even in social activities. This case report presents an alternative oral rehabilitation treatment of a 4-year-old boy with ectodermal dysplasia. A removable space maintainer with artificial teeth in maxillary arch was delivered according to the initial treatment plan. However, the child failed to adapt the appliance because of his masticatory habit. Then a fixed-type space maintainer was delivered on the patient's esthetic demand and it restored function, esthetics and self-esteem of the patient. The treatment described here suggests that individually customized considerations are essential for the oral rehabilitation of a patient with ectodermal dysplasia.
