Clinical Results of Arthroscopic Treatment for Infection after Total Knee Arthroplasty (슬관절 전치환술 후 발생한 감염에 시행한 관절경적 치료의 임상 결과)

  • Purpose: To evaluate the efficiency of arthroscopic treatment for infected total knee arthroplasty (TKA), and to investigate the factors affecting the outcomes. Materials and Methods: We analyzed 17 cases which underwent arthroscopic treatment to treat infection after TKA. After confirming infection by hematologic examination and analysis of joint fluid, we performed arthroscopic debridement, synovectomy and irrigation with normal saline mixed with antibiotics. Through routine examination after operation, we checked failure of treatment or recurrence of infection. If there is no recurrence until 2 years after the operation, we considered it as a success of treatment. Results: Of the 17 cases taken arthroscopic treatment, 13 cases were treated successfully with primary arthroscopic treatment only, but 4 cases had to undergo re-operation because of persistence or recurrence of infection. Analyzing the factors affecting the results, we found that symptom duration of the re-operation group is longer than the treated group after arthroscopy (p<0.05). Conclusion: Arthroscopic treatment can be effective when we performed appropriate selection of patients, careful and extensive arthroscopic irrigation and debridement, and suitable use of antibiotics. However, we have to do early arthroscopic surgery as soon as possible when infection after TKA is suspected.

  • Synovial hemangioma of the knee joint has been reported as a rare cause of pain, limitation of motion and hemarthrosis, often seen as an internal derangement of the knee. A 39-year-old woman presented with spontaneous hemarthrosis of the left knee joint and physical exam revealed a small painful mass adjacent to the medial side of the patella. Magnetic resonance imaging revealed an intra-articular tumor. Arthroscopic excision was performed and the histology was characteristic of hemangioma. Through this case, we discuss the diagnosis and treatment options for the synovial hemangioma of the knee.

  • An irreducible dislocation of the knee joint is quite rare. Most irreducible knee dislocations are posterolateral dislocations and result from the soft tissue interposition. To the best of our knowledge, there is no report of an irreducible knee dislocation result from interposition of the detached cartilage from the medial femoral condyle. We present a case of 51 years old female with irreducible knee dislocation which was treated with an arthroscopic debridement of the detached cartilage, result in reduction of the joint, which is failed in closed reduction. And then we perform the delayed arthroscopic reconstructions for the ruptured anterior and posterior cruciate ligaments. Debridement of the interposed structure using the arthroscope allows for reduction of the joint and good result without the need for an open procedure.

Analgesic Effect of Intraarticular Morphine or Ketorolac Injection after Arthroscopic ACL Reconstruction (관절경을 이용한 전 십자 인대 성형술 후 관절강내로 투여한 Morphine과 Ketorolac의 진통효과)

  • In order to obtain a good result in Arthroscopic ACL Reconstruction by immediate postoperative physical therapy, sufficient analgesia was needed. This study analyzes the analgesic effect of the intra-articular injection with ketorolac, Morphine together with bupivacaine in 80 male patients who had Arthroscopic ACL Reconstruction. On completion of the surgery under spinal anesthesia, the knee was injected with 30ml of 0.25% bupivacaine. Each of the study group received ketorolac and/or morphine, either through parenteral or intra-articular. Total amount of the drug used by Patient Controlled Analgesia(PCA) and Visual Analgesia Scale(VAS) for pain were measured and analyzed. The group which received intra-articular ketorolac or Morphine had a better analgesic effect than other group which received none. The group which received both did not do better in analgesic effect. Intra-articular infusion with either ketorolac or Morphine improved postoperative analgesia in Arthroscopic ACL Reconstruction surgery. However, combined injection did not offer more advantage.

Arthroscopic Synovectomy of the Knee in Hemophilic Patients (혈우병성 슬관절염의 관절경적 활액막 제거술)

  • Purpose : To validate the arthroscopic synovectomy for the treatment of hemophilic knee. Materials and Methods: From January 1996 to January 2001, 28 arthroscopic synovectolny were performed in 26 patients with hemophilic arthropathy of the knee. The mean age was 17.8 years. The mean follow-up period was 3 years 11 months. We used six portals (two anterior, two suprapatellar and two posterior) and posterior trans-septal portal in all cases. Result : The mean frequency of hemarthrosis was 4 times per month preoperatively and 2 postoperatively. The mean amount of factor replacement was 4,633 units preoperatively and 1,505 postoperatively. The mean range of motion was $112^{\circ}$ preoperatively and $107^{\circ}$ postoperatively. On radiographic evaluation, three cases were progressed at the latest follow-up. On the subjective evaluation, significant or moderate improvement were in 19 cases $(68\%)$, and no improvement or deterioration in 9 cases $(32\%)$. Conclusion : With complete synovectomy through the appropriate arthroscopic portals, arthroscopic synovectomy of the knee in hemophilic patients is the successful method in decreasing bleeding episodes, amount of factor replacement, knee pain and preventing or delaying onset of end-stage hemophilic arthropathy.

  • Meniscal ossicle in the knee is very rare disease and is important to distinguish with loose body. We experienced a case of meniscal ossicle that had knee joint pain and clicking on standing up and met with a good result with arthroscopic meniscectomy. Therefore, we report this case with a review of relevant literature.

  • Trans-posterior septal posterior arthroscopic portals are useful in performing arthroscopic diagnosis and treatment of the lesions located in posterior aspect of knee joints. Except inferior surface of posterior horn of lateral meniscus, we easily observed entire lateral compartment, especially inferior surface of anterior horn and lateral tibial condyle by figure of 4 position through posteromedial trans-posterior septal arthroscopic portal. And it was possible to obtain visual field from posterior aspect and we could use anteromedial and anterolateral portals for instrumentation without clouding. So, this procedure may be useful in performing surgery of lateral compartment especially, anterior aspect in the knee joints.

  • Arthroscopy is a surgical procedure orthopedic surgeons use to visualize, diagnose and treat problems inside a joint. In usual, since the arthroscopic surgery is more accurate and minimally invasive than through 'open' surgery, the surgery of knee, shoulder, and wrist using auto-shaver is increasing in recent. However, repetitive use of arthroscopic-shaver may induce the second infection, hence the disposable arthroscopic-shaver should be developed. In this paper, a unique power transmission mechanism using DC motor, gear trains, and link mechanism is proposed for disposable arthroscopic-shaver and some test results of tissue cutting are introduced.
