Fenestrated Medial Plica Syndrome - A Case Report - (공혈형 내측 활막추벽 증후군 - 1례 보고 -)

  • Fenestrated medial patellar plicae are unusual. These fenestrated medial plica may vary in size and shape from being small circular and 5mm in diameter to being large opening 3-4cm long. Arthroscopic resection of the painful medial plica can provide lasting and satisfactory relief of symptoms. There is high percentage of associated medial knee symptoms that are relieved by complete resection. We had performed arthroscopic excision of the symptomatic fenestrated medial plicae present in both knees. The symptoms were dramatically relieved after arthroscopic surgery.

  • Osgood-Schlatter's disease is generally treated conservatively. However, surgical treatment is necessary for some patients with recurrent or persistent pain that does not respond to conservative treatment. Most authors recommend the excision of the loose ossicles present around the distal end of the patellar tendon. The authors report the technique of arthroscopic removal of the ossicles for Osgood-Schaltter's disease instead of the conventional technique through the incision at the tibial tuberosity. The advantages of this technique are less damage to the patellar tendon, early postoperative recovery, making no incisional scar in front of the tuberosity which causes the scar discomfort in kneeling, and more cosmetic result.

  • Partial thickness rotator cuff tears (PTRCTs) are now a well-established clinical entity, yrt there is no consensus in regards to surgical management. Recommended treatment varies widely from simple debridement of the torn tendon to subacromial decompression to excision of the degenerative tendon and repair. Surgery may be performed using an open, arthroscopically assisted (miniopen), or entirely arthroscopic procedure. The advantages and disadvantages of open and arthroscopic procedures may lie in the balance between precision and morbidity. Visualization of both the articular and bursal surfaces is mandatory to determine the level of rotator cuff disease and formulate a treatment strategy. Because these tear often are secondary finding, recognition of the primary pathology is critical. Treatment of the primary problem usually remedies the partial tear. However, when there is significant PTRCT, an arthroscopic or open repair of the diseased tendon will give the most consistent results.

Phase-dependent Progress of Recalcitrant Calcific Tendinitis of the Shoulder after Removal (불응성 견관절 석회성 건염의 제거술 후 시기별 경과)

  • Purpose: To evaluate the results of arthroscopic removal of calcific deposit depending on the phase-dependent progress of recalcitrant calcific tendinitis of the shoulder. Materials and Methods: From January, 2003 to September, 2007, arthroscopic treatment of calcific tendinitis of the shoulder joint, especially supraspinatus was performed in 27 consecutive patients who had had typical symptoms and failed with the conservative treatment and all patients could be followed up at least 6 months. We evaluated the visual analogue scale at preoperative, postoperative 2 weeks and 6 months. The results were compared according to the arthroscopic findings. Results: 11 cases were chalky calcium deposit of arthroscopic finding and 16 cases were toothpaste-like appearance. The VAS was 8.0 of the group with chalky deposit and 8.3 with toothpaste-like deposit preoperatively, which was not significantly different. However, at postoperative 2 weeks, it was 6.7 with chalky deposit and 2.7 with toothpaste-like, which is significantly different. At postoperative 6 months, the symptoms of all patients were resolved. Conclusion: Resorptive phase is well respond to surgical excision at early postoperative evaluation. The phase of calcific tendinitis is one of important factors for rapid resolution of the shoulder pain and functional improvement after arthroscopic treatment.

Resection of Ganglion Cyst of the Posterior Cruciate Ligament using Arthroscopic Posterior Trans-septal Approach (관절경하 후격막 통과 도달법을 이용한 후방십자인대 결절종의 치료)

  • Purpose: We reviewed the effectiveness and the result of arthroscopic resection for ganglion cyst of posterior cruciate ligament(PCL) through the posterior trans-septal approach. Material and Method: Seven patients with symptomatic ganglion cyst of the PCL were treated with arthroscopic resection using posterior trans-septal approach. We analyzed advantages of the posterior trans-septal approach and presence of intra articular problem. Knee function was assessed using the lysholm knee scale. Results: The overall result for the 7 knees were very satisfied in all. We found that four of seven patients had intra articular problems, and past history of trauma could be found in 3 cases. The Lysholm knee scale was increased from average 93.3 points preoperatively to average 97.8 points postoperatively. Conclusion: Arthroscopic posterior trans-septal approach shows an effective technique for complete resection of cyst and evaluation of the combined posterior compartment injury.

Arthroscopic Management in the Athletic Elbow Lesion (주관절 병변이 있는 운동 선수에서 관절경적 처치)

  • Purpose : To describe clinical results and improvement of range of motion and pain after arthroscopic treatment for the athletic elbow lesion. Materials and Methods : We reviewed 13 cases of functional restriction of the elbow motion, who all patients were treated by conservative treatment during than 6 months but failed. We performed arthroscopic osteophytectomy, capsular release, spur excision. The interval from injury to operation was average of 13 months (range, eight to sixteen months). We used the Broberg and Morrey functional rating score system in evaluating result. The follow-up period averaged 6 months. Results : Arthroscopic finding that moderative to severe synovitis in all cases, intraarticular loose body in 9 cases, olecranon bony spur in 2 cases, was observed. All cases were successfully improved in range of motion and pain without in any further operation. Preoperative average score was 65 and postoperative average score was 89.4 in Broberg and Morrey functional rating score. Conclusions : Arthroscopic treatment appear to be satisfactory management modality due to diagnose the casative factor and reduce the injury around the elbow joint in painful elbow lesion. Elbow arthroscopy appears to be a safe and effective treatment for athletes, allowing easy return to participation in sports.

Unusual Isolated Complete Tear of Subscapularis and Biceps Long Head Tendon Associated with Heterotopic Ossification

  • Heterotopic ossification (HO) within the substance of the subscapularis tendon is a rare lesion which remains a poorly described condition with little known of the exact mechanisms involved. Furthermore, its clinical importance remains still unclear. To our knowledge, there are no studies present to data regarding HO within the substance of the subscapularis tendon, even with resultant isolated complete tear of the subscapularis tendon. Here we present a case of huge HO associated with unusual isolated complete tear of subscapularis tendon concomitant with tear of biceps long head tendon. After arthroscopic debridement for the complete tear site of biceps long head tendon, mini-open excision of the ossification and subscapularis repair with suture anchor fixation were performed. The patient showed complete recovery of strength and function of the subscapularis at subsequent 24 months follow up.

  • The subtalar joint is a complex joint that is functionally responsible for inversion and eversion of the hindfoot. Advances in optical technology and surgical instrumentation have allowed the arthorscocpic surgeon to investiagate the small joints including the subtalar joint. Indications for subtalar arthroscopy include pain, swelling, stiffness and locking. Therapeutic indications include treatment of chondromalacia, osteophytes, arthrofibrosis, synovitis, loose bodies, osteochondral lesions, excision of a painful os trigonum, arthrodesis, and FHL tendinopathy. Contraindications to subtalar arthroscopy include infection, advanced osteoarthritis with deformity, severe edema, poor vascularity and poor skin quality. Subtalar arthroscopy is a technically demanding and difficult procedure that should only be performed by experienced surgeons. With proper instrumentation and careful operative techniques, satisfactory results may be obtained with minimal morbidity.

Povidone-Iodine related Chemical Burn under the Tourniquet (A Case Report) (지혈대 내측의 포비돈-요오드 용액에 의한 화학 화상 (1예 보고))

  • In the extremity surgery, pneumatic tourniquet and povidone-iodine solution are commonly used to provide an aseptic, bloodless field, and their complication rate has remained low. However, chemical burn under tourniquet has been rarely reported. Patients sustained burn injuries over the dependent, weight-bearing regions such as posterior neck, back, buttocks and posterior thighs. This rare adverse complication occurred in a 22-year-old man who underwent modified Brostrom operation with arthroscopic os trigonum excision. 10% povidone-iodine was used as topical antiseptic, and full thickness burn occurred underneath the area of tourniquet application. Main causes of povidone-iodine related chemical burn are considered maceration, irritation of the skin, long term use of the tourniquet and pressure. To reduce the complications like chemical burn, awareness of the risk and the possible pathogenesis as well as the preventive measures is important in surgical practice.

Synovial Osteochondromatosis of the Subtalar Joint in an Adolescent Baseball Player (청소년 야구 선수에서 발생한 거골하 관절의 활액막 골연골종증)

  • Synovial chondromatosis is an uncommon disorder characterized by cartilaginous proliferation within the synovial membrane of the articular joint. Smaller joints are rarely affected and it may be progressed to osteochondromatosis after ossification or calcification of metaplastic cartilage. It is commonly presented in the third to fourth decade of life, but rarely presented in adolescence. We report a unique case of synovial osteochondromatosis of the subtalar joint in 14-year-old baseball player. Arthroscopic removal of loose body and complete excision of the osteochondral mass with concomitant synovectomy resulted in satisfactory outcome without recurrence at final follow-up.
