Purpose : We evaluated clinical result of arthroscopic and open Bankart repair in anterior shoulder instability to identify factors iuluencing operative result and prognosis. Materials & Methods . We reviewed 24 patients of anterior shoulder instability treated with arthroscopic Bankart repair in 16 cases and open Bankart repair in 8 cases. Average age was 26 years old and involved in dominant arm in 15 cases. Patients were suffered instability for 3.1 years before operation and mean follow-up was 2 year 9 months ( 1 you 9 months -4year 10 months). Results : Post operative pain was subsided in 2 weeks in arthroscopic surgery and 3 weeks in open surgery. The final range of motion after arthroscopic repair were flekion in 168" , external rotation in 54" , and internal rotation in 79, and after open repair 168" ,49" , and 78 respectively. In arthroscopic surgery,2 cases (13%) were redislocated, and 4 cases(25%) showed mild instability. In open case,1 case (11%) showed mild instability. According to function- al result by Rowe grading scale, satisfactory results were 12case (76%) in arthroscopic repair and 7 cases (88%) in open cases. Conclusions Both arthroscopic or open Bankart could get good results in the treatment of anterior instability of shoulder. In arthroscopic repair, perioperative morbidity was lower than open repair, but it needs careful rehabilitation program to prevent redislocation and to return to sports activity.
Purpose: This study reports the clinical results of the arthroscopic Bankart repair in traumatic anterior instability of the shoulder with bio-knotless anchor. Materials and Methods: 21 cases of 21 patients (20 male and 1 female) were included in this study. The average age was 24.8 years old and the period from the first injury to operation was average 37.2 months. All cases had Bankart lesion and 12 cases had Hill-Sachs' lesion. The SLAP lesion was associated in 6 cases. Preoperative Rowe score was average 29.1. Arthroscopic Bankart repair with bio-knotless anchor were performed in all cases; 3 anchors at 3, 4, 5 O'clock position of the glenoid were used in 11 cases and 2 anchors at 4, 5 O'clock position were used in 10 cases. All the associated SLAP lesions were repaired arthroscopically with bio-knotless anchor. Thermal capsular shrinkage at the anterior and inferior shoulder capsule after the Bankart repair was performed in 3 cases. The average follow up period was 20.2 months. Results: The Rowe score improved to 92.8, excellent in 17 cases and good in 4 cases, at last follow up period and 20 cases had full range of motion of the shoulder. 1 case had mild limited range of motion of the shoulder (150 degrees in flexion, 60 degrees in external rotation and T12 level in internal rotation) without any problem in normal activity. The arthroscopic revision surgery of the shoulder was performed in 1 case because of multiple traumatic injuries of the shoulder with pain postoperatively. Conclusion: Arthroscopic Bankart repair with bio-knotless anchor in traumatic anterior shoulder instability is one of the good methods because of the good clinical results.
Background: Arthroscopic fixations for large and comminuted bony Bankart lesions are technically difficult. We developed an arthroscopic multiple pulled suture (MPS) technique to restore large and comminuted bony Bankart lesions. Methods: Ten patients (mean age, 49.8 years; range, 31-79 years) underwent bony Bankart repair using the illustrated MPS technique and were then followed for a mean of 27.3 months. A plain radiograph series and three-dimensional computed tomography scans were taken at the initial clinical evaluation and 3 months postoperatively. Outcome measurements included the American Shoulder and Elbow Surgeons (ASES) score, Rowe score, University of California at Los Angeles (UCLA) score, and subjective patient satisfaction, along with surgical complications. Results: Union of an osseous fragment with the glenoid rim was confirmed in all patients on a computed tomography scan 3 months after operation. The osseous fragment was restored to proper articular congruence and reduction. The affected shoulder was stable in nine of the 10 patients. One patient presented with a redislocation after a sports injury 3 years postoperatively. The ASES, Rowe, and UCLA scores improved at the final evaluation, and median patient satisfaction at the final follow-up was 9 of 10 points (range, 6-10 points). Conclusions: The arthroscopic MPS technique for bony Bankart lesions with large or comminuted osseous fragments was a relatively easy and safe method for stable fixation of the osseous fragment. Therefore, the arthroscopic MPS technique resulted in good restoration of stability with high patient satisfaction and low complication rates.
목적 : 재발성 전방 견관절 탈구 환자에서 비흡수성 봉합사를 사용하여 suture-anchor 방식을 이용한 관절경적 Bankart 술식의 임상적 결과와 병변의 예후에 영향을 미칠것으로 사료되는 여러 인자들을 술 전, 술 후 Rowe 점수를 통해 평가, 비교하였다. 대상 및 방법 : 1997년 6월부터 2001년 5월까지 외상으로 인한 재발성 견관절 탈구로 인해 관절경적 Bankart 병변 봉합술을 시행 후 최소 1년 6개월 이상 추시가 가능했던 29예를 대상으로 하였다. 28명 모두 남자였으며 수상 당시의 평균나이는 21.3세다. 술 전, 술 후 견관절의 기능평가는 Rowe점수를 사용하였으며 예후에 영향을 미칠 것으로 사료되는 9개의 인자를 설정한 후 술 전, 술 후 Rowe 점수를 이용하여 공변량 분석을 이용한 통계학적 검증을 실시하였다. 결과 : 술전 Rowe 점수는 평균 28.4점(보통 11예, 불량 17예)이었는데 반해 술 후 Rowe점수는 87.1점(우수 12예, 양호 15예, 보통 1예)이었고 통계학적으로 유의한 점수의 증가를 나타냈다(p=0.000). 추시상 나타난 합병증으로는 재탈구 1례, 경미한 견관절 운동범위 장애를 나타내는 2례가 있었다. 술 전, 술 후 Rowe점수를 통한 예후 인자 분석에 있어서는 수상 당시의 나이가 20세 미만인 군에서 20세 이상인 군보다 통계학적으로 유의하게 (p=0.023) 예후가 좋은 것으로 나타났다. 결론 : 관절경을 이용한 Bankart 술식은 재발성 견관절 탈구환자에 있어 유용한 치료방법중의 한가지로 사료된다. 또한 수상당시의 나이는 술 전, 술 후 Rowe점수를 비교한 결과 예후에 영향을 미치는 인자로 사료되었다.
목적: 외상성 재발성 견관절 탈구환자에서 Bankart 병변에 대해 금속 봉합나사못을 이용한 관절경적 봉합술의 유용성을 알아보고자 한다. 대상 및 방법: 5년 이상 추시가 가능하였던 환자 85명(남 80명, 여 5명)을 대상으로 후향적 연구를 시행하였다. 수술 당시 환자의 평균 연령은 26세(15세~52세)였고, 이환 기간은 6개월에서 38개월까지 평균 20개월이었다. 모든 예에서 Bankart 병변이 존재하였고, 동반 병변으로 Hill-Sachs 병변과 SLAP 병변이 각각 44예, 10예였으며, 7예에서 회전근개 부분 파열이 관찰되었다. 평균 추시 기간은 7년 5개월(5년 8개월~9년)이었다. 결과: 술 전 Rowe 점수는 평균 29.3점(25점~50점)이였으며, 최종 추시 시의 Rowe 점수는 평균 86.8점(40~100)으로 우수 28 예(32.9%), 양호 46예(54.1%)로 74예(87.%)의 환자에서 양호 이상의 결과를 얻었으며 견관절의 운동 범위는 70예(82.4%)에서 정상으로 회복되었다. 재탈구가 3예(3.5%)에서 발생하였고 관절경적 재수술을 시행하였다. 결론: 외상성 재발성 견관절 탈구로 인한 전방 불안정성 환자에서 Bankart 병변에 대한 금속 봉합나사못을 이용한 관절경적 봉합술은 우수한 임상 결과와 낮은 재발율을 보였고, 합병증을 낮출 수 있는 효과적인 술식 중에 하나라고 사료된다.
Purpose: In the traumatic anterior shoulder instability, the laxity of joint capsule and ligament is frequently demonstrated. Although a arthroscopic procedure to address anterior instability with joint capsular redundancy have generally provided good results, its recurrence rate is higher than open procedure. By reducing the capsular redundancy, thermal shrinkage is likely to improve the outcome of arthroscopic anterior stabilization. The objective of this study was to evaluate additional thermal capsular shrinkage as a treatment of joint capsular redundancy in anterior shoulder instability. Materials and Methods: From March 1999 to June 2000, 25 shoulders of 23 patients of recurrent anterior shoulder dislocation underwent arthroscopic Bankart repair with shrinkage procedure. The mean follow up was 29 months and average age at the time of operation was 26 years. Of these patients, 20 were male and 3 were female who had been experienced the average 8 times of dislocation before operation. Thermal shrinkage alone without Bankart repair was performed in two cases who did not have Bankart lesion. The clinical result was evaluated in according to Modified Rowe Score. Results: The Modified Rowe Score was improved from preoperative 35 points to postoperative 88 points. None of cases showed recurrence of dislocation. But, in two cases, temporary sensory hypesthesia of the axillary nerve was developed and in two cases of postoperative stiffness, arthroscopic capsular release and brisement were performed. Conclusion: Additional capsular shrinkage in arthroscopic technique to address recurrent anterior shoulder instability could treat effectively the capsular redundancy.
목적: 견관절의 전방 불안정성이 있는 환자에서 관절경하 생체흡수형 knotless anchor를 이용한 치료 결과를 객관적 및 주관적 평가를 통하여 알아보는데 있다. 대상 및 방법: 2003년 1월부터 2003년 6월까지 본원에서 외상성 견관절 전방 불안정성에 대하여 관절경하 생체흡수형 knotless anchor를 이용하여 Bankart복원술을 시행하고 평균 12개월 추시된 15예를 대상으로 하였다. 환자 구성은 남자가 14명, 여자가 1명이었으며, 평균 연령은 24세 (16세${\sim}$42세)였다. 평균 추시 기간은 14개월(12개월${\sim}$18개월)이었다. 수술시간은 같은 저자에 의해 2002년 1월부터 2002년 10월까지 suture anchor 술기를 이용하여 관절경하 Bankart 복원술을 시행한 20예와 비교하였다. 결과: 수술 후 탈구의 재발이나 아탈구의 발생은 없었으며 Rowe등에 의한 기능적 평가에서 평균 89.4점이었고 환자의 주관적 만족도는 평균 87.5였다. 최종 추시시의 견관절 운동 범위는 각각 굴곡이 평균 171도, 외회전이 평균 54도로 측정되었고 전 예에서 결과에 만족하였으나 3예에서 운동시 불안감각이 남아 있었다. 수술시간은 suture anchor 술기를 이용한 Bankart 복원술에서 1개의 매듭 당 평균 25.5분이 소요되었지만, 생체흡수형 knotless anchor를 이용시 1개의 anchor당 소요 시간은 평균 16.5분으로 의미 있게 감소됨을 알 수 있었다(P<0.05).결론: 견관절 전방 불안정성의 원인인 Bankart병변의 치료에 있어 knotless anchor 술식은 관절낭의 적절한 긴장을 획득하고 수술 시간을 단축하는 장점이 있으며 탈구나 아탈구의 재발없이 견관절 불안정성을 치료할 수 있는 유용한 방법으로 사료된다.
목적: 견관절의 제 5형 SLAP 병변에 대하여 bio-knotless 봉합 나사못을 이용하여 관절경적 봉합을 시행한 후 추시한 결과 만족할 만한 임상적 결과를 얻었기에 이를 보고하고자 한다. 대상 및 방법: 지속적인 추시가 가능하였던 10례에 대하여 임상적 결과를 분석하였으며 평균 연령은 32.7세($20{\sim}49$)였고 모두 남자였다. 손상의 원인은 7례에서 스포츠 손상이었으며 1례가 산업 재해, 2례가 교통 사고였다. 처음 수상한 날로부터 수술까지의 기간은 평균 47.2개월이었으며 수술 전 Rowe 점수는 평균 37.5점($30{\sim}55$)이었다. 모든 예에서 SLAP 병변을 먼저 관절경적으로 1개 또는 2개의 bio-knotless 나사못을 사용하여 봉합한 다음 Bankart 병변을 봉합하였는데 3개의 bio-knotless 나사못을 삽입한 경우가 7례, 2개를 삽입한 경우가 3례였다. 수술 후 3주간 상지 외전 보조기를 착용시키고 매일 간헐적인 수동적 견관절 굴곡-외회전-내회전 운동을 시행하였고, 관절의 운동 범위가 정상이 되면 회전근 개의 근력 회복을 위한 운동 치료를 시행하였고, 마지막 단계에서는 삼각근 및 견갑골 주위 근육 강화를 위한 근력 강화 운동을 시행하였다. 결과: 평균 추시 기간은 15.7개월($6{\sim}27$)이었으며 최종 추시시 견관절의 운동 범위는 8례에서 완전한 회복을 보였으나 산업 재해에 의한 손상의 환자에서는 굴곡 170도, 상지 내전 상태에서의 외회전 60도, 내회전은 제 12 흉추 범위를 보였으며 교통사고로 수상한 1례에서 견관절의 능동적 운동 범위가 굴곡 160도, 상지 내전 상태에서의 외회전 45도, 내회전은 제 12 흉추 범위를 보였으나 일상 생활에는 문제가 없었다. 최종 추시시 Rowe 점수는 평균 93점($75{\sim}100$)으로 모두 우수한 결과의 소견을 보였다. 결론: 제 5형 SLAP 병변에 대하여 bio-knotless를 이용한 관절경적 봉합 수술이 만족할 만한 임상적 결과의 소견을 보여 매우 좋은 수술 방법으로 사료된다.
Purpose: The purpose of this study is to evaluate the short-term clinical outcome of the capsulolabral repair using absorbable suture anchor in traumatic anterior instability of shoulder. Materials and Methods: From June 2000 to September 2001, 15 shoulders with recurrent anterior instability were operated with arthroscopic Bankart repair using absorbable suture-anchor 'PANALOK' (Mitek, westwood, MA), and were followed up over 1 year (average; 13 months). The mean age was 23-years. There were fourteen males and one female. The mean duration from the initial symptoms to the operation was 24 months. Associated pathologies were Hill-Sachs defect in 12 cases, SLAP in 6 cases, and partial rotator cuff tear in 2 cases. The results were evaluated by patien's satisfaction, Modified Rowe Score in regard to joint stability, mobility, pain and function in comparison with the preoperative ones, and other complications. Results: At the last follow-up, the total Rowe Score increased from 38 points to 92 points. There were no recurrence and 14 patients among 15 patients gained pre-operative level of sports activity and no other complications. Conclusion: Effective capsulolabral repair could be obtained by the absorbable anchoring without any untoward complications. This procedure is simple and safe one and this system can be a good substitute for the metallic anchor.
Superior labrum anterior to posterior (SLAP) lesions of the shoulder has recently been a popular issue to shoulder surgeons. Now we are correlating many shoulder symptoms to this SLAP lesion. A 45 year-old female patient injured her shoulder when her arm sleeve was entrapped in moving automobile door. A forceful pull of the arm in external and abduction position was suspected. She complained continuous shoulder pain with limited range of motion for 2 months. Magnetic resonance image showed possible SLAP lesion but no definite diagnosis were made prior to the operation. Arthroscopic evaluation revealed SLAP type Ⅱ lesion with concomitant avulsion of the superior glenoid cartilage. In addition anterior labrocapsular tear was seen from 7 to 9 o'clock of anterior glenoid. The SLAP lesion and the anterior capsulolabral lesion were repaired properly to the glenoid. We report a case of glenoid-cartilage avulsion type of SLAP Ⅱwith anterior labrocapsular lesion.
