관절염 환자의 통증과 심리.사회적 상태와의 관계 (The Relationship between Pain and Psychosocial Factors of Patients with Arthritis)

  • Purpose: This study was conducted to examine the relationship among pain, family support, self-efficacy, depression of patients with arthritis. Method: The subjects of this study consisted of 96 patients with arthritis. The data was analyzed ANOVA. Duncan test. Pearson Correlation Coefficients using SAS. Result: Pain showed significant differences according to depression(F=8.12, p=0.001), self efficacy(F=11.52, p<.000) and not significant differences family support(F=0.34, p=0.714). There was a significant negative correlation between family support and depression(r=-.48, p<.000), depression and self efficacy(r=-.41, p<.000). Conclusion: Therefore, for the management of arthritis patient it is required continuous self-management and proper program on self-efficacy promotion.

세포교정영양요법(OCNT)을 이용한 류마티스 관절염 개선 사례 (Improvement of symptoms in patients with rheumatoid arthritis using ortho-cellular nutrition therapy)

  • Objective: Improvement of symptoms in patients with rheumatoid arthritis using OCNT Methods: The patient was a Korean woman in her 50s who was experiencing foot pain due to rheumatoid arthritis and was experiencing discomfort in her daily life. OCNT was applied to the patient for approximately 6 months. Results: After starting OCNT, the patient's joint pain and chest pain gradually decreased, and after about six months, she no longer felt discomfort due to these symptoms. Conclusion: For rheumatoid arthritis patients suffering from joint pain and chest pain, OCNT can help relieve symptoms.

류마티스 관절염 환자의 원인지각에 대한 연구 - Q방법론적 접근 - (An Inquiry to the Causal Perceptions & Emotions of Rheumatoid Arthritis Patients)

  • This study was undertaken to find out the causal perception of rheumatoid arthritis patients, and to understand the typology. The Q-population consisted of 236 statements of causal perception were collected. Thirty eight Q-samples of causal perception were selected. The P-sample for this study were made up of 28 first visiting female rheumatoid arthritis patients from a rheumatoid arthritis specialty hospital. Each respondent responded Q-set of causal perception according to 9-point scale. The result of Q-sorting were coded and analyzed using QUANL PC program. 1) Typological Observation on Causal Perception (1) Physical Fatigue Type : Type 1 perceived that the illness occurred due to excessive work requiring physical labor or strain that had occurred from not resting after excessive physical labor, therefore, thinking the origin of the illness was from physical strain. (2) Physical origin Type : Type 2 perceived that the major cause for the illness is not only excessive physical labour but also fecundity and old age. (3) Causality to Environment Type : Type 3 perceived that rheumatoid arthritis occurred from injury to the joints or bad and humid weather. (4) Conscience of Guilty Type : Type 4 consisted of people with guilty conscience for lack of religious commitment. They perceived that the illness was a punishment from God for not praying or because of bad luck. (5) Rationally Perceiving Type : People who belong in type 5 perceived the cause of illness in light of scientific facts such as genetics, unbalanced diet or lack of exercise. (6) Psychological Stress Type : People who belong in type 6 believed that excessive stress was the cause of the illness. 2) Emotions of Rheumatoid arthritis patients Rheumatoid arthritis patients' positive emotions included determination, courage, coping, acceptance, hope, and adoption ; and their negative emotions were prostration, worry, stupor, conflicts, grievance, giving-up, resignation, depression, loss, solitariness, fear, anxiety, avoidance, anger and loneliness. Rheumatoid arthritis patients experience different level of emotions from their suffering experience from the severe pains. Rheumatoid arthritis patients also experience negative emotions when they could not perform self-care and lose their self-esteem from painful suffering ; however, they regain positive emotions when they recover from pain with the use of drugs, physical therapy or exercise. Their emotional states are closely connected to level of and presence of pain.

An Inquiry to the Causal Perceptions & Health Seeking Behaviors of Rheumatoid Arthritis Patients

  • This study was undertaken to investigate the causal perceptions and health seeking behaviors of Rheumatoid arthritis patients, define and understand the typology, and find the relationship between causal perceptions and health seeking behavioral types. There were six types(Physical Fatigue, Dispensation of Nature, Causality to Environment, Conscience of Guilty, Rationally perceiving, Psychological Stress) of subjective opinion about Causal Perceptions of Rheumatoid Arthritis Patients. And there were four types(Oriental medical Treatment, Information Seeking, Dietary Control. Western Medical Treatment) of subjective opinion about Health Seeking Behaviors. In the relationship between types of the causal perceptions and health seeking behaviors, oriental medical treatment and information seeking type were common health seeking behaviors of all six causal perception types. Only difference for internal causal perception types was related to hospital instructions and external causal perception types were related to dietary control. The result of this study can help health care providers, especially nurses to understand the types of causal perceptions and health seeking behaviors of Rheumatoid arthritis patients to gain treatment compliance from patients according to their causal perceptions of the illness, and use it to develop educational nursing intervention to aid health care.

만성 관절염 환자의 운동행위 변화단계에 따른 관련변인에 관한 연구 (A Study on the Stage of Change of Exercise Behavior and Related Factors in Patients with Chronic Arthritis)

  • Purpose: This study was done to identify the process of change, decisional balance and self-efficacy corresponding to stage of exercise behavior based on the Transtheoretical model in patients with chronic arthritis. Method: The participants were 151 patients with chronic arthritis using an out-patient pain clinic and hospital in C city. The collected data were analyzed using ANOVA, Sheffe test and discriminant analysis with SPSS/WIN. Results: The patients were distributed as follows for each stage of exercise behavior 21.8% in the precontemplation stage, 26.5% in the contemplation stage, 25.2% in the preparation stage, 7.3% in the action stage, and 19.2% in the maintenance stage. Significant effects for stage of change were found for nine of the 10 processes of change and Pros, Cons, self-efficacy. Scores on these variables were found to increase across the stage of exercise behaviors from precontemplation to maintenance. Discriminant analysis revealed that self-efficacy and stimulus control, and reinforcement management were the major correlates of the stage of exercise behaviors. Conclusion: The findings of this study give useful information for exercise intervention, by using strategies of exercise for chronic arthritis patients in different stages of exercise behavior.

관절염 환자의 수중운동 지속/중단 요인에 관한 연구 (Influencing Factors on Aquatic Exercise Adherence and Dropout in Patients with Arthritis)

  • The purpose of this study was to identify influencing factors adherence and dropout of aquatic exercise in patients who had been diagnosed chronic arthritis. Subjects were 54 patients with rheumatoid arthritis and osteoarthritis who had participated in the 6-week aquatic exercise program in Taejon metropolitan city. Data was collected from march 25 to April 25, 1999 through face to face and telephone interview. Result are as follows. Fourteen subjects(26.0%) were adherer who was keeping aquatic exercise. The reasons of aquatic exercise adherence were improvements of physical and affectional factors. And the major reasons of dropout was an environment factors(65.8%) and second reasons was physical factors(34.2%). Participants of friends(r=.34, p=.014) was significantly associated with the total duration of an aquatic exercise adherence. For these results, the nurse who care to patients with arthritis have to encourage to maintain aquatic exercise within six month after they begin aquatic exercise.

사상체질에 따른 류마티스 관절염의 위해요인 및 TNF-${\alpha}$(G308A) 유전자 다형성간의 연관성 연구 (A Study of TNF-${\alpha}$(G308A) Genetic Polymorphism and Risk Factors Associated with Rheumatoid Arthritis by Sasang Constitution)

  • Objectives: The purpose of this study is to examine the risk factors and the genetic polymorphism of TNF-alpha associated with rheumatoid arthritis by Sasang constitution Methods : This study was planned to detect the susceptibility of the patients diagnosed by rheumatoid arthritis to Sasang Constitution and to examine the risk factor such as life style and environmental stress (smoking, environmental tobacco smoke, alcohol intake and so on). The genetic polymorphism of TNF-alpha (G308A) were analyzed by PCR-RFLP in rheumatoid arthritis patients and controls. Rheumatoid arthritis patients and matched controls are assessed with QSCCII question for Sasang Typology. Then the genetic polymorphism of patients by Sasang constitution are compared to those of control, which are statistically analyzed and adjusted by age, sex, smoking status, alcohol intake, BMI, and econocmic status. Results: Differential effect of passive smoking on the association between Sasang constitution and rheumatoid arthritis risk was found. This study showed that the genetic polymorphism (TNF-${\alpha}$(G308A)) of rheumatoid arthritis patients and controls associated with the susceptibility to rheumatoid arthritis by sasang constitution was analyzed. Differential effects of TNF-${\alpha}$(G308) genetic polymorphism on the association between rheumatoid arthritis risk and Sasang constitution were found. Conclusion : It is suggested that the genetic polymorphism correlated with susceptibility to rheumatoid arthritis by specific sasang constitution used as its susceptibility marker and further as basic data to prevent the risk factors for rheumatoid arthritis. But larger studies will be needed to confirm these preliminary findings.

수중운동 프로그램이 류마티스 관절염 환자의 우울과 무기력감에 미치는 영향 (Effects of Aquatic Exercise Program on Depression and Helplessness in Patients having Rheumatoid Arthritis)

  • An aquatic exercise program is one of the nursing intervention for the patients having rheumatoid arthritis. The purpose of this study was to obtain the effect of aquatic exercise program on depression and helplessness in the rheumatoid arthritis patients. Thirty five patients with rheumatoid arthritis were the subjects of this study. who were treated to the rheumatism center at a hospital in Seoul from December 1992 to April 1993. The data were collected before and after aguatic excercise. All data collected were analyzed using percent, t-test, ANCOVA with SPSS $pc^+$ program. The findings of this study were as follows : 1. The score of depression was not different between experimental group and control group(P=0.178). 2. There was significant difference in the helplessness score between experimental group and control group(F=4.60, P=0.039).

재가 관절염환자의 건강상태 장애정도에 관한 연구 (The Health Status of Arthritis Patients at Home in K City Using Aims 2 Instrument)

  • This study was carried out in order to examine the health status of arthritis patients living in K city of southern part of Korea. The data were collected with AIMS2 instrument from 56 subjects who were asked to complete the questionnaire by the trained surveyor. The AIMS2 has been approved useful instrument for assessing the outcome of various treatments and programs in rheumatic diseases. The component variables of the AIMS2 are Mobility, Walking and Bending, Hand and finger function, Arm function, Self-care tasks, Household tasks, Social activity, Support from family and friends, Arthritis pain, Work, Mood, Level of tension, The total number of items of AIMS 2 are 78. The results are as follows: The average age of the subjects was 58 years old and no gender difference. The mean scores of the 12 sub-concepts of AIMS2 are 15.0 of mobility, 12.16 of walking and of bending, 9.32 of hand and finger function, 9.03 of arm function, 13.53 of self-care tasks, 10.19 of household tasks, 15.09 of social activity, 19.58 support from family and friends, 13.82 of arthritis pain, work, 14.85 of level of tension, 15.12 of mood. The Cronbach Cronbach $\alpha$. is 0.72-91 for RA patients and 0.74-96 for OA patients. This results showed that the health status of Arthritis patients at home in K-city was almost not so serious, but the level of tension and mood were quite higher than the average score. Considering the results of this particular subjects, an appropriate rehabilitation program should be developed and implemented.

태극권 운동이 류마티스관절염 환자의 주관적 증상, 체력, 우울 및 자기효능감에 미치는 효과: 3개년 자료를 바탕으로 (Comparison of Subjective Symptoms, Physical Fitness, Depression and Self-efficacy before and after Tai Chi in Patients with Rheumatoid Arthritis: Based on 3 Years' Data)

  • Purpose: The purpose of this study was to evaluate whether subjective symptoms, physical fitness, and emotional variables were improved after participating in a 6-week Tai Chi program in patients with rheumatoid arthritis. Methods: 23 rheumatoid arthritis patients participated in the Yang style Tai Chi for arthritis patients 90 minutes session conducted twice a week at the Arthritis Center of D University Hospital. The main outcome of current studies included self-reported scores of subjective symptoms(pain, sleep satisfaction, and fatigue), physical fitness tests, and depression (PHQ-9) and self-efficacy investigated with questionnaire before and after participating in Tai Chi. Results: After participating in Tai Chi, participants perceived lesser pain (t=2.61, p=.016) and fatigue (Z=-2.32, p=.020), and reported improved sleep satisfaction (t=-3.56, p=.002). In the physical fitness test, there were significant improvement in muscle strength (upper limb: t=-7.19, p<.001, lower limb: t=-4.86, p<.001), and balance (right t=-3.70, p=.001; left t=-2.30, p=.031). Depression (t=3.87, p=.001) and self-efficacy (Z=-3.44, p=.001) improved significantly after participating in Tai Chi. Conclusion: Tai Chi had beneficial effects on subjective symptoms and emotional variables as well as physical fitness. Therefore, the program could be recommended in the patients with rheumatoid arthritis.
