This is a report of successful conversion arterial switch operation for failed Sunning procedure in transposition of the great arteries(TGA) with ventricular septal defect(VSD). A 15 month-male patient was admitted due to intractable congestive heart failure after Sunning operation was done at the age of 8 months. Angiography revealed marked dysfunction of the morphologic right ventricle with tricuspid regurgitation and residual VSD. The pulmonary ventricle 1 systemic ventricle pressure ratio' of 75/85 at catheter study enabled us to do the take down of denning repair, patch closure of VSD and arterial switch without pulmonary artery banding. After the operation, the baby showed good growth with normal ventricular function.

Measuring the pulse wave velocity (PWV) is a non-invasive method for evaluating the stiffness of the vessel wall. While previous studies investigated the association of uric acid level with arterial stiffness, most did not consider smoking status and obesity as covariates. The objective of this study was to investigate the relationship between uric acid levels and arterial stiffness, considering smoking status and obesity. We studied 3390 subjects (1940 males and 1450 females). Abnormal PWV was also defined as the highest quartile of values in the subjects. Data were analyzed using Pearson correlation, t-test, and multiple regression analysis. Approximately 41.9% of men and 3.7% of women were current smokers. Prevalence of hypertension was 11.4% in men and 7.0% in women. In women, hyperuricemia was associated with abnormal PWV (OR 1.58; 95% CI 1.03~2.43). Hyperuricemia was also associated with abnormal PWV (OR 2.37; 95% CI 1.01~5.59) in non- or light male smokers with normal blood pressure. This study suggests that uric acid is associated with arterial stiffness in women and non-smoking men.
Sixty four children [aged 2 days to 9 years] , 58 with complete transposition of the great arteries, 5 with Taussig-Bing double outlet right ventricle, and 1 with double outlet left ventricle plus left ventricular type single ventricle, have undergone anatomic correction from November 1987 to August 1992. Eleven underwent previous operations: pulmonary artery banding[7], modified Blalock-Taussig shunt[2], coarctoplasty[2], aortic arch reconstruction[1] . Of 58 patients with TGA, Type A coronary arteries of Yacoub were seen in 50[86%]. U-shaped coroanry arterial flaps were transfered to the neoaorta using trap door technique, and neopulmonary arterial tract was constructed using glutaraldehyde fixed autopericardium with Lecompte maneuver. There were 18 hospital deaths [28.1%] with no late mortality. Mean follow-up of 20.4\ulcorner11.9 months were achieved in all survivors. Postoperative cardiac catheterizations were done in 14 cases. Mean pressure gradients of pulmonary and aortic outflow tract were 15.0 $\pm$2.6 and 4.2$\pm$1.4mmHg, mild aortic valve insufficiencies were found in 2, and mean cardiac index was 5.18$\pm$0.19 L/min/M2. We conclude that we should continue anatomic correction for the complex congenital heart anomalies with the malposition of the great arteries because myocardial function seems to be well preserved, though we are still on the learning curve.
Steady and physiological flows of a Newtonian fluid and blood in the bifurcated arterial vessel are numerically simulated. Distributions of velocity, pressure and wall shear stress in the bifurcated arterial vessel are calculated to investigate the differences between steady and physiological flows. For the given Reynolds number physiological flow characteristics of a Newtonian fluid and blood in the bifurcated arterial vessel are quite different from those of steady flows. No flow separation or flow reversal in the bifurcated region in the downstream after stenosis appears during the acceleration phase. Also, no recirculation region is seen for steady flows. However, during the deceleration phase the flow began to exhibit flow reversal, which is eventually extended to the entire wall region.
Blood pressure (BP) measurement plays a pivotal role in veterinary medicine for diagnosing cardiovascular disorders and monitoring anesthesia of animals. Although indirect BP measurement has been widely applied to monitor BP because of its convenience and non-invasiveness, it is still unclear whether indirect BP measurement is compatible with direct BP measurement in minipigs. In addition, the effect of animal posture during BP measurement is not well understood in minipigs despite its importance to cardiovascular performance. Therefore, both systolic and diastolic arterial BPs in minipigs were measured via femoral artery catheterization for direct BP measurement and using a compressive cuff as an indirect BP measurement under the dorsal or right lateral recumbent postures. Numerical values were processed by the Bland-Altman method to calculate the bias ${\pm}$ SD and the limits of agreement (LOA). In accordance with the American College of Veterinary Internal Medicine guidelines, the results between direct and indirect BP measurements were determined as apparent disagreements in both systolic and diastolic arterial BPs under all postures because of large bias ${\pm}$ SD and wide LOA. The results of the present will help prevent misinterpretation of the anesthetized patient's condition during monitoring of BP by indirect measurement.

This study was designed to investigate the effects of Acanthopanax sessiliflorus 5.,M (ASS) on the regional cerebral blood flow (rCBF) and mean arterial blood pressure (BP) in normal rats and ischemic rats. Experimental groups of rCBF and BP in normal and ischemic rats as follows ; ALE was injected ASS leaves 45% ethyl alcohol extract, ASE was injected ASS stems 45% ethyl alcohol extract, ARE was injected ASS roots 45% ethyl alcohol extract. The results were as follows ; In normal rats, ALE and ARE significantly increased rCBF in a dose-dependent manner, but ASE significantly decreased rCBF and MABP in a dose-dependent manner, ALE increased MABP In ischemic rats, rCBF was significantly and staDly improved by ARE (10 mg/kg, i.p.) during the period of cerebral reperfusion, which was contrasted with the findings of rapid and marked increase in control group. From the above results, it was thought that all part of Acanthopanax sessiliflorus $S_{EEM}$ was effective for hemodynamics and especially ARE was more effective than other parts.
Purpose: The aim of this retrospective study was to analyze the characteristics of peripheral arterial ischemia and tissue necrosis in premature babies, as well as the effects of nitroglycerin. Methods: In total, 513 newborns were enrolled who were admitted to the neonatal intensive care unit with a gestational age of 34 weeks or younger. Data were collected on general personal and clinical information, peripheral arterial ischemia, and nitroglycerin patch application in the premature infants. The collected data were analyzed using the χ2 test, t-test, Mann-Whitney U test, logistic regression. Results: Thirty-six (7.0%) infants had peripheral arterial ischemia, while 477 (93.0%) infants did not. Lower gestational age (χ2=35.97, p<.001), lower birth weight (χ2=29.40, p<.001), lower blood pressure (χ2=23.10, p<.001), and insertion of an umbilical artery catheter (p<.001) were significantly associated with the occurrence of peripheral arterial ischemia. Among the preterm infants in whom nitroglycerin patches were applied, 30 (83.3%) premature infants without necrosis improved without complications, 4 (11.1%) showed hypotension, and 2 (5.6%) showed skin damage. Conclusion: Based on a review of our experiences with nitroglycerin patches, we recommend closely observing skin color and using nitroglycerin patches on the skin to help improve flow in premature infants with peripheral arterial ischemia.

This study was undertaken to define the effect of DaeSiHo-Tang extract on the hypertension in spontaneous hypertensive rat and norepinephrine-induced arterial contraction in rabbit. Systolic blood pressure and blood velocity were significantly attenuated by administration of DaeSiHo-Tang extract. but blood flow and renin-angiotensin-aldosterone system unaffected by DaeSiHo-Tang extract. The relaxation effect of DaeSiHo-Tang extract was dependent on the presence of endothelium, showing that DaeSiHo-Tang extract-induced relaxation was not observed in the strips without endothelium. The endothelium-dependent relaxation induced by DaeSiHo-Tang extract was decreased by the pretreatment of $N{\omega}$-nitro-L-arginine or methylene blue, but it was not observed in the strips pretreated with indomethacin or tetraethylammonium chloride. When $Ca^{2+}$ was applied, the strips which were contracted by norepinephrine in a $Ca^{2+}$-free solution, arterial contraction was increased. But pre-treatment of DaeSiHo-Tang extract inhibited contractile response to $Ca^{2+}$. These results indicate that antihypertensive effect of DaeSiHo-Tang extract is due to descend arterial resistance by the arterial relaxation through the formation of nitric oxide in the vascular endothelial cells.
Cerebrovascular disease is one of the three major causes of death in Korea. Since these diseases are associated with atherosclerosis, the diagnosis of atherosclerotic factors should be presented. In this study, we evaluated the relationship between brachial-ankle arterial pulse wave velocity, cerebral artery vascular stenosis, blood pressure, obesity, and abdominal obesity by age group. The significance of cerebral artery stenosis and age group. The risk factors of atherosclerosis, such as blood pressure, obesity, and abdominal obesity, were significant in all age groups. When the pulse wave velocity of the brachial-ankle artery was increased, the cerebral artery stenosis was distributed in 57.3% of the total test subjects. If the arterial stiffness is suspected during the measurement of the pulse wave velocity of the brachial ankle artery, We recommend suspected vascular stenosis and perform a cerebral artery angiography. It is suggested that the data will be used as a baseline data for similar studies after evaluating the significance of blood pressure, obesity, and abdominal obesity as risk factors of atherosclerosis.
To determine the frequency of past and present obesity among patients with NIDDM and to identify the differences of body fat, blood pressure and C-peptide/glucose ratio according to obese diabetic patients (BMI$\geq$25 kg/$m^2$) and nonobese (BMI<25 kg/$m^2$). Also the final factor is to observe the anthrometric change patterns in the study. Method: The weight at 20 years-old, previous maximal body weight, and acute weight loss were queried. Current height, body weight, BMI, waist & hip circumferences, waist-hip ratio, skinfold thicknesses, blood pressure, fasting blood glucose, and fasting C-peptide were measured in one hundred sixty-seven NIDDM patients. The differences of the parameters ccording to obese and nonobese, and three anthropometric change patterns were analyzed. Result: Results were as follows: 1. 66.5 % of the NIDDM patients had a history of past obesity as assessed by their maximum weight, while only 33.2% of them were currently obese (p's < 0.001). 2. The waist & hip circumferences, skinfold thicknesses, systolic, diastolic & mean arterial blood pressure in obese patients were greater than those of nonobese patients (all p's < 0.001). 3. The waist and the hip circumferences, and skinfold thicknesses (subscapula & triceps) were highest among the obese-obese group. WHR and abdominal skinfold thickness in the obese-obese and obese-nonobese groups were higher than those in the nonobese- nonobese group. Systolic & diastolic and mean arterial blood pressures in the obese-obese group were higher than those of obese-nonobese and nonobese-nonobese groups(all p's < 0.005). 4. The abdominal and subscapular skinfold thicknesses in female diabetic patients were greater than those of male patients (all p's <0.0001). Conclusion: Although most Korean NIDDM patients were previously obese, many of them were not obese during the course of the study. Greater central and upper body adiposicity and higher blood pressure was shown in obese diabetic patients. Also, greater central and upper body adiposicity was demonstrated in female diabetic patients.
