중심정맥관의 동맥 내 거치: 증례보고 (Inadvertent Arterial Catheterization of Central Venous Catheter: A Case Report)

  • Central venous catheterization is one of the most important procedures for initial resuscitation of hemodynamically unstable patients including multiple trauma patients. Inadvertent arterial placement of the large caliber central venous catheter can results in resuscitation failure as well as unnecessary invasive treatment. Here, we report an arterial puncture during central venous catheterization which may lead to inadvertent arterial catheterization. We recommend that arterial catheterization should be evaluated before dilator insertion during Seldinger's method. Ultrasound can help in preventing the inadvertent arterial catheterization of central venous catheter.

심도자술후 발생한 대퇴동맥 혈전증 환아에서 동맥내 Urokinase 국소 주입요법의 효과 (Intraarterial Catheter-directed Urokinase Infusion for Femoral Artery Thrombosis after Cardiac Catheterization in Infants and Children)

  • 목 적 : 대퇴동맥 혈전증은 하지 절단같은 극단적인 재앙뿐만 아니라 하지의 성장 장애를 초래할 수 있는 심도자술의 심각한 합병증이지만, 소아에서 이에 대한 표준적 치료법은 아직 정립되지 못한 상태이다. 저자는 심도자술후 발생한 대퇴동맥 혈전증에서 urokinase의 동맥내 국소 주입요법의 유용성을 검토하고자 본 연구를 시행하였다. 방 법 : 1994년 1월부터 2002년 8월까지 심도자술 후 발생한 대퇴동맥 혈전증으로 동아대학교병원 소아과에서 동맥내 urokinase 국소 주입법을 이용해 혈전용해술을 받은 9명, 12례를 대상으로 하여, 병력지와 혈관조영 소견을 후향적으로 분석하였다. 결 과 : 1) 대퇴동맥을 이용한 심도자술이 행해진 391례 중 전신적 헤파린 또는 urokinase에 반응을 보이지 않았던 대퇴동맥 혈전증의 발생빈도는 2.8퍼센트였다. 2) 대상 환아들의 연령은 기하평균 5.8개월(1-71개월)이었고 체중은 $8.5{\pm}4.6kg$(3.5-20.5 kg)였다. 3) Urokinase는 1,000-4,400 unit/kg/hr로 $50.6{\pm}29.2$시간(18-110시간)에 걸쳐 주입하였는데, 치료중 2례에서 환측의 천자부위로 출혈이 있었으며, 한명은 수혈이 필요했다. 심도자술후 4일 이내에 치료를 시작했던 환아들은 모두 혈전의 완전 소실을 보였다. 혈전 형성후 각각 12일과 19일째 치료를 시작했던 2례는 호전되지 않아 풍선 혈관성형술을 실시하였는데 부분적으로 도움이 되었다. 결 론: 심도자술후 발생한 대퇴동맥 혈전증에서, 전신적 혈전용해제 투여로 회복되지 않으면, 반대측 대퇴동맥을 통한 국소적 동맥내 혈전용해제 투여를 조기에 시행하는 것이 효과적이다.

  • 15년전 심방중격 결손증 진단을 받은 47세 남자 환자가 심도자 검사를 위해 입원하였다. 환자는 입술과 손톱에서 명확한 청색증을 보이고 있었으며 심한 폐동맥 고혈압을 나타내고 있었다. 본 환자는 지난 수년간 아이젠멩거 증후군으로 진단되어 대증적 치료만을 받아오고 있었다. 심도자 검사 후 환자는 흡입형 Iloprost 치료를 시작 하였으며 성공적으로 심방중격결손증 수술을 받을 수 있었다. 환자는 수술 후에도 치료를 지속하였다.

Deep Vein Thrombosis Due to Hematoma as a Rare Complication after Femoral Arterial Catheterization

  • Hematoma is quite a common complication of femoral arterial catheterization. However, to the best of our knowledge, there have been no previous studies regarding deep vein thrombosis (DVT) caused by compression of a vein due to a hematoma. We report a case of a hematoma developing after femoral arterial catheterization and causing extensive symptomatic DVT. A 59-year-old male was seen in our Emergency Department with right lower leg swelling 15 days after coronary stent implantation performed using right femoral artery access. Computed tomographic (CT) scanning revealed a large hematoma (45 mm in its longest diameter) compressing the common femoral vein and with DVT from the right external iliac vein to the popliteal vein. Due to the extensive DVT involvement, we decided to release the compressed common femoral vein by surgical evacuation of the large hematoma. However, even following evacuation of the hematoma, as the DVT did not resolve soon, further mechanical thrombectomy and catheter-directed thrombolysis were performed. Angiography then showed nearly resolved DVT, and the leg swelling was improved. The patient was discharged with the anticoagulation medication, warfarin.

심실중격결손증을 동반한 대혈관 전위증 및 양대동맥 우심실 기시증에 대한 동맥전환술 (Arterial Switch Operation for Transposition of The Great Arteries with Ventricular Septal Defect and for Double Outlet Right Ventricle with Subpulmonary Venricular Septal Defect)

  • Arterial switch operation for repair of nineteen cases of transposition of the great arteries associated with ventricular septal defect and three cases with double outlet right ventricle with subpulmonary ventricular septal defect[Taussig-Bing type DORV] was performed from November 1987 to September 1990 at the Seoul National University Children`s Hospital. Sixteen of them were under six months of age, and three were under one year of age with body weight ranged from three to fourteen kilograms. Preoperative cardiac catheterization was done in eighteen patients, in which the pressure of the left ventricle was greater than 70% of the right ventricle in all but one. Patent ductus was associated in thirteen cases[68.4%] of TGA+VSD, and atrial septal defect or patent oval foramen was in sixteen cases. Four atrial septostomy, one modified Blalock- Taussig shunt, one pulmonary artery banding, one coarctoplasty using subclavian arterial flap, were perfomed before arterial switch operation. There were five hospital deaths, all in the. patients with transposition of the great arteries with ventricular septal defect[overall mortality rate 22.7%]. Lecompte Maneuver was used in all patients, and in all patient the U-shaped flap of coronary arteries were transposed to V-shaped cleavage created in the neoaorta. Arterial defect in the neopulmonary artery was covered with 0.0625% Glutaraldehyde fixed autogenous pericardium There have been no late deaths, Postoperative cardiac catheterization and angiocardiogram in four patients has revealed no stenosis in the neopulmonary artery or neoaorta with reasonable P[RV/LV], Anatomic correction for transposition and double outlet right ventricle with subpulmonary ventricular septal defect would seem to be a good operative alternative to intraatrial switch procedures, with the advantage of incorporating the left ventricle to systemic circulation.

폐동맥 고혈압을 동반한 후천성 심질환의 술후 폐동맥압의 변화에 대한 연구 (A study of postoperative pulmonary arterial pressure change in the acquired heart disease with pulmonary hypertension)

  • To see the change of pulmonary arterial pressure after mitral valve replacement, postoperative cardiac catheterization and echocardiographies were performed in 12 patients of mitral valvular disease with pulmonary hypertension[systolic pulmonary arterial pressure>50 mm Hg]. The mean follow-up duration was 35.4[range: 15-47] months per patient. The following results were obtained. 1] Preoperative systolic pulmonary arterial pressure value of 66.17\ulcorner10.73mmHg decreased significantly to 29.17\ulcorner6.86mmHg postoperatively[p<0.01]. 2] Preoperative Pp/Ps value of 0.67\ulcorner0.13 decreased significantly to 0.28\ulcorner0.06 postoperatively[p<0.01]. 3] Preoperative PAWP value of 29.00\ulcorner4.02mmHg decreased significantly to 9.92\ulcorner4.27 mmHg postoperatively[p<0.01]. 4] Preoperative LAD value of 5.58\ulcorner1.20cm decreased significantly to 4.37\ulcorner0.67cm postoperatively [p<0.01]. In conclusion, pulmonary arterial hypertension secondary to mitral valvular disease could be reduced to normal range after successful mitral valve replacement.

우발적인 척추동맥으로의 중심정맥 카테터의 삽관 (Accidental Vertebral Artery Cannulation as a Complication of the Central Venous Catherization)

  • Central venous catheterization through a subclavian approach is indicated for some special purposes but it may cause many complications such as infection, bleeding, pneumothorax, thrombosis, air embolization, arrhythmia, myocardial perforation, and nerve injury. A case involving a mistaken central venous catheterization into the right vertebral artery through the subclavian artery is presented. A 33-year-old man who had deteriorated mentality after head injury underwent an emergency craniotomy for acute epidural hematomas on the right frontal and temporal convexities. His mentality improved rapidly, but he complained of continuous severe pain in the right posterior neck even though he had no previous symptom or past medical history of such pain. Three-dimensional cervical spine computed tomography (3D-CT) was performed first to rule out unconfirmed cervical injuries and it revealed a linear radiopaque material intrathoracically from the level of the 1st rib up to the level of C6 in the right vertebral foramen. An additional neck CT was performed, and the subclavian catheter was indwelling in the right vertebral artery through right subclavian artery. For the purpose of proper fluid infusion and central venous pressure monitoring, the subclavian vein catheterization had been performed in the operation room after general anesthesia induction before the craniotomy. Sufficient anatomical consideration and prudence is essential because inadvertent arterial cannulation at a non-compressible site is a highly risky iatrogenic complication of central venous line placement.

Folded-Loop Guidewire Remodeling Technique: 색전술 시 급격한 분지 각도를 가지는 혈관의 선택적 진입 방법 (Folded-Loop Guidewire Remodeling Technique: Catheterizing Markedly Angulated Branches during Intravascular Embolization)

  • 목적 경피적 혈관내 시술 시 급격한 분지 각도를 가지는 혈관의 선택적 진입이 어려운 경우에 활용할 수 있는 새로운 guidewire shaping 방법인 folded-loop guidewire remodeling 방법을 소개한다. 대상과 방법 0.014 inch 미세 guidewire tip을 pigtail loop 형태로 만든 후, metal introducer를 이용하여 guidewire를 microcatheter로 진입시킨다. 혈관 내에서 guidewire를 회전시키면 기존의 pigtail loop 형태로 쉽게 guidewire tip을 변형시킬 수 있다. Guidewire를 뒤로 당기면 guidewire tip이 작은 U형으로 변형되면서 분지 혈관으로 쉽게 진입된다. 결과 2019년 12월부터 2022년 1월까지 동맥 색전술 시 기존 방법으로 분지동맥의 선택적 진입이 어려웠던 64명의 환자(남/여, 49/15; 평균나이 66.8 ± 9.5세)에서 본 방법을 시행하였으며, 98%의 성공률을 확인하였다. 색전술의 적응증은 transcatheter arterial chemoembolization, 위장관 출혈, 객혈, 외상출혈, 종양출혈이었다. 결론 Folded-loop guidewire remodeling 방법은 기존의 보편적 방법으로 선택이 어려웠던 급격한 분지 각도를 가지는 혈관의 선택적 진입에 효과적인 방법이다.

심실중격결손증에서 술전 심도자검사치와 결손크기의 상관관계에 대한 연구 (The Correlation between Cardiac Catheterization Data and Defect Size in Ventricular Septal Defect)

  • We clinically evaluated 121 cases of ventricular septal defect which we operated from April, 1986 to December, 1989 at Inha General Hospital, Seong-Nam, Department of Thoracic and Cardiovascular Surgery, College of Medicine, Inha University. These patients were occupied 54.8% of all congenital heart diseases operated on its same period. Of the 121 patients, 63 patients were male[52.1%] and 58 patients were female[47.9i]. The two most common symptoms were frequent upper respiratory infection and dyspnea on exertion. By Kirklin s anatomical classification, type I constituted 34.7%, type II 61.98%, type III 0.03% and type IV not occupied. Associated cardiac anomalies were found in 34 cases, and PDA was most common associated anomaly, occupied in 22 cases. On the cardiac catheterization data, there were statistically significant correlation between VSD size[cm2 /BSAm2] and systolic pulmonary arterial pressure[sPAP], pulmonary to systemic flow ratio[Qp/Qs] & pulmonary to systemic pressure ratio[Pp/Ps] respectively, Type II [r=0.53, p<0.01] was more correlated than type I [r=0.49, p<0.05] between VSD size and Qp /Qs. We could not found the correlationship between age and Qp/Qs [Type I; r=0.16, Type II; r=-0.15] All cases were operated under cardiopulmonary bypass and 58 cases[46.3%] were operated through the right atrial approach, and 34 cases[28.1%] through the pulmonary arterial approach. Operative mortality rate was 4.13%[5 cases].

Ultrasound-guided internal jugular vein catheterization in critically ill pediatric patients

  • Purpose: Continuous intravenous access is imperative in emergency situations. Ultrasound-guided internal jugular vein (IJV) catheterization was investigated in critically ill pediatric patients to assess the feasibility of the procedure. Methods: Patients admitted to the pediatric intensive care unit between February 2011 and September 2012 were enrolled in this study. All patients received a central venous catheter from attending house staff under ultrasound guidance. Outcome measures included successful insertion of the catheter, cannulation time, number of cannulation attempts, and number and type of resulting complications. Results: Forty-one central venous catheters (93.2%) were successfully inserted into 44 patients (21 males and 23 females; mean age, $6.54{\pm}1.06$ years). Thirty-three patients (75.0%) had neurological disorders. The right IJV was used for catheter insertion in 34 cases (82.9%). The mean number of cannulation attempts and the mean cannulation time was $1.57{\pm}0.34$ and $14.07{\pm}1.91$ minutes, respectively, the mean catheter dwell time was $14.73{\pm}2.5$ days. Accidental catheter removal was observed in 9 patients (22.0%). Six patients (13.6%) reported complications, the most serious being catheter-related sepsis, which affected 1 patient (2.3%). Other complications included 2 reported cases of catheter malposition (4.6%), and 1 case each of arterial puncture (2.3%), pneumothorax (2.3%), and skin infection (2.3%). Conclusion: The results suggest that ultrasound-guided IJV catheterization can be performed easily and without any serious complications in pediatric patients, even when performed by visiting house staff. Therefore, ultrasound-guided IJV catheterization is strongly recommended for critically ill pediatric patients.
