Cemento-osseous dysplasia: clinical presentation and symptoms

  • Objectives: The purpose of this study was to evaluate risk factors and symptoms in cemento-osseous dysplasia (COD) patients. Materials and Methods: In this study, 62 patients who were diagnosed histologically with COD were investigated from 2010 to 2020 at the author's institution. We compared clinical and radiological characteristics of symptomatic and asymptomatic patients. The factors were sex, age, lesion size, site, radiologic stage of lesion, apical involvement, sign of infection, and history of tooth extraction. Statistical analysis was performed using Fisher's exact test and the chi-square test. Results: COD was more prevalent in female patients. With the exception of three cases, all were focal COD. The majority of patients presented with symptoms when the lesion was smaller than 1.5 cm in size. Symptoms were observed when the apex of the tooth was included in the lesion or there was a local infection around the lesion. The history of tooth extraction and previous endodontic treatment were evaluated, and history was not a significant predictor for the onset of symptoms. Conclusion: In this study, risk factors associated with symptomatic patients were size of lesion, apical involvement, and local infection.

Cutting efficiency of apical preparation using ultrasonic tips with microprojections: confocal laser scanning microscopy study

  • Objectives: The purpose of this study was to compare the cutting efficiency of a newly developed microprojection tip and a diamond-coated tip under two different engine powers. Materials and Methods: The apical 3 mm of each root was resected, and root-end preparation was performed with upward and downward pressure using one of the ultrasonic tips, KIS-1D (Obtura Spartan) or JT-5B (B&L Biotech Ltd.). The ultrasonic engine was set to power-1 or -4. Forty teeth were randomly divided into four groups: K1 (KIS-1D / Power-1), J1 (JT-5B / Power-1), K4 (KIS-1D / Power-4), and J4 (JT-5B / Power-4). The total time required for root-end preparation was recorded. All teeth were resected and the apical parts were evaluated for the number and length of cracks using a confocal scanning micrscope. The size of the root-end cavity and the width of the remaining dentin were recorded. The data were statistically analyzed using two-way analysis of variance and a Mann-Whitney test. Results: There was no significant difference in the time required between the instrument groups, but the power-4 groups showed reduced preparation time for both instrument groups (p < 0.05). The K4 and J4 groups with a power-4 showed a significantly higher crack formation and a longer crack irrespective of the instruments. There was no significant difference in the remaining dentin thickness or any of the parameters after preparation. Conclusions: Ultrasonic tips with microprojections would be an option to substitute for the conventional ultrasonic tips with a diamond coating with the same clinical efficiency.

Push-out bond strength and marginal adaptation of apical plugs with bioactive endodontic cements in simulated immature teeth

  • Objectives: This study evaluates the bond strength and marginal adaptation of mineral trioxide aggregate (MTA) Repair HP and Biodentine used as apical plugs; MTA was used as reference material for comparison. Materials and Methods: A total of 30 single-rooted teeth with standardized, artificially created open apices were randomly divided into 3 groups (n = 10 per group), according to the material used to form 6-mm-thick apical plugs: group 1 (MTA Repair HP); group 2 (Biodentine); and group 3 (white MTA). Subsequently, the specimens were transversely sectioned to obtain 2 (cervical and apical) 2.5-mm-thick slices per root. Epoxy resin replicas were observed under a scanning electron microscope to measure the gap size at the material/dentin interface (the largest and smaller gaps were recorded for each replica). The bond strength of the investigated materials to dentin was determined using the push-out test. The variable bond strengths and gap sizes were evaluated independently at the apical and cervical root dentin slices. Data were analyzed using descriptive and analytic statistics. Results: The comparison between the groups regarding the variables' bond strengths and gap sizes showed no statistical difference (p > 0.05) except for a single difference in the smallest gap at the cervical root dentin slice, which was higher in group 3 than in group 1 (p < 0.05). Conclusions: The bond strength and marginal adaptation to root canal walls of MTA HP and Biodentine cement were comparable to white MTA.

수 종의 Ni-Ti 회전 기구들을 이용한 치근단 폐쇄 향상을 위한 근관 확대 평가 (Evaluation of canal preparation for apical sealing with various Ni-Ti rotary instruments)

  • 연구목적: 본 연구의 목적은 원형의 치근단 삭제를 제공하는 능력에 관한 수 종의 Ni-Ti 회전 기구들을 평가하는 것이다. 연구 재료 및 방법: 실험은 50개의 단근치를 선택하여 백악상아질 경계에서 절단하였고, 근관의 치관부 1/3은 Gates Glidden bur (#2, #3, #4)를 이용하여 확대하였다. 시편들은 각 군당 10개씩 5개의 실험군들로 나누었다. 1군은 GT file, Profile .04, Quantec #9, #10을 사용했고, 2군은 1군과 유사하나 Quantec 파일 대신 Lightspeed를 사용했고, 3군은 Orifice shaper, Profile .06 종류들, Lightspeed를 사용했고, 4군은 3군과 유사하나 Lightspeed 대신 Quantec #9, #10를 사용했고, 5군은 근관 확대를 위해 GT file, Profile .04를 사용했다. 모든 치아 시편들은 치근첨으로부터 1 mm, 3 mm, 5 mm에서 절단하여 현미경하에서 관찰했다. 결과: 본 연구의 결과 실험군 2, 3군(Lightspeed file군)이 실험군 1, 4군(Quantec), 실험군 5군(GT file, profile .04종류들)에 비하여 치근단 1 mm에서 더 원형에 가까운 치근단 삭제를 보였다(p < 0.05). 치근단 3, 5 mm들에서는 각 군들 사이에서 통계학적 유의차가 없었다. 치근단 5 mm 시편들에서는 대부분의 시편들이 완전한 원형을 나타내었으며 불규칙한 모양은 관찰되지 않았다. 결론: 치근단 1 mm 시편에서 Lightspeed를 사용한 경우에서 다른 Quantec, GT와 Profile .04 종류들을 사용한 것보다 더 원형의 삭제를 얻었고, 근관장에서 최초의 Lightspeed에 맞는 크기보다 ISO 크기보다 3단계 크기 확대로는 모든 치근단 1 mm에서 원형의 삭제를 얻었을 수 없었고 단지 35%에서만 원형의 삭제를 얻을 수 있었다는 것이다. 따라서 ISO 크기보다 3단계 크기 확대가 원형 삭제를 얻기에는 불충분하며 좀 더 큰 크기로 확대하는 것이 필요함을 알 수 있으나 치아에 따라 치근단 부위 치질의 양이 다를 수 있으므로 일률적으로 적용하기에는 무리가 있을 수 있다.

개방 근첨 치아의 근관 충전방법에 따른 치근단 폐쇄효과에 관한 연구 (THE SEALING ABILITY OF OBTURATION TECHNIQUES IN OPEN APEX)

  • The purpose of this study was to compare the leakage of four different obturation techniques in conjunction with immediate apical barrier of ${\beta}$-tricalcium phosphate(TCP) in teeth with open apex. Eighty single-rooted human premolar teeth were prepared and sectioned horizontally, so maximum diameter in apex was 4mm. Apical defects that were similar to open apex, were created with #1/2 round bur and SF104R bur. The apical foramen were opened to a size 80 file extended 3mm beyond the apex. The teeth were placed into the oasis block soaked saline to simulate periapical tissue often associated with pulpless teeth and received apical barriers consisting of TCP followed by obturation using lateral condensation technique, vertical condensation technique, continuous wave technique and thermoplasticized gutta-percha injection technique. Two unobturated teeth served as positive and negative controls. Teeth were immersed in resorcinol-formaldehyde resin for S days at $4^{\circ}C$, and the resin was allowed to polymerize completely for 4 days at room temperature. Teeth were then sectioned horizontally at 1.5mm(level 1), 2.5mm(level 2) and 3.5mm(level 3) from the apex, and examined under a stereomicroscope at ${\times}40$ magnification. The photographs were taken at ${\times}40$ magnification of the filling in each level and scanned. The leakage length in tooth/resin interface was measured at each of the three levels. Each ratio of leakage was obtained by calculating the ratio of the leakage length of canal wall infiltrated with resin to the total length of the canal and was analyzed statistically(One-way ANOVA and Scheffe test). The result were as follows : 1. At the level 1, there was the least leakage in the thermoplasticized gutta-percha injection technique group(group 4), but there was statistically significant(p<0.05). 2. At the level 2, there was the least leakage in the thermoplasticized gutta-percha injection technique group(group 4), and the most leakage in the continuous wave technique group(group 3). There was statistically significant difference between the thermoplasticized gutta-percha injection technique group and the continuous wave technique group(p<0.05). 3. At the level 3, there was the least leakage in the thermoplasticized gutta-percha injection technique group(group 4), but there were no statistically significant differences between other groups(p>0.05). These results suggest that thermoplasticized gutta-percha injection technique which had 1mm apical gutta-percha matrix after the formation of TCP apical barrier, can demonstrate favorable apical sealing.

Continuous wave of obturation technique에서 플러거의 다양한 적용 깊이에 따른 근단부 폐쇄효율 비교 (COMPARISON OF APICAL SEALING EFFICACIES USING DIFFERENT PLUGGING DEPTH IN CONTINUOUS WAVE OF OBTURATION TECHNIQUE)

  • 이 연구의 목적은 continuous wave of obturation technique으로 근관 충전을 할 때, 다양한 heat source plugger의 깊이에 따른 근단부 근관 폐쇄능력을 비교하는 것이었다. 직선 형태 발거 소구치를 #35/.06 크기까지 니켈티타늄 파일을 사용하여 성형 및 세정하였다. 성형과 세정이 완료된 치아를 heat source plugger의 적용 깊이에 따라 15개씩 5군으로 분류하였다. 3 mm, 4 mm, 5 mm, 6 mm, 7 mm, 양성 대조군을 제외한 모든 근관은 E&Q plus(Meta Biomed Co., Chungju, Korea)를 사용하여 #35/.06 gutta-percha cone으로 충전하였다. 모든 치아의 근첨공 주변 1 mm를 제외하고 nail varnish를 도포하였고 음성 대조군은 근첨공을 포함하여 전체 면을 도포하였다. 모든 시편을 1% 메틸렌블루 용액에 72시간 동안 보관한 다음 치근첨으로부터 1, 2, 3 mm 높이에서 수평으로 절단하였다. 각 단면을 $12.5{\times}2.5$배 현미경하에서 관찰하고 디지털 카메라로 촬영하였다. 염색제 누출 평가를 위해 aye-degree scoring system을 사용하였다. 각 절단면에서의 점수를 모두 합하여 한 시편의 점수로 사용하고 통계학적 분석을 하였다. 이 실험 조건하에서, 3-7 mm까지의 heat source plugger의 적용 깊이는 근단부 폐쇄에서 유의한 차이를 나타내지 않았다. 5-7 mm의 Buchanan's protocol을 포함한 3-7 mm를 모두 임상 적용에서 받아들여질 만하다고 보인다.

근관형성 방법에 따른 근관변위와 잔존치질의 두께에 관한 연구 (EVALUATION OF ROOT CANAL TRANSPORTATION AND REMAINING DENTIN/CEMENTUM THICKNESS FOLLOWING HAND AND ULTRASONIC INSTRUMENTATION)

  • The purpose of this study was to evaluate the root canal transportation and remaining dentin / cementum thickness after using hand and ultrasonic instrumentation in the curved mesial root canals of extracted human mandibular molars. Fourty - six clear polyether blocks were made and randomly divided into two groups: hand instrumentation group with K - Flex files and ultrasonic instrumentation group with Suprasson SP unit. All root canals were instrumented to a size corresponding to a # 30 K - Flex file 1mm short from the radiographic apex. The roots were then sectioned perpendicular to the long axis so the apical and middle third could be evaluated with the Zoom stereomicroscope. The results were as follows : 1. In the total amount of removed dentin at middle third level, there was not significant difference between the hand instrumentation and ultrasonic instrumentation (P>0.05). 2. In the total amount of removed dentin at apical third level, there was more removed by the ultrasonic instrumentation than hand instrumentation(P<0.005). 3. In the transportation width, there was not significant difference between the two groups at both sectioned levels(P>0.05). 4. It was suggested that the canal was transported distally at middle third level and mesially at apical third level by booth techniques.

근관내(根管內) 근관충전제(根管充塡劑)의 유입성(流入性)에 관(關)한 실험적(實驗的) 연구(硏究) (AN EXPERIMENTAL STUDY ON THE FLOW OF CANAL SEALER BY MEANS OF LENTULO)

  • Artificial canals were prepared by means of engine reamer on the slab which was made of clear acrylic resin. The shape of this canal is similar to the natural canal, the size is almost same as Kerr File No. 70, and the length is about 20mm. Sealer, Zine oxide eugenol mixture was prepared as matched to spatula test. Lentulo filler was inserted as deep as middle portion and apical one-third portion of the canal and the dental engine was driven in the speed of 1,000 and 3,000 r.p.m. in 30 seconds and 60 seconds with feeding of the sealer. The author measured the excess flow of the canal sealer from Lentulo end. Following are the results obtained from this study. 1. Lentulo insertion at the middle portion 1,000 r.p.m. in 30 seconds case: 0.21mm Same as above in 60 seconds: 0.20mm 2. Lentulo insertion at apical-thirds. 1,000 r.p.m. in 30 seconds: 0.22mm. Same as above in 60 seconds: 0.20mm. 3. Lentulo insertion at middle portion. 3,000 r.p.m. in 30 seconds: 0.14mm Same as above in 60 seconds: 0.25mm. 4. Lentulo insertion at apical-thirds, 3,000 r.p.m. in 30 seconds: 0.11mm Same as above in 60 seconds: 0.27mm.

Anatomical factors of the maxillary tuberosity that influence molar distalization

  • Objective: To examine the areas of the maxillary tuberosity (MT) (coronal, apical, width, and height) with respect to the presence or absence of the third molar to establish possible anatomical limitations for molar distalization. Methods: A total of 277 tuberosities were evaluated through sagittal computed tomography (CT) images, divided for measurement into coronal (free of bone), apical (area of influence of the maxillary sinus), and tuberosity (bony area) zones, and stratified by the presence or absence of the third molar, sex, and two age subgroups. Mann-Whitney U test was used to compare the groups considering the third molar. Results: The medians of the width and height of the tuberosity decreased significantly in the absence of the third molar (P < 0.001). The apical area also showed differences, with negative values in the absence of the third molar and positive values in the presence of the third molar (P < 0.001). However, no differences were observed for the coronal area (P > 0.05). Conclusions: In the absence of the third molar, the size of the MT, represented by its width and height, was smaller and negative values (decrease) were observed for the maxillary sinus. The sagittal CT provides useful information regarding the amount of bone tissue available for distalization and relationship of the second molar with respect to the maxillary sinus, which allows individualizing each case in relation to the amount and type of movement expected.

Efficacy of reciprocating and rotary retreatment nickel-titanium file systems for removing filling materials with a complementary cleaning method in oval canals

  • Objectives: This study aimed to evaluate and compare the efficacy of the S1 reciprocating system and the D-Race retreatment rotary system for filling material removal and the apical extrusion of debris. Materials and Methods: Sixty-four freshly extracted maxillary canines were shaped with size 10 and size 15 K-files, instrumented using ProTaper Gold under irrigation with 2.5% sodium hypochlorite (NaOCl), obturated according to the principle of thermo-mechanical condensation with gutta-percha and zinc oxide eugenol sealer, and allowed to set for 3 weeks at 37℃. Subsequently, the teeth were divided into a control group (n = 4), the D-Race rotary instrument group (n = 30), and the S1 reciprocating instrument group (n = 30). After classical retreatment, the canals were subjected to a complementary approach with the XP-Endo Shaper. Desocclusol was used as a solvent, and irrigation with 2.5% NaOCl was performed. Each group was divided into subgroups according to the timing of radiographic readings. The images were imported into a software program to measure the remaining filling material, the apical extrusion, and the root canal space. The data were statistically analyzed using the Z-test and JASP graphics software. Results: No significant differences were found between the D-Race and S1 groups for primary retreatment; however, using a complementary cleaning method increased the removal of remnant filling (p < 0.05). Conclusions: Classical removal of canal filling material may not be sufficient for root canal disinfection, although a complementary finishing approach improved the results. Nevertheless, all systems left some debris and caused apical extrusion.
