K-File과 초음파기구의 근관확대 및 세척효과에 대한 비교연구 (A COMPARATIVE STUDY OF THE EFFECT K - FILE AND ULTRASONIC INSTRUMENT IN CLEANING AND SHAPING ROOT CANAL)

  • The purpose of this study was to compare the effectiveness of hand instrumentation with K - file and ultrasonic instrumentation and irrigation system in removing pulpal debris and canal wall planing. 20 mandibular molar teeth were instrumented to size 30 K - file and 20 teeth were instrumented with ultrasonic Suprasson. And Normal Saline and 2.5% NaOCl were used as irrigation solution. All specimens were viewed at the coronal, middle, and apical third of the root canals for the evaluation of the cleaning effect under the multiview microscope. The result were as follows : 1. All of the technique and irrigation solution was effecient in the debris removal and canal wall planing at the cervical and middle thirds of the root canal. 2. All of the techniques and irrigation solutions was less efficient in the debris removal and canal planing at the apical third of the root canal. 3. The debris removal and canal wall planing was depended more on the anatomical variations of the root canal than on the techniques and irrigation solutions.

만곡근관의 확대시 기구에 따른 형태변화에 관한 연구 (TRANSPORTATION OF CURVED CANAL AFTER CANAL ENLARGEMENT ACCORDING TO FILING INSTRUMENTS)

  • The purpose of this study was to evaluate the amount of transportation of original canal, zip formation, permanent deformation and fracture of instruments after canal enlargement. In this study, the 60 resin blocks that have curved canals were randomly divided into 3 experimental groups with 20 teeth each according to instrument types and filling methods for canal enlargement. The curved canals of each experimental groups were enlarged to No 40 ISO size with the K-flexo stainless steel file (Group 1), Engine-driven Ni-Ti Profile new series(Group 2) and Engine-driven Ni-Ti Quantec 2000 series(Group 3) according to the manufacturer's recommendation. Pre- and postoperative X-rays were taken at same position and the films were scanned and the canal images were traced to determine the canal curvature according to the method of Schneider. The amount of reduction in canal curvature were calculated between pre- and postoperative X-rays. In addition to zip formation, permanent deformation and fracture of instruments were examined after canal enlargement. The results were as follows : 1. All experimental groups showed some loss of canal curvature after instrumentation. There was a significant change in curvature between before and after instrumentation in each group(p<0.001). 2. Engine-driven Ni-Ti instrumentations resulted in an average loss of curvature of 2.36 degrees for Profile new series, 3.43 degrees for Quantec series, and hand instrumentation showed an average loss of curvature of 6.48 degrees for K-flexo file. There was a statistical significant difference between hand instrumentation and engine-driven Ni-Ti instrumentations(p<0.05). But there was no statistical difference between Profile new series and Quantec series. 3. There were many apical zip formations in group 1(Hand instrumentation). But there were no apical zip formations in group 2,3(Engine-driven Ni-Ti instrumentation). 4. The instrument deformation occured 9 cases in group 1(K-flexo file), 2 cases in group 2(Profile new series) and 3 cases in group 3(Quantec) after instrumentation. And the instrument fracture occured 1 case in each group. The results showed that the engine-driven Ni-Ti instruments, if we use carefully according to manufacturer's recommendations, can be use effectively for instrumenting the curved root canals in case of the MAF was over size 40.

치과용 단층촬영을 이용한 Stainless-Steel File과 MFile-System® 전동식 기구의 근관 성형 능력에 대한 비교 연구 (A Comparison of Stainless-Steel File and MFile-System® Ni-Ti Rotary Instrument in Canal Preparation using Dental Computed Tomography)

  • Objectives The aim of this study is to compare the quality of root canal preparation completed using MFile-$System^{(R)}$ instrument ( Komet, Gebr.Brsaseler, Germany) and conventional stainless steel file in the canals of Maxillary molar teeth that had a canal curvature between $25^{\circ}$ or more Materials & Methods Buccal canals of 24 first and second maxillary molar teeth, extracted for periodontal and prosthetic reasons were used. Tissue fragments and calcified debris were removed from teeth by scaling and the teeth were stored in 10% formalin solution for 24 hour. Then, teeth were stored in saline until used. To be included the roots had to have completed formed apices and angle of curvature ranging between $25^{\circ}$ or more according to the criteria described by Schneider(1971). Palatal and Second mesiobuccal canals were not included. Teeth were embedded into transparent acrylic. The teeth were randomly divided into two experimental groups. All teeth were scanned by Dental CT (PSR9000N, Asahi, Japan) to determine the root canal shape before instrumentation. Image slices were prepared from the apical end point to the pulp chamber. The first two sections were 2 mm from the apical end of root and 2 mm below the orifice. Further section was recorded, dividing the distance between the sections of apical and coronal levels into two equal lengths. 12 teeth were instrumented using stainless steel fileand another 12 teeth were instrumented using MFile-$System^{(R)}$. Following the completion of the instrumentation, the teeth were again scanned and compared with the cross sectional images taken prior to canal preparation. Amount of transportation and centering ability was assessed. Student's t-test was used for statistical analysis. Result Less transportation occurred with MFile-$System^{(R)}$ rotary instrumentation than stainless steel instrument. MFile-$System^{(R)}$ had better centering ability than stainless steel instrument. Conclusion MFile-$System^{(R)}$ rotary instrumentation transported canals less and had good centering ability.

Comparison of shaping ability of the Reciproc Blue and One Curve with or without glide path in simulated S-shaped root canals

  • Objectives: This study aimed to assess the impact of a glide-path on the shaping ability of 2 single-file instruments and to compare the centering ability, maintenance of original canal curvatures and area of instrumentation in simulated S-shaped root canals. Materials and Methods: Forty simulated S-shaped root canals were used and were prepared with One Curve (group OC), One G and OC (group GOC), Reciproc Blue (group RB) and R-Pilot and RB (group PRB) and scanned before and after instrumentation. The images were analyzed using AutoCAD. After superimposing the samples, 4 levels (D1, D2, D3, and D4) and 2 angles (Δ1 and Δ2) were established to evaluate the centering ability and modification of the canal curvatures. Then, the area of instrumentation (ΔA) was measured. The data were analyzed using 2-way analysis of variance and Tukey's test for multiple comparisons (p < 0.05). Results: Regarding the centering ability in the apical part (D3, D4), the use of the glide-path yielded better results than the single-file groups. Among the groups at D4, OC showed the worst results (p < 0.05). The OC system removed less material (ΔA) than the RB system, and for Δ1, OC yielded a worse result than RB (p < 0.05). Conclusions: The glide-path improved the centering ability in the apical part of the simulated S-shaped canals. The RB system showed a better centering ability in the apical part and major respect of the canal curvatures compared with OC system.

기계적 협착부를 갖는 근관에서 Root-ZX의 정확도에 관한 연구 (A STUDY ON THE ACCURACY OF THE ROOT-ZX IN THE CANAL WITH MECHANICALLY FORMED CONSTRICTION)

  • Currently electronic apex locators have been widely used to determine working length in endodontic treatment. According to Manufacture's recommendation, it is beneficial to find the working length before instrumenting the canal. However, in crown-down pressureless technique, working length of tooth is established following coronal instrumentation 3mm short of radiographic apex. In narrow canals, mechanically formed constriction might be established by coronal instrumentation in some distance from anatomical constriction. The purpose of this study was to evaluate the accuracy of Root-ZX in the canal with mechanical constriction following considerable coronal enlargement with ProFile .06 series. The 40 root canals in 30 extracted mandibular molars were accessed, and their actual length (AL) established by passing a size 10 file just through the minor apical foramen. The teeth were then embedded in an acrylic container with normal saline. The initial canal length(IL) was measured with Root-ZX by negotiating a size 10 file to the apical constriction. The canal was sequentially enlarged to size 40 with ProFile .06 file 3mm short of actual length. The enlarged final canal lengths (FL) were obtained with a size 15 file. The average values of IL, FL were calculated and compared using Repeated measures Analysis of Variance followed Turkey's Studentized Range test. The results were obtained as follows: 1. The initial canal length was 0.12mm shorter than actual canal length(P>0.05). 2. The differences between initial canal length and final canal length were not significant(P>0.05). 3. As a result of this study, regardless of mechanically formed constriction. Root-ZX differentiated between mechanical and anatomic constriction.

근관형성법이 근관잔Δ사의 치근단 정출에 미치는 영향 (EFFECT OF CANAL PREPARATION METHODS ON THE APICAL EXTRUSION OF DEBRIS)

  • 근관 형성중 잔사의 치근단 정출은 치근단 조직의 반응을 유발하여 동통과 부종을 일으키며 치유를 지연시킬 수 있다. 본 연구의 목적은 수종의 기구와 방법을 사용하여 근관을 형성하고 이에 따른 근관 잔사의 치근단 정출량을 비교함으로써 잔사의 정출을 최소화 할 수 있는 기구 조작법을 알아보고자 하는 것이다. 120개의 발거된 사람의 하악 절치에서 근관형성중 치근단으로 정출되는 잔사의 양을 비교하였다. 첫 번째 실험에서는 40개의 치아를 10개씩 4군으로 나누어 2.52% 치아염소산나트륨용액으로 근관을 세척하면서 다음의 4방법 중 하나를 이용하여 근관을 형성하였다. 수동 스테인레스 스틸 K-화일을 이용하여 step-back technique, crown-down pressureless technique으로 근관을 형성하였고 전동 니켈 티타늄 기구로는 Quantec SC series 2000과 Profile .04 taper series 29를 이용하여 근관을 형성하였다. 두 번째 실험에서는 80개의 치아를 16개씩 5군으로 나누고 근관 세척에 의한 잔사의 치근단 정출 영향을 배제하고자 근관 세척을 시행하지 않고 근관을 형성하였다. 이 때에는 첫 번째 실험의 방법중 Quantec SC series 2000 대신에 Quantec SC 및 Quantec LX를 이용하여 근관을 형성하였고 나머지는 첫 번째 실험과 동일하게 시행하였다. 각 치아로부터 정출되는 잔사를 용기에 모아 24 시간동안 건조시킨 후 전자 저울로 무게를 측정, 비교하여 다음과 같은 결과를 얻었다. 근관 세척을 시행한 경우와 시행하지 않은 경우 공히 step-back technique을 사용한 군에서는 다른 군에 비해 현저히 많은 량의 치근단 정출 잔사가 나타났다. 그러나, crown-down technique을 이용하여 근관 형성한 군과 수종의 Ni-Ti 파일을 이용하여 전동으로 근관 형성한 군들 사이에는 정출 잔사량에 유의한 차이가 나타나지 않았다. 따라서, 본 실험에 사용된 수동 및 전동 기구의 종류는 근관 잔사의 치근단 정출에 영향을 미치지 않는 것으로 생각되며, 근관잔사의 치근단 정출을 최소화하기 위해서는 수동 및 전동 기구 공히 step-back 방법에 의한 filing 동작보다는 crown-down technique의 reaming 동작을 사용하여 근관을 형성하는 것이 바람직하다고 사료된다.

  • A model system was used which enabled the same root canal system to be measured before and after coronal flaring of 51 extracted mandibular molars. The concavity of the distal surface of the mesial root was measured and the amount of reduction was compared after coronal flaring using step-back flared preparation, Gates-Glidden dirll or ultrasonic system(Quick-$\varepsilon$) at the furcation and apical 3mm from the furcation. The results were as follows: 1. The mean concavity of mesial root of manchbular molar was $0.73{\pm}0.27mm$ at the bifurcation and $0.65{\pm}0.23mm$ at the 3.0mm apical from the bifurcation. 2. The thickness of the root canal wall of the mesiobuccal canal was $1.08{\pm}0.26mm$ at the bifurcation and $1.00{\pm}0.23mm$ at the 3.0mm apical from the bifurcation. 3. The thickness of the root canal wall of the mesiolingual was $1.09{\pm}0.21mm$ at the bifurcation and $0.98{\pm}0.29mm$ at the 3.0mm apical from the bifurcation. 4. In the amount of reduction at the furcation and at the 3.0mm apical from the furcation there was no statistically significant difference between the step-back preparation and Gates-Glidden drill preparation, and ultrasonic preparation(P>0.05).

도말층이 가온 수직가업 근관 충전후 치근단 누출에 미치는 영향 (EFFECT OF SMEAR LAYER ON THE APICAL LEAKAGE OF WARM VERTICAL CANAL FILLING)

  • 본 연구는 근관형성시 생성되는 도말층의 존재가 System-B를 이용한 가온 수직가압충전 후의 미세누출에 미치는 영향을 관찰하고자 시행되었다. 최근에 발거된 20개의 상악전치를 이용하여 Ni-Ti 전동화일의 한 종류인 Quantec series 2000$^{TM}$(Tycom, USA) 과 수통 K-화일로 근관형성시 생성되는 도말층의 차이와 EDTA 용액 및 EDTA/차아염소산나트륨 용액의 2종 세척액에 따른 도말층의 제거양상을 주사전자현미경으로 관찰하였다. 44개의 상악전치를 이용하여 System-B를 이용한 가온 수직가압충전후의 미세누출의 정도를 근관형성시 생성되는 도말층의 존재유무에 따라 색소침투법과 치아투명화법을 이용하여 비교한 결과 다음과 같다. ${\cdot}$ 수동 K-화일이나 전동 Ni-Ti 화일로 근관형성한 군 모두에서 EDTA 용액을 사용한 군은 도말층이 완전히 제거되어 상아세관이 깨끗하게 개방되었지만, 차아염소산나트륨 용액만 사용한 군은 근관의 대부분이 도말층으로 덮혀 있었고 개방된 상아세관은 발견할 수 없었다. ${\cdot}$ 수동 K-화일 군과 전동 Ni-Ti 화일 군 사이에는 생성된 도말층의 차이를 관찰할 수 없었다. ${\cdot}$ EDTA 용액을 이용하여 도말층을 제거한 경우와 차아염소산나트륨 용액만을 사용한 경우간의 비교에서는, sealer를 사용했을 때에는 도말층을 제거한 경우가 유의하게 적은 색소 침투를 나타냈으며(p<0.01), sealer를 사용하지 않았을 때에는 EDTA 용액과 차아염소산나트륨 용액으로 세척한 경우가 다소 적은 색소 침투를 나타냈으나 통계적 유의성은 없었다(p>0.05). ${\cdot}$ EDTA 용액를 이용하여 도말층을 제거한 후 sealer사용 유무에 따른 비교에서는 sealer를 사용한 경우에서 통계적으로 유의하게 적은 색소침투를 나타냈다(p<0.01). ${\cdot}$ EDTA 용액을 사용하지 않고 차아염소산나트륨 용액만으로 근관형성한 후 sealer사용 유무에 따른 비교에서는 sealer를 사용한 경우에서 통계적으로 유의하게 적은 색소 침투를 보였다(p<0.01).

Effect of repetitive pecking at working length for glide path preparation using G-file

  • Objectives: Glide path preparation is recommended to reduce torsional failure of nickel-titanium (NiTi) rotary instruments and to prevent root canal transportation. This study evaluated whether the repetitive insertions of G-files to the working length maintain the apical size as well as provide sufficient lumen as a glide path for subsequent instrumentation. Materials and Methods: The G-file system (Micro-Mega) composed of G1 and G2 files for glide path preparation was used with the J-shaped, simulated resin canals. After inserting a G1 file twice, a G2 file was inserted to the working length 1, 4, 7, or 10 times for four each experimental group, respectively (n = 10). Then the canals were cleaned by copious irrigation, and lubricated with a separating gel medium. Canal replicas were made using silicone impression material, and the diameter of the replicas was measured at working length (D0) and 1 mm level (D1) under a scanning electron microscope. Data was analysed by one-way ANOVA and post-hoc tests (p = 0.05). Results: The diameter at D0 level did not show any significant difference between the 1, 2, 4, and 10 times of repetitive pecking insertions of G2 files at working length. However, 10 times of pecking motion with G2 file resulted in significantly larger canal diameter at D1 (p < 0.05). Conclusions: Under the limitations of this study, the repetitive insertion of a G2 file up to 10 times at working length created an adequate lumen for subsequent apical shaping with other rotary files bigger than International Organization for Standardization (ISO) size 20, without apical transportation at D0 level.

ProTaper와 ProFile을 사용한 Hybrid instrumentation method의 근관 형성 전, 후 근관 단면적과 근관벽에서 치근외면까지 최단거리의 변화에 미치는 영향에 대한 실험적 연구 (The experimental study of the effect of the hybrid instrumentation method with ProTaper and ProFile on the change of root canal area and distance from the canal to the root surface after canal shaping.)

  • The aim of this study was to investigate the effect of the hybrid instrumentation method with ProTaper and ProFile on the change of root canal area and distance from the canal to the root surface after canal shaping. The mesial canals of twenty extracted mandibular first molars having $10-20^{\circ}\Delta$ curvature were scanned using X-ray microcomputed tomography (XMCT)-scanner before root canals were instrumented. They were divided into four groups (n=10 canals ter group). In Group 1, root canals were instrumented by the step-back technique with stainless steel K-Flexofile after coronal flaring. The remainders were instrumented by the crown-down technique with, ProTaper system (Group 2), ProFile (Group 3) or ProTaper (Group 4). All canals were prepared up to size 25 at the end-point of preparation and scanned again. Pre- and post-operative cross-sectional images of 1, 3, 5, and 7 mm from the apical foramen were compared. For each level, change of cross-sectional canal are and distance to the nearest external root surface was calculated using Adobe Photoshop 6.0 and image software program. In the change of cross-sectional area, Group 4 was less than Group 2 at 3 mm and 5 mm level (p<0.05). In the difference of the distance from the canal to the root surface after canal shaping, Group 4 was least among the other groups at 7 mm level (p<0.05). According to the results, the methods using ProFile or K file only and the hybrid instrumentation technique using ProTaper and ProFile are more appropriate methods of canal preparation than ProTaper system for narrow of curved canals.
