Impact of Applied Progressive Deep Muscle Relaxation Training on the Level of Depression, Anxiety and Stress among Prostate Cancer Patients: A Quasi-Experimental Study

  • Background: The aim of this study was to determine the impact of applied progressive muscle relaxation training on the levels of depression, anxiety and stress among prostate cancer patients. Materials and Methods: A quasi-experimental study was conducted at the University Malaya Medical Centre (UMMC) and Universiti Kebangsaan Malaysia Medical Centre (UKMMC) over six months. Prostate cancer patients from UMMC received the intervention and patients from UKMMC were taken as controls. The level of depression, anxiety and stress were measured using Depression, Anxiety Stress Scales - 21 (DASS-21). Results: A total of 77 patients from the UMMC and 78 patients from the UKMMC participated. At the end of the study, 90.9% and 87.2% of patients from the UMMC and UKMMC groups completed the study respectively. There were significant improvements in anxiety (p<0.001, partial ${\eta}^2$=0.198) and stress (p<0.001, partial ${\eta}^2$=0.103) at the end of the study in those receiving muscle training. However, there was no improvement in depression (p=0.956). Conclusions: The improvement in anxiety and stress showed the potential of APMRT in the management of prostate cancer patients. Future studies should be carried out over a longer duration to provide stronger evidence for the introduction of relaxation therapy among prostate cancer patients as a coping strategy to improve their anxiety and stress.

소방관의 우울, 불안, ADHD, 충동성 (Depression, Anxiety, ADHD, and Impulsivity with Fire Officers)

  • Objective : This study aimed to evaluate the association of exhaustion, ADHD, self-esteem, impulsivity, suicidal tendencies and quality of life as well as depression and anxiety in fire officers in Korea. Methods : This study was undertaken for the first time in Korea, from September 2016 to December 2016. A set of questionnaires was provided to a total of 113 fire officers and 284 other participants as a comparison group. All subjects were evaluated using the Beck's Depression Inventory, Beck's Anxiety Inventory, Malach Burnout Inventory, Korean Adult Attention Deficit Hyperactivity Disorder Scales, Rosenberg's Self-esteem Inventory, Baratt's Impulsivity Scale, Beck's Scale of Suicide Ideation, and Quality of Life Scale. Furthermore, the odds ratio of depression, anxiety, exhaustion, ADHD, self-esteem, impulsivity, suicidal tendency and quality of life were evaluated the fire officer group and control group were compared through logistic regression analysis. Result : The results of this study indicated that the fire officers were more likely to show depression, anxiety, exhaustion, ADHD, low self-esteem and higher impulsivity. Also, the association between the fire officer group and depression, ADHD and impulsivity was confirmed through logistic regression analysis. Conclusion : Timely and effective evaluation and treatment of impulsivity, as well as depression is required for fire officers.

무릎 전치환술 환자의 통증과 심리적 안녕에 관한 연구 (A Study of Pain and Psychological Well-Being in Total Knee Replacement Patients)

  • Purpose: The study investigated pain and falls as psychological factors in Total Knee Replacement patients and examined the relationship between these factors. Methods: Ninety-six total knee replacement patients aged 69.31±7.01 (male: 21, female: 75) participated in the study. Post-surgery pain was assessed on a visual analog scale, and fall risk scores were evaluated on fall fear and fall efficacy scales. Other psychological factors assessed included national anxiety and trail anxiety, fear prevention, Beck depression score, and psychological health measures. The data were analyzed using SPSS version 22.0. Results: A statistically significant correlation was found between pain before surgery and FES fall fear score before surgery (p < 0.05). A statistically significant correlation was found between pain after surgery and PWBS fall fear score after surgery (p < 0.05). A statistically significant correlation was found between state anxiety and PWBS, Trail anxiety (p < 0.05). A statistically significant correlation was also found between trail anxiety and BDI, PWBS (p < 0.05). State anxiety and fear avoidance before surgery were found to affect trail anxiety, and pain before surgery and fall fear score before surgery were found to affect FES. In addition, PWBS was found to affect BDI. Conclusion: The study confirms that increased pain in Total Knee Replacement patients can be a risk factor for falls. In addition, fall fears increase fear avoidance and cause psychological deterioration. This suggests that efforts should be made to reduce pain and to manage psychological factors.

장루미보유 직장암 환자의 불안과 우울 및 관련요인 (Anxiety, Depression and their Correlates in Rectal Cancer Patients Without Ileostomy)

  • Purpose: This study aimed to identify the relationships among bowel function, health locus of control, anxiety, and depression in patient with rectal cancer. Methods: This study utilized a descriptive correlational design. Subjects were 200 rectal cancer patients who attended out-patient clinic in a colorectal cancer center. The study instruments included the scales for bowel function, health locus of control, anxiety, and depression. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, and Pearson's correlation coefficient. Results: The mean score of bowel function was $34.44{\pm}5.73$. The mean scores of internal HLOC, chance HLOC, doctor HLOC, and powerful others HLOC were $29.06{\pm}6.81$, $22.41{\pm}6.96$, $15.88{\pm}2.85$, and $10.72{\pm}4.68$ respectively. The mean scores of anxiety and depression were $4.49{\pm}3.95$ and $6.25{\pm}3.98$, respectively. There were significant negative correlations between bowel function and anxiety (r=-.180, p=.011); between bowel function and depression (r=-.267, p<.001); and between internal health locus of control and depression (r=-.149, p=.035). There were significant positive correlations between chance health locus of control and depression (r=.146, p=.039), and between anxiety and depression (r=.651, p<.001). Conclusion: It is suggested that anxiety and depression for the patients with low bowel function after colorectal cancer surgery should be evaluated and nursing interventions to enhance internal health locus of control need to be developed.

폐쇄성 수면무호흡증 환자에서 수면무호흡 정도, 수면 및 기분관련 척도, 수면중 활동도 간의 연관성 (The Correlation between Severity of Sleep Apnea, Sleep and Mood Related Scales, and Activity During Sleep in Obstructive Sleep Apnea Syndrome Patients)

  • 목 적 : 본 연구는 폐쇄성 수면무호흡증후군 환자에서 수면무호흡지수, 수면관련 척도, 기분관련 척도, 그리고 수면중 신체활동도 간의 연관성을 보고자 하였다. 방 법 : 폐쇄성 수면무호흡증후군을 보이는 122명(평균연령 $43{\pm}11$세)의 남자환자를 선정하였다. 폐쇄성 수면무호흡증후군 진단은 수면무호흡지수>5인 경우로 하였고, 수면무호흡지수의 평균은 $39.6{\pm}26.0$이였다. 수면관련척도로 Stan-ford Sleepiness Scale(SSS), Epworth Sleepiness Scale(E-SS), Pittsburg Sleep Quality Index(PSQI), Morningness-Ev-eningness Scale(MES)이 사용되었고, 기분관련척도로 Beck Depression Inventory(BDI), Beck Anxiety Inventory(BAI), State-Trait Anxiety Inventory(STAI) I, II, Profile of Mood States(POMS)가 사용되었다. 수면중 활동도를 측정하기 위해 양측 손목에 손목 액티그라프(wrist actigraphy, WATG)를 착용한 채 1일밤 야간수면다원검사(nocturnal polysom-nography, NPSG)를 시행하였다. WATG에서 산출된 지수는 총활동도(total activity score), 평균활동도(mean activity score) 및 분절지수(fragmentation index)였다. 수면무호흡지수, 수면 및 기분관련척도, 수면중 활동도 간의 상관관계를 분석하였다. 결 과 : ESS는 PSQI, BDI, BAI, STAI I, II와 각각 유의한 양의 상관관계를 보였다(p<0.01). SSS는 PSQI, BAI와 유의한 양의 상관관계를 보였다(p<0.05, p<0.01). BAI는 총활동도, 평균활동도 및 분절지수(fragmentation index)와 유의한 양의 상관관계를 보였다(p<0.05, p<0.01, p<0.05). 수면중 총활동도는 ESS, BAI와 각각 유의한 양의 상관관계를 보였다(p<0.05). 수면 분절지수는 ESS, PSQI, BAI와 유의한 양의 상관관계를 보였다(p<0.05, p<0.01, p<0.05). 수면무호흡지수는 수면 및 기분관련 척도와 유의한 상관관계를 보이지 않았다. 결 론 : 주간 졸리움을 호소하는 정도가 수면무호흡증의 심한 정도와 연관이 있기 보다는 오히려 야간수면 만족도, 우울 및 불안의 정도, 수면중 활동도와 더 연관성이 있었다.

우울증 자살시도자들에서 표준우울평가척도들의 요인분석 : CRESCEND 연구 (Factor Analysis of Standardized Depression Scales in Suicidal Attempters among Patients with Depressive Disorders : CRESCEND Study)

  • Objective : The aim of this study was to find associations between specific symptoms and suicidal behavior by using global severity on depression scales. Methods : Data were obtained from 1,183 patients of CRESCEND study. Factor analysis of the 17-item Hamilton Rating Scale for Depression (HAM-D) and the Beck Depression Inventory-II (BDI-II) was used to compare symptom clusters between the past suicide attempters and the non-suicide attempters, among the patients with depressive disorders. Results : Factor analyses of HAM-D and BDI-II extracted three factors. Suicide attempters had significantly higher scores on core factor of HAM-D, including depressed mood, feeling of guilt, suicide, work, and activities. The higher scores on all factors of BDI-II or scores correlated with a total of 7 suicide attempts and with known risk factors for suicidal behavior. Conclusion : These findings suggest that the suicide attempters complained of more cognitive, affective, somatic symptoms on BDI-II, but the core factor on HAM-D was the only differentiated factor between the two groups. Clinician-rated scales as well as self-reported questionnaires were valid in measuring suicidal attempts, and the clinical profile may help in guiding the studies of biological correlates and the treatments to reduce suicide risk.

인지행동치료 후 약물 중단 예측 요인에 관한 연구 (Predicting Factors of Discontinuation of Medication after Cognitive Behavioral Therapy for Panic Disorder)

  • Objective : The authors experienced that cognitive behavioral therapy(CBT) could replace medication for controlling panic attacks and anticipatory anxiety symptoms. The objective of this study was finding out predicting factors of discontinuation of medication after CBT for patients with panic disorder. Method : A hundred forty-eight patients who met DSM-IV criteria for panic disorder with or without agoraphobia for at least 3 months had completed 12 weekly sessions of Panic Control Therapy(PCT ; Barlow et al). Eighty-one patients who could discontinue medication and sixty-seven patients who could not discontinue medication were measured with several scales as the pre- and post-treatment aassessment. The scales were Beck Depression Inventory(BDI), Clinical Global Impression(CGI), Spielberger State Anxiety Inventory(STAI-state), Anxiety Sensitivity Index(ASI), Body Sensation Questionnaire (BSQ), Panic Belief Questionnaire(PBQ), Agoraphobic Cognition Questionnaire(ACQ), Fear Questionnaire(FQ), Toronto Alexithymia Scale(TAS). Results : At the pre-treatment assessment, the scores of BDI, CGI, STAI-state, ACQ, BSQ were higher in the patients who could discontinue medication than in the patients who could not discontinue medication(t=-2.68, t=-4.88, t=-3.07, t=-3.68, t=-3.35, p<0.01). Conclusion : Patients with panic disorder who were less depressed, less anxious, less agoraphobic and who had less negative cognitions for the bodily sensation and who had higher scores in the therapist's assessment could discontinue their medications.

임플란트 수술을 위한 미다졸람 정주와 아산화질소 흡입 병용 진정법의 효과와 안전성에 대한 전향적 무작위 대조군 연구 (A Prospective, Randomized and Controlled Study for the Efficacy and Safety of Sedation Technique for Implant Surgery by Combining Nitrous Oxide and Intravenous Midazolam)

  • Background: The purpose of this study is to investigate the efficacy and safety of the sedation technique for implant surgery by combining the use of inhalation of nitrous oxide/oxygen with intravenous midazolam. Methods: Patients requiring surgery for the placement of dental implants were randomly allocated to two groups receiving intravenous midazolam or a combined technique using nitrous oxide/oxide and intravenous midazolam. Safety parameters, cooperation scores, anxiety scales, total amount of midazolam administered and recovery time were recorded and compared. Results: There were a statistically significant reduction in the amount of midazolam required to achieve optimal sedation (P<0.01), an overall significant reduction in recovery time (P<0.01), a significant reduction in anxiety scales (P<0.05), and a significant improvement in cooperation (P<0.05) and peripheral oxygen saturation (P<0.05) when a combined technique of inhalational $N_2O/O_2$ and midazolam was used. Conclusions: For implant surgery, this combining sedation technique could be safe and reliable, demonstrating reduction of total dose of midazolam and level of patient's anxiety and improvement in patient's recovery and cooperation.

CP 기반 정신 평가 및 진단을 위한 심리검사의 활용 (Utilization of Psychological Tools for Critical Pathway Based Mental Evaluation and Diagnosis)

  • Objectives: For diagnosis and evaluation, evaluation tools are needed. Various tools can be used to diagnose and evaluate mental disorders. Among them, psychological tests are widely used. For Korean Standard Classification of Diseases (KCD) diagnosis, psychological tests are also required. Currently developed critical pathway (CP) presented tools for diagnosis and evaluation of mental disorders. The CP suggests the use of tools based on the Clinical Practice Guideline (CPG). Therefore, CPG-based tools should be able to be used in the clinical scene of Korean Medicine for diagnosing and evaluating mental disorders according to CP. Methods: Tools suggested by CPs are summarized. The degree of utilization of tools in CPGs is also summarized. A review was conducted by Korean Medicine neuropsychiatrist experts on tools and user's usage plan. Results: As a result, developed CPs suggested using 19 tools for anxiety disorder, 13 tools for insomnia), 12 tools for Hwabyeong, and 9 tools for dementia. In CPG, 48, 34, 44, and 44 tools were used for anxiety disorder, insomnia, Hwabyeong, and dementia, respectively. Among tools presented in CP, HAM-A, HAM-D, CGI, SAS, and TESS for anxiety disorder, CPG, ISI, and PSQI for insomnia disorder, CPG, STAI, and STAXI for Hwabyeong were frequently used in CPG. For dementia CPG, MoCA, MMSE, HDS, ADL, and ADAS-cog were frequently used. Among them, MoCA, ADL, and ADAS-cog were suggested tools in CP. Conclusions: As a result of analyzing tools suggested and used in the developed CPs and CPGs, it was verified that various tools were used in each study. Most of them were symptom and behavioral evaluation scales. Therefore, symptoms and behavior evaluation scales used more frequently should be able to be used in the clinical scene of Korean Medicine.

Functional Connectivity with Regions Related to Emotional Regulation is Altered in Emotional Laborers

  • Emotional labor, characterized by a dysfunctional type of emotional regulation called surface acting, has detrimental psychological consequences on employees, including depression and social anxiety. Because such disorders exhibit psychological characteristics manifested through brain activation, previous studies have succeeded in distinguishing individuals with depression and social anxiety from healthy controls using their functional connectivity characteristics. However, it has not been established whether the functional connectivity characteristics associated with emotional labor are distinguishable. Thus, we obtained resting-state fMRI data from participants in the emotion labor (EL) group and control (CTRL) group, and we subjected their whole-brain functional connectivity matrices to a linear support vector machine classifier. Our analysis revealed that the EL and CTRL groups could be successfully distinguished on the basis of individuals' connectivity patterns, and confidence in the classification was correlated with the scores on the depression and social anxiety scales. These results are expected to provide insight on the neurobiological characteristics of emotional labor and enable the sorting of employees undergoing adverse emotional labor utilizing neurobiological observations.
