Clinical implications of anti-thyroglobulin antibody measurement before surgery in thyroid cancer

  • Thyroglobulin antibody (TgAb) is a class G immunoglobulin and a conventional marker for thyroid autoimmunity. From a clinical perspective, TgAb is less useful than thyroid peroxidase antibodies for predicting thyroid dysfunction. However, TgAb is found more frequently in differentiated thyroid cancer (DTC) and can interfere with thyroglobulin (Tg) measurements, which are used to monitor the recurrence or persistence of DTC. Recent studies suggested a small but consistent role for preoperative TgAb in predicting DTC in thyroid nodules, and in reflecting adverse tumor characteristics or prognosis, including lymph node metastasis, but this is still controversial. Postoperative TgAb can serve as a biomarker for remnant thyroid tissue, so follow-up measures of TgAb are useful for predicting cancer recurrence in DTC patients. Since high serum TgAb levels may also affect the fine needle aspiration washout Tg levels from suspicious lymph nodes of DTC patients, it is important to use caution when interpreting the washout Tg levels in patients who are positive for TgAb.

  • Recently, research and development for a number of repairing material like an epoxy-based material and polymer-cementitious material as well as anti-washout underwater concrete have been carried out. But, the study on tile materials for the concrete structure exposed to a wetted condition is at a standstill and there are not any suitable reference data at a repairing work for the concrete structure at a splash as well as a structure under severe moisture condition. In this study, the material, called as “ceramic metal”, with an excellent mobility and plasticity as well as with a high bond strength and durability of freezing-thawing resistant properties under any environmental conditions was developed. And, the experimental evaluations for the utility wert widely performed.

콘크리트 하수관거 보수용 초속경 수중불분리 모르타르의 재료적 특성 (Material Properties of Ultra Rapid Hardening Mortar for Repairing Sewage Treatment Concrete Pipes)

  • 국내에 설치된 하수관거 중에서 20년을 초과한 콘크리트관의 연장은 66,334km로서 전체의 42.5%를 차지한다. 열화된 콘크리트 하수관거에서는 내부 하수의 유출로 인해 관로 주변의 동공을 확장시켜 땅꺼짐 등의 문제도 발생시키기 때문에 이에 대한 보수를 통한 유지관리가 필요한 실정이다. 본 연구에서는 초속경시멘트 및 분리저감제를 적용한 수중불분리성 모르타르를 하수관거 보수에 적용하기 위한 기초연구를 수행하였다. 배합조건에 따른 응결시험 결과 분리저감제 혼입에 따라 종결이 최대 172%정도 지연되는 것으로 나타났다. 수중분리도는 분리저감제 0.3% 이상 혼입시 pH 12 이하로 측정되었고, 현탁액량은 분리저감제 0.2% 이상 혼입시 50mg/l이하로 측정되어 국내 KCI-AD102 기준을 만족하였다. 기건조건과 수중조건에서 타설하였을 때의 강도비 평가 결과, 분리저감제 혼입시 80% 이상의 강도비를 나타내어 국내 KCI-AD102 기준을 만족하였다. 부착강도는 분리저감제 혼입시 기건조건보다 수중조건에서 타설하였을 때 높은 강도로 측정되었다. 내수성 검토는 분리저감제 혼입량에 따른 흡수율과 투수성으로 평가하였고, 각각 42.6%, 36.6%의 성능개선을 확인하였다.

거가대교 케이슨기초 그라우트 충전 사례연구 (A Case Study on Caisson Foundation Grouting in Geo-Ga Bridge)

  • During construction of a sea-crossing bridge grouting was used to fill densely the space between the bottom of caisson and the ground. This grout mixture was mixed with an anti-washout admixture after locating accurately the pre-cast caisson on three concrete landing pads. This method differs significantly from the costly conventional method, for bridge foundations offshore, where concrete is placed in situ after excavating inside of a temporary concrete coffering wall. To verify the grouting method in advance, the full-scale field tests were performed twice on land. After identifying the fluidity of the grout material to be filled, finding some possible problems with the main construction and revising the original design, the main construction has been continuing successfully with 20 caissons completed to date. The purpose of this paper is to introduce for the first time in Korea the grouting method including the automatic and the manual monitoring system based on the main construction of the caisson foundation.

  • 일반적인 그라우팅 재료는 물속에서 재료 분리가 발생하여 수중 그라우팅 작업시 시공 품질 확보가 매우 어렵다. 최근에는 다양한 혼화제를 첨가하여 수중에서 재료분리가 발생하지 않는 수중 불분리성의 그라우팅 재료들이 개발되고 있다. 본 연구에서는 계면활성제계 개질제를 이용하여 수중에서 재료분리가 발생하지 않으면서도 유동성을 충분히 갖는 점탄성 그라우트재를 개발하고 실내시험을 통해 해수 및 담수에 수중 불분리성 그라우팅 재료와 일반 그라우팅 재료를 시공하였을 때의 공학적 특성을 분석하였다. 본 연구에서 개발한 수중 불분리성 그라우팅 재료는 일반 그라우트재와 비교하여 해수와 담수 상태에서 충분한 강도가 발휘되었으며 수중에서 재료 분리가 발생하지 않아 수중에서도 좋은 시공 품질을 확보할 수 있을 것으로 평가되었다.

Evaluation of Mixing Conditions for the Production of Optimized High Flowing Concrete

  • Most difficulties of inducing high fluidity on the concrete mixing design with a strength range of 210 to 240kg/$\textrm{cm}^2$ result from the segregation of aggregates due to the shortage of cementitious binders. To solve the problem, this study concentrated on finding the optimized amount of binder material which does not affect the concrete strength and is also economical. Also there were studies on the use of intermediate sized aggregates to avoid the gap-grading between coarse and fine aggregates so that the material segregation in high flowing concrete was and minimalized the fluidity and penetration capacity of the reinforcing bars was enhanced. Throughout the parametric study with respect to water/binder ratio. superplasticizer. replaceable mineral admixture, the size of coarse aggregate and mixing methods, the effect of each constituent on the characteristics of high flowing concrete could be observed. As a result or partially using stone powder or an intermediate class of aggregate (max. diameter 13mm) . it was fund that the fluidity of concrete significantly increased without material segregation and any change of compressive strengths. It was also proved in this study that proper mixing time and speed are significant factors influence the performence of high flowing concrete.

지반함몰 저감을 위한 속경형 하수관거 뒤채움재료 개발 (Development of Rapid Hardening Backfill Material for Reducing Ground Subsidence)

  • 하수관거 시공시 부적절한 뒤채움재 및 다짐불량으로 인해 하수관로 파손 및 지반침하 현상이 발생한다. 최근들어 이러한 문제점을 해결하기 위해 유동성 채움재에 대한 관심이 증가하고 있다. 본 연구에서는 속경성, 가소성, 유동성, 수중불분리성을 갖춘 뒤채움재의 최적배합 도출과 뒤채움재의 경제성 향상을 위한 기초 연구를 수행하였다. 속경성 결합재의 최적배합 도출 실험 결과를 통해 뒤채움재의 최적 배합의 특성을 평가하였다. 시험결과, 속경성 결합재 페이스트의 W/M을 100%까지 증가시켜도 가소제 첨가량을 적절히 조절하여 사용하면 요구 성능을 만족시킴과 동시에 경제성 또한 확보 할 수 있을 것으로 사료된다.

  • It was applied the grouting method to fill densely the space between the bottom of the caisson and the ground with the grout mixture mixed with anti-washout admixture after locating accurately the precast caisson on three concrete landing pads but it is far different from a costly conventional method, which place concrete to build the foundation of reinforcement concrete on the spot after excavating inside of the temporary coffering wall for the bridge foundation in the sea. To verify the grouting method in advance, the full-scale trial test was performed twice on the land. After confirming the fluidity of material for the injection and some possible problems during construction and then enhancing the original design, the main process is ongoing and it has been finished 12 spots until now. The purpose of this study is to introduces for the first time in Korea the grouting method including the automatic and the manual monitoring process applied to, based on the main process of the caisson foundation finished already in the site. In a similar construction it is sincerely expected to be referred to in the future.

Lipomyces starkeyi KSM 22 glucanhydrolase의 추가가 구강세정액의 치태 억제 효과에 미치는 영향 (Effect of mouthrinse containing Lipomyces starkeyi KSM 22 glucanhydrolase on plaque formation during a 4-day period)

  • A novel glucanhydrolase from Lipomyces starkeyi KSM 22 has been suggested as a promising anti-plaque agent because it has been shown to have additional amylase activity and mutanase activity besides dextranase activity and to strongly bind to hydroxyapatite. Mouthrinsing with Lipomyces starkeyi KSM 22 glucanhydrolase solution was comparable to 0.12% chlorhexidine mouthwash in inhibition of plaque accumulation and gingival inflammation and local side effects were less frequent and less intense in human experimental gingivitis. In this study, Lipomyces starkeyi KSM 22 glucanhydrolase mouthrinses (1 and 2 unit/ml) were compared with a control mouthrinse (commercial 0.01% benzethonium chloride mouthrinse, $Caregargle^{(R)}$, Hanmi Pharmaceuticals) in the ability to inhibit plaque formation. A 3-replicate clinical trial using 4-day plaque regrowth model was used. Fifteen volunteers were rendered plaque-free on the 1st day of each study period, ceased toothcleansing, and rinsed 2X daily with allocated mouthrinse thereafter. On day 5, plaque accumulation was scored and the washout periods was 9 days for the next trial. Lipomyces starkeyi KSM22 glucanhydrolase(1 unit and 2 unit)- containing mouthrinse resulted in Significantly lower plaque formation in plaque area and thickness, compared to the control mouthrinse. There was no significant difference in plaque inhibition between enzyme-mouthrinses at 2 different concentrations used. This glucanhydrolase- containing mouthwash resulted in significantly lower plaque area severity index score and tended to have lower plaque thickness severity index score than those of control mouthrinse. But there was no significant difference according to the enzyme concentration. From these results, Lipomyces starkeyi KSM 22 glucanhydrolase-containing benzethonium chloride mouthrinse has greater anti-plaque effect than the commercial mouthrinse alone. Therefore this glucanhydrolase preparation is a promising agent for new mouthwash formulation in the near future.

Effects of Gamma Irradiation on Chemical Composition, Antinutritional Factors, Ruminal Degradation and In vitro Protein Digestibility of Full-fat Soybean

  • The aim of this study was to evaluate the effects of gamma irradiation (${\gamma}$-irradiation) at doses of 15, 30 and 45 kGy on chemical composition, anti-nutritional factors, ruminal dry matter (DM) and crude protein (CP) degradibility, in vitro CP digestibility and to monitor the fate of true proteins of full-fat soybean (SB) in the rumen. Nylon bags of untreated or ${\gamma}$-irradiated SB were suspended in the rumens of three ruminally-fistulated bulls for up to 48 h and resulting data were fitted to a nonlinear degradation model to calculate degradation parameters of DM and CP. Proteins of untreated and treated SB bag residues were fractionated by sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE). Digestibility of rumen undegraded CP was estimated using the three-step in vitro procedure. The chemical composition of raw and irradiated soybeans was similar. Results showed that phytic acid in ${\gamma}$-irradiated SB at dose of 30 kGy was eliminated completely. The trypsin inhibitor activity of 15, 30 and 45 kGy ${\gamma}$-irradiated SB was decreased (p<0.01) by 18.4, 55.5 and 63.5%, respectively. From in sacco results, ${\gamma}$-irradiation decreased (p<0.05) the washout fractions of DM and CP at doses of 30 and 45 kGy, but increased (p<0.05) the potentially degradable fractions. Gamma irradiation at doses of 15, 30 and 45 kGy decreased (p<0.05) effective degradability of CP at a rumen outflow rate of 0.05 $h^{-1}$ by 4.4, 14.4 and 26.5%, respectively. On the contrary, digestibility of ruminally undegraded CP of irradiated SB at doses of 30 and 45 kGy was improved (p<0.05) by 12 and 28%, respectively. Electrophoretic analysis of untreated soybean proteins incubated in the rumen revealed that ${\beta}$-conglycinin subunits had disappeared at 2 h of incubation time, whereas the subunits of glycinin were more resistant to degradation until 16 h of incubation. From the SDS-PAGE patterns, acidic subunits of 15, 30 and 45 kGy ${\gamma}$-irradiated SB disappeared after 8, 8 and 16 h of incubation, respectively, while the basic subunits of glycinin were not degraded completely until 24, 48 and 48 h of incubation, respectively. It was concluded that ${\gamma}$-irradiated soybean proteins at doses higher than 15 kGy could be effectively protected from ruminal degradation.
