Benefits of Metformin Use for Cholangiocarcinoma

  • Metformin is an oral anti-hyperglycemic agent, which is the most commonly prescribed medication in the treatment of type-2 diabetes mellitus. It is purportedly associated with a reduced risk for various cancers, mainly exerting anti-proliferation effects on various human cancer cell types, such as pancreas, prostate, breast, stomach and liver. This mini-review highlights the risk and benefit of metformin used for cholangiocarcinoma (CCA) prevention and therapy. The results indicated metformin might be a quite promising strategy CCA prevention and treatment, one mechanism being inhibition of CCA tumor growth by cell cycle arrest in both in vitro and in vivo. The AMPK/mTORC1 pathway in intrahepatic CCA cells is targeted by metformin. Furthermore, metformin inhibited CCA tumor growth via the regulation of Drosha-mediated expression of multiple carcinogenic miRNAs. The use of metformin seems to be safe in patients with cirrhosis, and provides a survival benefit. Once hepatic malignancies are already established, metformin does not offer any therapeutic potential. Clinical trials and epidemiological studies of the benefit of metformin use for CCA should be conducted. To date, whether metformin as a prospective chemotherapeutic for CCA is still questionable and waits further atttention.

Berberine Inhibits the Production of Lysophosphatidylcholine-induced Reactive Oxygen Species and the ERK1/2 Pathway in Vascular Smooth Muscle Cells

  • Lysophosphatidylcholine (lysoPC) induces vascular smooth muscle cell (VSMC) proliferation and migration, which has been proposed to initiate the intimal thickening in coronary atherosclerotic lesions. Berberine is an alkaloid in Berberis aquifolium and many other plants. Recently, it has been shown to have beneficial effects on the cardiovascular system, such as anti-hyperglycemic and cholesterol-lowering activity. In this study, we investigated its effects on lysoPC-induced VSMC proliferation and migration. Berberine inhibited lysoPC-induced DNA synthesis and cell proliferation in VSMCs, as well as migration of the lysoPC-stimulated VSMCs. It also inhibited the activation of extracellular signal-regulated kinases (ERKs) and reduced transcription factor AP-1 activity and the lysoPC-induced increases in intracellular reactive oxygen species (ROS). These results indicate that the inhibitory effects of berberine on lysoPC-stimulated VSMC proliferation and migration are attributable to inhibition of ROS generation and hence of activation of the ERK1/2 pathway. This suggests that berberine has potential in the prevention of atherosclerosis and restenosis.

제2형 당뇨모델 db/db 마우스에서 밀순 물추출물의 항당뇨 효과 (Anti-diabetic Effects of Triticum aestivum L. Water Extracts in db/db Mice as an Animal Model of Diabetes Mellitus Type II)

  • We evaluated the anti-diabetic effects of Triticum aestivum sprout water extract (TA) in diabetes mellitus type 2. For the experiments, the diabetic animal model db/db mice were divided to 3 groups: diabetic control (db/db) and two experimental groups orally treated with 25 and 100 mg/kg single dose of TA (TA-25 and TA-100, respectively). The lean mice were used as the non-diabetic normal control. All mice have free access to water and AIN-93 diet. TA was administrated to diabetic mice for 5 weeks and the diabetic clinical markers, including blood glucose level, body weight, food intake and insulin level, were measured at a time. After administration for 5 weeks, the blood glucose level was decreased 1.10 and 1.98 folds in TA-25 and TA-100 groups, respectively, compared with db/db group. The body weight and diet consumption were significantly reduced by TA treatment in dose-dependent manner. The treatments of TA-100 also significantly decreased remarkedly liver weight and slightly serum insulin levels when compared with them of the diabetic control group. However the immunohistochemical staining for insulin clearly showed high expression of insulin in the pancreatic islet cells derived from all db/db mice, even if TA was administrated. Moreover, TA-100 treatment significantly improved impaired glucose tolerance in diabetic db/db mice. The results suggest that TA has anti-hyperglycemic effect attenuating blood glucose in the animal model of type 2 diabetes and might be useful as a functional diet for human diabetic diseases.

  • 본 연구는 동충하초(Isaria tenuipes)를 유전적 당뇨 질환 동물모델인 OLETF 랫드에서 4주간 경구 투여하여 항당뇨 효과에 대한 연구결과로써, OLETF 랫드 동충하초(Isaria tenuipes) 투여군은 당뇨대조군과 비교하여 투여 4주에는 사료 섭취량과 체중이 증가하였다. 이는 동충하초(Isaria tenuipes)를 섭취한 OLETF 랫드는 고지방식 사료만 섭취한 OLETF 랫드에 비해 당뇨 유발이 개선된 결과로 판단된다. 혈중 포도당 농도, 인슐린 농도 및 인슐린 저항성 지표인 HOMA-IR 수치가 용량 의존적으로 감소하였다. 또한 GLUT4 유전자 발현을 측정한 결과 대조군에 비해 동충하초(Isaria tenuipes) 투여군의 mRNA 및 단백질이 용량 의존적으로 증가하는 경향을 보였다. 따라서, 동충하초(Isaria tenuipes)의 섭취가 제2형 당뇨에서 인슐린의 감수성이나 저항성에 영향을 미치고, 혈당을 강화시키는 것으로 사료되므로 당뇨병환자의 혈당저하에 기여할 것이라 기대한다. 이에 본 연구는 천연물질을 이용한 당뇨병의 예방 및 치료에 동충하초(Isaria tenuipes)를 사용할 수 있는 이론적 토대를 제공할 것으로 생각된다.

Effect of diabetes-specific oral nutritional supplements with allulose on weight and glycemic profiles in overweight or obese type 2 diabetic patients

  • BACKGROUND/OBJECTIVES: Diabetes-specific oral nutritional supplements (ONS) have anti-hyperglycemic effects, while D-allulose exerts anti-diabetic and anti-obesity effects. In this study, we investigated the efficacy and safety of diabetes-specific ONS, including allulose, on glycemic and weight changes in overweight or obese patients with type 2 diabetes mellitus (T2DM). SUBJECTS/METHODS: A single-arm, historical-control pilot clinical trial was conducted on 26 overweight or obese patients with T2DM (age range: 30-70 yrs). The participants were administered 2 packs of diabetes-specific ONS, including allulose (200 kcal/200 mL), every morning for 8 weeks. The glycemic profiles, obesity-related parameters, and lipid profiles were assessed to evaluate the efficacy of ONS. RESULTS: After 8 weeks, fasting blood glucose (FBG) level significantly decreased from 139.00 ± 29.66 mg/dL to 126.08 ± 32.00 mg/dL (P = 0.007) and glycosylated hemoglobin (HbA1c) improved (7.23 ± 0.82% vs. 7.03 ± 0.69%, P = 0.041). Moreover, the fasting insulin (δ: -1.81 ± 3.61 μU/mL, P = 0.017) and homeostasis model assessment for insulin resistance (HOMA-IR) (δ: -0.87 ± 1.57, P = 0.009) levels decreased at 8 weeks, and body weight significantly decreased from 67.20 ± 8.29 kg to 66.43 ± 8.12 kg (P = 0.008). Body mass index (BMI) also decreased in accordance with this (from 25.59 ± 1.82 kg/m2 to 25.30 ± 1.86 kg/m2, P = 0.009), as did waist circumference (δ: -1.31 ± 2.04 cm, P = 0.003). CONCLUSIONS: The consumption of diabetes-specific ONS with allulose in overweight or obese patients with T2DM improved glycemic profiles, such as FBG, HbA1c, and HOMA-IR, and reduced body weight and BMI.

Influence of Extraction Method on Quality and Functionality of Broccoli Juice

  • This study was performed to compare the quality and functionality of broccoli juice as affected by extraction method. Broccoli juice was extracted using method I (NUC Kuvings silent juicer), method II (NUC centrifugal juicer), and method III (NUC mixer), and the quality properties of the broccoli juices were analyzed using three different methods. Additionally, the antioxidative, anticancer, and anti-hyperglycemic activities of broccoli juice prepared by the three different methods were investigated in vitro. The broccoli juice made by method I contained the highest polyphenol and flavonoid contents at 1,226.24 mg/L and 1,018.32 mg/L, respectively. Particularly, broccoli juice prepared by method I showed higher DPPH and ABTS radical scavenging activities than those of the other samples. Additionally, broccoli juice made by method I showed the highest growth inhibitory effects against HeLa, A549, AGS, and HT-29 cancer cells. Broccoli juice prepared by method I had the highest ${\alpha}$-glucosidase inhibitory effects. These results indicate that there are important differences in chemical and functional qualities between juice extraction techniques.

고혈압쥐의 과일과 야채의 섭취에 따른 저혈당 효과 (Hypoglycemic Effects of Fruits and Vegetables in Hyperglycemic Rats for Prevention of Type-2 Diabetes)

  • 고혈당 쥐에(Sprague-Dawley rat) 각종 야채와 과일을 섭취(1g/kg body weight) 한 후 oral glucose tolerance test(OGTT)을 실시하였다. 0분, 5분, 15분, 30분, 60분, 90분, 120분 후에 blood glucose levels(${\Delta}BGLs$)을 측정하였다. Under glucose curve(${\Delta}AUCs$)은 OGGT의 120분 후 계산하였으며 과일과 야채의 Total phenolic content(TPC)과 anti-oxidant activity (AOA)는 Folin Ciocalteu and DPPH(2, 2-diphenyl-1-picrylhydrazyl)을 통하여 특정하였다. 실험의 마지막은 Pearson's correlations을 사용하여 TPC, AOA and ${\Delta}AUC$ 간의 상관관계를 분석하였으며 모든 통계수치는 unpaired Student's t-test를 실시하였다. 과일중에서는 탠저린, 자두, 배가 저혈당 효과를 보였으며 야채중에서는 푸른잎 머스타드와 양배투, 치커리, 브로콜리가 감소된 ${\Delta}BGLs$와 유효한 ${\Delta}AUC$수치를 보여서 저혈당 효과에 효과적이었다. 효과적인 ${\Delta}AUC$의 범위는 $5548.2{\pm}462.1$에서부터 $3823.3{\pm}282.0mg{\cdot}min/dL$이며, TPC와 AOA의 범위는 $0.063{\pm}0.00$에서부터 $0.913{\pm}0.14mg/g$ GAE, $01.05{\pm}0.08$에서부터 $75.46{\pm}0.06%$이다. 전체적으로 과일의 50%와 양체의 60-65%가 높은 TPC와 효과적인 AOA의 수치를 나타내었다. 우리는 이번 연구를 통하여 저혈당 효과가 있어 제2형 당뇨를 예방할 수 있는 과일과 채소류를 선별할 수 있었다.

  • To investigate the antioxidant activitys of Biofuntional Foods Prescriptions (BFP), we were measured radical scavenging activity with 1,1-diphenyl-2-picryl-hydrazyl (DPPH) method and anti-lipid peroxidative efficacy on human LDL with thiobarbituric acid reactive substances (TBARS) assay. The DPPH and TBARS assay were revealed that Two BFP had significantly activity when compared to that of contrast drug. The antidiabetic effect of Dicollow-B (Rx B) were investigated in streptozotocin-induced diabetic Spraque-Dawley (SD) rats. When each 100,200 kg/mg fraction of Rx B was administrated. the blood glucose showed 155.7 mg/dl, 124.7 mg/dl and inhibitive activity of the serum total cholesterol showed 67.8 mg/dl, 61.5 mg/dl and the serum triglyce of hyperglycemic rats revealed 66.3 mg/dl, 62.3mg/dl. Those were decreased do dependently when compared with control group. The effect of Rx B on The body weight was not dose-dependently decreased dosedependently when by the administration of 100 mg/kg or 200 mg/kg fractions. But it was dose-dependantly decreased from $22.2{\times}4.9$ g to $18.9{\times}4.4$ g by the administration of 400 mg/kg fraction.

관찰연구에서 확인된 SGLT2 억제제의 심혈관질환 예방효과: 한국인의 결과를 중심으로 (Preventive Effect of an SGLT2 Inhibitor on Cardiovascular Disease in an Observational Study: Results from a Korean Population)

  • The sodium-glucose cotransporter-2 inhibitor (SGLT2i) is a new anti-hyperglycemic agent that have function to concomitantly inhibit the reabsorption of glucose and sodium in the renal proximal convoluting tubule. Recent two cardiovascular outcome trials showed that a lower risk of cardiovascular events with SGLT2i in people with type 2 diabetes. In addition, prior real-world data demonstrated similar SGLT2i effects, but these studies were limited to the United States and Europe. Thus, the CVD-REAL (Comparative Effectiveness of Cardiovascular Outcomes in New Users of Sodium-Glucose Cotransporter-2 Inhibitors) 2 Study was investigated cardiovascular outcomes in those initiated on SGLT2i versus other glucose-lowering drugs (oGLDs) across 6 countries in the Asia Pacific, the Middle East, and North American regions. In Korea, 336,644 episodes of initiation in SGLT2i or oGLD group between September 2014 and December 2016 were identified in Korea National Health Insurance database after propensity score matching. SGLT2i users was associated with a lower risk of all-cause death (hazard ratio [HR], 0.72; 95% confidence interval [CI], 0.67~0.77), hospitalization for heart failure (HHF) (HR, 0.87; 95% CI, 0.82~0.92), all-cause death or HHF (HR, 0.81; 95% CI, 0.78~0.85), myocardial infarction (HR, 0.81; 95% CI, 0.74~0.89), and stroke (HR, 0.82; 95% CI, 0.78~0.86) compared with oGLD users. In conclusion, initiation of SGLT2i had a lower risk of cardiovascular events in people with type 2 diabetes compared with oGLDs.

소토사자환이 ob/ob mouse의 혈당, 고지혈증, Polyol Pathway 및 항산화작용에 미치는 영향 (Effects of Sotosajahwan on Blood Glucose, Hyperlipidemia, Polyol Pathway and Antioxidative Mechanism in ob/ob Mouse)

  • Effects of Sotosaja hwan on Blood Glucose, Hyperlipidemia, Polyol Pathway and Antioxidative Mechanism in ob/ob Mouse Diabetes is a disease in which the body does not produce or properly use insulin. Etiological studies of diabetes and its complications showed that oxidative stress might play a major role. Therefore, many efforts have been tried to regulate free oxygen radicals for treating diabetes and its complications. Sotosaja-hwan has been known to be effective for the antiaging and composed of four crude herbs. In male ob/ob mouse in severe obesity, hyperinsulinemia and hyperlipidemia, which are features of NIDDM, the hyperglycemic activites and mechanisms of Sotosaja-hwan were examined. Mice were grouped and treated for 5 weeks as follows. Both the lean (C57/BL6J black mice) and diabetic (ob/ob mice) control groups received standard chow. The experimental groups were fed with a diet of chow supplemented with 30 and 90 mg Sotosaja-hwan per 1 kg of body weight for 14 days. The effects of Sotosaja-hwan extract on the ob/ob mice were observed by measuring the serum levels of glucose, insulin, lipid components, and the kidney levels of superoxide anion radical $({\cdot}O_2)$, MDA+HAE, GSH/GSSG ratio, and also the enzyme activities involved in polyol pathway. Sotosaja-hwan lowered the levels of serum glucose and insulin in a dose dependent manner. Total cholesterol, triglyceride and free fatty acid levels were decreased, while the HDL-cholesterol level was increased, in Sotosaja-hwan treated groups. Renal aldose reductase and sorbitol dehydrogenase activities were increased in the ob/ob mice, whereas those were inhibited in the Sotosaja-hwan-administered groups. Sotosaja-hwan inhibited the generation of ${\cdot}O_2$ in the kidney. Finally, MDA+HAE levels was increased and GSH/GSSG ratio was decreased in the ob/ob mice, whereas those were improved in the Sotosaja-hwan-administered groups. Sotosaja-hwan showed the antidiabetic and anti hyperlipidemic activities by regulating the activities of polyol pathway enzymes, scavenging reactive oxygen species and reducing the MDA+HAE levels in the ob/ob mice.
