  • Systemic lupus erythematosus (SLE) is an episodic, multi-system, autoimmune disease characterized by widespread inflammation of blood vessels and connective tissues and by the presence of antinuclear antibodies (ANAs), especially antibodies to native (double-stranded) DNA (dsDNA). Its clinical manifestations are extremely variable, and its natural history is unpredictable. Untreated, SLE is often progressive and has a significant fatality rate. The most widely used criteria for the classification of SLE are those of the American College of Rheumatology (ACR), which were revised in 1982 and modified in 1997. The presence of four criteria have been diagnosed as a SLE. Rashes are common at onset and during active disease. The oral mucosa is the site of ulceration with SLE. Arthralgia and arthritis affect most children and these symptoms are short in duration and can be migratory. Lupus nephritis may be more frequent and of greater severity in children than in adults. The initial manifestation of nephritis is microscopic hematuria, followed by proteinuria. The most common neuropsychiatric symptoms are depression, psychosis(hallucination and paranoia) and headache. CNS disease is a major cause of morbidity and mortality. Pericarditis is the most common cardiac manifestation. Libman-Sacks endocarditis is less common in children. The most frequently described pleuropulmonary manifestations are pleural effusions, pleuritis, pneunonitis and pulmonary hemorrhage. During the active phase ESR, CRP, gamma globulin, ferritin and anti-dsDNA are elevated. Antibodies to dsDNA occur in children with active nephritis. Antibodies to the extractable nuclear antigens (Sm, Ro/SS-A, La/SS-B) are strongly associated with SLE. Specific treatment should be individualized and based on the severity of the disease. Sepsis has replaced renal failure as the most common cause of death.

The Involvement of AMPA Receptor in the Antidepressant-like Effects of the Portulaca Oleracea L. Extract in Mice

  • Objectives : The development of natural drugs with antidepressant effects is important and needed. This study was performed to investigate the antidepressant-like effects of the distilled water extract of Portulaca oleracea L. (POL) in a mouse model and to investigate the role of ${\alpha}$-amino-3-hydroxy-5-methyl-4-isoxazole-propionic acid (AMPA) receptors in producing these antidepressant-like effects. Methods : The forced swim test (FST) and tail suspension test (TST) were used to investigate the behavioral anti-depressive-like effects of POL in mice. Additional behavioral experiments with 2,3-dihydroxy-6-nitro-7-sulfamoyl-benzo[f]quinoxaline-2,3-dione, an AMPA receptor antagonist, were undertaken to determine the involvement of the antidepressant-like properties of POL in AMPA receptor throughput. Results : Oral administration of the POL extract (100 mg/kg) 1 h prior to testing significantly reduced the immobility times in the FST and TST. The antidepressant-like effects of the POL extract were not increased in a dose-dependent manner. Pre-treatment with NBQX significantly attenuated the reduction in immobility time induced by the POL extract in the FST. Conclusions : The distilled water extract of POL has antidepressant-like effects, which may be related to AMPA receptor. Pre-treatment with NBQX significantly attenuates the reduction in immobility time induced by the POL extract in the FST.

Effects of So-Ochim-tang-Gagam-bang on Oxidative Stress and Serotonin Metabolism in P815 Cells (소오침탕가감방(小烏沈湯加減方)의 산화스트레스와 serotonin 대사 과정에 미치는 영향)

  • This experiment was designed to investigate the effects of So-Oochim-tang-Gagam-bang (SOCT-G) on oxidative stress and serotonin metabolism in P815 Mast Cells The effects of SOCT-G on activity of 2,2-Diphenyl-1-picrylhydrazyl (DPPH) radical-scavenging and Super Oxide Dismutase (SOD) in P815 mast cells were investigated. The effect of SOCT-G on content of serotonin in P815 mast cells was investigated. The effects of SOCT-G on expression of 5-hydroxytryptamine transporter (5-HTT), Tryptophan hydroxylase 1 (TPH-1) mRNA in P815 mast cells were investigated. The SOCT-G increased DPPH radical-scavenging activity in P815 mast cells. The SOCT-G increased SOD activity in P815 mast cells. The SOCT-G decreased the intracellular content of serotonin in P815 mast cells. The SOCT-G decreased 5-HTT and TPH-1 mRNA expression in P815 mast cells. This experiment shows that So-Ochm-Tang-Gagam-bang has a significant effect of oxidative stress that help prevent free radical damage. And So-Ochim-Tang-Gagam-bang decreased the intracellular content of serotonin and mRNA expression of 5-HTT and TPH-1. Therefore, further researches are suggested to reveal the anti-depressive effectiveness of So-Ochim-Tang-Gagam-bang.

Effects of Rehmanniae Radix on HPA Axis System and Catecholamienrgic System in FST (생지황(生地黃)이 강제수영부하시험에서 HPA Axis System과 Catecholaminergic System에 미치는 영향)

  • Object : The effects of Rehmanniae Radix(RR) on HPA Axis system and Catecholaminergic system was investigated. Methods : we performed the Forced Swimming Test(FST). Also the expression of Corticotropin -Releasing Factor(CRF), c-Fos and Tyrosine Hydorxylase(TH) was measured by immunohistochemical method at Paraventricular Nucleus(PVN), Locus Coeruleus(LC) and Ventral Tegmental Area(VTA). Results : 1. The duration of immobility in the Forced Swimming Test was significantly decreased in the Rehmanniae Radix 400 mg/kg groups in comparison with the control group(p<0.05). 2. Comparing to the control group, CRF expression was significantly decreased in the Rehmanniae Radix 400 mg/kg treated group in comparison with the control group(p<0.01). 3. c-Fos expression was significantly decreased at PVN in the Rehmanniae Radix 100 mg/kg treated group in comparison with the control group(p<0.01). 4. TH expression was significantly increased at LC in the Rehmanniae Radix 100 mg/kg and 400 mg/kg treated group in comparison with the control group(p<0.001). 5. TH expression was significantly decreased at VTA in the Rehmanniae Radix 100 mg/kg and 400 mg/kg treated group in comparison with the control group(p<0.001). Conclusion : According to the results, it can be considered that Rehmanniae Radix has antidepressant effect by showing the reduction of immobility in FST through the decreased expression of CRF, c-Fos in PVN and TH in VTA.

Lactococcus lactis Culture Methods for the Enhanced Depression of Inducers in Atopic Diseases (아토피유발인자 억제효과를 증대하는 Lactococcus lactis의 배양방법)

  • We conducted a screening and checked the cultivation methods of lactic acid bacteria, which have anti-atopic dermatitis functions, by determining the lactic acid bacteria's immune enhancement by FACS, and antimicrobial activity against Staphylococcus aureus. The increase of Tcell CD4+/CD25+/foxp3+ was bigger in Lactobacillus plantarum than Lactococcus lactis subsp. lactis (Lc. lactis) and the antimicrobacterial activity against S. aureus was the opposite. The antimicrobial activity of Lb. plantarum culture with medium containing Lc. lactis culture broth was not enhanced, but the antimicrobial activity of Lc. lactis cultured in a medium containing Lb. plantarum culture broth was enhanced. As the optimal method caltivation of Lc. lactis in a medium containing 10% of heat-killed Lb. plantarum culture broth was chosen. By this method, the antibacterial activity of the pure Lc. lactis culture increased sharply at the end of the log phase, while a restraint effect on the growth of S. aureus increased 1.29 times.

The Protective Effects of Development Assets on Health Risk Behaviors among Korean Adolescents (청소년 성장발달자산의 건강위험행태 방어효과)

  • Objectives: This study aims to examine the positive effects of developmental assets on the prevention of health risk behaviors among Korean adolescents. The framework of developmental assets was introduced as building blocks for healthy child and adolescent development by Benson and his colleagues. These assets represent positive external environmental factors and internal personal qualities which strongly influence the quality of life of adolescents. Methods: In 2001, we administered self-reported questionnaires to 3,739 girls and boys in grade 9 at eight schools in two cities, Chinju and Suwon in South Korea. The data was compiled into a 'Korean Youth Health Related Behaviors and Developmental Assets Survey' and then analyzed Youth health risk behaviors, which we measured in terms of their frequency, included tobacco, alcohol, and substance use, sexual intercourse, anti-social behaviors, violence, feelings of social isolation at school, depression or attempted suicide, absenteeism from school, gambling. Results: Overall, with respect to developmental assets, the Korean adolescents surveyed only 12.6 of the 40 assets, and 88 percent of adolescents had 20 or fewer of the assets. Based on their number of developmental assets(DA), adolescents were categorized into four groups: Group I(DA 0-10): Group II(DA 11-20): Group III(DA 21-30): Group IV(DA 31-40). We examined the differences in the frequency of each health risk behavior across the four groups, Group I-IV, and found that the frequency of most health risk behaviors decreased as the number of developmental assets increased. Conclusion: We therefore concluded that in order to make effective approaches for the prevention of health risk behaviors among adolescents, we should consider and develop more comprehensive and adolescents-based policy and programs to promote various aspects of adolescents' health and quality of life.

Psychotropic Prescription Patterns for Inpatients with Schizophrenia : 10-Year Comparison in a University-Affiliated Hospital in South Korea (조현병 환자의 입원 치료시 약물처방 경향의 변화 : 일 대학병원에서 1996~2000년과 2006~2010년의 차이 비교)

  • Objectives Previous literature on the prescription change among patients with schizophrenia mainly focused on antipsychotics. This study investigated chronological change in the patterns of discharge medication among inpatients with schizophrenia at a psychiatric inpatient unit of a university-affiliated hospital. Methods All admission records at a psychiatric unit of Hanyang University Guri Hospital with discharge diagnosis of schizophrenia during two different five-year time frames (1996-2000 and 2006-2010) were reviewed including the demographic and clinical data and discharge medications. The data were gathered from a total of 207 patients (95 in 1990s and 112 in 2000s). Results The frequency in use of atypical antipsychotics (p < 0.01), antidepressants (p < 0.05), beta-blockers (p < 0.01), and benzodiazepine (p < 0.01) was significantly higher in 2000s. Anticholinergic drugs were less likely used in 2000s (p < 0.01). We did not find significant differences in the equivalent dose of antipsychotic drugs, the use of mood stabilizers and cholinergic drugs between two time frames. Conclusions Increased proportion of atypical antipsychotics and decreased use of anti-parkinsonian drugs are in line with literature. Our results show that more diverse classes of psychotic medications are used for schizophrenia in recent years. It is likely that psychiatrists are becoming more conscious of negative symptoms, anxiety, and depression in the pharmacotherapy of schizophrenia as well as positive symptoms of the illness.

Experimental Study on the Antidepressant Effect of Ginseng Radix Alba and Cyperi Rhizoma (인삼(人蔘)과 향부자(香附子)의 항우울효과(抗憂鬱效果)에 관한 실험적 연구)

  • Ginseng Radix Alba and Cyperi Rhizoma were investigated for their anti-depressant effects. For this purpose, forced-swimming test, tail suspension test, hot plate test, reserpine-induced hypothermia, aggressive behavior test were performed. In addition, the brain content of 5-hydroxyindoleacetic acid(a metabolite of serotonin), the monoamine oxidase activity, anticonvulsant effect, sleep enhancement effect were determined. The results are as follows: In the forced swimming test, Ginseng Radix diminished the duration of immobility by 45.5% compared to the control group, while Cyperi Rhizoma showed weaker effect (12.4% reduction) at 2g/kg. In the tail suspension test, the effect of Ginseng Radix(43.7% reduction) are also better than that of Cyperi Rhizoma(15.6% reduction) at 2g/kg. In the hot plate test, Ginseng Radix showed no difference as compared to control, while Cyperi Rhizoma increased the jump latency time by about 25% after administration for 10 days. In the reserpine-induced hypothermia test, both drugs slowly dropped the body temperature compared to the control group, especially the rate of hypothermia of Ginseng Radix was 24.0% at 1g/kg. In the aggressive behavior test, both drugs delayed the onset time, decreased the duration and frequency, of which effects were better in Cyperi Rhizoma. The content of 5-hydroxyindoleacetic acid in mice brain was slightly increased in Ginseng Radix, while Cyperi Rhizoma increased its level almost to the control group. Both drugs inhibited the monoamine oxidase activity in a dose-dependent manner, but the effect(51.2%) of Cyperi Rhizoma was more potent than the effect(11.8%) of Ginseng Radix. In the pentobarbital-induced sleep test, Cyperi Rhizoma exhibited no significant difference against the control group, while Ginseng Radix showed about two-fold enhancement at 2g/kg. The anticonvulsant effect of both drugs delayed the onset time, shortened the duration of convulsion and diminished the lethality, but Ginseng Radix were better than Cyperi Rhizoma.

Effects of Ethanol Extract from Magnolia Officinalis on Anxiety and Cognitive Function (후박 에탄올 추출물의 불안 및 인지기능에 대한 효과)

  • The bark of Magnolia officinalis has been used in traditional oriental medicine to treat a variety of mental disorders including anxiety and depression. The purpose of this study was to examine the effect of M. officinalis ethanol extract on stress-induced alterations in learning and cognitive function using a passive avoidance test (PAT) and also on anxiety-related behavior using the elevated plus-maze test (EPM) in female rats . The degree of Tyrosine hydroxylase (TH) in the region of the ventral tegmental area (VTA) and the locus coeruleus (LC) was measured using an immunohistochemical method. Corticosterone concentrations in serum were also measured. The ethanol extract from Magnolia officinalis was orally administered to female rats 30 minutes before evaluating their immobilization stress and anxiety-related behavior using an elevated plus-maze test and a passive avoidance test. Time spent in the open arms of the EPM increased in the M. officinalis-treated group compared with that of the saline-treated control group. In the passive avoidance test, the memory and cognitive function improved in the M. officinalis extract-treated group. M. officinalis extracts reduced elevated corticosterone concentrations in serum. Also, stress-induced TH increases were suppressed in the M. officinalis extract-treated group in the LC and the VTA region. These results suggest that M. officinalis might prove to be an effective anxiolytic anti-stress agent.

Factors Related to Smoking Behavior among High School Students (일 지역 고등학생의 흡연경험 관련 요인)

  • Purpose: The study investigated factors related to the smoking behavior among 639 high school students in Geoje, Korea. Methods: Data collected by self-reported questionnaires was analyzed by the SPSS WIN 14.0 program with Chi-test, t-test, and multiple logistic regression analysis. Results: Smoking behavior differed by gender (p=.002), school type (p=.001), experience with alcohol consumption (p=.005), family harmony (p=.003), father's smoking (p=.001), academic achievement (p=.010), self-esteem (p=.010), stress (p=.004), and attitude to smoking (p=.0001). Smoking behavior was significantly associated with gender (odds ratio (OR)=0.66, 95% confidence interval (CI)=0.46-0.95), school type (OR=1.52, 95% CI=1.03-2.23), experience with alcohol consumption (OR=0.56, 95%CI= 0.39-0.82), academic achievement (OR=1.98, 95-I=1.20-3.29), attitude to smoking (OR= 0.76, 95%CI=0.69-0.84) and father's smoking (OR=1.59, 95%CI=1.11-2.29). Conclusion: Anti-smoking programs for high school students should especially consider those at higher risk (males in vocational high school with no drinking experience of drinking, lower academic achievement, favorable attitude to smoking, and whose fathers smoke). Community efforts to reduce adult smoking would be advantageous.
