The Effect of Eating Behavior on Being Overweight or Obese During Preadolescence

  • Objectives: Being overweight or obese is central to metabolic syndrome, and these characteristics constitute dominant risk factors for chronic diseases. Although behavioral factors, including eating habits and sedentary lifestyles, are considered to be determinants of obesity, the specific childhood factors that contribute to this condition have not been clearly defined. Methods: The subjects consisted of 261 children aged 7- 9 years who were recruited from an elementary school during October 2003. Information was obtained from their parents using a questionnaire focused on eating behaviors and lifestyle factors, additional data were also collected via anthropometric measurements and biochemical examinations, including blood tests. Results: A total of 48 (18.4%) of the 261 children were overweight, and 34 (70.8%) had at least one other component of metabolic syndrome. Eating behaviors emerged as significant lifestyle-related risk factors for being overweight or obese. Those who engaged in overeating more than twice per week had three times the risk of being overweight (odds ratio [OR], 3.10, 95% confidence interval [CI], 1.39 to 6.92), and those who ate rapidly had three times the risk of being overweight (OR, 3.28; 95% CI, 1.68 to 6.41). Those who had fewer family meals (fewer than 2?3/month) had a nine times higher risk of being overweight than those who had family meals more frequently (at least 1/day) (OR, 9.51; 95% CI, 1.21 to 74.82). Conclusions: This study showed that being overweight or obese during preadolescence is associated with a higher risk of metabolic syndrome and is related to unhealthy eating behaviors. Thus, weight-control strategies and healthy eating behaviors should be developed early in life to reduce the risk for metabolic syndrome.

농촌보건사업지역(農村保健事業地域)의 아동영양(兒童營養) 실태조사(實態調査) (Child Nutrition Survey in Rural Health Project Areas)

  • The main purposes of the nutrition and clinical surveys were to provide baseline information on the nutritional status of pre-school children in rural health demonstration project areas of the Korea Health Development Institute (KHDI) for nutrition guidance services for the MCH target group. The survey covered a total of 222 pre-school children and 135 mothers in Okgu Gun, Cholla Pukto Province from August 10 to August 17, 1979. The survey results are summarized as follows: 1) Family Environment Seventy percent of the households had more than three children, and the mean family sire was 6. Sixty-nine percent of the mothers and 47% of the fathers of the surveyed households were educated at or below the primary school level. The majority, 70% of the mothers, were aged between 20 years and 35 years. 2) Anthropometric Measurements and Hemoglobin Value 4.5% of the children were lower than 80% weight for age of the Korean standard, and 5.4% were lower than 85% arm circumference for age of the Jelliffe's standard resectively, and those were suffering from protein-energy malnutrition. Angular stomatitis were observed on 66.2% of the subjects. Mean hemoglobin value was 11.1g/100m1, and 44.2% of the subjects were categorized as anaemia. 3) Food and Nutrient Intake of animal foods was very low, ranging from 2.9 to 17%. Consumption of eggs was less than 2% of total food intake, and intake of legumes was also very scanty, between 0.8 to 3.7%. These data present evidence of very poor protein intake, quality as well as quantity. Energy intake of children was 60.0 to 64.4% of the recommended allowance, and mean protein intake only met 47.4% of the recommendation. Low intake of vitamins except thiamin were also found. 4) Mother's Nutrition Knowledge Eighty-five percent of the mothers were entirely ignorant regarding the 'five basic food group' which is most important fact on food and nutrition guidance. Mean knowledge score from 14 basic questions about food and nutrition was as low as 5.1. There was a significant positive correlation between mother's educational level and nutrition knowledge score. 5) Family Planning Variable There were significant correlation among maternal, family planning variables, and some of the nutritional and physical measurements. The study revealed that the mother's educational level and nutrition knowledge score are more crucial factors than the family planning variables on effecting food intakes on children. Recommendation : According to the results of the surveys, there were high incident rates of nutritional anaemia and angular stomatitis among pre-school children, and most of rural women had very limited knowledge about food and nutrition. As a main part of the health education activities, the community health workers should provide nutrition education to the village mothers to improve the nutrional status of young children in rural areas. Nutrional promotion at the primary health care level should be mainly based on appropriate nutrition education.

한국 노인의 식생활 지침 실천도와 알츠하이머형 치매의 관련성 (Association between compliance with dietary guidelines and Alzheimer's disease in Korean elderly)

  • 우리나라의 노인 대상으로 식생활지침 실천도 점수에 따른 치매 유병률의 차이를 분석한 결과, 치매 노인군이 정상노인군에 비해 나이가 유의하게 많았고, 성, 체격, 그리고, 식사속도, 식사를 거르는 횟수, 편식 등 식생활 특성에는 정상 노인군과 유의한 차이가 없었다. 식생활지침 세부 실천지침 중에서 다양한 식품군 섭취, 규칙적이고 안전한 식사, 음주, 가벼운 운동부분에서 치매노인군의 실천도가 정상노인군에 비해 낮았고, 그 외의 문항에서는 유의한 차이가 없었다. 식생활지침 실천도 점수가 높을수록 치매 유병률이 낮은 경향을 보였다. 요약하면 어르신을 위한 식생활지침의 실천도가 높을수록 알츠하이머형 치매의 위험이 감소하므로, "어르신을 위한 식생활지침 (보건복지부, 2011)" 실천도를 높이는 것이 알츠하이머형 치매 예방에 도움이 될 수 있으며, 향후 알츠하이머형 치매 예방을 위한 식생활 지침의 기초자료로 활용할 수 있을 것으로 사료된다.

미숙아의 초기 영양섭취 및 성장상태에 관한 연구 (Nutritional and Growth Status of Premature Infants During Neonatal Period)

  • 본 연구는 미숙아들의 영양공급 및 성장상태를 조사하기 위하여 출생시 체중이 2500g이하이며 在胎期間이 37주 미만인 신생아중 20일이상 입원한 미숙아를 대상으로 그들의 역학적 조사, 영양 섭취량과 공급 방식, 체위의 변화, 혈액학적 검사등을 비교 분석한 그 결과는 다음과 같다. 1) 출생체중별 在胎期間과 apgar score를 보면 1499g이하 미숙아군은 31주, 4/7(1min/5min)이며 1500~1999g, 2000~2500g 미숙아군은 각각 33주, 8/9(1min/5min)이었다. 2) 미숙아들의 체중별 1일 kg당 열량과 수분 섭취량을 평균적으로 보면 1499g 이하 미숙아군은 102.0Kcal, 150.2ml이며 1500~1999g 미숙아군은 119.3Kcal, 136.4ml이며 2000~2500g 미숙아군은 101.7Kcal, 141.5ml이었다. 3) 1999g이하 미숙아군의 영양 공급 빈도 중 dextrose와 dextrose/hyperal 공급시일 즉 관주입법으로 공급한 시일을 평균적으로 보면 1499g이하 미숙아군은 7.7일이여 1500~1999g 미숙아군은 3.8일이었다. dextrose/formula와 dextrose/hyperal/formula공급시일은 1499g이하 미숙아군은 평균 14.2일, 1500~1999g 미숙아군은 10.9일이었다. 4) 미숙아들의 초기 체중 감소율을 보면 1499g이하 미숙아군은 5.4%, 1500~1999g 미숙아군은 6.4%, 2000~2500g 미숙아군은 11.4%로 출생체중이 높은 미숙아군의 감소율이 높게 나타났다. 5) 혈액학적 검사시 1499g 이하 미숙아군의 10일째 Hb은 12.1g/dl, Hct은 36.7%, MCHC는 33.2%이며 1500~1999g 미숙아군의 수치는 15.4g/dl, 46.7%, 33.8%이며 2000~2500g 미숙아군은 14.3g/dl, 42.5%, 351.1%로 조사기간중 20일째까지 Hb과 Hct수치는 점차 감소하였다. 6) Amino acid 투여의 영향을 보면 amino acid투여군의 초기 체중 감소율이 9.0%로 amino acid투여 안한군(9.4%)보다 단백질과 열량을 적게 섭취하였음에도 초기 체중 감소율의 유의적 차이는 없었다.

국내 영양조사(1960-1990년)에서 적용된 영양평가 방법의 내용 및 추이분석 (Evaluation of Methods Used in Nutrition Surveys in Korea(1960-1990))

  • 1960년에서 1990년까지 발표된 영양조사 관련논문 336편을 9(영유아, 학동기아동, 청소년, 대학생, 임신수유부, 주부, 노인, 근로자, 환자)개 대상 집단별로 나누어 각 대상집단의 영양조사에 적용된 조사방법을 분석한 결과는 아래와 같다. 영양조사의 대상별 분포를 보면 영유아, 학동기아동, 대학생, 청소년, 주부, 노인, 근로자, 수유부, 환자의 순이었다. 조사건수의 85%가 식이조사를 포함였으며, 식이섭취조사법의 이용이 증가되었다. 설문지의 조사표를 이용하여 식습관, 영양지식등 식생활 상황을 파악한 논문이 가장 많았으며 그 내용도 대상집단에 따라 다양하였다. 논문의 53%가 신체계측조사를 적용하였으며 영유아, 학동기 아동의 경우는 신장과 체중을 계측하여 발육표준치와 비교하여 성장발육을 측정하였고, 성인의 경우는 신장과 체중을 이용하여 body mass index를 구하거나 피부투겹두께를 측정하여 체조성을 관찰하였으며, 최근에는 영양부족으로 인한성장 지연이나 열세한 체위보다는 영양과잉섭취로 인한 비만 쪽으로의 연구가 증가하고 있었으며 따라서 정확한 체지방측정 방법을 모색하는 연구논문이 다수 있었다. 생화학적 조사를 수행한 연구논문은 32%로서 숫자에 있어서는 거의 빈혈에 관한 생화학적 조사가 대부분을 차지하였다. 단백질 영양상태 판정을 위한 조사가 다수 있었으며, 무기질에 있어서는 Na과K이 주를 이루었고 비타민에 관한 생화학적 조사는 매우 부진하였다. 임상조사는 26%로서 그 적용빈도가 가장 낮았으며 특히 영양결핍성 신체징후를 관찰한 경우는 5%정도에 그쳤다. 반면에 대상자의 자각증상에 관한 설문 또는 조사표를 이용한 건강상태자각정도와 피로도 등을 측정하는 사례가 많았다.

남성 근로자의 혈중 항산화 영양소 상태와 대사증후군 인자와의 관계 (The Relationship Between Plasma Antioxidant Levels and Metabolic Syndrome Risk Factors in Male Workers)

  • The principal objective of this study was to evaluate the association between plasma antioxidant levels and metabolic syndrome in male workers, and to provide basic information regarding the control and prevention of metabolic syndrome. We analyzed 163 male workers who had participated in annual medical examinations from January to December 2007. The subjects were classified into normal and metabolic syndrome groups according to the NCEP-ATP III criteria and the Asia-Pacific criteria for waist circumference. Anthropometric parameters, lifestyles, blood lipid profiles, and antioxidant levels were evaluated. As compared to the normal group, the metabolic syndrome group evidenced significantly higher plasma levels of $\alpha$- tocopherol(p<0.05) and retinol(p<0.05), but significantly lower plasma levels of lycopene(p<0.05) and $\beta$-carotene(p<0.05). This tendency was found to be predominantly attributable to increases in the number of metabolic syndrome components. In our simple regression analysis, higher plasma levels of $\alpha$-tocopherol($\beta$=0.001, p<0.01) and retinol($\beta$=0.021, p<0.001) were associated with significantly higher risks of metabolic syndrome, but lycopene($\beta$=-1.499, p<0.01) and $\beta$-carotene($\beta$=-0.048, p<0.01) were associated with significantly lower risks of metabolic syndrome. Retinol($\beta$=0.013, p<0.05) and $\beta$-carotene($beta$=-0.044, p<0.01) were associated significantly with metabolic syndrome, when adjusted for age and BMI. These data indicate that the plasma levels of $\alpha$-tocopherol, retinol, lycopene, and $\beta$-carotene are associated with metabolic syndrome. Specifically, low lycopene and $\beta$-carotene levels in the plasma appear to increase the risk of metabolic syndrome. Therefore, proper nutritional education programs for male workers are required to increase dietary intakes of antioxidant vitamins. Further studies will be necessary to determine whether antioxidant levels can be utilized as a predictive or a preventive factor.

사춘기 남녀 청소년의 인체 특징에 관한 비교 - 만 $10{\sim}14$세를 중심으로 - (The Comparison of the Physical Characteristics between Boys and Girls at Puberty)

  • The purpose of this study was to analyze the body measurements of boys and girls at puberty and to provide the fundamental data for pubescent apparel manufacturers to produce clothing that reflect their physical characteristics. A total of 549 boys and 529 girls aged between 10 and 14 were measured in the capital area from March 4 to April 3, 2004. Data were collected from 35 anthropometric items and 12 photographic items per a person. SPSS Ver. 12 program was used in data analysis including means, standard deviation, t-test and Duncan test. The main results of this study were as follows. They showed the significant difference of their growth in accordance with the increase of their ages. There were also the difference between boys and girls. As for height and length items, boys showed a slow growth at the age of $10{\sim}11\;and\;12{\sim}13$. Those at the age of $11{\sim}12\;and\;13{\sim}14$ showed rapid growth. That is, an active growth was followed by a slow growth and that phenomenon repeatedly occurred. On the other hand, girls showed remarkable growth at the age $10{\sim}11$ and the growth rate gradually slowed down afterward. Regarding circumference items, boys at the age of $11{\sim}12\;and\;13{\sim}14$ showed remarkable growth. This results showed that boys at the age of $11{\sim}12$ had vertical growth and horizontal growth at the same time and for those at the age of $13{\sim}14$, growth was more conspicuous in horizontal direction. Meanwhile, for girls, the growth rate was high at the age of $11{\sim}12$, somewhat later than the age of the growth of height and length. As for breadth-related items and depth-related items, for both sexes two items grew steadily throughout the ages, breadth-related items showed a higher growth rate than that of depth-related items. This study analyzed the body measurements of pubescent boys and girls and the results showed that, for boys, an active physical growth took place at the age of 13 according to previous studies, but the findings of this study suggested that the phenomenon now occurred at the age of $11{\sim}12$, which proved that physical growth took place earlier than before. Also, an active growth was followed by a slow growth. Girls at puberty showed remarkable growth of height at the age of $10{\sim}11$ that is consistent with previous studies and then showed horizontal growth at the age of around 12, having a voluminous body shape.

비만도에 따른 사상체질별 체형 특성 분석 (A Study on the Sasang Constitutional Characteristics by Obesity Grade)

Objectives The objective of this study is to offer some standards for the distinction of Sasang Constitutions through analyzing the characteristics of their body shapes that are classified by BMI. 2. Methods The subject of this study were 1341 female and 927 male patients who aged from 17 to 80 in Seoul, Pusan, Daeku and Jeonju. They were treated with Sasang Constitutional medicine. 8 circumferences, 5 widths, weight, height of their body were measured with measuring tape, large sliding caliper, scale and anthropometer. Collected 15 anthropometric datas were analyzed by Analysis of Contingency table, ANOVA and Duncan test. 3. Results (1) The Body shapes according to obesity grade are classified Underweight type that BMI is less than 18.5, Normal weight type that BMI is $18.5{\sim}23.0$ and Overweight type that BMI is mote than 23.0. (2) Soeumin represents Underweight type and Taeeumin represents Overweight type regardless of gender differences. Soeumin stands for Normal weight type in women and Soyangin stands for Normal weight type in men. (3) In case of Underweight type, 13 measurements are not suitable to estimate Sasang Constitutions regardless of gender differences. (4) In case of Normal weight type, 12 measurements except for W3 in women and W7 in men are suitable to estimate Sasang Constitutions. And there are no gender differences in Soyangin and Soeumin, but there are gender differences in Taeyangin and Taeeumin. (5) In case of Overweight type, 9 measurements except for C2, C5, C6, W7 in women and 12 measurements except for W3 in men are suitable to estimate Sasang Constitutions. And there are no gender differences in 4 Sasang Constitutions. 4. Conclusions From the above results, we have to consider not only gender differences and age groups but also obesity grade when we distinguish Sasang Constitutions.

농촌 국민학교 아동의 혈압, 짠맛에 대한 역치, 최적 염미도, 뇨 중 배설성분 및 혈압에 관한 영양지식 (Blood Pressure, Salt Threshold, Salt Preference, Urinary Excretions and Nutrition Knowledge About Blood Pressure of Elementary School Children in Rural Area)

  • The purpose of this study was to observe the relationship of dietary factors to blood pressure in 5th and 6th grade school children. Salt threshold, salt preference and nutrition knowlege about blood pressure were tested. Twenty-four hour urines were collected for the measurements of the volume and concentrations of sodium, potassium, chloride, calcium, phosphorus, creatinine and urea nitrogen. 1) Mean systolic and diastolic blood pressure of elementary school children were 106.8/67.6mmHg in males and 108.7/69.5mmHg in females. Seven children(4%) of total subjects were found to be hypertensive. Their mean blood pressure was 130.0/86.4mmHg. 2) Urinary excretions of creatinine and urea nitrogen during twenty-four hours were 621.1mg and 1524mg, respectively. The mean daily urinary sodium excretion was 2940mg, which is equivalent to 7.37g NaCl. 3) The daily urinary excretions of other minerals were as follows; potassium was 1301mg, chloride, 4991mg, calcium, 55.7mg and phosphorus, 700.03mg. 4) Salt preference of hypertensive children90.473$\pm$0.275) was significantly higher than those of normotensive children(0.473$\pm$0.213%), but salt threshold was lower in hypertensives(0.105$\pm$0.04%) than normotensives(0.081$\pm$0.022%) (p<0.05). INdices for assessing obesity, such as body weight, triceps skinfold, weight for heigth and body surface area, were high in hypertensive children than normotensive children(p<0.05). But there was no significant difference in energy and nutrient intakes between two groups. 5) Various anthropometric measurements had positive correlations with blood pressure, but body muscle mass(%) had a negative correlation with blood pressure. And daily energy and nutrient intakes were not related to blood pressure. 6) Blood pressures-both systolic and diastolic-were significantly correlated with urinary excretions of potassium and chloride. Daily sodium excretion was related to systolic blood pressure, but no related to diastolci blood pressure. There was no relationship of salt threshold to salt preference, and neither threshold nor preference was related to blood pressure. Results of this study suggest that nutrition education program for children including the information about desirable food habits for the prevention of hypertension should be developed.

뇌혈관질환의 예측인자로서의 악력 (Grip Strength as a Predictor of Cerebrovascular Disease)

  • Background: Cerebrovascular disease is included in four major diseases and is a disease that has high rates of prevalence and mortality around the world. Moreover, it is a disease that requires a high cost for long-term hospitalization and treatment. This study aims to figure out the correlation between grip strength, which was presented as a simple, cost-effective, and relevant predictor of cerebrovascular disease, and cerebrovascular disease based on the results of a prior study. And furthermore, our study compared model suitability of the model to measuring grip strength and relative grip strength as a predictor of cerebrovascular disease to improve the quality of cerebrovascular disease's predictor. Methods: This study conducted an analysis based on the generalized linear mixed model using the data from the Korea Longitudinal Study of Ageing from 2006 to 2016. The research subjects consisted of 9,132 middle old age people aged 45 years or older at baseline with no missing information of education level, gender, marital status, residential region, type of national health insurance, self-related health, smoking status, alcohol use, and economic activity. The grip strength was calculated the average which measured 4 times (both hands twice), and the relative grip force was divided by the body mass index as a variable considering the anthropometric figure that affects the cerebrovascular disease and the grip strength. Cerebrovascular diseases, a dependent variable, were investigated based on experiences diagnosed by doctors. Results: An analysis of the association between grip strength and found that about 0.972 (odds ratio [OR], 0.972; 95% confidence interval [CI], 0.963-0.981) was the incidence of cerebral vascular disease as grip strength increased by one unit increase and the association between relative grip strength and cerebrovascular disease found that about 0.418 (OR, 0.418; 95% CI, 0.342-0.511) was the incidence of cerebral vascular disease as relative grip strength increased by unit. In addition, the model suitability of the model for each grip strength and relative grip strength was 11,193 and 11,156, which means relative grip strength is the better application to the predictor of cerebrovascular diseases, irrespective of other variables. Conclusion: The results of this study need to be carefully examined and validated in applying relative grip strength to improve the quality of predictors of cerebrovascular diseases affecting high mortality and prevalence.
