Dual-innervated multivector muscle transfer using two superficial subslips of the serratus anterior muscle for long-standing facial paralysis

  • Recent reports have described several cases of double muscle transfers to restore natural, symmetrical smiles in patients with long-standing facial paralysis. However, these complex procedures sometimes result in cheek bulkiness owing to the double muscle transfer. We present the case of a 67-year-old woman with long-standing facial paralysis, who underwent two-stage facial reanimation using two superficial subslips of the serratus anterior muscle innervated by the masseteric and contralateral facial nerves via a sural nerve graft. Each muscle subslip was transferred to the upper lip and oral commissures, which were oriented in different directions. Furthermore, a horizontal fascia lata graft was added at the lower lip to prevent deformities such as lower lip elongation and deviation. Voluntary contraction was noted at roughly 4 months, and a spontaneous smile without biting was noted 8 months postoperatively. At 18 months after surgery, the patient demonstrated a spontaneous symmetrical smile with adequate excursion of the lower lip, upper lip, and oral commissure, without cheek bulkiness. Dual-innervated muscle transfer using two multivector superficial subslips of the serratus anterior muscle may be a good option for long-standing facial paralysis, as it can achieve a symmetrical smile that can be performed voluntarily and spontaneously.

A Development of the Preschool Teacher Play Support Scale Based on the Play Practice Resources for Teachers of the 2019 Revised Nuri Curriculum (2019 개정 누리과정 놀이실행자료에 기초한 유아교사용 놀이지원 척도 개발)

  • Objective: The goal of this study is to develop an instrument for measuring the quality of teacher support for children's play (age 3-5) based on the Play Practice Resources for Teachers presented in the 2019 revised Nuri Curriculum. Methods: The subjects of this study were 333 early childhood educators. Exploratory and confirmatory factor analysis, as well as correlation analysis were all performed to confirm construct validity and concurrent validity. The reliability of each factor was verified using Cronbach's alpha. Results: The Preschool Teacher Play Support Scale (PTPSS) was finalized with 25 items, which included the following four sub-factors: interaction, play materials, play space, and play safety. The concurrent validity was also confirmed through significant correlations between the existing measures such as the Teacher Self-Efficacy Scale, the Teaching Efficacy Scale for Play, and the Teacher-Toddler Interaction Scale. The reliability of each factor was also desirable. Conclusion/Implications: The PTPSS was verified as a sound measure quantifying the play support behavior of teachers who respect the autonomy and initiative of young children which is being pursued by the 2019 revised Nuri Curriculum.

Research trend of Korean Medicine for Musculoskeletal Disorders of the Hand and Wrist (국내 의료기관에서 시행된 손/손목 근골격계 질환의 한방치료 연구동향)

  • Objectives The purpose of this study is to investigate the research trend of Korean traditional medicine for musculoskeletal disorders related to wrist and hand under Korean healthcare facilities. Methods We collected data from search engines for research including foreign and domestic online database using the keywords ligament tendon, wrist sprain, tendinitis, trigger finger, carpal tunnel, ganglion cyst, de Quervain's and Raynaud. Results A total of 45 articles were about ligament tendon, wrist sprain, tendinitis, tenosynovitis, trigger finger, carpal tunnel syndrome, ganglion cyst, de Quervain's tenosynovitis and Raynaud disease. Out of 45 articles, 20 articles were about carpal tunnel syndrome and the most commonly used measurement was visual analog scale. Conclusions This study shows the research trend of musculoskeletal disorders related with the wrist and hand. Through the collected data, the treatment methods and specific information of treatments were organized. This study can be used in clinical environments and will contribute for further study in musculoskeletal disorders related with the wrist and hand.

The use of augmented reality navigation technology in combination with endoscopic surgery for the treatment of an odontogenic cyst of the upper jaw: A technical report

  • Purpose: This report presents the first known use of a rigid endoscope with augmented reality technology for the removal of an odontogenic cyst that penetrated the maxillary sinus and illustrates its practical use in a patient. Materials and Methods: In the preoperative period, cone-beam computed tomography was performed in a specially designed marker holder frame, and the contours of the cyst and the nearest anatomical formations were segmented in the 3D Slicer program. During the operation, a marker was installed on the patient's head, as well as on the tip of the endoscope, which made it possible to visualize the mass and the movement of the endoscope. The surgical intervention was performed with the support of augmented reality in HoloLens glasses (Microsoft Corporation, Redmond, WA, USA). Results: The use of this technology improved the accuracy of surgical manipulations, reduced operational risks, and shortened the time of surgery and the rehabilitation period. Conclusion: With the help of modern technologies, a navigation system was created that helped to track the position of the endoscope in mixed reality in real time, as well as to fully visualize anatomical formations.

  • The interaction between UN and CdCl2 in the LiCl-KCl molten eutectic was studied at 1023 K. The chlorination was monitored by sampling and recording the redox potential of the medium. At 1023 K the chlorination of UN with cadmium chloride in the molten LiCl-KCl eutectic proceeds completely and results in the formation of uranium chlorides. The melts of the LiCl-KCl-UCl3 or LiCl-KCl-UCl4 compositions can be obtained by the end of experiment depending on the presence of metallic cadmium in the reaction zone. The higher the concentration of the chlorinating agent, the faster the reaction rate. At [CdCl2]/[UN] = 1.65 (10% excess) the reaction proceeds to completion in about 7.5 h. At [CdCl2]/[UN] = 7 the complete chlorination takes 2.5-3 h.

  • The emergence of technologies like Big data analytics, Industrial Internet of Things, Internet of Things, and applicability of these technologies in various domains leads to increased demand in the underlying execution environment. The demand may be for compute, storage, and network resources. These demands cannot be effectively catered by the conventional cloud environment, which requires an integrated environment. The task of finding an appropriate service provider is tedious for a service consumer as the number of service providers drastically increases and the services provided are heterogeneous in the specification. A service broker is essential to find the service provider for varying service consumer requests. Also, the service broker should be smart enough to make the service providers best fit for consumer requests, ensuring that both service consumer and provider are mutually beneficial. A service broker in an integrated environment named Global Manager is proposed in the paper, which can find an appropriate service provider for every varying service consumer request. The proposed Global Manager is capable of identification of parameters for service negotiation with the service providers thereby making the providers the best fit to the maximum possible extent for every consumer request. The paper describes the architecture of the proposed Global Manager, workflow through the proposed algorithms followed by the pilot implementation with sample datasets retrieved from literature and synthetic data. The experimental results are presented with a few of the future work to be carried out to make the Manager more sustainable and serviceable.

  • The purpose of the article is to identify priority trends of technological innovations and strategic opportunities for using the smart potential to the benefit of the Russian industrial production development in the context of digital transformation. The article substantiates the demand for technological process automation at industrial enterprises in Russia and considers the possibilities of using artificial intelligence and the implementation of smart manufacturing in the industry. The article reveals the priorities of the leading Russian industrial companies in the field of digitalization, namely, an expansion of the use of cloud technologies, predictive analysis, IaaS services (virtual data storage and processing centers), supervisory control, and data acquisition (SCADA), etc. The authors give the characteristics of the monitoring of the smart manufacturing systems development indicators in the Russian Federation, conducted by Rosstat since 2020; presents projected data on the assessment of the required resources in relation to the instruments of state support for the development of smart manufacturing technologies for the period until 2024. The article determines targets for the development of smart technologies within the framework of the Federal Project "Digital Technologies".

Predicting the unconfined compressive strength of granite using only two non-destructive test indexes

  • This paper reports the results of advanced data analysis involving artificial neural networks for the prediction of the unconfined compressive strength of granite using only two non-destructive test indexes. A data-independent site-independent unbiased database comprising 182 datasets from non-destructive tests reported in the literature was compiled and used to train and develop artificial neural networks for the prediction of the unconfined compressive strength of granite. The results show that the optimum artificial network developed in this research predicts the unconfined compressive strength of weak to very strong granites (20.3-198.15 MPa) with less than ±20% deviation from the experimental data for 70% of the specimen and significantly outperforms a number of available models available in the literature. The results also raise interesting questions with regards to the suitability of the Pearson correlation coefficient in assessing the prediction accuracy of models.

  • Objectives : The purpose of this study is to investigate the research trend of Korean traditional medicine for edema under Korean healthcare facilities. Methods : We collected data from search engines for research including domestic online database using the keywords edema, Hydrocele, emphysema and Pneumocranium Results : Total of 41 articles were about Lower body edema, General edema, edema of the eyelid, Lymphedema, Hydrocele, Bone marrow edema, hand edema, Brain edema, emphysema, Pneumocranium The treatment methods used were acupuncture and pharmacopuncture treatment, moxibustion treatment, herbal medicine treatment, cupping treatment, massage treatment, radio frequency treatment, and Chuna treatment. Twenty-nine studies used only oriental medicine treatment, and 12 studies combined oriental medicine and western medicine. the most commonly used measurement was Measurement of the perimeter of the area. Conclusions : This study shows the research trend of edema. Through the collected data, the treatment methods and for specific information of treatments were organized. This study can be used in clinical environments and will contribute for further study in edema.

Influencing Factors of COVID-19 Stress, Impulsiveness, and Family Strength on Suicidal Ideation of Public Nursing Officials in Community Health Center (간호직 공무원의 코로나19 스트레스, 충동성, 가족건강성이 자살생각에 미치는 영향요인)

  • Purpose: This study was conducted to identify factors of COVID-19 stress, impulsiveness, and family strength on suicidal ideation of public nursing officials in community health centers. Methods: This study was a descriptive analysis study the subjects of this study were a total of 145 public nursing officials from community health centers in Busan. The data were analyzed using IBM SPSS/WIN 23.0 version. Result: The result of the predictor analysis showed that the motor impulsiveness (β=.383, p<.001), workload (β=.222, p=.003), cognitive impulsiveness (β=-.205, p=.012), bond (β=-.169, p=.033). The regression model showed an explanatory power of 24.1%. Conciusion: it is necessary to increase impulse and stress control ability, and to adjust the workload. In addition, it is necessary to systematically guarantee a working environment where practical vacation can be used, but it may not be a realistic alternative in a disaster crisis such as COVID-19, so indirect alternatives such as reinforcing infectious disease experts, improving the work environment, and psychological support to prevent suicide in advance are required.
