족관절 관절염이 있는 환자에서 체중 부하상태의 양측 족관절과 환측 족관절 전후면 방사선 사진의 방사선학적 측정치의 비교 (Comparison of Radiologic Parameters between Weight Bearing Affected Single Ankle Anteroposterior View and both Ankle Anteroposterior View in Ankle Osteoarthritis)

  • Purpose: This study was performed to analyze the results of radiologic parameters between weight bearing affected single ankle anteroposterior (AP) view and both ankle AP view in ankle osteoarthritis (OA). Materials and Methods: Between January 2009 and August 2010, 41 patients (50 ankles) who visited our institution to treat ankle OA were reviewed retrospectively. In radiographic assessment, weight bearing affected single ankle AP view and both ankle AP view were checked, and measured tibial anterior surface angle (TAS), tibial medial malleolar angle (TMM), talar tilting angle (TT), maximum and minimun joint space width (JSW) of ankle, width between articular surface of medial malleolar and medial articular surface of talus as radiologic parameters. Results: On weight bearing both ankle AP view, TAS was $85.4{\pm}4.1^{\circ}$, TMM was $33.1{\pm}9^{\circ}$, TT was $5.4{\pm}6.1^{\circ}$, maximum JSW was $3.2{\pm}2.7$ mm, minimum JSW was $1.1{\pm}1.3$ mm, width between articular surface of medial malleolar and medial articular surface of talus was $1.8{\pm}1.8$ mm and on weight bearing affected ankle AP view, TAS was $85.3{\pm}3.9^{\circ}$, TMM was $34.3{\pm}10.9^{\circ}$, TT was $5.4{\pm}6.5^{\circ}$, maximum JSW was $3.2{\pm}2.7$ mm, minimum JSW was $1.1{\pm}1.3$ mm, width between articular surface of medial malleolar and medial articular surface of talus was $1.6{\pm}1.7$ mm. Conclusion: There is no statistical significance in radiologic parameters between weight-bearing affected single ankle AP view and both ankle AP view in ankle OA.

PACE 프로그램이 퇴행성관절염 노인의 자기효능감과 통증 및 관절기능에 미치는 효과 - 미국이민 한국노인을 중심으로 - (The Effects of PACE Program on Self-efficacy, Pain and Joint Function in Korean Immigrant Elderly with Osteoarthritis.)

  • The PACE (People nth Arthritis Can Exercise) is an exercise program developed by the Arthritis Foundation to improve muscle strength and joint flexibility for patients with arthritis. The purpose of this study was to explore the effects of PACE program on self-efficacy, pain, and joint function in the Korean immigrant elderly. The PACE program was held twice a week for 6 weeks for Korean immigrant elderly who had osteoarthritis. Twenty four subjects completed the program, who were recruited in two places : 10 elderly in a senior residential apartment, and 14 elderly in a senior center supported by Congregated Meal Program for Korean Elderly. Self-efficacy(Sherer et al., 1982), pain severity(by using Visual Analogue Scale), and number of painful joints were measured before and after the PACE program. To examine the joint flexibility and strengthening, the followings were measured : the extent of the upward arm reach in both sides(flexibility of shoulder), the ability to touch fingertips of the both hands in back pat and rub(flexibility of arm), the degree of range of motion(ROM) of both ankles in their dorsiflexion(flexibility of ankle) and plantarflexion with standing with toe(strengthening of ankle), and the degree of knee extension. Wilcoxon signed rank test was used for data analysis and the significance of the differences in the variables was examined to compare the data obtained before and after the PACE program. After the PACE, followings were found : 1. Self-efficacy was significantly increased. 2. Pain severity and number of painful Joints was significantly decreased. 3. The flexibility of both shoulders and arms were significantly improved, but the flexibility of knee was not changed. 4. The flexibility and strengthening of both ankle was significantly improved. In conclusion, PACE was clearly proved to be an effective exercise program to promote self-efficacy, to reduce pain, and to enhance joint function in the elderly with osteoarthritis. It is suggested that the PACE program should be recommended as one of the useful and appropriate nursing interventions for elderly with osteoarthritis.

거골하 관절염으로 오인된 골관절 결핵 (1예 보고) (Osteoarticular Tuberculosis Misdiagnosed as Subtalar Osteoarthritis (A Case Report))

  • Osteoarticular tuberculosis is often misdiagnosed as other disease because of a rare incidence and nonspecific clinical and radiographic presentation. Therefore, it is important to know clinical and radiographic presentations of osteoarticular tuberculosis and to diagnose in early phase. Especially, fistula formation is one of the most important clinical features which suspects osteoarticular tuberculosis. We report a case of ankle tuberculosis to be misdiagnosed as subtalar osteoarthritis and performed subtalar arthrodesis.

  • Purpose: We evaluated the clinical and radiological results of arthroscopic ankle fusion using 2 medial screws which had advantages of less morbidity, early weight-bearing and high union rate. Material and Methods: From April, 2002 to March, 2004, 8 patients who had ankle osteoarthritis were treated by ankle fusion using 2 medial screws under arthroscopy; five patients with post-traumatic osteoarthritis, two with post-infectious arthritis and one with paralytic foot. There were 5 male and 3 female. Average age was 67 years old ranging from 57 to 71 years. We evaluated them clinically preoperative and postoperative using AOFAS score, VAS pain scale and patient's satisfaction. In regard to radiological fusion, we checked them by simple AP, lateral and mortise view. Follow up period was average 11 months (range, $6{\sim}24$ months). Results: All ankles were successfully fused with 2 medial screws under arthroscopy. The mean time of fusion was 10.5 weeks (range, $8{\sim}14$ weeks). Patient's satisfaction checked at 6 months after operation had favorable results (excellent and good 75%). One case had pain on medial malleolar area because of screw's protrusion. Conclusion: Arthroscopic ankle fusion using 2 medial screws was good modality of ankle fusion with less morbidity and early weight-bearing in some cases of ankle arthritis.

관상면 변형이 동반된 족관절염에서의 인공관절 치환술 (Total Ankle Arthroplasty in Ankle Arthritis with Coronal Plane Deformity)

  • Total ankle arthroplasty has become a viable motion-preserving alternative to ankle arthrodesis, especially in the last two decades. Recent improvements have been achieved in the strength of implant design and surgical technique. Nevertheless, addressing preoperative deformities is essential for successful outcomes of total ankle arthroplasty. Residual malalignment can produce instability and edge loading, causing acceleration of polyethylene wear, followed by osteolysis and an increased risk of revision surgery. Therefore, the accompanying deformities and their correction techniques need to be comprehensively elucidated and understood. In this article, we provide a review of the application of total ankle arthroplasty in arthritis with coronal plane varus and valgus deformities.

Effects of a 12-week Combined Exercise Program on Gait Parameters in Elderly Women with Osteoarthritis

  • Objective: The purpose of this study was to investigate the effects of a 12-week combined exercise program on gait parameters in elderly women with osteoarthritis. Method: The subjects of this study were 11 elderly women (age: $67.09{\pm}2.47$, height: $157.35{\pm}4.30cm$, weight: $62.49{\pm}6.36kg$) with knee osteoarthritis. The combined exercise program of this study was divided into aerobic exercise and lower extremity strengthening exercises. The exercises were performed for 60 minutes per session, three times a week, for a total of 12 weeks. The maximum joint moments of the hip, knee, and ankle joints with walking were obtained with 8-3D cameras (MX-T20, Vicon, USA) and 2-force plate (AMTI OR6-7-400, AMTI, USA). SPSS Windows version 23.0 was used for statistical analysis. A paired t-test was used for pre-post comparison. An alpha level of .05 was utilized in all tests. Results: The maximum extension moment was significantly higher in the hip joint after P1 on the X axis. The maximum joint moment of P3 in extension was statistically significantly lower after intervention. On the Z axis, the maximum joint moment was significantly lower after the exercise intervention at P3. There was a statistically significant increase in the extension moment of the left and right knee joints in the X axis after exercise intervention. In the right ankle joint, P1 (plantar flexion moment) showed a statistically significant high moment after exercise intervention. Conclusion: These results suggest that combined exercise, including lower limb and aerobic exercise, may have a positive effect on mobility and walking moments in patients with osteoarthritis of the knee.

족부족관절 질환에서 히알루론산과 혈소판 풍부 혈장 주사치료 (Hyaluronic Acid and Platelet-Rich Plasma Injections in Foot and Ankle Disorders)

  • Numerous nonoperative treatments have been used to reduce pain and improve the clinical outcomes of patients with foot and ankle disorders. Among these treatments, hyaluronic acid and platelet-rich plasma (PRP) injections have been used increasingly. This review analyzes the recent literature reporting the efficacy and indications for hyaluronic acid and PRP injections. Hyaluronic acid and PRP can be used safely as adjuncts. Hyaluronic acid injections have shown efficacy and have a promising role as an adjuvant treatment method. In addition, its indication has expanded to various foot and ankle diseases over osteoarthritis. On the other hand, the efficacy of PRP injection has not yet been clarified, and the lack of standardization of procedures for the preparation and administration of PRP makes it difficult to establish definitive treatment indications.

족관절 기립 측면 사진에서 측정한 경골축-거골비 (Tibial Axis-Talar Ratio Measured on Standing Ankle Lateral Radiographs)

  • Purpose: To present tibial axis-talar ratio and tibia-ankle surface angle in lateral measured on standing ankle lateral radiographs of adults who did not have specific ankle pathology. Materials and Methods: On Fifty-six radiographs without ankle osteoarthritis and malalignment, radiographic measures were performed with use of a custom dizitizing program based on $PV-WAVE^{(R)}$. AP ankle alignment was quantified by tibia axis-talar ratio (TTR) and tibia ankle surface angle in lateral (TLS angle). The data was compared with previously reported american data and analyzed using a t-test. Results: The average TTR and TLS angle of our series were $33.9{\pm}3.3%$, and no significant difference compared with reported american adults (p=0.152). Conclusion: The T-T ratio and TLS angle measured on standing ankle lateral radiographs of our series were $33.9{\pm}3.3%$ and $80.4{\pm}3.3^{\circ}$. This measures appear to be a useful measure for determining AP ankle alignment.

무릎관절 뼈관절염 환자의 발과 무릎관절 자세 특성간의 상관관계 (The Correlation Between Foot and Knee Posture Characteristics in Knee Osteoarthritis People)

  • PURPOSE: This study investigated to correlation between foot and knee posture characteristics in knee osteoarthritis patients. METHODS: The subjects of this study were 103 patients with clinically and radiographically-confirmed knee osteoarthritis was investigated using the quadriceps angle (QA), distance of both knee (DBK), ankle dorsiflexion angle (ADA), navicular drop (ND). One-way ANOVA and Pearson's correlation were used to investigate the correlation between foot and knee posture measurements. All of whom agreed to participate in the study. In order to assure the statistical significance of the results, we used for SPSS ver. 18.0 for windows. RESULTS: The results of this study were as follows : 1) There were statistically significant difference in the correlation between QA and DBK. 2) There were statistically significant difference in the correlation between DBK and ADA. CONCLUSION: According the results of this study, patients with osteoarthritis exhibit more genu varus knee posture. Moreover patients with osteoarthritis were not correlation between foot and knee Posture characteristics.

관절경하 유관 나사를 이용한 족관절 관절 고정술 (3례 보고) (TAnkle Arthrodesis Using Cannulated Screws under Arthroscopy -Three cases report-)

  • There have been numerous methods to obtain a stable arthrodesis of the tibiotalar joint. However, a controvercy has arisen over the most successful and reproducible method to obtain an arthrodesis. The objective of this report is to show that an ankle arthrodesis can be performed successfully and advantageously by using cannulated screws under arthroscopic control and that our csaes will be a part of the multicentric study of arthroscopic ankle arthrodesis in korea. Between May 1996 and May 1997, we perfomed 3 ankle arthrodesis under arthroscopy using cannulated screws. Two patients had post-traumatic osteoarthritis and one had rheumatoid arthritis. Clinical ankylosis in one case and radiological union in two cases were obtained in 12 weeks postoperatively. We propose that arthroscopic ankle arthrodesis is effective procedure in patient who has minimal deformity and bone loss, advantages like small incision, minimal morbidity, low infection rate and rapid return to normal daily activity, but more cases and comparision with open tecnique will be needed.
