Vascular Malformation of Flexor Hallucis Longus Muscle Associated with a Flexion Deformities of Toes: A Case Report (장무지 굴곡근의 혈관성 기형에 동반된 발가락의 굴곡 변형: 증례 보고)

  • Vascular malformations may typically present with palpable mass that can be either asymptomatic or can present with symptoms including swelling and pain. On rare occasions, vascular malformation of muscle may produce joint deformities caused by contracture of the involved muscle. When vascular malformation involves the flexor muscle of the leg, ankle equinus deformity may occur. However, there are no reports of toe deformities secondary to intermuscular or intramuscular vascular malformations of flexor muscles of toe. Thus, we report a case of vascular malformation of flexor hallucis longus muscle with flexion contracture of toes in a 40-years-old woman who was treated with surgical excision.

Microsurgical Selective Obturator Neurotomy for Spastic Hip Adduction

  • Objective : Cerebral palsy may induce harmful spastic hip adduction. We report the result of microsurgical selective obturator neurotomy, performed on 12 spastic hip adductions of 6 patients, followed clinically for at least 26 months postoperatively. Methods : Microsurgical selective obturator neurotomies, involving microsurgical resection of the anterior obturator nerve branches were performed on 6 patients from January 2000 through June 2003. All patients presented with the inability to sit and 2 patients complained of persistent, intractable pain. We used intraoperative bipolar stimulation to identify selected motor branches. Results : The procedure was performed bilaterally in all patients. In the 3 patients in whom contractures were present, microsurgical selective obturator neurotomies were accompanied by an additional tenotomy of the adductor muscles. Selective tibial neurotomy was performed on three of six patients who originally presented with a spastic ankle. Postoperatively, all spastic hip adductions were corrected more than 60 degrees in passive abduction-adduction amplitude. However, one patient who did not receive active postoperative physiotherapy demonstrated a decreased passive abduction-adduction amplitude upon follow-up. There were no surgical complications. Conclusion : We think microsurgical selective obturator neurotomy may be an effective procedure in the treatment of localized, harmful spastic hip adduction after failure of well conducted conservative treatment. As muscular contractions are often associated with spasticity of the hip adductors, an adjunctive tenotomy may be an option. Comprehensive postoperative physiotherapy is essential to improve long-term results.

Effects of Vibratory Stimulus on Postural Balance Control during Standing on a Stable and an Unstable Support (안정판과 불안정판에서 자세 균형 조절에 대한 진동자극의 영향)

  • The purpose of this study was to analyze the effects of vibratory stimulus as somatosensory inputs on the postural control in human standing. To study these effects, the center of pressure(COP) was observed while subjects were standing on a stable and an unstable support with co-stimulated mechanical vibrations to flexor ankle muscles(tibialis anterior tendon, achilles tendon) and two plantar zones on both foot. The COP sway measurement was repeated twice in four conditions: (1) with visual cue and vibration, (2) without visual cue and vibration, (3) with visual cue and without vibration, (4) without visual cue and with vibration. The calculated parameters were the COP sway area and the distance, the median frequency and the spectral energy of COP sway in three intervals $0.1{\sim}0.3,\;0.3{\sim}1,\;1{\sim}3Hz$. The results showed that vibratory stimulus affect postural stability. The reduction rate of the COP sway with vibratory stimulus were higher on the unstable support because the effect of postural stability increases when afferent nervous flow is more activated by vibration on unstable support. If unclear visual or vibratory information is received, one type of information is compared with the other type of sensory information. Then the input balance between visual and vibratory information is corrected to maintain postural stability. These findings are important for the rehabilitation system of postural balance control and the use of vibratory information.

The Effect of Increased Running Speed on the Magnitude of Impact Shock Attenuation during Ground Contact (착지 시 달리기 속도 증가가 충격 쇼크 흡수에 미치는 영향)

  • Objective: The purpose of this study was to investigate the effect of increased running speed on the magnitude of impact shock attenuation in high frequency (9~20 Hz) at support phase on the treadmill running. Method: Twenty-four healthy male heel-toe runners participated in this study. Average age, height, mass, and preference running speed were 23.43±3.78 years, 176.44±3.38 cm, 71.05±9.04 kg, and 3.0±0.5 m/s, respectively. Three triaxial accelerometer (Noraxon, USA) were mounted to the tuberosity of tibia, PSIS (postero-superior iliac spine), and forehead to collect acceleration signals, respectively. Accelerations were collected for 20 strides at 1,000 Hz during treadmill (Bertec, USA) running at speed of 2.5, 3.0, 3.5, and 4.0 m/s. Power Spectrum Density (PSD) of three acceleration signals was calculated to use in transfer function describing the gain and attenuation of impact shock between the tibia and PSIS, and forehead. One-way ANOVA were performed to compare magnitude of shock attenuation between and within running speeds. The alpha level for all statistical tests was .05. Results: No significant differences resulted for magnitude of the vertical and resultant impact shock attenuation between the tibia and PSIS, and forehead between running speeds. However, significant differences within running speed were found in magnitude of the vertical shock attenuation between tibia and PSIS, tibia and forehead at speed of 2.5, 3.0 m/s, respectively. Conclusion: In conclusion, it might be conjectured that muscles covering the knee and ankle joints and shoe's heel pad need to strengthen to keep the lower extremities from injuries by impact shock at relatively fast running speed that faster than preferred running speed.

Rectus Abdominis free Muscle Transplantation (복직근 유리 조직 이식술)

  • The rectus abdominis muscle is versatile alone or as a musculocutaneous flap and useful for defects of moderate size requiring well vascularized tissue in the extremities. The muscle is long, thin and thus well suites for the hand, anterior tibial and ankle defects. The anatomical location makes dissection convenient for the working teams simultaneously in the same field. Authors have performed rectus abdominis free muscle transplantation in 10 cased to fit defects or cavities in the lower extremities at Chonbuk National University Hospital from June 1992 through August 1994. The results were as follows: 1. 7 cases of the 10 were exposed lower extremities from the vehicle trauma and rectus abdominis free muscles were transplanted at average of the 40th hospital day. 2. In chronic osteomyelitis, saucerization and parenteral antibiotics infusion therapy were combined with in 2 cases. 3. 9 of 10 cases were in complete success except 1 case, 67-yrs-old female, who was sustained the high-energery motor vehicle trauma. 4. Split thickness skin graft was performed on the well-grown surface of the rectus abdominis muscle in the extremities at the 4th postoperative week and taken well without any complications.

Exoskeleton Based on Counterbalance Mechanism for Arm Strength Assistance (중력보상장치 기반의 근력보조 외골격 장치)

  • Workers in industrial fields are highly exposed to accidents or injuries caused by long working hours. An exoskeleton that is able to support the arm muscles of the worker and thereby reduce the probability of an accident and enhance working efficiency could be a solution to this problem. However, existing exoskeletons demand the use of high-priced sensors and motors, which makes them difficult to use in industrial fields. To solve this problem, we developed an arm assisting exoskeleton that consists only of mechanical components without any electronic sensors or motors. The exoskeleton follows the movement of the human arm by shoulder joint and ankle joint. In addition, counterbalance mechanisms are installed on the exoskeleton to support arm strength. The experimental validation of the exoskeleton was conducted using an EMG sensor, confirming the performance of the exoskeleton.

The Comparison of Lower Limb Muscle Activities and VMO/VLO Ratio according to Direction for Using the Ramp in the Normal Adult (정상성인에서 경사로의 사용 방향이 다리근육의 활성도 및 안쪽/가쪽넓은근의 비율에 대한 연구)

  • Purpose : The purpose of study was to find out the environmental risk factor that can be easily occurred imbalance muscle activities according to direction for using the ramp during one legged standing. Method : The subjects were 20 normal adults with a mean age of $23.15{\pm}2.14years$ and a Body Mmass Index (BMI) of $22.74{\pm}1.07$. Participants were measured muscle activities on vastus medialis, vastus lateralis, tibialis anterior, peroneus longus during one legged stance at four conditions ramp (down ramp, up ramp, medial ramp, lateral ramp). The statistical analyses were performed using IBM SPSS(Ver. 23) and p-value less than .05 were considered statistically significant for all cases. Result : In this study, the activity of the lower extremity muscle and the ratio of the vastus medial and lateral muscles according to the direction of use of the ramp were investigated. The changes in the muscle activity of the lower limbs along the direction of the ramp were significantly different between the vastus medial muscle and the peroneus longus muscle. Conclusion : For a short time on a ramp or a pedestrian crossing, a clerk in a ramp can move or stand by placing the lower limbs in various directions, but if performed in consideration of the individual's disease characteristics or unstable foot position, It is thought that there will be an effect to prevent on the ankle and knee unstability.

Sural Artery Flap for Skin Defect of Lower Legs (비복동맥 피판술을 이용한 하지 피부결손의 치료)

  • Purpose: We reconstructed the skin defect of lower legs exposing muscles, tendons and bone with fasciocutaneous sural artery flap and report our cases. Materials and Methods: Between March 2005 and September 2006, 8 cases of skin defect were reconstructed with fasciocutaneous sural artery flap. Defect site were 4 case of ankle and foot and 4 cases of lower leg. The average defect size was $4{\times}4\;cm^2$. There were 5 men and 3 women and mean age was 52.2 years. We evaluated the viability of flap, postoperative complication, healing time, patient's satisfaction. Results: There was no flap failure in 8 cases. But recurrent discharge in 2 cases was healed through several times adequate debridement and delayed suture without complication. Flap edema may be due to venous congestion was healed through leg elevation and use of low molecular weight heparin. Mean time to heal the skin defect was 4 weeks. No infection and recurrence in follow up period. Cosmetic results as judged by patients were that 5 cases are good and 3 cases are fair. Conclusion: Sural artery flap is good treatment method among the numerous methods in the cases of skin defect, with soft tissue exposed, which is not covered with debridment and skin graft. Sural artery flap is useful method for the skin defect of lower legs because it is simple procedure, has constant blood supply and relatively good cosmetic effect.

A Study on the Measurement and Shape of Lower Body of Sportmen (슬랙스 설계를 위한 스포츠전공 남자대학생의 하반신 체형 연구)

  • The aim of this study is to provide fundamental data on the development of ready-to-wear clothing appropriate for the lower body types of male students who play sports in college. The study was conducted by means of targeting 151 male sports majors between $18{\sim}27$ years of age. Characteristics of the respondents were ascertained by means of questionnaires and an evaluation of a total of 36 lower body features. The results of the study are as follows. 1. A comparison of anthropometric measurements with the National Anthropometric Survey of Korea (1997) revealed that the main characteristics of the lower somatotypes of male sports college students are large circumferences and developed muscles of the lower body. 2. Sports were classified into the following five categories : soccer, bodybuilding, wrestling, judo, and taekwondo. In particular, bodybuilding students demonstrated a somatotype with an increased circumference, depth, and width of the thigh and lower body. 3. Somatotypes were classified into 3 types by means of a cluster analysis employing S factors. Type 1 somatotype exhibits a large circumference in the lower body. Type 2 is a lower somatotype of a middle size. Finally, Type 3 encompasses a smaller group in weight and circumference with a developed calf and ankle. 4. Each group was evaluated using a discriminatory analysis as a check to see if the groups had been discriminated with accuracy. The total accuracy rate was 96.0%.

Effects of Aquatic Exercise on Skinfold Thickness and Circumference of Upper and Lower Extremities in Patients with Rheumatoid Arthritis (수중운동 프로그램이 류마티스 관절염 환자의 사지 피부두겹 두께와 둘레에 미치는 영향)

  • Many arthritis patients experience weakness of muscles in extremities mainly due to disuse atrophy and weight gain because of the limited activities and exercises. This study examines the effects of the 6-week aquatic-exercise program on the body fat and the muscle of the rheumatoid arthritis patients. Seventeen patients in the experimental group and 18 in the control group were assigned depending on their preference and physical condition. These patients had more than 3 points of pain out of 10, deformities in knee, wrist and ankle joints. The amount of aquatic exercise increases from 35 minutes in the first week to 60 minutes in the 6th week. In the resting period they discussed their own experiences about exercise, personal and family affairs, and performed some recreation programs in order to increase the self-efficacy and promote the relationship with other patients by the group activities. Skinfold thickness and circumferences of both extremities were measured before and after experiment to compare the difference. For testing the body fat Saham Model was used. Prior to the experiment two group's body weight, skinfold thickness and skin circumferences were not significantly different which indicates the homogeneity of two groups. Body weight and most parts of skinfold thickness of the experimental group were significantly lower than the control group after 6-week aquatic exercise program. Circumference was not significantly lower than the control after the program. These findings indicate the in-crease of muscle sizes and the reduction of the body fat. Therefore a more active application of aquatic exercise into a variety of clients is strongly suggested.
