The effects of 8-week spinal stabilization exercise program on NDI, postural balance and body shape change in patients with chronic neck pain (8주간의 척추 안정화 운동 프로그램이 만성 경부통 환자의 NDI, 균형 능력 및 자세 변화에 미치는 영향)

  • This study examined how the eight-week spinal stabilization exercise program affects the cervical disability index, postural balance ability, and body shape change. The exercise program performed 60 minutes of spinal stabilization exercise three times a week for eight weeks. Sixteen patients with chronic neck pain, who complained of neck pain for six months, were classified into exercise group (n=8 patients) and control group (n=8 patients). The results before and after the eight-week exercise program were observed. Significant differences were observed in the time, group, and interaction of the neck disability index (p<.05). The balance ability showed significant interaction effects between the groups and periods (p<.05). Significant differences were noted in the timing and interaction in the pelvic inclination angle in posture change (p<.05), and there were significant differences in the group, timing, group, and interaction in the cervical and shoulder position angles (p<.05). The above results showed that the spinal stabilization exercise significantly improved the cervical disability index, balance ability change, and body shape change in patients with chronic neck pain. Future studies will analyze the specific changes in spinal structure through radiographic imaging to increase the validity of spinal stabilization exercise.

A Comparison of pre and post-surgical characteristics in skeletal Class III malocclusion patients using counterpart analysis (구조적 대응체 분석법에 의한 골격성 II급 부정교합 환자의 악교정 수술전후의 비교)

  • Enlow's counterpart analysis explains the complex with anatomic and developmental characteristics where craniofacial aspect of Individuals has been developed. Counterpart analysis does not compare individual measurement with the normal value from the average of majority but analyzes by comparison of values that each individual has. In this study we examined surgical changes in skeletal Class III malocclusion patients(male 40, female 40) and compared them with normal occlusion patients using counterpart analysis. The results indicated that : 1. Skeletal anterior-posterior discrepancy was relieved by shortening of the ramus width(B3). 2. The ramus alignment(R3, R4) was displaced posteriorly and the occlusal plane angle(R5) was rotated clockwise. 3. Skeletal Class III pattern was relieved in the post-operative group, but differences in the level of the cranium(R1, R2) was remaining compared to the normal occlusion patients. 4. In the comparison of surgery methods, the two-jaw surgery group presented changes In the maxillary length(A4), ramus alignment(R3, R4) and occlusal plane angle(R5) compared to the one-jaw surgery group, but the differences were not significant. In the past study about Korean skeletal Class m patients, the skeletal characteristics are upward backward rotation of the cranial base, posterior displacement of the maxilla, forward inclination of the ramus and lengthening of the mandibular body, but in this study, skeletal Class m pattern was relieved by shortening of the ramus width and maxillary advancement by orthognathic surgery, because orthognathic surgery is usually performed on limited areas in the maxilla and the mandible.

A study on the change of head posture and hyoid bone position before and after rapid maxillary expansions (상악골 급속확장술식 전후 두경부 자세와 설골위치의 변화에 관한 연구)

  • The present study assessed the effects of Rapid Maxillary Expansion on head posture and hyoid bone position. For this study, 32 Angle's class III patients - hellman 3c $\~$ adult stage, mean age 12y9m - were selected and divided into two groups, A,B according to craniocervical angulation. Craniocervical angulation Increased in Group A and decreased in Group B after the therapy. And 23 Angle's class I persons $\~$ same hellman stage, mean age 12y7m $\~$ were selected (or the control group. Cephalometric analysis of skeletal pattern, pharyngeal space, head posture, hyoid bone position was performed. The results were as follows, 1. Comparison of skeletal pattern and pharyngeal space 1) All two groups(A,B) had Mandibular plane inclined inferiorly and no pharyngeal space change was obseved after RME therapy 2) Skeletal pattern and pharyngeal space of Group A, B were normal before and after treatment. 2. Comparison of head posture 1) Craniocervical angulation of Group A was increased after treatment. That of Group B was decreased and mandibular plane was inclined inferiorly after treatment. 2) Before treatment, craniocervical inclination was normal in Group A but larger than normal in Group B. After treatment, all two groups(A, B) had normal craniocervical angulation. 3. Comparison of hyoid bone position 1) After treatment, long axis of hyoid in Group A, B was not changed. Antero- posteriorly, hyoid position was changed posteriorly in Group A but no change was founded in Group B after treatment. Vertically, hyoid bone position were not changed in two groups except increase in APHFH in Group A after treatment 2) Long axis of hyoid bone was normal in Group A, B before and after treatment. Anteroposteriorly, hyoid bone position was more anterior than Group B, C before treatment but all the position of two groups had normal position after treatment. Vertical position of hyoid bone was normal in all two groups before and after treatment.

Comparison of the Outcomes after Primary Total Hip Arthroplasty Using a Short Stem between the Modified Anterolateral Approach and Direct Anterior Approach with a Standard Operation Table (일반 수술 침대와 짧은 대퇴 주대를 이용한 인공 고관절 전 치환술의 직접 전방 도달법과 변형된 전 측방 도달법에 따른 결과 비교)

  • Purpose: Total hip arthroplasty was performed using a direct anterior approach (DAA) on an ordinary operation table and a short femoral stem. The clinical radiographic results were evaluated by a comparison with those performed using the modified hardinge (anterolateral approach, ALA) method. Materials and Methods: From January 2013 to November 2015, 102 patients who underwent total hip arthroplasty using DAA (DAA group) and the same number of patients using ALA (ALA group), both performed by a single surgeon, were compared and analyzed retrospectively. The operation time and amounts of bleeding were compared, and the improvement in post-operative pain, ambulatory capacity and functional recovery of the hip joint were checked. The location of insertion of the acetabular cup and femoral stem were evaluated radiologically, and the complications that occurred in the two groups were investigated. Results: The amount of bleeding was significantly smaller in the DAA group (p=0.018). Up to 3 weeks postoperatively, recovery of hip muscle strength was significantly higher in the DAA group (flexion/extension strength p=0.023, abduction strength p=0.031). The Harris hip score was significantly better in the DAA group for up to 3 months (p<0.001) and the Koval score showed significantly better results in the DAA group up to 6 weeks (p≤0.001). The visual analogue scale score improvement was significantly higher in the DAA group by day 7 (p=0.035). The inclination angle (p<0.001) and anteversion angle (p<0.001) of the acetabular cup were located in the safe zone of the DAA group more than in the ALA group, and there was no statistically significant difference in the position of the femur stem and leg length difference. During surgery, two cases of greater trochanter fracture occurred in the DAA group (p=0.155). Conclusion: The DAA performed in the ordinary operation table using a short femoral stem showed post-operative early functional recovery. Because a simple to use fluoroscope was used during surgery with an anatomical position familiar to the surgeon, it is considered to be useful for the insertion of implants into the desired position and for an approach that is useful for the prevention of leg length differences.

A Study on Improving Survival of Bombina orientalis through Escape Facilities in Artificial Canals (무당개구리의 인공 수로 내 수로 탈출시설을 통한 생존성 향상에 대한 연구)

  • Amphibians are a taxonomic group that ecologically connects terrestrial ecosystems and aquatic ecosystems. They play a very important role in the food chain of the ecosystem. It is known that there are about 5,948 species distributed all over the world, but after the Industrial Revolution, due to industrialization and urbanization, there has been a decrease in species and populations. In particular, it is becoming a factor in exacerbating habitat fragmentation or fragmentation due to artificial canals. In orderto improve the survivalrate of wild animals in artificial canals, escape facilities are installed to reduce it. This study analyzed the slope, height of the escape facility, escape rate, and travel distance in the operating facility for Bombina orientalis, which mainly inhabits near forests. The slope of the escape facility showed a relatively similar escape success rate regardless of height at 50° and 60°, while at 70°, it showed a relatively high escape success rate at only 40cm in height. The success rate of escape from the waterway escape facility in operation was 14.71%, showing a very low utilization rate, and the recognition rate of the artificial canal escape facility was found to be very low as it moved along the side wall of the artificial canal. Therefore, in the case of a waterway escape facility for Bombina orientalis, it is possible to construct it at an angle of 60°, and if the side walls of the artificial canals are built within 60°, Bombina orientalis can move freely in both directions, overcoming the low utilization rate of existing waterway escape facilities. It is expected to minimize the impact of movement and death of artificial canals. In addition, if the spacing between escape facilities is narrowed from the installation standard of 30m and ramps are constructed in both directions upstream and downstream, the escape success rate of amphibians,reptiles, and small mammals otherthan lady frogs is expected to improve.

A STUDY ON SOFT TISSUE FACIAL PROFILE CHANCES IN ORTHODONTIC TREATMENT WITH FOUR PREMOLAR EXTRACTIONS (4개 소구치 발치를 통한 교정치료시 나타나는 안모 연조직 변화에 관한 연구)

  • The purpose of this study was to evaluate the differences of soft tissue profile changes between the growing patients and the adult who had passed the growth peak, in orthodontic treatment with four premolar extractions. The results which was taken by correlating the soft tissue changes with hard tissue changes, lip thickness, molar relationship and arch length discrepancy in both groups was like the followings. 1. Significant hard tissue changes were decrease of VIs, VIi, UlPP, LlMP, HIi and increase of HPog'in adults and decrease of VIs, VIi and increase of VA, VPog'and all the vertical measurements in adolescents. 2. Significant soft tissue changes were decrease of VLs, VLi, and VILS in adults and increase of VSn, VSLS, VLs, VPog' and almost all vertical measurements in adolescents, and ${\Delta}LsE,\;{\Denta}LiE$ in adults and ${\Delta}LsE,\;{\Denta}LiE,\;{\Delta}LiSP\;and\;{\Delta}Mang$ was also significant 3. Correlation coefficient between ${\Delta}VIs\;and\;{\Delta}VLs$ was the greatest in adults and the next was ${\Delta}Ii/{\Delta}Li,\;{\Delta}Ii/{\Delta}ILS,\;{\Delta}ID/{\Delta}Li\;and\;{\Delta}ID/{\Delta}ILS$. In contrast all the vertical and horizontal measurements of hard and soft tissue in adolescents showed statistically significant corerrlation. 4. There were differences in correlation between soft tissue changes and incisor inclination and retraction at both groups, but the lower lip, nasolabial angle and mentolabial angle were commonly less influenced by the hard tissue changes in both groups. 5. The thinner the upper lip was, the more the ${\Delta}LsSP$ was in both groups, and the thinner the lower lip was, the more the ${\Delta}LiE\;and\;{\Denta}LiSP$ was in adolescents. 6. Molar relationship didn't influence the soft tissue profile changes. 7. Arch length discrepancy didn't influence the soft tissue profile changes.

A study on treatment effects of different activator types in Angle's Class II div.1 malocclusion patients (2급 1류 부정교합 환자에서 activator유형에 따른 치료 효과에 관한 연구)

  • This research was carried out to compares the treatment effects of Horizontal and Vertical type activators in Angle's Class II div. 1 maloccusion patients with mandibular retrusion dand to find out whether different treatment effects or growth pattern were observed between sexes in each study groups. The results were as follows: 1. In Horizontal activator group, forward positioning of mandible and vertical increase in anteror face as examplified by increase of LAFH and AEM were observed when pre and post-treatment datas were evaluated. 2. Males samples in Horizontal activator group showed increase in mandiular length accmpanied by posterior positioning of maxilla, wheras female samples in Horizontal activator group showed increase in mandibular body length, labial inclination of mandibular incisors and increase in lower anterior facial height .3. In vertical activator group, increase in AFH, LAFH, PFH and LPFH were observed when pre and post treatment datas were evaluated. 4. Male samples in Vertical activator group showed increase in mandibular body length and anterior and posterior facial heights, whereas females samples of Vertical activator group showed mainly increase in anterior facial height. 5. When pre and post treatment datas of Horizontal and Vertical activator groups were compared, skeletal difference were mainly observed in pretreatment datas but dental difference were observed in post treatment datas ,indicating that two actiators differ only in their effects to dental variables. 6. Difference between sexes were noted after treatment although no difference were observed between sexs in each groups before treatmentt. This indicates that inherent growth effects in each sex exerts more influence 1km appliances used for treatment.

A Study on Effect of Stabilizing Pile on Stability of Infinite Slope (무한사면의 안정성에 미치는 억지말뚝의 영향에 대한 이론적 연구)

  • To analyze an infinite slope that is reinforced with stabilizing piles, the forces on the stabilizing pile were estimated by the theory of plastic deformation and the theory of plastic flow and the effects of diverse factors on the factor of safety of an infinite slope were investigated. According to the results of the analyses, the factor of the safety of the slope reinforced with stabilized piles were increased tremendously and the factor of safety decreased as the center to center distance of the stabilizing pile increased. The effect of the existence of seepage of the infinite slope with stabilizing piles on the factor of safety appears to be insignificant. Considering the formulated factor of safety of an infinite slope with stabilizing piles, the width and length of the element of the infinite slope and force on the stabilizing pile influence the factor of safety of the infinite slope with a stabilizing pile including the soil strength parameter, inclination of the slope and depth of the slope, which are important for calculating the factor of safety of a non-reinforced infinite slope. The factor of safety of an infinite slope with stabilizing piles derived from the theory of plastic deformation were increased significantly with the internal friction angle of the soil, and the minimum and the maximum factor of safety under the conditions considered in this study were 13.7 and 65.6, respectively. As the diameter of the stabilizing pile increased, the forces on the stabilizing pile also increased but the factor of safety of the infinite slope with stabilizing piles decreased due to the effects of the width and the length of the element of the infinite slope. The factor of safety of the infinite slope with stabilizing piles derived from plastic flow were much larger than that of the non-reinforced infinite slope and the factor safety of the infinite slope with a stabilizing pile increased with increasing product of the flow velocity and plastic viscosity ( ) and the factor of safety of the infinite slope with stabilizing piles decreased with increasing center to center distance of the pile.

THREE-DIMENSIONAL FINITE ELEMENT ANALYSIS OF THE PHENOMENON PRODUCED DURING RETRACTION OF FOUR MAXILLARY INCISORS (상악 4절치의 후방견인시 나타나는 현상에 관한 유한요소법적 분석)

  • This study was designed to investigate force systems and tooth movements produced by retraction archwire during retraction of four maxillary incisors after the maxillary canine retraction into the maxillary first premolar extraction space using the computer-aided three-dimensional finite element method. A three-dimensional finite element model, consisting of 2248 elements and 3194 nodes, was constructed. The model consisted of maxillary teeth and surrounding periodontal membranes, .022'$\times$.028'-slot brackets, and 5 types of retraction archwires(.019'$\times$.025' stainless steel archwire) modeled using the beam elements. The contact between the wire and the bracket slot was modeled using the gap elements because of the non-linear elastic behaviors of the contact between them. The forces and moments, End displacements produced by retraction archwire were measured at various conditions to investigate the difference according to types of loops, magnitudes of activation force, gable angle, and anterior lingual root torque. The results were expressed quantitative and visual ways in the three-dimensional method. The following conclusions can be drawn from this study.1. When the tear-drop loop archwire was activated, the mesio-distal and lingual translational movements of the teeth helped to close the extraction space, but unwanted movements of the teeth including intrusions and extrusions, and rotational movements in each direction occurred. 2. Activation of T-loop archwire compared with those of other types of retraction archwires produced the least translational movements of the teeth helped to space closure and also the least unwanted movements of the teeth. 3. Increasing amount of activation in the tear-drop archwire led not only to increase of translational movements of the teeth helped to space closure, but also to increase of unwanted movements of the teeth. 4. Addition of gable bend in the tear-drop archwire helped anterior teeth to translational movements in the mesio-distal direction, but increased unwanted movements of the teeth 5. Addition of anterior lingual root torque in the tear-drop archwire helped central and lateral incisor to improve their facio-lingual inclination, but increased unwanted movements of the teeth.

A Study on the date of the black glazed bowls with "Gongyu (供御)" or "Jinzhan (進琖)" of Jian ware (건요(建窯) "공어(供御)"·"진잔(進琖)"명흑유완(銘黑釉碗)의 제작시기 문제)

  • This study specifically examined the date of the black glazed bowls marked with "Gongyu (供御)" or "Jinzhan (進琖)" of Jian ware, one of the key research topics related to the bowls. The results of this study can be summarized as follows. Jian Kiln produced various shapes of black glazed bowls, but almost all of the inscriptions of "Gongyu (供御)" or "Jinzhan (進琖)" are found only in two certain type bowls: Type I, the Shukou type bowls (束口碗), or Type II, the Piekou type bowls (撇口碗). Of these, there are significantly more of the former in existence. For Type I bowls marked with "Gongyu (供御)" or "Jinzhan (進琖)", the mouth of the bowls is slightly evaginated outwards, and the inclination angle of the side slope is about 50°. The shape feature, Shukou (束口) is formed which is slightly indented around the bowl about 0.3~0.5cm below the mouth of bowl. And the height of the bowls is relatively low compared to other Type I black glazed bowls produced by Jian Kiln, so the height divided by the diameter is 0.5 or less. There is little difference in shape between the black glazed bowls marked with "Gongyu (供御) and those with "Jinzhan (進琖)". However, taking into consideration the excavation situation of both type bowls, the former is considered to be ahead of the latter in terms of production date. On the other hand, the black glazed bowls of Jian ware, which have the same shape features as the Type I bowls marked with "Gongyu (供御)" or "Jinzhan (進琖)" have not been found in the tombs dating from the end of the 12th to the beginning of the 13th century from which typical Jian kiln black glazed bowls of the same type were excavated. For the Hakata site (博多遺址) in Japan, the black glazed bowls with such a shape feature were excavated from early 12th century sites, rather than from the late 12th to early 13th century sites at which the typical black glazed bowls of Jian kiln were found. Considering that the black glazed bowls from Fujian province were imported into Hakata with almost no time gap, it is very unlikely that the production time of Type I black glazed bowls marked with "Gongyu (供御)" or "Jinzhan (進琖)" will deviate from the early 12th century. In conclusion, it is considered that the black glazed bowls marked with "Gongyu (供御)" or "Jinzhan (進琖)" of Jian ware were produced in the early 12th century.
