Surgical Treatment Using a Lateral Approach in Intra-articular Fractures of the Calcaneus (외측 도달법을 이용한 관절내 종골 골절의 수술적 치료)

  • Purpose: We report the radiologic and the clinical results for surgical treatment of calcaneal fractures involving the subtalar joint. Methods: We evaluated the findings of radiographs and computed tomographs of 39 patients (40 cases) with intra-articular calcaneal fractures. The fractures were treated with open reduction via an extended lateral approach and internal fixation using a plate. We assessed the radiologic results, such as the $B{\ddot{o}}hler$ angle, the Gissane angle, and the height/width ratio. We assessed the clinical results based on the critieria of Salama et al. Results: According to the Essex-Lopresti classification, 9 cases were classified as tongue type and 31 cases as joint depression type. According to the Sanders classification, 10 cases were classified as type IIA, 4 cases as IIB, 16 cases as IIIAB, 4 cases as IIIAC, and 6 cases as type IV. The mean preoperative $B{\ddot{o}}hler$ angle was 7.7, the mean postoperative $B{\ddot{o}}hler$ angle was 21.1, and the mean last follow-up $B{\ddot{o}}hler$ angle was 16.8. Clinical results classified as 10 excellent, 13 good, 11 fair, and 6 poor. Conclusion: In the treatment of intra-articular fractures of the calcaneus, open reduction via an extended lateral approach seems to be a useful method in that it can provide direct exposure of the subtalar joint with little morbidity.

Factor Affecting Mandibular Rotational Troque Movements (하악의 비틀림회전운동에 영향을 미치는 요인)

  • This study was performed to investigate the factor that might affect mandibualr body rotation. For the study, 115 patients with temporomandibular disorders and 35 dental students without angy signs and symptoms of temporomandibular disorders were randomly selected as the patient group and the contreol group, respectively. Preferred chewing side, Angle' classification, lateral guidance pattern, and affected side were clinically recorded, and the amount of Mandibular body rotational torque movement was measured in wide opening and closure, in right and left excursion with vertical and lateral distance in frontal plane, right and left rotational angel in horizontal and in frontal plane. Masticatory muscle activity of anteriorocclusal contact pattern on maximal hard biting were also observed synchronously with BioEMG and T-Scan , respectively. The observed items were muscle activity of anterior temporalis and superficial masseter, and tooth contact status related to contact number, force, duration, and occlusal unbalance between right and left arch. The data collected were analyzed by SAS statistical program. The results of this study were as follows : 1. Mean value of vertical distance in frontal plane in wide opening and closure was more in control subjects than in patients, but there was no difference for rotational angle. In right excursion, rotational angles were greater in patient group than in control group. 2. Comparison among the subjects by preferred chewing side did not reveal any significant difference, but comparison among patients by affected side showed more rotational amount in bilaterally affected patients than in unilaterally affected patients. 3. Comparison among the subjects by Angle's classification or lateral guidance pattern revealed no difference. There was also no difference between preferred chewing side and contralateral side, and between affected side and contralateral side. 4. Positive correlation in madibular rotational torque movements were observed among vertical distance, total horizontal rotation angle, electromyographic activity of anterior temporalis, tooth contact number, and tooth contact force but total frontal rotation angle almost did not show any correlation with other variables except vertical distance.

Correlations between the Clinical Results and Radiologic Evaluation after Surgical Treatment of Calcaneal Fracture (종골 골절의 수술적 치료 후 방사선학적 평가와 임상적 결과의 상관 관계: 종골 골절의 술 후 방사선학적인 평가)

  • Purpose: The purpose of this study is to find out the correlation factors for prognosis of calcaneal fractures. Materials and Methods: 120 cases (feet) of calcaneal fractures, all of them followed up for more than 1 year after surgical procedures, consisted of 101 men (105 feet) and 13 women (15 feet) were reviewed retrospectively. The collected clinical data were as follows : injury mechanism, surgical procedures, time to procedure, time to work and the radiologic data: Bohler angle, heel width, displacement of posterior facet. AOFAS hindfoot score and VAS score were checked. With ANOVA test and multiple regression analysis, the data processed statistically. Results: According Sanders classification, type II was 37 cases (31%), type III 66 cases (55%), and type IV 17 cases (14%). On plane radiography, the Bohler angle improved to average 28.4 degree from 5.6 degree, and the displacement of posterior facet was corrected to average 1.2 mm. AOFAS hindfoot score was checked average 81.7 points postoperatively, and the meaningful difference existed between types of Sanders classification. The Bohler angle represented the outline of the calcaneus had the better correlation with the clinical outcome of calcaneal fractures rather than the anatomical reduction of the posterior facet did. And the width of calcaneus had good correlation with the clinical score. Conclusion: We should also concern about the outline of calcaneus, the width of calcaneus and the Bohler angle representing anatomical reduction, not only the acute reduction of the posterior facet.

  • High-resolution images using remote sensing (RS) is importance to secure for spatial classification depending on the characteristics of the complex and various factors that make up the river environment. The purpose of this study is to evaluate the accuracy of the classification results and to suggest the possibility of applying the high resolution hyperspectral images obtained by using the drone to perform spatial classification. Hyperspectral images obtained from study area were reduced the dimensionality with PCA and MNF transformation to remove effects of noise. Spatial classification was performed by supervised classifications such as MLC(Maximum Likelihood Classification), SVM(Support Vector Machine) and SAM(Spectral Angle Mapping). In overall, the highest classification accuracy was showed when the MLC supervised classification was used by MNF transformed image. However, it was confirmed that the misclassification was mainly found in the boundary of some classes including water body and the shadowing area. The results of this study can be used as basic data for remote sensing using drone and hyperspectral sensor, and it is expected that it can be applied to a wider range of river environments through the development of additional algorithms.

  • This paper proposes an efficient algorithm which recognizes the mixed document image consisting of the images, texts, tables, and straight lines. This system is composed of three steps. The first step is the detection of rotation angle for complementing skewed images, the second is detection of erasing an unnecessary background region and last is the classification of each component included in document images. This algorithm performs preprocessing of detecting rotation angles and correcting documents based on the detected rotation angles in order to minimize the error rate by skewness of the documentation. We detected the rotation angie using only horizontal and vertical components in document images and minimized calculation time by erasing unnecessary background region in the detecting process of component of document. In the next step, we classify various components such as image, text, table and line area included in document images. we applied this method to various document images in order to evaluate the performance of document recognition system and show the successful experimental results.

  • To classify the foot type, direct measurements, metatarso-phalanx angle and foot print angle of the right and left foot were measured and analyzed. The results are as follows. 1. The correlation coefficients between right and left foot were high degree, and except medial malleous height, the diffences between right and left foot are not significant. 2. The correlation coefficient among direct measurements of the foot are high degree, tut the correlation coefficients between direct measurements and metatarso-phalanx angle and foot print angle are no or low degree. Therefore, to recognize the foot type, the direct measurements, metatarso-phalanx angle and foot print angle need to be measured independently. 3. According to foot width/foot $length{\times}100$ which is the slender degree of foot, three groups are distinguished: one is slender type of which foot width/foot $length{\times}100$ is less than $40.14\%$, two is standard type of which foot width/foot $length{\times}100$ is $40.14\%{\~}44.30\%$, three is broad type of which foot width/foot $length{\times}100$ is more than $44.30\%$. 4. On the photographs metatarso-phalanx angle was measured, and two groups are distinguished: one is normal type of which the metatarso-phalanx angle is more than $160^{\circ}$ and the other is the deformed type of which the metatarso-Phalanx angle is less than $159^{\circ}$. 5. By foot print angle $30^{\circ}$ which need reformation of the foot, two groups are distinguished: one is normal foot print angle of which the angle is more than $30^{\circ}$ and the other is flat foot print angle of which the angle is less than $30^{\circ}$. 6. Classifications by foot width/foot $length{\times}100$, metatarso-Phalanx angle and foot Print angle are put together, and then foot types are classified into 12 groups such as Table 11.

A STUDY ON THE FACIAL ESTHETIC PREFERENCES AMONG KOREAN YOUTHS: ASSESSMENT OF PROFILE PREFERENCES (한국 젊은이의 안면미 선호경향에 관한 연구 : 얼굴의 측모평가를 중심으로)

  • This study was designed to assess profile preferences among Korean youths in the year 1992. Facial esthetics was evaluated by means of silhouette profiles, eliminating the influence of a number of aspects that may affect judgment when normal lateral photographs are used. The main points of preference to be clarified here are as follows. First, on facial convexity, Second, on nasion depth, Third, on mentolabial sulcus depth, Fourth, on the position of upper and lower lips, Fifth, on facial type according to Angle's classification of malocclusion, Sixth, on Song's tangents. The 54 subjects printed in questionnaire as black and white silhouettes were selected from 300 tracings from cephalometric radiographs of people whose age ranging from 11 to 20 years. Photographs of six female subjects were retouched by computer graphic software and printed in color and black/white photographs which were used for adaptation of eyes of participants in selecting profiles in silhouette. They constitute 2 questions. The 54 subjects were grouped as 22 questions, each of them composed of 6 subjects, according to the aspects to be clarified. Twenty four questions in total were asked to assess profile preferences. For the assessment, the profile line, the facial esthetic triangle, Song's tangents, and Angle's classification of malocclusion were introduced. The profile line is composed of 11 component points which are Trichion, Glabella, Nasion, Pronasale, Subnasale, Labrale superius, Stomion, Labrale inferius, Supramentale, Pogonion, and Gnathion. The facial esthetic triangle is composed of 3 tangents: A-tangent which is the tangent of dorsum of nose, B-tangent which is the line passing through Sn and Ls, and C-tangent which is drawn on the turning point of the curve which lies between mentolabial sulcus (Sm) and pogonion (Pg). Angle's classification has 3 types of malocclusion which are Class I, Class II, and Class III. Class II malocclusion is subdivided into Division 1 and Division 2. The participants of the survey were composed of 861 college students (448 male students, 413 female students) whose majors grouped as Fine Arts. Liberal Arts, and Natural Sciences, and whose mean age 21.8 years. The statistics program SPSS/PC + of SPSS Inc. was used to analyze answers of participants. Crosstabulation, Chi-square test, and Kendall test were done. The conclusions are as follows: First, Korean youths have a tendency to prefer the slightly convex face to the flat or concave face. Second, they prefer a moderately deep nasion. Third, they prefer a moderately deep mentolabial sulcus. Fourth, they prefer the position of lips which are near to Ricketts' E-line. The position of the upper lip which is slightly posterior to E-line is preferred. The upper lip which lies too far anterior or posterior to the lower lip is not perferred. Fifth, they prefer most, according to Angle's Classification of Malocclusion, Class I facial profile which has a slight inclination to Class II division 2. The order of preference is Class I, Class II division 2, Class III, and Class II division 1. Sixth, they prefer the type 2 and 3 of Song's tangents. The facial profile within which A-and B-tangent meet is preferred. The facial profile which has Cotangent that .meets with A-tangent slightly posterior to the crossing point of A-and B-tangent or that parallels with B-tangent is preferred.

Classification of Upper Body Somatotypes according to the Age Group : Using 3D-Body Scan Data

  • Two hundreds of female aged 19 years old and up were recruited to evaluate the postural changes and bilateral variation of asymmetry over age. To find out the differences among the age group, subjects were classified into 5 groups, early young age(19-29), late young age(30-39), early middle age(40-49), late middle age(50-59), and old age(60-). 35 body measurements were taken by the 3-D body scanner which allowed us to take measurements which cannot be measured using traditional methods, including the shape of a cross section, slice area surface are, and volume. Bilateral variations were observed as a function of age; Depth of scapular point level, scapular point to center back, and blade angle. Postural change of anterior cervical angle, upper anterior thoracic angle, upper posterior thoracic angle, posterior cervical angle, and center back/center front ratio were also exhibited. In each measurements, subjects were classified into normal, and abnormal group. Percentiles of abnormal in shoulder line angle, blade angle, neck point $\∼$ acromial point $\∼$ scapular point, posterior cervical angle, and upper posterior thoracic angle were increased over age group. The upper body of lateral view was classified into 3 types of posture based on the previous research; straight, erect(leaning back), and stooped(bent forward). The percentiles of subjects who have straight postures were decreased as a function of age, but those of stooped postures were increased. Subjects who have erect postures did not so. The stooped posture group shows the big cervical fossa angle, anterior cervical angle, posterior cervical angle, upper posterior thoracic angle, and the small upper anterior thoracic angle comparing to the straight and erect posture group. These results could be apply for clothing construction reflecting the changes in back, shoulder, neck, and the bilateral asymmetry according to the target age group.

Forensic Classification of Median Filtering by Hough Transform of Digital Image (디지털 영상의 허프 변환에 의한 미디언 필터링 포렌식 분류)

  • In the distribution of digital image, the median filtering is used for a forgery. This paper proposed the algorithm of a image forensics detection for the classification of median filtering. For the solution of this grave problem, the feature vector is composed of 42-Dim. The detected quantity 32, 64 and 128 of forgery image edges, respectively, which are processed by the Hough transform, then it extracted from the start-end point coordinates of the Hough Lines. Also, the Hough Peaks of the Angle-Distance plane are extracted. Subsequently, both of the feature vectors are composed of the proposed scheme. The defined 42-Dim. feature vector is trained in SVM (Support Vector Machine) classifier for the MF classification of the forged images. The experimental results of the proposed MF detection algorithm is compared between the 10-Dim. MFR and the 686-Dim. SPAM. It confirmed that the MF forensic classification ratio of the evaluated performance is 99% above with the whole test image types: the unaltered, the average filtering ($3{\times}3$), the JPEG (QF=90 and 70)) compression, the Gaussian filtered ($3{\times}3$ and $5{\times}5$) images, respectively.

Operative Treatment with the Plate Fixation in Intraarticular Calcaneal Fractures (관절 내 종골 골절에서 금속판 고정을 이용한 수술적 치료)

  • Purpose: We evaluated the results of operative treatment with F or H plates and screws using extensile lateral approach in intraarticular calcaneal fractures. Materials and Methods: From August 2003 to July 2006, twenty intraarticular calcaneal fractures which were operated with open reduction and internal fixation with F or H plates and screws were evaluated retrospectively. According to the Essex-Lopresti classification, 3 cases were tongue type and 16 were joint depression type. With the Sanders classification, 2 cases were IIA type, 7 were IIB, 5 were IIIAB, 2 were IIIAC and 3 were IV. We have analysed the $B{\ddot{o}}hler$ angle, Gissane angle, and calcaneal width in radiologic evaluation, and evaluated clinical result according to the Creighton-Nebraska Health Foundation Score. Results: Radiologic changes showed as follows: $B{\ddot{o}}hler$ angle improved from $5.8^{\circ}$ to $25.9^{\circ}$, Gissane angle from $119.0^{\circ}$ to $113.3^{\circ}$, and calcaneal width from 50.4 mm to 37.8 mm. In the clinical results, excellent cases were noted in 8 cases, good in 8 cases, fair in 2 cases, poor in 1 case. Conclusion: Operative treatment with F or H plates and screws using extensile lateral approach in intraarticular calcaneal fractures was thought to be a useful operative method allowing anatomical reduction.
