부정교합자의 하악안정위에 관한 두부 X선학적 연구 (A ROENTGENOCEPHALOMETRIC STUDY ON THE REST POSITION OF THE MANDIBLE IN MALOCCLUSION)

  • In orthodontic view points, there were a few attempts to describe on the mandibular rest position with roentgenocephalometric studies for the persons with malocclusion. The purpose of the present study was to evaluate and interpret the difference in each of the Angle's classification of the malocclusion. The author has selected 115 malocclusion patients of the both sexes as subjects, whose dental ages were from III A. Two lateral cephalometric head films were taken mandible at rest and with the teeth in occlusion utilizing the Broadbent-Bolton cephalometer. Their linear and angular measurements were made directly. The author obtained the following results: 1) The change in the incisal overbite was of $-2.32\~-3.36mm$, and the overjet decreased similiarly in each class of the malocclusion. 2) The range of free-way space was of $1.97\~3.56mm$, and showed prominent in the Angle's class III malocclusion. 3) In the rest position, the mandible showed backward displacement, and it was prominent in the Angle's class III malocclusion. 4) In the change of Ar-Gn, the class I and II showed more increase of distance-than the class III. 5) In the change of ${\angel}SNL_1L_1'$, the class III showed smaller angle than the class I and II. 6) There was no significant difference between the male and female in each class of the malocclusion.

앵글씨 분류에 의한 성인 골격구조 및 하악운동량 평가 (The Assessment for Mandibular Movement and Adult Facial Skeletal Structure According to Angle's Classcification)

  • The purpose of this study was to identify the difference of vertical movement of mandible according to Angle's molar relationship and by skeletal factors affect to vertical movement of mandible. 172(age ranged from 20 to 30) subjects who go to college within territory of Kwangju city without any experience of temporomandibular disorder, extraction and orthodontic treatment. were selected for this study. The subjects were classified into class I(male:30, female:49), class II(male:18, female:24) and class III(male:18, female:33) according to Angle's molar relationship. The distance was measured between incisal edge of maxillary and mandibular central incisor and between bottom of central fossa of maxillary and mandibular 1st molar with ruler. The arch length and width were measured on the diagnostic cast. Cephalometrics were taken and then traced. Landmarks were identified and analyzed. 1. Maximal interincisal opening of male is larger than that of female in class I, class II and class III. Among each group maximal interincisal distance is the largest in class III. Maximal intermolar distance of male is superior to that of female in class I, class II, and class III, but there is no siginficant difference among them. 2. On maximal opening movement of Angle's classification class I and class II, total mandibular length, mandibular ramal length, madibular inferior border length and upper arch width were important variables and facial length, upper arch length and lower arch length had negative relationship to that. On maximal opening movement of Angle's class III, the upper arch length, the lower arch length and anterior facial length were important variables especially when compared with class I and II, and upper arch width had negative relationship. These results suggest that maximal opening movement is affected by facial morphology in all classes, but each group is affected by different facial skeletal variables. Accordingly, facioskeletal variables might be considered as diagnosis and treatment to improve the amount of mouth opening.

한국성인 정상교합자와 부정교합자의 연조직 측모에 관한 두부방사선 계측학적 비교연구 (A CEPHALOMETRIC COMPARATIVE STUDY ON SOFT TISSUE PROFILE BETWEEN NORMAL OCCLUSION AND MALOCCLUSION IN KOREAN ADULTS)

  • The purpose of this artic1e is to compare soft tissue profiles between Korean adults with normal occ1usion and malocclusin and to identify the differences between them. The subjects of this cephalometric study were 40 males with normal occlusion(Group 1), 27 females with normal occlusion(Group 2), 28 adults with Angle's Class II malocclusion(Group 3) and 41 adults with Angle's Class III malocclusion(Group 4). The results of this study were as follows ; 1) People with Angle's Class II malocclusion had tendency to have more labial tipping of lower teeth than people with normal occ1usion. Through NOA angle measurement, it was determined that people with Angle's Class II malocclusion had more protruding midface than people with normal occlusion and people with Angle's Class III malocclusion had retruding midface. 2) Through Powell's esthetic triangle analysis, it was determined that people with Angle's Class II malocclusion had retruding chin and protruding nose. 3) No significant differences between people with normal occlusion and maloclusion could be identified by measuring soft tissue profile angle basis of S-NS plane. 4) There were significant differences between groups with normal occlusion and malocclusion by measuring Facial convexity angle(Significance level 99%). 5) By measuring the distance between each landmark basis of N-Pog plane, People with Angle's Class II malocclusion were identified as having more protruding midface, but there were no significant differences between people with normal occlusion and Angle's Class III malocclusion. 6) By measuring the vertical dimension of the face, it was determined that the lower facial height was higher than the upper facial height in all groups, particularly in group with Angle's Class III malocclusion. 7) By measuring the lips basis of E-line and S-line, it was determined that people with Angle's Class III malocclusion had more, protruding lower lips than people with normal occlusion, while people with normal occlusion, while people with Angle's Class II malocclusion had more protruding upper lips. By measuring the distance between the superior sulcus and inferior sulcus basis of H-line, people with Angle's Class II malocclusion had thicker upper lips than the other's.

Short-term and long-term treatment outcomes with Class III activator

  • Objective: The purpose of this retrospective study was to investigate short-term and long-term skeletodental outcomes of Class III activator treatment. Methods: A Class III activator treatment group (AG) comprised of 22 patients (9 boys, 13 girls) was compared with a Class III control group (CG) comprised of 17 patients (6 boys, 11 girls). The total treatment period was divided into three stages; the initial stage (T1), the post-activator treatment or post-mandibular growth peak stage (T2), and the long-term follow-up stage (T3). Cephalometric changes were evaluated statistically via the Mann-Whitney U-test and the Friedman test. Results: The AG exhibited significant increases in the SNA angle, ANB angle, Wits appraisal, A point-N perpendicular, Convexity of A point, and proclination of the maxillary incisors, from T1 to T2. In the long-term follow-up (T1-T3), the AG exhibited significantly greater increases in the ANB angle, Wits appraisal, and Convexity of A point than the CG. Conclusions: Favorable skeletal outcomes induced during the Class III activator treatment period were generally maintained until the long-term follow-up period of the post-mandibular growth peak stage.

III급 부정교합자의 안모유형에 관한 연구 (THE CEPHALOMETRIC STUDY OF FACIAL TYPES IN CLASS III MALOCCLUSION)

  • It is the aim of this study to observe the distribution of various facial types in class III malocclusion and to characterize the craniofacial features of the very facial types. Cephalometric headptates of a hundred and ten persons showing bilateral class III malocclusion whose mean age was 12.51 years and sixty nine persons of normal occlusion whose mean age was 12.23 years were measured and statistically analyzed. The following summary and conclusions were drawn. 1. Affording the bases for SNA and SNB, $35.45\%$ of sample showed normally positioned maxilla and protruded mandible, $30.00\%$ for retruded maxilla and normally positioned mandible, $15.45\%$ for retruded maxilla and protruded mandible, $10.90\%$ for both maxilla and mandible within normal range and $8.20\%$ for miscellaneous types were arranged in class III malocclusion. 2. $52.72\%$ of sample showed neutrodiveigent, $35.45\%$ for hyperdivergent and $11.81\%$ manifested hypodivergent mandible in class III malocclusion. 3. Providing the bases for facial and mandibular planes, $33.63\%$ of sample showed prognathic and neutrodivergent, $20.90\%$ for mesognathic and hyperdivergent, $17.27\%$ for prognathic and hyperdivergent and $15.45\%$ for mesognathic and neutrodivergent were arranged in class III malocclusion. 4. The class III malocclusion brought out shorter cranial base, smaller saddle angle, and larger articular and genial angle. It showed retropositioned maxilla and forward positioned mandible in spite of no significant differences in linear measurements of mandible. Anterior lower facial height was significantly larger in class III malocclusion, while posterior total facial and anterior total facial heights exhibited no significant differences. 5. It is suggested class III malocclusion was attributed to shorter cranial base, smaller saddle angle, maxillary deficiency and/or retrusion, mandibular excess and/or protrusion, excessive vertical growth of the anterior lower face, and their complex as well.

개방교합과 과개교합에서 구치의 근원심 치축경사도에 관한 두부방사선계측학적 연구 (A CEPHALOMETRIC STUDY ON MESIODISTAL AXIAL INCLINATION OF POSTERIOR TEETH IN OPEN BITE AND DEEP BITE)

  • For the purpose of investigating mesiodistal axial inclination of posterior teeth in normal occlusion group, open bite and deep bite group and investigating the correlationship between the axial inclination of posterior teeth and overbite of anterior teeth, a cephalometric study was performed on the subjects consisted of normal occlusion group(40), open bite group(71 : Angle's Class I, Class II, division 1 25, Class III 25) and deep bite group(64 : Angle's Class I 23, Class II, division 1 21, Class III 20). Mesiodistal axial inclination of posterior teeth to occlusal, mandibular and palatal plane were measured. The findings of this study were as follows : 1. Upper and lower posterior teeth were more mesially inclined to occlusal plane in open bite group than in deep bite group. 2. Lower posterior teeth were more mesially inclined in deep bite group than in open bite group in Angle's Class II, division 1 malocclusion but there were no significant differences in Angle's Class I and Class III malocclusion. 3. There was no significant correlationship between the axial inclination of posterior teeth to each plane and overbite of anterior teeth in open bite group. 4. There was a significant correlationship between the axial inclination of upeer and lower second premolar to occlusal plane and overbite of anterior teeth in Angle's Class I, Class II, division 1 and Class III malocclusion.

Angle III급 부정교합자의 악안면골격 특성에 관한 연구 (STUDY ON THE CHARACTERISTICS OF CRANIOFACIAL SKELETON OF ANGLE'S CLASS III MALOCCLUSION CASES)

  • This investigation was designed to categorize Angle's class III malocclusion groups through analyzing horizontal and vertical components of craniofacial skeleton in Angle's class III malocclusion. The material selected for this study consisted in standard lateral cephalogram of one hundred and fifty five children, seventy four boys and eighty one girls, aged 6 through 15 years, having Angle's class III malocclusion. On the basis of findings of this study, the following results were obtained. 1. In horizontal skeletal classifications,16 groups were classified FMN-A-B, SE-FMN-A according to the Ba-SE-Me, Ba-SE/R. 2. The sequences that have relatively high frequency are as follow; a) Horizontal Group l b) Horizontal Group 3 c) Horizontal Group 5 d) Horizontal Group 9 e) Horizontal Group 4 3. In vertical skeletal classification, 8 groups were classified according to the PMV/PP, PMV/OP, PMV/MP. 4. The sequences that relatively high are as follows; a) Vertical Group 1 b) Vertical Group 3 c) Vertical Group 4 d) Vertical Group 8.

골격성 III급 부정교합의 골격유형에 따른 치아치조특성 (Dentoalveolar Characteristics according to facial types of Class III Malocclusion)

  • 본 연구는 골격성 III급 부정교합자의 두부방사선사진 분석을 통하여 수평적, 수직적 골격 형태에 따른 치아치조특성의 차이를 알아보기 위하여 시행되었다. 전북대학교 치과대학병원 교정과에 내원한 골격성 III급 부정교합자중 교정 치료의 경험이 없는 남자 29명, 여자 31명, 합계 60명(평균나이 :남자 19.4세, 여자 20.2세)의 표본을 연구 대상으로 하여 III급 부정교합의 치아, 치조의 보상적 변화 양상을 분석하여, 다음과 같은 결론을 얻었다. 1. 골격성 III급 부정교합자군는 남녀 모두 정상교합보다 작은 IMPA를 보였다(p<0.01). 2. 골격성 III급 부정교합자 남녀군 모두에서 하악골의 전후방적 위치를 나타내는 SNB 및 NtoPog은 하악전치의 전후방 경사를 나타내는 IMPA와 음의 상관관계를 보였다(p<0.01). 남자군에서는 SNB가 SNU1, FHU1, PalU1과 양의 상관관계를 보였으나(p<0.01), 여자군에서는 SNU1만이 상관관계를 보였다(p<0.01). 3. 골격성 III급 부정교합자의 남자군에서 하악골의 수직적 위치관계를 나타내는 SNMP, FMA, PalMP와 하악전치의 전후방 경사를 나타내는 IMPA는 음의 상관관계를 보였으나(p<0.01), SNU1, FHU1, PalU1과는 상관관계를 보이지 않았다. 골격성 III급 부정교합자의 여자군에서는 FMA, PalMP와 IMPA가 음의 상관관계를 보였다(p<0.01). 4. 골격성 III급 부정교합자군에서 high angle군에서 low angle군과 비교하여 SNU1, IMPA가 작은값을 보였다(<0.05).

골격성 III급 부정교합자의 악골 부조화가 구치부 치성보상에 미치는 영향 (The relationship between posterior dental compensation and skeletal discrepancy in class III malocclusion)

  • 골격성 III급 부정교합자는 부적절한 악골 위치에서 기능 교합을 유지하기 위해 치아의 경사도와 수직적 위치가 변화하게 된다. 이러한 치성보상은 전치부 및 구치부 전반에 걸쳐 일어나며, 현재 보편적으로 시행되고 있는 악교정수술을 위한 술전교정시 이를 제거하여 각 골격 구조내에서 치아들이 적절히 위치되도록 하여야 한다. 전치부 치성보상에 관한 측모 두부방사선 사진을 이용한 연구는 많이 시행되어졌으나, 구치부 경사도에 관한 연구는 미미 한 실정이다. 이에 본 연구는 성인 골격성 III급 부정교합자의 전후방적, 수직적 악골 부조화에 따른 폭경 부조화 및 구치 경사도 변화를 알아보기 위해 진단 모형상에서 상하악 견치, 제1소구치, 제1대구치 부위의 기저골 폭경을 측정하여 상악에 대한 하악의 비율을 구하였으며, 모형 을 삭제하여 좌우 구치간 경사도를 측정하였다. 악골 부조화 심화에 따른 기저골 폭경 비율과 제1대구치 협설 경사도 변화의 상관관계를 조사하여 다음과 같은 결론을 얻었다. 1. 골격성 III급 부정교합자의 악골의 전후방적 부조화가 심화될수록 상하악 구치간 각도합이 증가하여 치성보상이 많이 일어났으며, 특히 하악 구치가 설측 경사되는 치성보상을 보였다(p<0.001). 2. 골격성 III급 부정교합자의 악골의 수직적 부조화 및 폭경 부조화와 구치부 치성보상 간에는 유의할만한 상관성이 없었다. 3. 골격성 III급 부정교합자의 전후방적, 수직적 부조화의 심화와 기저골 폭경 부조화는 유의할만한 상관성이 없었다.

측모 두부방사선 계측법에 의한 혼합 치열기 아동의 연조직에 관한 연구 (A ROENTGENOCEPHALOMETRIC STUDY ON THE SOFT TISSUE OF THE CHILDREN IN MIXED DENTITION)

  • The Purpose of this study was to investigate the differences in soft tissue characteristics according to the dental or skeletal dysplasia. For this purpose, lateral cephalogram of 153 children (Hellman dental age IIIB: control group 32, Angle CIII. div. 1 malocclusion group 55, Angle Cl III group 66) were traced and measured. For these measurements, following conclusions were made. 1. FH A, FH Sn, FH UL, AA' of the Class III group were thicker than those of the normal and Class II group, but FH B, FH LL, BB' of the Class III group were not significantly different from those of the normal group. 2. FH B, FH LL, BB' of the Class II group were thicker than those of the normal and Class III group, but FH A, FH Sn, FH UL, AA' of the Class II group were not significantly different from those of the noraml group. 3. Ans-Sn, FH P were not significantly different in three groups, while PP' of the Class III group was thicker than those of the other groups. 4. The lower lips of the Class II group were more anterioly everted with respect to the lower incisor inclination than those of the other groups. 5. The severity of skeletal dysplasia was partly camouflaged by the soft tissue.
