The relationship between uric acid and anemia in Korean adults with chronic kidney disease (국내 만성신장질환자에서 나타나는 요산과 빈혈과의 관련성)

  • The present study was conducted to assess the relationship between hyperuricemia and anemia in Korean adults with chronic kidney disease (CKD). Data from 557 adults (age ≥ 20 years) in the Eighth Korean National Health and Nutrition Examination Survey (2019-2021) were analyzed. Several key findings were identified. First, after adjusting for the related variables, the odds ratio (OR) of anemia (hemoglobin [Hb] < 13.0 mg/dL in men or < 12 mg/dL in women), using the normouricemia group (uric acid [UA] < 7.0 mg/dL in men or < 6.0 mg/dL in women) as a reference, was positively significant for the hyperuricemia group (uric acid [UA] ≥ 7.0 mg/dL in men or ≥ 6.0 mg/dL in women) in overall population (odds ratio [OR], 1.719; 95% confidence interval [CI], 1.371-2.942), men (OR, 2.008; 95% CI, 1.013-3.165), and women (OR, 2.246; 95% CI, 1.315-3.835). In conclusions, hyperuricemia was associated with an increase in anemia in both Korean men and women with chronic kidney disease.

Clinical Significance of Immature Reticulocyte Fraction and Reticulocyte Cellular Indices in Pediatric Anemia Patients (망상적혈구 지수 및 미성숙망상적혈구 분획의 소아 빈혈에서의 임상적 의의)

  • Purpose : Flow cytometric automated reticulocyte analysis is a superior method to manual reticulocyte counting, with respect to precision and sensitivity. Furthermore, flow cytometric analysis is able to measure immature reticulocyte fraction(IRF) and reticulocyte cellular indices(RCI : cell hemoglobin content : CHr, mean cell volume : MCVr, cell hemoglobin concentration mean : CHCMr, distribytion width : RDWr, HDWr, CHDWr). In this study, we investigated the mean values and clinical significances of IRF and RCI in healthy children and pediatric anemia patients. Methods : IRF and RCI were measured with an automated blood cell analyzer, ADVIA 120(Bayer, USA) using oxazine 750 dye, in 57 healthy children and 61 children with anemia. The anemia group consisted of 27 iron deficiency anemia(IDA) patients and 34 patients with anemia associated with acute infection(AAI). We compared the mean values of IRF and RCI in the control group classified according to age, between anemia groups and the control group, and between the IDA group and the AAI group. Results : For the normal control group, the mean values of IRF, CHr, MCVr and HDWr were higher in neonates when compared to older children. The mean values of IRF and RDWr were significantly higher, and the mean values of CHr and CHCMr were significantly lower in the IDA group when compared to the control group. The mean value of IRF was significantly higher, and the mean value of CHDWr was significantly lower in the AAI group when compared to the control group. The mean values of IRF, CHr and CHCMr were significantly lower in the IDA group when compared to the AAI group. Conclusion : We could determine the normal mean values of IRF and RCI in healthy children classified according to age for understanding of hematopoietic response differences according to age. The evaluation of IRF and RCI by automated reticulocyte analyzer seemed to be accurate and clinically useful for the early diagnosis of anemia and the differentiation of IDA from AAI.

The relationship between the prevalence of anemia and dietary intake among adults according to household types based on data from the 7th (2016-2018) Korea National Health and Nutrition Examination Survey (국민건강영양조사 제7기 (2016-2018년)에서의 가구 유형에 따른 성인의 빈혈 유병율과 식이 섭취)

  • Purpose: In this study, data from the 7th Korea National Health and Nutrition Examination Survey (2016-2018) were used to examine the relationship between the prevalence of anemia and dietary intake among adults according to household types. Methods: Using data from a total of 10,646 subjects (4,428 men and 6,218 women), the general information, body measurements, results of biochemical examination, food and nutrient intake, and meal quality evaluation were analyzed according to the type of household. Results: The prevalence of anemia was higher in men belonging to single-person households (SPH) than in those from multi-person households (MPH), while anemia prevalence was higher among the women in the MPH than in the SPH. The men in SPH had a lower total food intake of nuts, vegetables, fruits, fish, and seaweed than the men in MPH, and consumed higher quantities of milk, oil, and processed foods. The women from SPH had a lower intake of seaweed and a higher intake of milk than those belonging to the MPH. In addition, the men in SPH had a lower iron intake and iron intake per 1,000kcal than the men in MPH, lower iron intake through plant-based foods, and a lower iron intake ratio compared to the reference nutrient intake. The total Korean Healthy Eating Index (KHEI) score was lower in both men and women in SPH than in those from the MPH. When analyzing the relationship between household type and anemia risk after correcting for the confusion variable, the risk of anemia in men in SPH increased compared to those belonging to the MPH. However, women showed no such significant correlation. There was no relationship between the total KHEI score and the risk of anemia by gender and household type. Conclusion: In conclusion, since anemia in men belonging to SPH is a matter of concern, it is essential to develop guidelines for anemia-related nutrition education for men living alone.

Experience of a Bloodless Two-Jaw Surgery and Care in Jehovah's Witnesses with Anemia (빈혈이 있는 여호와의 증인 환자에서 무수혈 양악교정 수술)

  • We report a case of 15 year-old Jehovah's Witness patient with mild anemia who underwent a successful orthognathic two-jaw surgery. Jehovah's Witness patients refuse transfusion of blood or blood products even in life threatening situations. The use of recombinant human erythropoietin and iron supplement increased hemoglobin during preoperative period. Intraoperatively, meticulous surgical hemostasis, acute normovolemic hemodilution and induced hypotension enabled the completion of the operation without the use of blood products.

  • Plummer-Vinson syndrome is also termed Paterson-Brown-Kelly syndrome and sideropenic dysphagia, because it was described originally by Paterson and Kelly in 19`19 and it was often combined with iron-deficiency anemia. The syndrome is encountered most often in middle-aged anemic female. It is characterized by dysphagia referred to the upper esophagus, atrophy of the oral or pharyngeal mucous membranes, koilonychia, and stenosis or webs of the upper esophageal mucosa. And it is also related to the late occurrence of the carcinoma of the upper esophagus. Here is presented a case of upper esophageal web with dysphagia and iron-deficiency anemia with review of literature, which was treated by the surgical esophagoplasty to relieve the symptom of dysphagia and to widen the upper esophageal narrowing.

Effect of Deer Blood on Aplastic Anemia Induced Mouse (녹혈이 빈혈유발 Mouse에 미치는 영향)

  • Hematopoietic stem cells in bone marrow form all kinds of blood cells. In traditional medicine, functions of bone marrow cells are very similar to those of Essence(精) which is a fundamental factor of physical development and reproduction. Our experiment examined the effect of deer blood on aplastic anemia induced mouse using cyclophosphamide 150 mg/kg i.p injection before experiment and then another cyclophosphamide 120 mg/kg i.p injection on day 10. Then we administrated dried deer blood in distilled water for 5 days, 9 days and 10 days. We examined blood and marrow samples. In results, deer blood showed a trend of effectiveness on recovery of red blood cells and erythropoietin although they were not statistical significant. And deer blood did not show changes in CD34.

  • Ferrokinetcs by Pollycove's method on 2 cases of Chronic myelogenous leukemia were presented in order to study the erythropoietic function and pathogenesis of anemia. The results are as follows. (1) The daily hemoglobin syntheses were slightly elevated. Both the mean effective hemoglobinization time and mean erythron life span were shortened. The maximum net ROC incorporations of $^{59}Fe$ weere decreased. The in vivo surface measurements of radioactivity over liver, spleen, and sacral bone marrow revealed findings consistant with hemolysis and extramedullary hematopoies. (2) Based upon those findings it is assumed that anemia in chronic myelogenous leukemia may result from hemolysis and inadequate compensation to the developing anemia.

Surgical Treatment of Hemolytic Anemia Induced by Residual VSD and PS (잔여 심실중격결손과 우심실 유출로 협착으로 유발된 용혈성 빈혈의 외과적 치험)

  • A patient with intravascular hemolysis due to residual shunt and right ventricular outflow track obstruction after total correction of TOF was presented. The patient was 29 years old female. She underwented VSD closure with dacron patch, infundibulectomy, pulmonic valvotomy and direct closure of PFO. 8 months after the operation, severe intravascular hemolysis and hemolytic anemia appeared. Conservative therapies were not effective, her general condition and laboratory finding got worse gradually. She underwent reoperation, the shunt was closed and right ventricular outflow tract obstruction was corrected by pulmonary valvotomy, infudibulectomy and transannular patch. After operation, hemolysis disappeared dramatically. Severe hemolysis may induce renal failure and necessitate transfusion frequently. If hemolytic anemia is not corrected by conservative treatment, early reoperation is required.

  • It has been known that garlic, one of the essential ingredient in korean food, has a hypotensive effect. and it is reported that they lower the level of triglycerides, cholesterol and glucose in blood. Especially, the sulfur containing amine acid and the derivatives of the garlic has the counteracting effect to heavy metals. Nowadays, the garlic is known for its efficiency for the various kinds of cancer, neoplasms, hypertension, arteriosclerosis and apoplexy. But, it is reported that the intake of the excessive amount of garlic causes hemolytic anemia recently. The hemolytic anemia is more severe especially in HK phenotype dogs which has a Na-K-ATPase activity. Therefore, this study was performed to examine the effect on the blood of the HK phenotype Jindo dogs when administered the excessive amount of garlic. HK phenotype group showed the significant decrease on RBC, WBC, PCV, Hb, MCV, MCHC, GSH, Met-Hb but LK phenotype group didn't show the significant decrease.

Immune Mediated Hemolytic Anemia in a Dog (개에 있어서 면역매개성 용혈성빈혈 일례)

  • A 3-year-old, 4.0 kg, intact male shih-tzu dog with anorexia, depression, pale mucous membranes, tachypnea, tachycardia was referred to the Veterinary Medical Teaching Hospital. Autoagglutination was observed by naked eye when blood was collected in an EDTA-tube and many spherocytes were found on a Diff-Quik stained blood smear. PCV was 6% and indirect bilirubin was increased markedly. So the immune-mediated hemolytic anemia was diagnosed. Autoagglutination was too severe to perform cross-matching test. Blood was not transfused as it might accelerate or precipitate hemolytic crisis, and regeneration of erythrocytes was very good. Thus corticosteroid of immunosuppressive dose and fluid were administered and PCV was monitored. Although blood was not transfused, PCV increased from 6 to 15.9% in a day and to 30% 7 days later. Therapy for liver was concurrently conducted because liver enzyme activities were high. Corticosteroid tappering therapy was conducted for 75 days and PCV was recovered to 46% after 4 months form start of the treatment.
