Treatment for Distal Humeral Fractures by Modified Anatomical Y-Plate (변형된 Y형 해부학적 금속판을 이용한 상완골 원위부 골절의 치료)

  • Comminuted fracture of the distal end of the humerus in adults is very rare and difficult to treat. In operative treatment, an implant which can achieve accurate anatoraical reduction and rigid fixation is needed. But the pre­existing Y -shaped plate had wide and thick limbs and some problems in fixation for most distal humeral fractures. So we devised a modified plate which is more narrow and th1n and has a different hole distance and wing length (long lateral wing). The aim of the study was to evaluate the clinical result of this modified anatomical Y -plate. From 1991 to 1997, we treated 23 cases of distal humeral fractures using modified anatomical plate and the results were as follows. 1. Fracture type in 18 patients(78%) was C-type(C1,C2,C3) which were intraarticular and mostly displaced or comminuted. 2. Bony union was obtained in 22 patients(96%) through rigid fixation and observed radiologically at 3.5 months(2-6months) on the average. 3. In 23 patients, 19 patients(82%) showed satisfactory results after 34 months(6-73months) follow-up. So if the modified anatomical Y-plate is used in the treatment of distal humeral fracture, a satisfactory result can be obtained through a more accurate, easy, and rigid fixation than preexisting plate.

Analytic Study of 362 Bile Cytologic Materials (담즙 세포학 표본 362건의 검색)

  • A total of 362 bile samples from 104 patients with evidence of biliary tract obstruction were submitted for diagnostic cytology from January, 1989 to April, 1991. The patients were classified based on the obstructive cause, and the cytologic results were reviewed. 298 of the specimens were from patients with intrabiliary malignant structure, and 17 were from patients with extrabiliary malignant compression or benign biliary obstruction. Bile cytology was positive for carcinoma in 42 samples from patients with intrabiliary malignant stricture, and the diagnostic sensitivity was 14%. There were no false positive cases. We concluded that the cause of low sensitivity was degenerative change of cell due to prolonged sampling time. What is noteworthy was the high prevalence of Clonorchis sinensis eggs, being detected in 25 patients with intrabiliary malignancy, supporting the association between clonorchiasis and biliary tree malignancy.

A Comparative Study of Radiographic Images on Normal Anatomical Structures (정상 해부학적 구조물에 대한 X-선 영상의 비교 연구)

  • Purpose: To compare radiographic images of Digora/sup (R)/ system and Ektaspeed Plus film obtained from normal adults. Materials and methods: Storage phosphor plate(SPP) was placed in a film holder behind Ektaspeed Plus film package without lead foil. The effect of film on SPP was studied in a separate in vitro experiment. Forty-seven sets of images were prepared for the evaluaton. The regions of interest(ROI) for evaluation were designated at seven sites including normal anatomical structures. The image quality for each ROI was evaluated on enhanced and unenhanced storage phosphor(SP) images and Ektaspeed Plus film. Results: Two film-SPP configurations showed significantly different gray levels at each step of the aluminum step wedge(p<0.05). The contrasts were comparable. Enhanced SP images were significantly superior to unenhaned images and film in all anatomical sturctures(p<0.01). The differences between unenhanced SP images and film were significant(p<0.05) except root canal and cortical bone on alveolar crest. For anatomical items. there were statistically significant difference among five observers(p<0.05). Conclusions: The image quality of enhanced SP images were superior to Ektaspeed Plus film. and Digora system is potentially applicable to clinical diagnosis.

Comparing the precision of panoramic radiography and cone-beam computed tomography in avoiding anatomical structures critical to dental implant surgery: A retrospective study

  • Purpose: The aim of this study was to evaluate the correlations between measurements made using panoramic radiography and cone-beam computed tomography (CBCT) based on certain anatomical landmarks of the jaws, with the goal of preventing complications due to inaccurate measurements in the pre-surgical planning phase of dental implant placement. Materials and Methods: A total of 56 individuals who underwent panoramic radiography and a CBCT evaluation before dental implant surgery were enrolled in the study. Measurements were performed to identify the shortest vertical distance between the alveolar crest and neighboring anatomical structures, including the maxillary sinus, nasal floor, mandibular canal, and foramen mentale. The differences between the measurements on panoramic radiography and CBCT images were statistically analyzed. Results: Statistically significant differences were observed between the measurements on panoramic radiography and CBCT for all anatomical structures (P<.05). The correlation coefficients (r) between the paired samples obtained from panoramic radiography and CBCT were closely correlated (P<.05), with r values varying from 0.921 and 0.979 for different anatomical regions. Conclusion: The results of this study support the idea that panoramic radiography might provide sufficient information on bone height for preoperative implant planning in routine cases or when CBCT is unavailable. However, an additional CBCT evaluation might be helpful in cases where a safety margin cannot be respected due to insufficient bone height.

Pena Operation as a Redo Procedure for Anorectal Malformation (항문직장기형 재 수술로서 Pena술식)

  • The aim of this study was to evaluate the posterior sagittal anorectoplasty (PSARP) as a re-do operation in patients who failed initial repair of anorectal malformation. Nine patients (4 boys and 5 girls) who had previous failed surgery for anorectal malformation underwent secondary operations through posterior sagittal approach. The main reasons of surgery were constipation (n=3) and persistent anatomical derangement in spite of previous correction surgery (n=6). In addition to constipation, the former group (n=3) had various anatomical defects, and the latter group (n=6), of course, had constipation in some degrees. Patients ranged in age from 2 to 19 years (median 3 years) with only one over the age of 6 years. The primary procedures included PSARP (n=8) and anoplasty (n=1). The rectum was mobilized from surrounding structures through posterior sagittal approach and anatomical defects were corrected. The rectum underwent reconstruction, which involved relocation of the rectum and anus within the limits of the intact muscle complex. Patients underwent follow-up for periods ranging from 6 to 77 months (mean 37 months) after surgery. Anatomical corrections of all the defects were successfully fulfilled in 9 patients. All the patients were satisfied with the functional results after redo-PSARP compared with the preoperative defecatory function. This study suggests that (1) some of the patients with troublesome constipation may have anatomical defects, prominent or hidden, (2) surgeons should suspect the possibility of anatomical defect as the cause of incontinence and (3) preoperative thorough investigation to reveal the anatomical defects should be included in estimating patients with severe incontinence after previous surgery and planning the correction for failed previous surgery as well.

  • Objectives : The aims of this paper are finding the meaning of Yanluozi's visceral figures on the internal alchemy and searching the relevance of the internal alchemy and Oriental medicine. And we would like to compare Yanluozi's visceral figures and anatomical knowledge. Methods : The First, we have found existing researches of visceral figures. And then we have selected figures to need in-depth study. So we have compared visceral figures of huangdibashiyinanjingzuantujujie and Yanluozi's. And analyzed informations relationg to internal alchemy in the ten xiuzhen books of zazhujiejing 18th vol. And we were divided something to represent the anatomical knowledge and internal alchemic theories. Results & Conclusions : Visceral figures of huangdibashiyinanjingzuantujujie are that integrated and supplemented Yanluozi's figures. And those were draw a picture to express internal alchemy theories. Front visceral figure and rear visceral figure of both literatures were drawn based on the anatomical knowledge.

A Study on Pullout Characteristics of Pedicle Screw Design Considering Anatomical Structure of the Lumbar Spine (척추의 해부학적 요소를 고려한 척추경 나사못 디자인의 Pullout 특성 연구)

  • Recently, various types of pedicle screws have been developed considering the anatomical structure of the spine. The purpose of this study was to evaluate the pullout stiffness and strength of two types of commercial pedicle screws. The design of two type screws were single pitched thread (ST) pedicle screw and dual pitched thread (DT) pedicle screw, respectively. The tests were conducted in accordance with the ASTM standards using polyurethane (PU) test blocks which has anatomical structure of the spine. There was no significant difference in pullout stiffness between two types of screw. However, DT exhibited higher pullout strength than ST (p<0.05). Pedicle screw with dual pitched thread showed higher pullout strength without decrease in pullout stiffness compared to the standard pedicle screw. In conclusion, dual pitched thread design of the pedicle screw is considered to be more suitable than the single pitched thread for the anatomical structure of the spine.

PANAGRAPHIC STUDY OF MAXLLlOFACIAL REGION (Panagraph에 의한 악안면에 관한 연구)

  • The author has studied maxillo-facial anatomical landmarks using Status X with two methods. The one has performed by application of contrast media on the human dry skull, the other has performed on living human skull as control group. Comparing the panagraphs taken by two methods, the author has drawn following results: 1. The panagraphs revealed the undistorted, highly sharp panoramic shadows of each jaw on a film. 2. Diminishing the inserted anode tube overlapping-free representation of the anterior teeth was taken. 3. Alternating the head position of the objects, direction of anode tube and film placing, the shadows of temporo-mandibular joint and zygomatic arch were taken without overlapping the other bone tissues. 4. In the panagraphs applied various shaped contrast media to each anatomical landmark, a radio-anatomical atlas which is necessary to interpret various bone tissues was taken. 5. In order to interpret panagraphic shadows easily, the author has tried this study by comparing the films of the living human skull with the films of the human dry skull applied contrast media.

The Relationship Between Anatomical Characteristics and Compression Strength Parallel to Grain of Platanus orientalis L. (버즘나무의 해부학적 성질과 종압축강도와의 관계)

  • This study was carried out to get some basic information on mechanical properties of Platanus orientalis L. for the rational utilization of this wood. Relationship of anatomical characteristics with compression strength parallel to grain was analyzed using stepwise regression technique. All possible combination of 8 independent variables were regressed on compression strength parallel to grain. The summarized results in this study were as follows: 1. The compression strength parallel to grain increased with the increase of wood fiber length and wood fiber width. The strength, however, decreased with increase of number of pore per $\textrm{mm}^2$ and tangential diameter of pore. 2. The major factors affecting compression strength parallel to grain in heartwood were length of wood fiber and number of pore $per{\;}{\textrm{mm}^2}$ but width of wood fiber and length of vessel element were the important factors in sapwood.

Anatomical Characteristics of Paulownia tomentosa Root Wood

  • This study investigated several anatomical characteristics of Paulownia tomentosa roots. The root wood was separated into three parts from stem base (top, middle, and base) at different positions below ground. Qualitative anatomical data suggested that the growth rings in earlywood and latewood were structurally different. Furthermore, the root wood vessels were found having 2 to 3 radial multiples and they were appeared in the form of clusters. In addition, some sheath cells and septate axial parenchyma were observed. Regarding the quantitative anatomical characteristics, vessel and ray numbers per $mm^2$, as well as ray width and height differed significantly among the top, middle, and base rood wood parts. However, there were no significant differences in vessel diameters, cell wall thickness, and width and length of wood fibers among those parts. The crystallinity of the root top part was slightly higher than that of the middle and base parts. Furthermore, the vessel numbers, ray numbers, and ray width and height in the near pith (NP) area were higher compared to those in the near bark (NB) area. However, the fiber width and fiber length at NP were lower than those at NB. Overall, this study demonstrated some significant differences in the anatomical characteristics of the top, middle, and base parts of root wood from Paulownia tomentosa.
