사회적 기술 부족과 충동성으로 인해 또래갈등이 심한 분교아동의 상담사례 (Counseling Case Study of a Child with Peer Confliction due to Lack of Social Skills and Impulsiveness)

  • It seems common for students living at a small county and islands to experience psychological conflicts and be unaccustomed in the peer society because they are not familiar with peer interaction and social skills. This is a case study of L (hereinafter called L) who was grown up in the sheltered school at a small county. L was psychologically disturbed because he couldn't get along well in the transferred school. It is the reason why he had lived in the sheltered school at a small county, so he had not enough exposure to interact with peer and social skills. Sometimes he was obstinate irrationally and when he had trouble with friends, he threw something out or went out of school and tricked juniors dangerously. The fact of disperse with families, parent's indifference, and hate of older brother made L to have ill feeling against family. He had low motivation and low self confident in learning because of short attention time and accumulated poor learning progress. In this study, he was evaluated at various area, such as, intelligent, affective, personal and inter-personal, before counselling. To evaluated the effect of the counselling, K-WISC-III, KPRC, sentence filling test, social adaptation ability test, etc, were administered right after the counselling was over and 8 weeks later. For specific information gathering and analysing, observation diary and deepen counselling were accomplished by homeroom teacher, his mother, and his peers. To correct his problematic behaviors, 13 counseling sessions were accomplished for 6 months and those counselling sessions were recorded and analysed definitely. Followings are the result of this case study. First, he was recovered from the anxiety of inter-personal interaction and he started to interact with peers. The result of sac scale score of KPRC profile was lower than before as much as average student after counseling and 8 weeks later. This reveals that the distress against interpersonal relation have settled. Especially, through the result of sentence filing test, he seemed to feel attachment to peers and be positive, active in the relation of peer. For instance, he was active in the open class lesson and interacted well with peers. It could be said that he overcame the psychological distress comparing with previous time. Second, he could apologize to his peer and juniors for his fault. His attitude were well shown in the letter from an old friend at the sheltered school, average KPRC profiling score comparing with previous counseling time, and remarkable decrease of attack scale score of teacher and peer. Third, his view toward family turn out positive. He recognized his situation that he lived apart from family and even worried about his parent's financial difficulty. Through solving the confliction with his older brother, he could acquire the feeling of family reunion. Fourth, his learning motivation and self-confidence were increased. He confirmed his future positively and he might be judged more attentive because his intelligence index was higher than before as much as average student. With the main goal of this study, verification for effectiveness of counseling. understanding and helping problematic students such as L who lives at a small county and island through investigation of their real situation and problems with the method of counseling and socio-cultural analysis is worthwhile. Identification of ideal relationship with peer is related with positive self-conception, harmonic social adaptation and development of child. It is time to investigate easy adaptive in classroom and well-organised program to acquire general social skills for sheltered school students at a small county and islands.

초등학교 담임교사의 상담자로서 역할지각 및 수행실태와 아동의 기대 분석 (Analyses of Elementary School Homeroom Teachers' Role Percept ion and Performance as Counsellors and Children's Expectation for Teachers' Role as Counsellors)

  • The objective of this study is to conduct survey analyses of the role perception and performance of homeroom teachers in elementary schools in Seoul as well as their students' expectations for teachers' role as counsellors. The study also aims to analyze the causes behind the lackluster performance, to provide assistance in teachers' counselling and guidance activities and collect basic data for providing a plausible orientation for elementary school counselling. Research topics for achieving these study objectives are as follows. First, what is the status quo of counselling between elementary school students and teachers? Second, what is the role perception of elementary school homeroom teachers as counsellors and their current level of performance? Third, what are the differences in students' expectations for homeroom teachers' role as counsellors according to students' environmental variables such as gender and grade? Fourth, what are the discrepancies between the roles perception and performance of elementary school homeroom teachers and role expectation of students for homeroom teachers' role as counsellors? In order to answer these questions, surveys were conducted for 229 teachers and 385 students in grades 4, 5 and 6 in 11 elementary schools in Seoul, and the results were analyzed. The questionnaires used for this study were modified and supplemented according to the research objectives based on survey questions released by Gyung-Beom Lee(1989), Hak-Soo Lee(2001) and Gi-Nam Gwon(2005). Statistical analyses were peformed using the SPSS for Windows 10.0 program. The results of the study can be summarized as follows. First, most elementary school homeroom teachers were involved in counselling activities, and about half of them were providing counselling once a month or less. The classroom was the primary location of counselling, and more than half of the surveyed teachers were dissatisfied with their counselling activities. The teachers cited overwhelming teaching hours and excessive work as the factors that made counselling difficult. Second, it was revealed that most elementary school students have had experiences of anguish and most have had some form of counselling. They mostly sought counselling from their parents and friends, and the reasons behind such choices were that they were very understanding. Third, most students responded that they have had no experience of receiving counselling from their homeroom teachers. Among those with counselling experience with their homeroom teachers, most said that the counselling was helpful. The most significant reason for not receiving counselling from their homeroom teachers was that the students had no worries to talk about with their teachers. Fourth, as a result of categorizing the role of elementary school homeroom teachers as counsellors according to the areas of counselling, role perception for each area turned out to be generally high, while performance was substantially lacking. Fifth, in terms of the causes for the lackluster counselling performance, overwhelming teaching hours and excessive work were indicated for counselling areas of academic and personality issues. Sixth, the analysis of students' expectations for elementary school homeroom teachers as counsellors for counselling areas according to gender and grade revealed that there was no overall statistical significance. Seventh, from the general perspective, the level of role perception of the homeroom teachers were higher than the level of students' expectations. In conclusion, in order to enhance the teacher's role as a counsellor, there has to be a concrete perception of roles as a primary premise, calling for training sessions and programs dedicated to counseling for the teachers to take part in. Moreover, in order to alleviate the most significant causes for undermining teachers' counselling activities - overwhelming teaching hours and excessive work - there must be administrative consideration as well as provisions for effective counselling centers and dedicated school counsellors.

경사형 내부연결 임플란트 시스템 (SS $III^{(R)}$)에서 지대주 형태에 따른 응력분포의 3차원 유한요소 분석 (A 3-dimensional finite element analysis of tapered internal connection implant system (Avana SS $III^{(R)}$) on different abutment connections)

  • 연구 목적: 내부연결형 임플란트 고정체에 연결 방식이 다른 4종류의 지대주를 연결하여 교합력을 가하였을 때 각 구조에 발생하는 응력의 차이를 비교하고, 이들 연결방식이 임플란트 주위조직의 응력분포에 미치는 영향을 3차원 유한요소법을 통하여 알아보고, 지대주의 선택 기준을 마련하고자 하였다. 연구 재료 및 방법: 고정체로는 직경 4 mm, 길이 11.5 mm의 SS-$III^{(R)}$ (Osstem, Korea)를 사용하였으며, 사용된 지대주는 각각, 모델 1에서 Solid abutment, 모델 2에서 Com-Octa abutment, 모델 3에서 ComOcta Gold abutment, 모델 4에서 Octa abutment를 사용하였고, 네 가지 하중조건으로 치관중심와 (central fossa)에 fixture 장축에 평행하게 점하중으로100N의 수직하중, 협측교두에 대하여 fixture 장축에 평행하게 점하중으로 100N 수직하중, 치관중심와에서 설측으로 100N의 $30^{\circ}$ 경사하중, 협측교두 (buccal cusp)에 치아의 바깥쪽에서 내측으로 100N의 $30^{\circ}$ 경사하중을 주었고 3G.Author (Plasso Tech, USA)를 이용하여 분석하였다. 결과: 1. 골조직에서는 모든 모델에서 하중조건에 관계없이 가장 큰 응력이 고정체 상부에 집중되었고, 고정체 하부에서는 근단 부위에서 응력 집중을 보였으며, 그 외의 부위에서는 큰 응력 집중은 보이지 않았다. 2. 고정체에서의 응력은 모든 모델에서 하중조건에 관계없이 neck 부위에서 최대의 양상을 보였다. 3. 응력은 골에서보다 임플란트 내부에서 훨씬 높았다. 4. 중심와에 수직하중인 하중조건 1에서 가장 낮은 응력이 관찰되었으며, 이 때의 응력집중 현상도 가장 적게 나타났다. 서로 다른 지대주에서 응력분포 양상을 살펴본 결과, 같은 고정체를 사용한 경우에 지대주의 연결 형태에 따른 골조직에서의 응력 분포의 차이는 없었다.

버섯 수확 후 배지의 흰점박이꽃무지 사료화 연구 (Evaluation of Spent Mushroom Substrates as Food for White-spotted Flower Chafer, Protaetia brevitarsis seulensis (Coleoptera: Cetoniidae))

  • 항암작용과 간 질환을 예방하는 약용곤충으로 널리 알려진 흰점박이꽃무지 유충은 2016년 12월 일반식품으로 등록되었으나 판매가가 높아 소비가 활발하게 이루어지지 않고 있어 생산 단가를 낮추는 기술이 요구되고 있는 실정이다. 흰점박이꽃무지의 생산 비용을 절감하고 생산성을 향상시키기 위해 농업부산물인 새송이버섯 수확 후 배지와 표고버섯 수확 후 배지를 유충의 먹이로 급이하여 생육특성과 미량성분, 중금속함량을 조사하였고 대조구로는 관행으로 사용되고 있는 참나무 발효톱밥을 급이하였다. 새송이버섯 수확 후 배지를 급이한 처리구가 유충사육기간이 16.2일로 가장 짧았고 유충 무게 증가율과 고치 무게는 각각 156.3%와 4.1 g이었다. 새송이버섯 수확 후 배지를 급이한 처리구에서 용화율은 100%였고 우화율은 93.9%로 가장 높았지만 통계적 유의차는 없었다. 또한 총질소함량도 10.28%로 가장 높았고 미량원소 중 철은 145.8 mg/kg으로 다른 먹이와 비교해 높은 결과를 보였다. 성충의 산란선호성은 먹이 간 통계적 유의차가 없었다. 본 연구의 결과를 종합하였을 때 기존에 사용하고 있는 참나무 발효톱밥 대신 새송이버섯 수확 후 배지를 먹이로 사용하면 흰점박이꽃무지 유충 100 kg 당 667,960원의 비용이 감소할 것으로 추정된다.

강원도 지역 대학생들의 음식 기호도 및 영양 섭취 실태 조사연구 (Food Preferences and Nutrient Intakes of College Students in Kangwon Province)

  • 본 연구는 강원도 지역 대학생 200명을 임의로 추출하여 기호도 및 영양소 섭취실태를 조사한 것으로 그 결과를 요약하면 다음과 같다. 조사 대상자의 식사 섭취 상황은 '거의 먹는다'가 88%, '거의 먹지않는다'가 2%, '가끔 먹는다'가 10%로 나타났으며 타 지역보다 결식율이 낮은 것으로 조사되었다. 결식하는 이유는 '시산 부족'이 45%로 가장 컸으며, '식욕부족'이 29%로 나타났다. 우유 섭취 상황은 '꼭 마신다'가 19%, '가끔 마신다'가 48%, '거의 마시지 않는다'가 19%, '가끔 마신다'가 48%, '거의 마시지 않는다'가 33%로 나타났다. 우유는 거의 완전 식품으로 그 중에서도 칼슘섭취에 가장 큰 기여를 하며, 또한 유당, 카제인, 비타민 D 등 칼 흡수를 촉진하는 인자가 있으므로 식생활에 이로운을 인지해야 된다. 건강에 대한 관심도는 대학생을 조사대상자로 하여 그런지 '보통'으로 조사되었다. 주식에 대한 기호도에서는 밥, 빵, 국수 중 밥을 가장 선호하였다. 좋아하는 밥종류로는 쌀밥.콩나물밥을, 빵 종류로는 햄버거.샌드위치.야채빵을, 국수류로는 비빔국수.냉면.라면으로 나타났다. 싫어하는 주식으로는 수수밥.콩국수 등이었다. 쌀에 대한 아미노산 보족효과가 있는 콩밥에 대한 기호도는 저조하게 나왔는데 이에 대한 영양교육의 필요성이 요구된다. 부식에 대한 기호도에서는 가장 좋아하는 부식의 형태가 찌게이며, 그 다음으로 김치, 복음, 나물 순이었다. 기호도가 낮은 부식의 종류는 장아찌류이다. 기호도가 높은 부식은,국종류(고기국${\ulcorner}남{\Ircorner}$, 미역국.콩나물국), 김치류(배추김치.총각무김치.오이김치), 튀김류(오징어.고구마.감자.새우튀김), 볶음(오징어복음.김치볶음.고기볶음${\ulcorner}남{\Ircorner}$), 전류(고기전${\ulcorner}남{\Ircorner}$, 호박전.햄전), 구이(김구이.더덕구이.쇠고기 구이), 조림(오징어조림, 감자조림), 나물류(콩나물), 무침류(오징어.더덕.도라지무침), 장아찌(더덕.마늘장아찌${\ulcorner}남{\Ircorner}$,오이.깻잎장아찌${\ulcorner}여{\Ircorner}$) 등을 들었다. 기호도가 낮은 부식은 무국.부추김치.쑥튀김.간볶음.간전.가지구이.가지조림.가지나물.양배추무침.고추.양파장아찌 등이었다. 조리장법에 관계없이 좋아하는 식품으로는 오징어, 더덕, 쇠고기, 오이, 콩나물이었고, 조리방법에 관계없이 싫어하는 식품으로는 간.양배추.당근.가지.무우.돼지고기 등이었다. 기호도가 낮은 식품인 간.당근.양배추.무우 등은 각각 단백질, 비타민, 무기질의 좋은 급원이 되는 식품들이므로 영양교육 차원에서 크게 비중을 두어 강조하여야 하겠다. 일품요리 대한 기호도에서는 특별히 싫어하는 음식은 없었고, 오징어덮밥.만두국.비빔밥.김밥 등이 기호도가 높았으며 간식에 대한 기호도에서는 가장 높은 것이 과일유였다. 조사 대상자의 열량소, 단백질, 무기질(Ca, Fe), 비타민(비타민 A.비타민 C.리보플라빈.나이아신) 등의 평균 영양소 섭취량은 권장량과 비교하여 보았을 때 영양소와 철분, 칼슘을 제외하고는 권장량보다 높거나 비슷한 수준이었다. 권장량 이하를 섭취하는 비율은 열량이 가장 많았고, 그 다음이 철분이었다. 강원도 지역 대학생들의 기호도 및 영양실패 조사가 학기중에 실시되었으므로 대학생들의 생활 양식이 매우 유사함을 발견하였으며, 기록자로 하여금 섭취량을 중량으로 환산하기 어려우면 컵량으로 기록하게 하였는데 컵량 표시에 대한 기록자와 분석자간의 눈대중의 오차 또한 있을 것으로 생각된다. 그리고 본 조사는 5월부터 7월중에 실시하였는데, 한국의 경우 계절에 따라 섭취 식품의 종류가 다양하므로 타 계절과는 차이점이 있을 수 있다고 사료된다.고 사료된다.

장류를 이용하여 조리하는 한식 메뉴에 대한 성인 기호도 조사 (Survey on Menu Preferences of Adults for Korean Food Made from Korean Traditional Sauces)

  • 전국의 성인남녀 총 962명을 대상으로 장류를 이용하여 조리하는 한식 메뉴 65종에 대한 기호도를 조사한 결과 전체적으로 기호도가 높은 메뉴는 돼지갈비찜, 쇠갈비찜, 쇠갈비구이, 불고기, 떡갈비 순이었고, 전체적으로 기호도가 낮은 메뉴는 가지나물, 오징어무국, 우엉조림, 도라지오이생채, 마른새우볶음 순이었다. 그리고 여자의 기호도가 남자에 비해 유의적으로 높은 메뉴는 잡채, 도토리묵무침, 궁중떡볶이, 탕평채, 비빔밥, 쇠고기미역국, 숙주나물, 닭찜, 아귀찜이었고, 남자의 기호도가 여자에 비해 유의적으로 높은 메뉴는 더덕구이, 동태찌개, 고추장찌개, 추어탕, 콩비지찌개 등 총 19종이었다. 또한 20세 미만 그룹의 기호도가 다른 그룹보다 유의적 높은 메뉴는 콩나물국이었고 20세 이상에서 30세 미만 그룹의 기호도가 유의적으로 높은 메뉴는 육원전이었다. 한편 40세 이상 그룹의 기호도가 유의적으로 높은 메뉴는 갈치조림, 더덕구이, 조기양념구이, 동태찌개, 추어탕 등 총 15종이었다. 그리고 생산 서비스직군의 기호도가 다른 그룹보다 유의적으로 높은 메뉴는 동태찌개, 추어탕, 북어구이, 북어찜, 북어국, 가지나물, 조기양념구이였고, 사무 전문직군과 학생직군은 쇠갈비찜, 쇠갈비구이, 닭찜, 비빔국수, 육원전, 궁중떡볶이, 순두부찌개, 잡채, 탕평채, 비빔밥, 오징어볶음의 기호도가 생산 서비스직군보다 유의적으로 높았다. 본 연구 결과는 저나트륨 한식 메뉴 조리법 개발을 위한 기초자료로 활용할 수 있을 뿐만 아니라 급식 외식업체의 식단 개발 시에 유용하게 적용될 수 있을 것으로 기대된다.

High Glucose로 유도된 산화 스트레스에 대한 황칠나무 잎 추출물의 뇌신경세포 보호 효과 (Neuronal Cell Protective Effect of Dendropanax morbifera Extract against High Glucose-Induced Oxidative Stress)

  • 본 연구는 한국 고유 식용 자원인 황칠나무의 잎을 이용한 항산화 효과 및 고혈당으로 인한 신경/뇌신경세포 보호 효과를 알아보고자 실시하였다. 황칠나무 잎 추출물의 총폴리페놀 함량, ABTS와 DPPH 라디칼 소거 활성을 측정하였으며 활성이 가장 높은 80% 에탄올 추출물을 이용하여 극성 정도에 따라 분획을 하였다. 이들 중 ethyl acetate 분획물이 총 항산화력(FRAP assay)과 지질과산화물(MDA) 생성 억제 활성이 다른 분획물에 비하여 유의적으로 높은 값을 나타냈다. 이를 이용하여 신경세포로서의 PC12 세포와 인간 뇌조직 유래 뇌신경세포로서의 MC-IXC 세포에서 $H_2O_2$와 고혈당에 의한 세포생존율을 측정하였고, ethyl acetate 분획물은 산화적 스트레스로부터의 효과적인 세포 보호 효과를 나타냈다. 또한, 뇌신경말단에서 신경전달물질(ACh)의 분해를 유발하는 효소(AChE)의 저해 효과를 측정하였고, ethyl acetate 분획물은 유의적인 AChE 저해 효과를 보였다. 마지막으로 황칠나무 잎 ethyl acetate 분획물의 생리활성 물질을 확인하고자 HPLC 분석을 하였으며, 주요 생리활성 물질은 rutin으로 추정되었다. 이러한 연구 결과를 바탕으로 고려할 때 황칠나무 잎 추출물은 천연 항산화제의 역할을 가지고 있을 뿐만 아니라 이를 통해 산화적 스트레스로부터 뇌신경세포 등을 효과적으로 보호할 수 있으며 더불어 AChE 저해 활성을 통한 인지기능 개선 가능 소재로서 연구될 수 있을 것으로 판단된다.

간호교육 철학정립 및 교육과정 개발을 위한 기초조사 (A Preliminary Study on Setting Philosophy and Curriculum Development in Nursing Education)

  • The purpose of this study is to guide the direction of the Korean nursing education to analysize ⑴ the philosophy and objectives ⑵ curriculum, and ⑶ educational environment. This analysis is based on the data from 50 nursing schools (14 4-year colleges and 35 3-year colleges) The survey was conducted from Dec. 1986 through Jan. 1987 by mail. 1) Educational philosophy and objectives 10 4-year colleges and 8 3-year college program have curricular philosoph. Most popular curricular philosophies are human beings, health, nursing, nursology, nursing education, nurses role in the present and in the future. 10 nursing schools mentioned that human being is the subject to interact with : environment physically, mentally and socially. 2 schools mentioned that health is the state of functioning well physically, mentally and socially. 13 schools mentioned that the nursing is the dynamic act to maintain and to promote the highest possible level of health. 4 schools mentioned that the nursology is an applied science. 4 schools mentioned that nursing education is the process to induce the behavioural changes based on the individual ability. There is different opinion about the nurses' role between 4-year college and 3-year college. In the responses from 4-year colleges they focus on the leadership in effective changes, self-regulating and self-determining responsibilities, applying the new technology, continuing education, and participation in research to further nursing knowledge. In the responses from 3-year colleges, they focus on the education in college, primary health care nursing, direct care provider and public health education. Among 50 respondents 40 schools have educational goals which can be divided into two categories. One is to establish the moral and the other is to develop the professionalism. 2) Curriculm The analsis of curriculum is only based on the data from the 4-year colleges because the most of 3-year colleges follow the curriculum guideline set by the Ministry of Education. a) Comparison of the credits in cultural subject and in nursing major. The average required credit for graduation is 154.6 and the median credit is the range of 140-149. The average credit of cultural subjects is 43.4. In detail, the average number of credit of required course and elective courses are 24.1 and 19.3 respectively. The average credit for major subject is 111.2. In detail, the average credit for required courses and electives course are 100.9 and 10.4 respectively. In 5 colleges, students are offered even on elective course b) Comparison of the credit by class. The average earned credits are as follows : 41.1 in freshman, 400 in sophormore 38.3 in junior and 32.4 in senior. Cultural subjects are studied in early phases. c) Comparison of the compulsory and elective cultural subject by institute. The range of credit is 7-43 in compulsory cultural subjects and there are lot of differences among institutions. While all respondents require liberal arts as compulsary subjects, few respondents lists social science, natural science and behavioral science as required subjects. Social science-related subjects are frequently chosen as cultural subjects d) Distribution of creditsin cultural subjects by institute. The liberal art subjects are taught in 20 institute. English and physical education courses are taught in all instituions. The social science subjects are taught in 15 colleges and the basic Psycology and the Basic sociology are the most popular subjects. The natural science subjects are taught in 7 colleges and Biology and Chemistry are the most popular subjects among them. e) Distribution of credits in major basic courses by institute. Most of the institutes select Anatomy, Microbiology, Physiology, biochemistry and Pathology as basic major courses. f) Comparison of the required and elective courses for nursing major by institutions. Subjects and credit ranges in major are varing by institute. More than half of the respondents select the following subjects as required major subjects. (1) Adults Health Nursing and Practice (19.5 credits) (2) Mother and Child Care and Practice (8.9 credits) (3) Community Health Care and Practice (8.5 credits) (4) Psychiatric Nursing Care and Practice (8.1 credits) (5) Nursing Management and Practice (3.9 credits) (6) Fundamental of Nursing, Nursing Research and Health Assessment and Practice. Three institutions select Introduction to nursing, Rehabilitation Nursing, School Nursing, Public Health Nursing, Nursing English, Communication, Human Development as electives in nursing major. 3) Educational environment a) Nursing institution There are forty-three 3-year colleges and seventeen 4-year colleges and 81.4% of which are private b) Number of students and faculty 19.2% of the students are in 4-year colleges and 80.8% of the students are in 3-year colleges. In 4-year colleges, the number of nursing faculty members is in the other of assistant professor, instructor and professor. In 3-year colleges, the orderiis lecturer, associate professor, full time instructor and assistant professor. In 4-year colleges, 18.8 students are allocated per nursing faculty and in 3-year colleges, 33.1 students are allocated per nursing faculty. c) Clinical practices 66.7% of the 4-year colleges practice over 1201 hours in clinic and 28.5% of 3-year colleges practice over 1201 hours in clinic. In 4-year colleges, 11.5 students are allocated per nursing faculty and in 3-year colleges,17 students are allocated per nursing faculty The survey shows no difference in the procedure between 4-year colleges and 3-year colleges but 3-year colleges choose the more variety practicing site such as special hospital and community health clinic. d) Audiovisual facilities The survey shows a lot of difference in audiovisual facilities among institution and 3-year colleges are less equipped than 4-year colleges.

과도한 하안면 고경을 동반하는 하악전돌증환자의 하악골 후퇴술과 이부 감소 성형술식에 관한 연구 (A study on the mandibular setback osteotomy and reduction genioplasty in mandibular prognathism with long anterior facial height)

  • 본 연구는 과도한 하안면 고경을 동반하는 하악전돌증 환자의 악교정수술시 하악골후퇴 술과 이부 감소 성형술을 동시에 시행한 경우의 경조직과 연조직의 변화에 대해 살펴보고, 그 상관성을 평가하여 향후 진단시 보조자료로서 사용하고자 시행되었다. 골격성 III급 부정 교합으로 진단되어 술전 교정치료를 받고 하악골 후퇴술만을 시행한 성인 환자 20명을 대조군(A군)으로, 안모의 수평적 및 수직적 부조화가 심한 골격성 III급 부정교합으로 진단되어 술전 교정치료를 받고 하악골 후퇴술과 이부 감소 성형술을 함께 시행한 성인 환자 20명을 실험군(B군)으로 하였다. 두 군의 경조직 변화에 대한 연조직 변화 양상과, 경조직 변화에 따른 연조직의 이동 비율을 비교 분석하여 다음과 같은 결론을 얻었다. 1. A, B군 모두에서 경조직의 후방 이동과 하안면 하부 2/3의 연조직(Ils, Pog', Me')의 수평적 변화는 높은 상관성을 보였으며, B군에서는 A군에서보다 Ls, Stm, Li의 수평적 변화에도 중등도 이상의 상관성을 보였다. 2. B군의 경조직의 수직 이동에 따른 연조직의 수직적 변화의 상관성은 Point B-Ils, Me-Me'을 제외하고는 A군보다 적은 것으로 나타났다. 3. 경조직의 변화에 대한 연조직의 변화율을 비교해 보면, B군의 경우Pog'의 후방이동 비율이 A군에서보다 적게 나타났고, B군의 Ils, Pog', Me'의 상방이동 비율은 각각 32%, 54%, 60%로 나타났다. 4. 안면의 수직 고경에 대해서는 A, B군 모두에서 하안면 고경의 유의한 감소로 전안면 고경에 대한 하안면 고경의 비율이 감소하였으나, B군에서는 하안면 상부 1/3(Sn-Stm)의 비율은 변화 양상이 유의성있게 나타나지 않았다. 5. 이부 감소 성형술은 하악후퇴술과 함께 시행시 하안면 상부 1/3(Sn-Stm)의 안모비율에 대한 변화가 적고, 하안면 하부 2/3(St-Me')의 고경을 감소시킬 수 있는 술식이다.

농촌 일차 보건사업에 있어서 마을건강원 업무량 및 업적에 관한 연구 (A Study on Performance and Achievement of Village Health Workers in Rural Primary Health Care Program)

  • It is utmostly important to establish the efficient fitable way of peoples' active participation in primary health care especially in the areas where the public or governmental service input for the basic health care is insufficient like as in rural areas of Korea. In light of above reason, this study focused mainly on the evaluation of roles and activities of village health workers (VHWs) who were selected from grass- root level of village people in order to derive further motivation for active participation. This is believed to be a sort of feedback mechanisms. Actually, the authors collected the activity reports of VHWs who had been devoting themselves in the primary health care services of Jeomdong Area, of Yeoju Gun one of Korea University Community Health Action Programmes and survey record on the VHWs activity from correspondent people. 1 hose data were analyzed through computer programmed package. The activities performed by VHWs were limited to the performance in 1985 for conveniance. The summarized results were as follows; 1) General characteristics of VHWs. Among a total of 28 VHWs in the area, about 39.3g of them have been replaced up to the date since the implementation in 1983, because of moving out, occupational employment and of others. The age of majority (75.0%) lied between the range of 30-50, and educational background of 67.9% belonged to category of primary school graduation, about 50% of them experienced to be or were also entiled "chief of women club" of corresponding villages. 2) Work-load of VHWs. Each VHW was assigned for tasks of health care for average 55 households of 248 persons. They shared approximately 6 days a month for the activity in average and it covered 17 cases of basic health care in a month. A half of the VHWs performed home visits irregularly without solidified schedule. 3) Work performance analysis. Informations collected through VHWs were compared with data from official vital registration at local administration center "Myon Office" in 1985. VHWs collected 100.8 of new born, 116.2 of death, 58.3 of move in and 74.8 of move out in comparison with 100.0 of official registration each. Pregnant women of 79.8% of mothers among the total pregnancy of 94 which were confirmed as normally delivered or aborted cases by all means afterwards had been detected by VHWs as being pregnant and all of them received some of antenatal cares by VHWs. All(100%) of delivered women were detected by VHWs through home visits and they were cared postnatally. Whereas, according to the records of birth registration, the places of delivery were clinic in 33.7%, and mother's home in 66.3%, VHWs reported them to be clinic in 48.9%, midwifery in 20.2%. It was cleared that most of misinformation was caused by uncautious filling of birth registration at notification. Among the total of 717 eligible women under age 44 years, family planning status of 92.6% was reported by VHWs confirming practice of control to be 70.8% of reported fertile women. 4) Attitude of VHW on the roles and functions. Although 92.0% of VHWs expressed VHWs to be worthwhile, only 52.0% of them had dignity and satisfaction in their activity and 44.0% of them had passive attitude of working saying they followed direction regardlessly. Concerning difficulties in performance as a VHW, 60.7% of them pointed out lacking of medical and health related knowledge by themselves. Still, 64.0% of them thought visiting unfamilier house to be awful and 40.0% complained forms of activity to be difficult and hard. It was also revealed that 56.6% confessed lack of interest on community health service itself. Most of VHWs needed more educational training especially on clinical fields such as cares of gynecological diseases, hypertension, diabetes, and other chronic diseaes of the aged. Regular on-the-job basic trainings were said to be needed twice a year.
