Staining Response of KMnO4-Treated Animal Amyloid Proteins to Congo red (과망간산카리움 처리(處理)한 동물(動物) 아밀로이드 단백(蛋白)의 Congo red에 대한 염색반응(染色反應))

  • The characteristics of the amyloid proteins were histochemically investigated with the amyloidladen organs from bovine(15 cases), canine(1 case), feline(1 case), and artificially induced rabbit amyloidosis(1 case). The amyloid-laden organs from bovine, canine and experimental rabbit amyloidosis showed potassium-permanganate-senstive reaction, revealing complete disappearance for Congo red affinity, loss of brick-red colored fluorescence and green birefringence. From these findings, the amyloids from bovine, canine and experimentally induced rabbit amyloidosis were thought to be equivalent to amyloid protein A (AA protein). In the present feline case with amyloidosis, however, the amyloid proteins revealed potassium permanganate-resistant reaction, showing unchanged affinity for Congo red, and immunoglobulin was also deposited in the glomeruli of the kidney. From these findings, the amyloid proteins from feline case with amyloidosis were considered to be equivalent to amyloid light chain-related proteins (AL protein).

Delayed Enhancement Magnetic Resonance Imaging Findings in Cardiac Amyloidosis (심장 아밀로이드증의 지연 조영증강 MR 영상소견)

  • Purpose : To evaluate late gadolinium enhancement (LGE) pattern of left ventricular (LV) myocardium and presence or absence of LGE in other regions of the heart on cardiac magnetic resonance (CMR) imaging in patients diagnosed with cardiac amyloidosis. Materials and Methods: From 2009 to 2011, 9 patients who were suspected cardiac amyloidosis underwent CMR. We retrospectively analyzed the presence or absence of LGE and enhancement pattern in LV myocardium, and the presence or absence of LGE in other chambers as well. Also we measured interatrial septal thickness (IST), relative signal intensities of atrial septum and epicardial fat over the left atrial (LA) cavity on delayed enhanced images. MRI parameters in these patients were compared to those of control group of patients with ischemic heart disease by Wilcoxon rank sum test. Results: Of nine patients, LGE were found in 8; subendocardial circumferential pattern in 4 and diffuse pattern in 4. LGE in right ventricle was observed in 7. IST was significantly increased in patients with cardiac amyloidosis (P = 0.02). Ratio of atrial septum to LA cavity and ratio of epicardial fat to LA cavity showed a significant difference (P = 0.0002 and P = 0.0006, respectively). Conclusion: In LGE CMR, subendocardial or diffuse enhancement pattern is a typical finding for patients with cardiac amyloidosis. Atrial septum and epicardial fat show relatively increased signal intensities over LA blood cavity.

The Significance on Determination of Bovine Serum Amyloid Protein A(SAA) Concentration (소 혈청 아밀로이드 단백 A(SAA) 농도 측정의 의의)

  • The present study was performed in order to clarify the significance of serum amyloid A(SAA) estimation for the diagnosis of bovine amyloidosis and SAA as a useful parameter for the health status in herds. Twelve dutch dairy cows with final diagnosis(2 with amyloidosis, 3 with acute inflammatory disease and 7 with chronic inflammatory disease) were used to charify the significance of SAA determination for the diagnosis of bovine amyloidosis. The SAA concentration in the group of inflammatory disease was higher than that of amyloldotic group. Further the SAA value in the group of acute inflammatory disease was higher than that of chronic ones. To clarify the significance of SAA estimation as a useful parameter for the health status in herds, two Korean dairy farms(A and B) were selected and the SAA concentration was determined in total 76 cows(49 from A farm and 27 from B farm). The SAA concentration in cows from A farm was ranged with 0~169%. The cows with high level of SAA(31~169%) had the disease histories(1 with retained placenta, 3 with chronic mastitis, 2 with acute mastitis, 1 with abortion and acute mastitis, 1 with ovarian dysfunction, 1 with downer cow syndrome and 1 with laceration of the teat). The SAA value in the cows from B farm was ranged with 0~29% and disease history was not detected. In conclusion the SAA determination only is thought to be difficult for the diagnosis of bovine amyloidosis. Furthermore SAA estimation is thought to be a useful parameter for the health status in herds.

  • Introduction: We present a case of multiple myeloma with amyloidosis, which has features of peripheral neuropathy after induction chemotherapy before autologous peripheral blood stem cell transplantation, in a 56-year-old woman with Korean medicine. Case Presentation: For 17 days of hospitalization, the patient with complaints of numbness and a tingling sensation in the hands and feet was treated with acupuncture, herbal medicine. To reduce the symptoms, we provided Korean medicine treatments, including herbal medicine, acupuncture, and moxibustion. The Visual Analog Scale (VAS) was used to evaluate the results of the treatment. Until discharge, the VAS scores decreased for both hands and the foot tingling sensation. Conclusion: According to these results, Korean medicine treatment may be considered an effective treatment for tingling sensations in a patient with multiple myeloma with amyloidosis. Prospective studies are needed in the future to confirm and expand these findings.

Renal Amyloidosis in a Beagle (비글견에서 신장 아밀로이드증 증례)

  • An eight-year-old intact male Beagle had anorexia, vomiting, depression for two days. The dog had hypoalbuminenia and mild azotemia on hematologic and clinical chemistry examinations. Severe proteinuria was identified on urinalysis. On abdominal ultrasonographs, there were small amount of ascites and bilateral renomegaly with severe hyperechoic renal cortex and hyperechoic medullary rim sign. Renal biopsy and histopathology revealed renal amyloidosis. The quality of life in this dog was satisfactory with aggressive supportive care for three months. Euthanasia was performed due to deteriorated azotemia and nephrotic syndrome.

Pathologic changes on Renal Lesions in Sows (모돈의 신장에 관한 병리학적 관찰)

  • In order to investigate the prevalence and pattern of renal lesions in sows, 250 kidneys collected from abattoir were examined grossly and histopathologically. The prevalence of renal lesions in sows was 46.8% (l17/250). Main gross findings were consisted of congestion and/or petechiation (21.6%), cortical enlargement (15.2%), renal cysts (6.0%), abscessation (4.4%), and infarction (1.5%). Principle microscopic lesions were composed of interstitial nephritis (25.6% ), glomerulosclerosis (13.6%), glomerular thrombosis (3.6%), amyloidosis (2.0%) and glomerulosclerosis (2.0%) Sixty four kidneys with interstitial nephritis was classified by 46 chronic and 18 acute cases. Among 34 kidneys with glomerulonephritis, there were divided into 18 membranous type, 9 proliferative type and 7 membranoproliferative type. For these results, it was confirmed that sows raised in Kwangju and Chonnam areas had been affected by a variety of pathological renal lesions.

Backbone NMR chemical shift assignment of transthyretin

  • Transthyretin (TTR) is an important transporter protein for thyroxine (T4) and a holo-retinol protein in human. In its native state, TTR forms a tetrameric complex to construct the hydrophobic binding pocket for T4. On the other hand, this protein is also infamous for its amyloidogenic propensity, which causes various human diseases, such as senile systemic amyloidosis and familial amyloid polyneuropathy/cardiomyopathy. In this work, to investigate various structural features of TTR with solution-state nuclear magnetic resonance (NMR) spectroscopy, we conducted backbone NMR signal assignments. Except the N-terminal two residues and prolines, backbone 1H-15N signals of all residues were successfully assigned with additional chemical shift information of 13CO, 13Cα, and 13Cβ for most residues. The chemical shift information reported here will become an important basis for subsequent structural and functional studies of TTR.

  • Nodular pulmonary amyloidosis is one of the rare manifestation of amyloid disease. It is known to be caused by amyloid L fibrils in the majority of cases. We experienced an unusual case of a forty-one year-old woman who was presented with multiple nodular lesion on the chest X-ray. CT-guided core needle biopsy, performed on the lesion, showed apple green birefringes, when stained Congo red and examined under polarized light, Ultrastructurally, there are randomly oriented, forming densed networks, and consists of fine, 7.5 to 10nm diameter, rigid, non-branching filaments of various lengths in electron-microscopic finding. We report a case of primary diffuse nodular pulmonary amyloidosis only localized in the lung, which was confirmed by CT guided core needle biopsy.

Seed-Conjugated Polymer Bead for ${\beta}2$-Microglobulin Removal at Neutral pH

  • ${\beta}2$-Microglobulin (${\beta}2m$) is known to be a major factor for dialysis-related amyloidosis. We have studied ${\beta}2m$ removal through an aggregation process, which was initiated by a ligand that could catch the ${\beta}2m$ monomer and promote its aggregation into fibril. As a ligand, we have prepared ${\beta}2m$ fibril fragments and used them as a seed. The seed was coupled to PEGylated-PS beads to remove the monomeric ${\beta}2m$ from solution. The ${\beta}2m$ seed-conjugated resin effectively adsorbed the ${\beta}2m$ monomers with a capacity of 3.6 mg/ml via promoting their aggregation into fibrils on the resin at pH 7.4.
