Purpose: The short dental implant is considered as possible solution in the alveolar bone height deficient cases. The aim of this study was to evaluate clinical availability of short implants by measuring the marginal bone loss of short length implants and comparing with that of conventional length implants. Materials and Methods: The groups were composed of patients who had received at least one implant. The samples of this study were selected from patients who with functional loading after prosthetic treatment for 1 year follow up period. The implants with a length of 5.7 mm and 6mm were considered short. (Bicon Dental implants, USA). The experiment group was composed of $4.5{\times}6mm$, $5{\times}6mm$, $6{\times}5.7mm$ implants (total 18 implants were placed in 14 patients, 8 on maxilla, 10 on mandible). The control group was composed of $4.5{\times}8mm$, $5{\times}8mm$, $4.5{\times}11mm$, $5{\times}11mm$. All implants were selected only by implants placed on molar area. We evaluated marginal bone loss in radiographic images at baseline (implant loading) and 3, 6, 12 months after loading. Additionally, crown-to-implant ratio was evaluated, and marginal bone loss according to crown-to-implant ratio after functional loading was analyzed. Results: The short implant group had a mean marginal bone level of $-0.52{\pm}0.69mm$; the 8mm group, $-0.22{\pm}0.82mm$; and the II mm group, $-0.10{\pm}1.09mm$ after I year of functional loading. But significant differences were not detected between three groups at every follow-up period. Crown-to-implant ratio in short implant group was $1.55{\pm}0.23$; 8mm group was $1.15{\pm}0.18$; and 11mm group was $0.92{\pm}0.15$. Additionally, significant differences between three groups were founded. (P<.0001) The greatest marginal bone loss after 1 year follow-up was founded at crown-to-implant ratio 1~1.49 range in short implant. Conclusion: The marginal bone loss of short implants was comparable to that of long implants. So, the short implants can be a clinically acceptable option.
Tooth mobility is an important part of a periodontal examination and represents a function of the persisting height of the alveolar bone and the width of the periodontal ligament. The purpose of this study was to evaluate the changes of the tooth mobility over 4 week-period following surgical therapy on the periodontal disease. Thirty five patients presenting with moderate periodontal pockets were selected and tooth mobility was measured at weekly intervals using Periotest (Siemens Co., Germany) beginning at the pre-operation examination and ending four weeks following the modified Widman Flap. All data were statistically analyzed using the one-way ANOVA test. The results were obtained as follows; 1. All teeth exhibited the greatest change in mobility at 1 week post-op, mobility generally decreasing with time. 2. Comparison of the weekly tooth mobility data regarding the 1st premolars showed significant differences only between weeks 1 (9.94) and 4 (6.14) (p<0.05). 3. Comparison of the weekly tooth mobility data regarding the 1st molar showed significant changes in the intervals between pre-op (6.49) and week 1 (11.22), pre-op and week 2 (9.37), weeks 1 and 3 (7.65), weeks 1 and 4 (5.62), and weeks 2 and 4 (p<0.05). 4. Comparison of the weekly tooth mobility data regarding the 2nd premolar and 2nd molar showed significant differences between pre-op (6.91, 8.60) and week 1 (11.02, 12.62), weeks 1 and 3 (8.00, 8.05), weeks 3 ad 4 (6.22, 6.71), and weeks 2 (9.34, 11.01) and 4 (p<0.05).

Although physiologic abrasion in normal range need not to be corrected, when hard tissue of teeth are worn abnormally fast, it can cause severe damage and destroy esthetics and, functional structure of occlusion consequently. To establish a correct occlusal plane and space for the patient with worn dentition, it is necessary to increase vertical dimension. However, actual occlusal vertical dimension remains unhanged with compensation for the increase of alveolar bone height equivalent to the decrease of teeth length. A 74-year-old male presented with worn dentition and fractured tooth. Based on the assessment of OVD including clinical findings, full-mouth rehabilitation without increase of OVD was planned. This case presents that a satisfactory clinical result was achieved by restoring the worn dentition without changing occlusal vertical dimension.

In mixed dentition there exists many empty spaces in the arch due to eruption of permanent teeth and exfoliation of primary teeth. The empty spaces makes it difficult to apply fixed orthodontic appliances. Horseshoe Appliance can be used effectively at this stage, holding the whole dentition in one piece. It covers every surface of erupted teeth and prevents extrusion and rotation of single tooth. By using intermaxillary elastic force, remodeling of the alveolar bone is opposite in each arch. In patients who were treated with horseshoe appliance, forward growth of maxilla, labioversion of maxillary incisors and linguoversion of mandibular incisors were obtained. Minimum downward and clockwise rotation of mandible was shown, so increasing anterior facial height was minimized.

The purpose of this study was to evaluate clinical difference in teeth mobility after treatment with the modified Widman flap and the undisplaced flap in humans. Twenty males with moderate periodontal disease were selected. The severity of periodontal disease was evaluated with sulcular bleeding index, pocket depth and attachment level and tooth mobility was measured with Periotest(Siemens Co., Germany) at the initial examination, 1, 2, 4, 6, 8 and 12 weeks following the modified Widman flap and the undisplaced flap operation using the split-mouth technique. The relation of mobility to clinical parameters was statistically analyzed by multiple regression and the change of teeth mobility according to healing process by independent t-test using SPSS program. The results were as follows: 1. There was a strong relationship between the Periotest value(PTV) and attachment level. 2. The change of teeth mobility in both flap procedures was increased significantly at 1 week post-op. and was decreased to preoperative level at 4 weeks post-op, in modified Widman flap and at 6 weeks post-op. in undisplaced flap. 3. The change of teeth mobility in premolar teeth group in undisplaced flap compared to modified Widman flap was generally increased but these changes were not statistically significant. The changes of teeth mobility in molar teeth group in undisplaced flap was increased significantly at 2 weeks post-op.. 4. The change of teeth mobility following undisplaced flap was increased significantly compared to that of modified Widman flap at 2 weeks post-op.

Many of the patients with extensive abrasion need comprehensive restorative treatment. The abrasion is usually caused by attrition, besides of it, there are many reasons for it. The plan of treatment should be started on assessment of the type of attrition and the etiologic analysis. Patient with well-developed masticatory muscle, alveolar process, and high occlusal force and also with little muscle length difference between the stable and the contracted state should be carefully assessed for the vertical dimensional loss and the restoration should be carefully designed. Decrease of tooth length can be compensated by the growth of the alveolar bone height; therefore, consistency of the occlusal vertical dimension is maintained. Accordingly, a careless increase of the vertical dimension can produce muscle fatigue, depressed tooth and pain, and fracture of the restoration. In this case, the patient with multiple tooth abrasion and clenching habit, the edentulous maxillary area is restored with amalgam inserted RPD, and the dentulous area of the maxilla and mandible are treated with fixed restoration accompanying with the increase of vertical dimension. Consequently, we are going to report about the satisfying result in both functional and esthetic aspects.
목적 : 치조파열 환자에 있어서 이차 치조골 이식술을 시행한 후 그 결과를 알아보고자 방사선학적 분석을 통하여 평가하였다. 본 연구의 목적은 수술전 골결손부 인접치아의 치조정 높이와 수술후 골결손부 인접치아의 치조정 높이를 근심측과 원심측에서 각각 측정함으로써 치조정의 높이가 과연 통계적으로 유의성 있게 증가하는지와 치조골 이식술의 성공률이 치조열의 너비와 상관관계가 있는지의 여부를 알아보는 것이다. 환자 및 방법 : 1991년부터 1999년까지 서울대학교병원 구강악안면외과에서 자가장골 채취 후 분쇄피질망사골 형태 또는 block 피질망상골 형태로 이식한 편측성 치조열을 가진 환자 중 최소한 6개월이 경과한 환자 56명을 연구대상으로 하였고 두 술자에 의해 시술되었다. 수술전 골결손부 인접치아의 치조정의 높이와 치조열의 너비 및 수술후 이식한 골의 높이와 절흔의 양은 치과용 파노라마 방사선 사진을 이용하여 측정하였고, 치조열의 너비는 모델이나 환자의 골결손부 근심치아의 근원심 폭경을 이용하여 환산하였다. 그리고 이식한 골의 높이와 절흔의 양을 1995년 Long이 제시한 방법으로 측정하여 술전의 측정치와 비교하였다. 결과 : 치조열의 너비는 평균 6.9mm(1.9mm-12.1mm) 였다. 근심에서의 치아는 골이식 당시 중절치가 52개(92.9%), 측절치가 4개(7.1%)였고, 49명의 환자에서 완전맹출을, 6명의 환자에서 부분맹출(측절치 2개, 중절치 4개)을 보였다. 원심측에서의 치아는 골이식 당시 측절치가 25개(44.6%), 견치가 29개(51.8%), 소구치가 2개(3.6%)였고, 완전 맹출이 32.1%, 부분 맹출이 57.2%, 미맹출이 10.7%로서 완전히 맹출하기 전에 골이식한 경우가 67.9%였다. 모든 환자에 있어서 bony bridge가 나타났고, 절흔이 인접치 아래로 연장되지 않았으며, 치조골 이식술 후 oronasal fistula를 보인 환자는 한 명도 없었으므로 성공률은 100%였다. 술후 근심측에서의 치조정의 높이는 근심측 치아 치근길이의 79%(평균), 원심측에서의 치조정의 높이는 원심측 치아 치근길이의 87%(평균)로서 통계적으로 유의성 있게 증가하였다. 결론: 이차 치조골 이식술을 시행한 후 치조정의 높이는 골결손부 근심측과 원심측에서 모두 유의성있게 증가하였고, 근심측에서보다 원심측에서 통계학적으로 더 유의성 있게 증가하였다. 치조열의 너비와 절흔의 양, 치조정의 높이 및 치조골이식 성공률과는 유의성 있는 관련성이 없었다.

Tooth impaction is defined as a cessation of the eruption of a tooth at the level of the oral mucosa or alveolar bone by any causes. Any tooth in the dental arch can be impacted, but the teeth frequently involved in a descending order are the mandibular and maxillary third molars, the maxillary canines, the mandibular and maxillary second premolars, and the maxillary central incisors. In these teeth, impaction of maxillary incisor occurs in about 0.1-0.5% and major causes are trauma, supernumerary teeth and periapical inflammation of primary maxillary incisor. Delayed eruption of a maxillary central incisor results in midline shift, the space's being occupied by an adjacent tooth and different levels of alveolar height. Treatment options are observation, surgical intervention, surgical exposure and orthodontic traction, transplantation and extraction. These cases were about the patients with delayed eruption of maxillary central incisor. We surgically exposed impacted tooth and guided it into normal position by the orthodontic traction. At the completion of traction, the maxillary central incisor was positoned fairly within the arch and complications such as root resorption were not observed.
This study was designed to investigate the stress intensity and distribution produced by 1mm activation of retraction archwire with $0^{\circ},\;7^{\circ},\;14^{\circ}$ torque and application of high polk J-hook headgear during retraction of four maxillary incisors using the photoelastic stress analysis. The photoelastic model was made with a PL-3 type epoxy resin which was substituted by alveolar bone portion. Each retraction archwire was fabricated from .020' X .025' stainless steel wire which had vertical loops in 7mm height and hooks for high pull J-hook headgear between central and lateral incisors. The high pull J-hook headgear was applied 35 degree backward and upward to occlusal plane with 200gm pet each side The findings of this study were as follows: 1. In case of $0^{\circ}$ torque, the stress was distributed from cervical 1/8 to apex of roots of central and lateral incisors which were the forms of arc mode. When the high pull J-hook headgear was applied, the stress distributed by arc mode was presented from cervical 1/2 to apex of roots of central and lateral incisors. And the stress distributed by following the root surface was presented from alveolar crest to cervical 1/2 of roots of central and lateral incisors. The stress between apecies of central and Lateral incisors was presented also. 2. In case of $7^{\circ}$ torque, the stress distributed by arc mode was presented from cervical 1/2 to apex of roots of central and lateral incisors. And the stress distributed by following the root surface was presented from alveolar crest to cervical 1/2 of roots of central and lateral incisors. When the high pull J-hook headgear was applied, the stress distributed by following the root surface was presented mote apically than without headgear. The stress between apecies of central and lateral incisors was presented also. 3. In case of $14^{\circ}$ torque, the stress distributed by following the root surface was Presented from alveolar crest to apex of roots of central and lateral incisors. When the high pull J-hook headgear was applied, the stress distributed by following the root surface was presented stronger than without headgear The stress between apecies of central and lateral incisors was presented also.

It is well recognized that a cross-bite tends to occur more frequently among Japanese, Chinese and Koreans more than among Europeans. A variety of functional appliances have already invented for the treatment of cross-bites. One of them is the FR III(by Rolf $Fr\ddot{a}nkel$) which applies a quite different philosophy based on the using vestibular region compared with other methods which utilize muscular forces applied directly on the teeth. The FR III also increases both the lack of muscular tension against the dental arch and the dentition and muscular forces which can then influence the teeth indirectly. This mechanism can achieve favorable developments with the basal bone, teeth, and alveolar bone. After using FR III to the anterior cross-bite patient with the eruption of a maxillary incisor the results were as follows: 1. Forward growth of maxilla and proclination of the upper incisor 2. Downward and backward rotation of the mandible 3. Increase the facial height 4. The case which was normal mandible, underdevelopmental maxilla, deeper overbite and more nagative overjet exhibited good prognosis
